Significance of Membranous Urethral Length for Recovery From Postoperative Urinary Incontinence Following Holmium Laser Enucleation of the Prostate

        Shintaro Oka,Keita Kobayashi,Kenji Matsuda,Kimio Taka 대한배뇨장애요실금학회 2020 International Neurourology Journal Vol.24 No.4

Purpose: The aim of this study was to determine the significance of the membranous urethral length (MUL), including the thickness of the urethral sphincter, for recovery from postoperative stress urinary incontinence (SUI) following holmium laser enucleation of the prostate (HoLEP).Methods: We analyzed 78 patients who underwent HoLEP between June 2013 and September 2018, all of whom preoperatively received magnetic resonance imaging. MUL was measured using sagittal T2-weighted fast spin-echo images. The clinical and anatomical factors associated with MUL were evaluated. The recovery time of urinary incontinence was compared between patients with a long MUL (≥14 mm) and a short MUL (<14 mm). SUI included both stress and mixed urinary incontinence. Continence was defined as complete dryness. Results: The median MUL in patients without incontinence at 1 month postoperatively was significantly longer than the MUL in patients with incontinence (15.3 mm vs. 12.7 mm, P<0.001). The continence rates at 1 month after HoLEP in patients with longer MULs and shorter MULs were 80.4% and 30.0%, respectively. The recovery time of urinary incontinence in patients with longer MULs (≥14 mm) was significantly shorter than that in patients with shorter MULs (<14 mm) (log-rank test, P=0.001). After 6 months, the continence rates in patients with longer MULs and shorter MULs were similar (97%). MUL was significantly correlated with the recovery period of urinary incontinence (r=-0.459, P<0.001).Conclusions: MUL was useful for predicting early recovery from urinary incontinence following HoLEP. This study provides evidence that postoperative urinary incontinence following a transurethral procedure for benign prostatic hyperplasia was associated with anatomical factors. A long MUL was associated with better tolerance to urinary sphincter damage by the transurethral procedure.

      • Hot or Cold? The Effects of Anger and Perceived Responsibility on Sport Fans’ Negative Word-of-Mouth in Athlete Scandals

Shintaro Sato 글로벌지식마케팅경영학회 2018 Journal of Global Sport Management Vol. No.

Despite the frequent controversies in sport, little research has been conducted to understand sport consumers' emotional, cognitive, and behavioral reactions to athlete scandals. Drawing from attribution and expectancy–violation theories, this study examines the effects of athlete scandals on emotion (i.e. anger), cognition (i.e. perceived responsibility), and behavioral intention (i.e. negative word-of-mouth; NWOM). The results of an experiment showed that selected characteristics of athlete scandals (i.e. intentionality and performance relatedness) influence NWOM via perceived responsibility. The impact of perceived responsibility that is associated with NWOM intention was critical, while that associated with anger was much less crucial. This study contributes to the existing body of literature in the context of athlete scandals. When minimizing the negative impact is the main concern, managers must contemplate the effects of performance relatedness and intentionality of athlete scandals, and effectively communicate with spectators to reduce the perceived responsibility.

Asia-Only vs. Asia-Pacific Regionalism: The Regionalism Cycle in Summits and Financial Cooperation

Shintaro Hamanaka 인하대학교 국제관계연구소 2011 Pacific Focus Vol.26 No.2

While many believe that Asian regionalism lacks institutions, Asia, in fact, is home to a wide range of regional institutions and frameworks. The rise and fall of regional institutions in Asia is an extremely dynamic process. This paper argues that the dynamic nature of Asian regionalism can be explained by a "regionalism cycle" with regard to membership. The institutional outcomes of regionalism in Asia have been cyclical because the interplay between Japan and the United States has not had a stable equilibrium. This paper tests the hypothesized regionalism cycle based using actual cases of regional institutions in the areas of financial cooperation and summit meetings.

Promotion of Blood Fluidity by Inhibition of Platelet Adhesion Using Electroacupuncture Stimulation

        Shintaro Ishikawa,Makoto Murai,Takao Sato,Masataka Sunagawa,Erika Tokita,Steven K.H. Aung,Kazuhito Asano,Tadashi Hisamitsu 사단법인약침학회 2011 Journal of Acupuncture & Meridian Studies Vol.4 No.1

Stress applied to rats is known to result in a quick decrease in blood fluidity. Although electrical acupuncture stimulation (ACU) attenuates stress responses, the influence of ACU on blood fluidity has not been well examined. In the present study, the effect of ACU on blood fluidity and platelet adhesion was examined using a Micro Channel Array Flow Analyzer and a laser scattering platelet aggregometer (PA-20), respectively. Male Wistar rats (7−8 weeks old) were used. ACU (1 Hz, 3−5 V),which causes slight muscle twitching, was applied to acupoints for 60 minutes/day once or on 2 consecutive days. Stimulated acupoints were as follows: ZuSanli (ST-36), Sanyinjiao (SP-6), Hegu (L-I4), Neiguan (P-6), and Shenshu (BL-23). ACU applied to ST-36, SP-6, and L-14 revealed significant increases in blood fluidity while platelet adhesion activity decreased. No significant changes were observed when ACU was applied to P-6 and BL-23. Results indicate that ACU affects blood fluidity depending on the acupoints. Blood fluidity changed with ACU within 1 day. In other words, the effect of acupuncture has an immediate effect. In addition, platelet aggregation decreased with ACU, suggesting that an increase in blood fluidity is associated with platelet aggregation ability.

