        Emerging Pharmacologic Therapies for Constipation-predominant Irritable Bowel Syndrome and Chronic Constipation

        Irritable bowel syndrome with constipation and chronic functional constipation are common digestive disorders that negatively impact quality of life and account for billions of dollars in health care costs. Related to the heterogeneity of pathogenesis that underlie these disorders and the failure of symptoms to reliably predict underlying pathophysiology, traditional therapies provide relief to only a subset of affected individuals. The evidence surrounding new and emerging pharmacologic treatments, which include both luminally and systemically acting drugs, is discussed here. These include agents such as lubiprostone, bile acid modulations, guanylate cyclase-C receptor agonists, serotonin receptor modulators and herbal therapies.         A Practical Approach to Achieving the Highest Good In Philosophical Counseling

        A virtue based philosophical approach to counseling which frames itself around the highest good frees both the client and the counselor to immerse themselves more deeply in the counseling process. When they orient themselves and the process towards the highest good, a safe space is co-created for a dialogue about the client’s deepest concerns. Other unique benefits which can be derived from this approach include hope, safety, and the deepening of trust. Elements from phenomenology, pragmatism, and existentialism are utilized to support the client/counseling dyad that centers itself on a search for the highest good, a term used to describe the singular and overriding end that human beings ought to pursue. Existentially, the client gives real, sustained attention to the question of meaning, as well as to personal freedom, choice, and responsibility. Within a phenomenological framework, the counselor identifies meaningful connections from the client’s world which inform the ethical choices he or she needs to make. Pragmatically speaking, both begin to look for the best practical results, realizing them w ithin a f lex ible a nd p rovisional c ontext. This p luralistic approach allows for the use of a variety of methods by both counselor and client as they work towards outcomes grounded in virtue. For these reasons and more, this approach can be remarkably powerful for certain clients.

        A Template for Teaching 21st Century Technologies of the Self

        Technologies of the self, a termed coined by Michel Foucault, refers to the practices and strategies by which individuals represent to themselves their own ethical self-understanding, and through which they are enabled to become the subject of their lives. On their own, or with the help of others, they act upon their bodies, souls, thoughts and conduct in order to transform themselves and to attain a certain state of perfection or happiness. Within Greco-Roman philosophy, these practices fell under the general rubric of knowing and taking care of oneself. They were among the main rules for social and personal conduct. If western philosophy wishes to be of practical value once again, it must restore its original technologies and adapt them to 21st century man. Western philosophy students must be taught these practices so they can go out into the world and offer historically and philosophically grounded ways for people to know themselves and to care for themselves and their souls. The aim of this paper is to offer a pedagogy to support technologies borrowed from the Ancients and adapted for use by the modern person, as well as to present a model for teaching some of these technologies.

        A Novel Paramagnetic Nanoparticle T2 Magnetic Resonance Imaging Contrast Agent With High Colloidal Stability: Polyacrylic Acid-Coated Ultrafine Dysprosium Oxide Nanoparticles

        Until now, there have been limited studies on T2 magnetic resonance imaging (MRI) contrast agents (CAs) using paramagnetic nanoparticles (NPs). In this study, ultrafine dysprosium oxide (Dy2O3) NPs were synthesized as a T2 MRI CA. To obtain high biocompatibility and colloidal stability in aqueous medium for in vivo applications, they were grafted with polyacrylic acid. They displayed paramagnetism with an appreciable magnetization value at 300?K and consequently, an appreciable transverse water proton spin relaxivity (r2) value which increased as the applied MR field increased. Their potential as a T2 MRI CA was proved by taking in vivo T2 MR images in mice.

        Examination of the xanthosine response on gene expression of mammary epithelial cells using RNA-seq technology

        Background: Xanthosine treatment has been previously reported to increase mammary stem cell population and milk production in cattle and goats. However, the underlying molecular mechanisms associated with the increase in stem cell population and milk production remain unclear. Methods: Primiparous Beetal goats were assigned to the study. Five days post-partum, one mammary gland of each goat was infused with xanthosine (TRT) twice daily (2×) for 3 days consecutively, and the other gland served as a control (CON). Milk samples from the TRT and CON glands were collected on the 10th day after the last xanthosine infusion and the total RNA was isolated from milk fat globules (MEGs). Total RNA in MFGs was mainly derived from the milk epithelial cells (MECs) as evidenced by expression of milk synthesis genes. Significant differentially expressed genes (DEGs) were subjected to Gene Ontology (GO) terms using PANTHER and gene networks were generated using STRING db. Results: Preliminary analysis indicated that each individual goat responded to xanthosine treatment differently, with this trend being correlated with specific DEGs within the same animal’s mammary gland. Several pathways are impacted by these DEGs, including cell communication, cell proliferation and anti-microbials. Conclusions: This study provides valuable insights into transcriptomic changes in milk producing epithelial cells in response to xanthosine treatment. Further characterization of DEGs identified in this study is likely to delineate the molecular mechanisms of increased milk production and stem or progenitor cell population by the xanthosine treatment.

