      • 전기경련요법에서 remifentanil이 혈역학에 미치는 효과

        Objective: Electroconvulsive therapy (ECT) is often associated with transient hypertension and tachycardia. Acute hemodynamic change may produce serious complications. Remifentanil is a potent ultra short acting opioid and has been found to reduce blood pressure and heart rate. The goal of this study was to evaluate effects of remifentanil on seizure duration, recovery parameters and hemodynamic changes during ECT. Methods: Ten ASA Ⅰ, Ⅱ patients enrolled in this prospective, randomized trial, each receiving more than four electroconvulsive therapy. Patients were randomized to receive placebo, group P (propofol 1mg/kg+0.9% normal saline 1ml, n=10), and propofol with remifentanil, group R (propofol 1mg/kg+remifentanil 1ug/Kg n=10). Supplemental dose of propofol was given as required to achieve loss of consciousness. We recorded hemodynamic parameters, seizure duration, and recovery parameters. Results: Seizure duration and recovery parameters were similar in group R and group P. Blood pressure and Hear rate after ECT were significantly lower in group R. Conclusions: Remifentanil(1ug/kg) attenuated the acute hemodynamic response to ECT under propofol anesthesia without adversely affecting the duration of seizure activity and recovery profile. Adjunctive use of remifentanil may be good alternative regimens for ECT.

        Ascorbic acid가 두개관 세포군의 cAMP농도에 미치는 영향

        Ascorbic acid has long been known to influence the bone formation by hydroxylating the proline and lysine residues of collagen. Although there were no direct evidences, this effect might be due to the effect on the function of osteoblasts. This study was undertaken to investigate the effect of ascorbic acid on the cAMP accumulation in the calvarial bone cell populations. Fetal rat calvaria at 19th day of gestation were sequentially digested with enzyme mixture of collagenase, trypsin and EDTA for 10, 10, 10, 20 and 20 minutes. Each calvarial bone cell population was primarily cultured for 6-7 days, and then the effects of ascorbic acid, PTH and their combination on the cAMP accumulation were determined. The results were as follows. 1. Ascorbic acid, at concentration of 100㎍/ml, had no effect on the cAMP accumulation in all bone cell populations. 2. PTH, at concentration of 0.4U/ml, increased cAMP in population I, II, IV and V and decreased cAMP in population III, but the effect of PTH was great in population IV and V. 3. Ascorbic acid plus PTH increased the cAMP in population I, II, IV and V, and showed the synergistic effect in population IV.

      • 경막 천자 후 두통 치료 목적의 자가 혈액 봉합술 시행 중 발생한 기뇌증

        A 57-year-old woman who was suffered from chronic back pain and radiating pain to left leg visited the local pain clinic. An accidental dural puncture happened at the L4-5 intervertebral space during the procedure with the loss of resistance technique using air. The epidural needle was removed, and a new insertion was done at the L3-4 intervertebral space with administration of the 0.5% lidocaine 8 ml mixed triamcinolone 40 mg. On the next day, she visited again the local pain clinic with complaining of headache. During an epidural autologuous blood patch procedure, she complained of a severe headache, nausea, vomiting, and developed generalized tremor of abrupt onset. There was no evidence of a dural puncture during the procedure. She was referred to our hospital at once. A brain CT scan showed air-filled cavities in basal cistern. She was given conservative treatment during 14 days. After 10 days, follow up brain CT scan showed no air-filled cavity in basal cistern. She was discharged from our hospital without complications.

      • 합천군 정양지와 박실지의 곤충 및 저서성 대형무척추동물의 분포

        The community structures of benthic macro-invertebrates and insects of reservoirs Jeongyang and Pakasil in Hapcheon-gun were studied. The surveyed sites were three streams and waterfronts of each lake. The Collection was performed from April to October of 2005. As a result, total macreo-invertebrates collected in the reservoir Jeongyang were 3 phyla, 4 classes, 12 orders, 29 families and 56 species. In insects, 12 orders, 71 families and 167 species were identified. Total macro-invertebrates collectd in the reservoir Paksil were 2 phyla, 3 classes, 8 orders, 19 families and 36 species. In insects, 11orders, 50 families and 137 species were identified.Dominant species of benthic macro-invertebrates and insects in reservoir Jeongyang were Chiromomus sp and Nysius plebejus respectively. Dominant species of benthic macro-invertebrates and insects in reservoir Paksil were Physella acuta and Nysius plebejus respectively. The total species richness and species diversity of benthic macro-invertebrates in the reservoir Jeongyang was 16.88 and 1.43 respectively. The total species richness and species diversidty of benthic macro-invertebrates in the reservoir Paksil was 12.12 and 1.10 respectively. The total species richness and species diversity of insects in the reservoir Jeongyang was 56.65 and 1.99 respectively. The total species richness and species diversity of benthic macro-invertebrates in the reservoir Paksil was 47.65. and 0.90 respectively.