Role of A-Kinase Anchoring Protein 12 in the Central Nervous System

        Shintaro Kimura,Josephine Lok,Irwin H. Gelman,Eng H. Lo,Ken Arai 대한신경과학회 2023 Journal of Clinical Neurology Vol.19 No.4

A-kinase anchoring protein (AKAP) 12 is a scaffolding protein that anchors various signaling proteins to the plasma membrane. These signaling proteins include protein kinase A, protein kinase C, protein phosphatase 2B, Src-family kinases, cyclins, and calmodulin, which regulate their respective signaling pathways. AKAP12 expression is observed in the neurons, astrocytes, endothelial cells, pericytes, and oligodendrocytes of the central nervous system (CNS). Its physiological roles include promoting the development of the blood–brain barrier, maintaining white-matter homeostasis, and even regulating complex cognitive functions such as long-term memory formation. Under pathological conditions, dysregulation of AKAP12 expression levels may be involved in the pathology of neurological diseases such as ischemic brain injury and Alzheimer’s disease. This minireview aimed to summarize the current literature on the role of AKAP12 in the CNS.

Inverse association between sarcopenia and displacement in the early phase of fragility fractures of the pelvis

        Shintaro Honda,Satoshi Ota,Shinnosuke Yamashita,Tadashi Yasuda 대한골다공증학회 2022 Osteoporosis and Sarcopenia Vol.8 No.1

Objectives: Fragility fractures of the pelvis (FFP) commonly occur in the frail elderly. Displacement in the posterior pelvic ring is recognized as the key sign of instability. This study aims to elucidate the relationship between computer tomography (CT)-based frailty markers and displacement of the posterior pelvic ring within 7 days after injury. Methods: This retrospective study included 49 patients (42 females, 7 males) with FFP (type I 10, type II 24, type III 12, type IV 3). On a CT slice at the level of the third lumbar vertebra, skeletal muscle area, skeletal muscle radiation attenuation, and skeletal muscle index (SMI) were calculated as sarcopenia markers. Osteopenia was measured with trabecular region of interest attenuation technique on the same CT slice. Results: There was no difference in the demographics between non-displaced and displaced FFP. CT-based data showed that patients with FFP had osteopenia. However, no difference was found between non-displaced and displaced FFP. SMI was higher in FFP types III/IV than non-displaced FFP when CT-based data on sarcopenia were compared among all patients. Female patients with FFP demonstrated similar results. Logistic regression analysis using the demographics and CT-based markers on sarcopenia and osteopenia revealed that SMI was a potential determinant of displacement of the posterior pelvic ring fractures. Conclusions: There was inverse association between sarcopenia and displacement of the posterior pelvic ring in the early phase of FFP. Relatively preserved muscle may develop displacement in the elderly with osteopenia.

Fitting CAD data to scanned data with large deformation

Shintaro Suzuki,Yutaka Ohtake,Hiromasa Suzuki 한국CDE학회 2020 Journal of computational design and engineering Vol.7 No.2

Recently, largely deformed simulation models, such as car crash simulations, have been in high demand. To evaluate such models, it is necessary to use actual deformation results to compare and validate them. When measuring actual deformed objects, an X-ray CT is useful because it is non-destructive. However, matching undeformed CAD data and scanned deformation data is difficult. We propose a system for users to set control points or control lines on feature points and predict deformation using affine transformation with the moving least squares method. In the proposed method, undesirable distortions are reduced by evaluating scaling using singular value index and introducing offset control points. The deformed CAD data are obtained by matching predicted CAD data and a polygon mesh generated by deformed CT data. In addition, the surface elements of the generated deformed CAD data are evaluated for CAE.

      • DO NARCISSISTS ENGAGE MORE WITH LUXURY BRANDS? EXPLORING THE ROLE OF VANITY ON COMPULSIVE SOCIAL MEDIA USAGE

Shintaro Okazaki,Takumi Tagashira 글로벌지식마케팅경영학회 2018 Global Marketing Conference Vol.2018 No.07

In recent years, compulsive social media use has become an increasingly serious issue in the society. On the one hand, such compulsive behavior could even be viewed as a psychiatric disorder, as it may cause negative psychosocial and professional consequences (Aladwani and Almarzouqand, 2016). On the other hand, excessive involvement in social media may have a positive side—customer engagement which refers to “a customer’s behavioral manifestations that have a brand or firm focus, beyond purchase, resulting from motivational drivers” (van Doorn et al., 2010, p. 254). Of particular interest, a behavioral dimension of engagement consists of vigor or “a customer’s level of energy and mental resilience in interacting with a focal engagement object”, and interaction or “the two-way communications between a focal engagement subject and object. The latter two dimensions” (Brodie et al., 2011, p. 257). In a computer-mediated context, this behavioral dimension of customer engagement implies continuous and repeated contact via social media. However, to our knowledge, little research has addressed the relationship between compulsive social media use and customer engagement. Against this background, this study first tests an explanatory model focusing on customer engagement, compulsive social media usage, and compulsive buying. Then, the study examines the moderating role of narcissism and vanity on the model. The data is collected from an online survey with general consumer sample in the UK. On this basis, we validate the model via structural equation model. In closing, we offer a discussion of the theoretical and managerial implications.