        A Surprising Use of Philosophy-Empowering a Teenager to Become the Subject of Her Life-

        This paper presents the surprising story of successful philosophical counseling with a troubled teen who was cutting herself. I say “surprising” because nowhere is philosophical counseling recommended as a therapeutic approach to adolescent cutting. Utilizing a practical approach to philosophical counseling aimed towards the highest good, elements of phenomenology, existentialism and pragmatism were drawn upon, which enabled the client to become aware of what was meaningful to her, to become the subject of her life, and to flourish within it. The client and I made effective use of philosophy to help her reflect on herself and her life, to help her to stop hurting herself, and to become a budding philosopher. To achieve this end, the use of dialogue, reflective thinking, decision making skills, dream analysis, and parental coaching were also employed in the course of eighteen sessions conducted over less than a year. The client was given the opportunity to evaluate her own j udgments, to s tand back f rom her harmful behaviors, and to learn how her desires could be directed to the pursuit of her own good rather than toward her destruction. She acquired an ever-increasing degree of independence in practical reasoning, and a sense of herself as an independent person with agency.

        In Perfect Harmony – How Music Leads Us Towards the Highest Good

        In the Western world today, music is thought of more as an entertainment than as a source of enlightenment. So, it might seem odd to be connecting music with enabling a person to achieve their highest psychological and moral development, which in philosophical terms is known as the highest good. And yet, since the beginning of written history, great thinkers around the world have been interested in why music is such a powerful guiding force, both for individuals and whole cultures. This article takes a closer look at the world of music in order to better understand not only how it contributes to the highest good, but also how it creates greater harmony – not only within the individual, but also between people. It relies on philosophy, psychology, spirituality, and neuroscience to make its points. Perspectives from the East, and examples of music from other cultures, will also give insight into music’s value. One of the aims of these reflections is to move the reader towards a more conscious use of music. To that end, ways in which an individual can more fully extend the musical experience in his or her life are presented. When a larger consciousness surrounds it, more people will be able to take greater advantage of all the good that music has to offer. In turn, this will make our world a better, more harmonious place to live.

      • Precipitation of struvite by utilization of ammonium ions in flue gas scrubbing wastewater

        Struvite precipitation is a useful method to remove ammonium ions from de-NOx scrubbing waste water. This study aims to investigate the effect of process parameters such as pH, molar ratio, reaction time and crystal size on the synthesis yield. The results showed that over 90% NH4+ removal was achieved through struvite precipitation. The stoichiometric molar ratio of 1:1:1 and 30 minutes of reaction time was found sufficient to utilize the residual NH4+ effectively from the wastewater, of which removal efficiency from the waste water was 93 %. The obtained crystalline precipitates distributed from 14 μm to 55 μm depending on reaction condition.

      • Analysis of Community Empowerment Levels in Beef Cattle Breeding Business Based on Village Breeding Center in Karanganyar District, Indonesia

        The purpose of this study was to analyze the level of community empowerment in beef cattle breeding business based on village breeding center in Gondangrejo District, Karanganyar Regency, Indonesia. The study was conducted in Gondangrejo Subdistrict, Karanganyar Regency, Indonesia in April-May 2019. The selection of research locations was purposively conducted and the respondents were selected using census methods. Data collection techniques are carried out by literature study, observation, interview and focus group discussion. The research design uses descriptive analysis. The results showed that the level of community empowerment which included aspects of knowledge was in the medium category, the attitude aspect was in the good category and the skill aspect was in the medium category. The conclusion in this study is the levels of community empowerment in beef cattle breeding business based village breeding center in Gondangrejo District, Karanganyar Regency, Indonesia is in medium category. Therefore this study can be formulated community empowerment model to optimize the result research that still medium category through beef cattle breeding business.

        Modified versus conventional Tenzel semicircular flap in congenital bilateral upper eyelid coloboma occurring as an isolated finding: a case series

        Congenital eyelid coloboma is a full- or partial-thickness defect of the eyelids caused by failure of fusion of the embryonic fissure during embryological development. The aim of this study is to provide information regarding the modified reverse Tenzel semicircular flap procedure in the management of bilateral upper eyelid coloboma. The conventional Tenzel semicircular flap stabilizes the eyelid margins and ensures adequate eye opening and closing functions. However, a modified Tenzel semicircular flap provides enhanced cosmetic results.