        골흡수 기전에 관한 연구 : 파골세포의 활성화 기전 MECHANISM OF OSTEOCLAST ACTIVATION

        Although the osteoclast has long been recognized to be the cell responsible for bone resorption, little is known of the mechanisms by which its activity is controlled. Recently, it has been suggested that osteoblasts ─ the bone-forming cells ─ seem to be the target cells of PTH, the bone-resorbing hormone, and mediate osteoclastic bone resorption by producing the coupling factor(s). Because bone tissue consists of several types of cells, isolation of distinct bone cell populations is prerequisite for studying the mechanism of bone resorption in cellular level. This experiment was performed ⅰ) to isolate the metabolically distinct bone cell populations from fetal rat calvaria by sequential enzyme digestion and biochemical characterization and ⅱ) to identify the factor(s) produced by osteoblast that stimulate resorption employing organ culture of bone. Calvaria from rat fetus at 19 day of gestation, were sequentially digested by enzyme solution consisted of collagenase, trypsin and EDTA for 10 (population I), 10(II), 10(III), 20(IV) and 20 minutes (V). Each bone cell population was primarily cultured for 6-7 days and effects of PTH, calcitonin and PGE_2 on acid and alkaline phosphatase activity and cAMP level were determined. Basal level of acid phosphatase in populations released early were higher than late population. In contrast, basal level of alkaline phosphatase was reversed. PTH(0.4 unit/ml) increased the acid phosphatase activity only in population I with no effect on alkaline phosphatase. Calcitonin(150ng/ml) had no effect on acid and alkaline phosphatase activity in all bone cell populations. cAMP level of population IV and V were increased by PTH significantly while CT had no effect in all bone cell populations at all. PGE_2 increased cAMP in all populations, the acid phosphatase activity in population I and alkaline phosphatase activity in population IV and V. Taken together, these results indicate that population IV and V express typical osteoblastic phenotype while population I revealed some characteristics of osteoclast. Bone cell population IV and V were incubated with fresh MEM or MEM containing 0.4U/ml PTH for 2 hours. After 2 hour-incubation, both the control-conditioned media(control-CM) or PTH-conditioned media(PTH-CM) were collected. Both conditioned media were lyophyllized and redissolved as 2 fold concentrate. Ulnae and radii were removed from 19-day old fetal rats, prelabelled by subcutaneous injection of 200μCi ^45CaCl_2 into their mothers on the 17th day of gestation. After 24 hours, media was changed with fresh BGJb media or BGJb media containing 300μl of control-CM or PTH-CM and cultured for 5 days. Effects of control-CM or PTH-CM were observed by the ratios of %-release of ^45Ca between paired control and experimental group. Control-CM obtained from population IV and V had no or very little effect on bone resorption but PTH-CM obtained from population IV and V increased the ^45Ca release significantly after 3 and 5 days of culture. This result provides the evidence indicating that osteoblastic cells mediate osteoclastic bone resorption stimulated by PTH.

        The community structures of benthic macroinvertebrates and insects of reservoirs Yeoncho and Gucheon in Is. Geoje were studied. The surveyed sites were three streams and waterfronts each lake. The Collection was performed from May to October of 2004. As a result, total macroinvertabrates collected in the reservoir Yeoncho were 2 phyla, 2 classes, 8 orders, 15 families, 17 genera and 18 species. IN insects, 12 orders, 52 families, 89 genera and 95 species were identifies. Total macroinvertabrates collected in the reservoir Gucheon were 3 phyla, 4 classes, 10 orders, 15 families, 18 genera and 18 species. In insects, 8 orders, 45 families, 92 genera and 101 species were identified. Dominant species of benthic macroinvertebrates and insects in reservoir Yeoncho were Macrobrachium nipponense and Nysius(Nysius) plebejus respectively. The total species richness and species diversity of benthic macroinvertebrates in the reservoir Yeoncho was 32.78 and 1,77 respectively. The total species richness and species diversity of benthic macroinvertertebrates in the reservoir Gucheon was 36.48 and 1.88 respectively.

        Dibutyryl cAMP 와 theophylline이 조골세포군의 alkaline phosphatase에 미치는 영향에 관한 연구

        Effects of PGE_2, dibutyryl cAMP (DBcAMP) and theophylline on the acid and alkaline phosphatase of bone cell populations isolated from fetal rat calvaria were studied. Ten fetal rat calvaria of 19th day of gestation were sequentially digested with enzyme solution consisted of 0.1% collagenase, 0.05% trypsin and 0.5mM EDTA for 10, 10, 10, 20 and 20 minutes, and each bone cell population was primarily cultured for 6-7 days. After primary culture, the effects of PGE_2, DBcAMP and theophylline on acid and alkaline phosphatase were studied. The observed results were as follows. 1. Population IV and V of 5 bone cell populations isolated by sequential enzyme digestion were considered to be osteoblast-like cell population. 2. PGE_2 (500ng/ml) increased acid phosphatase in population I and increased alkaline phosphatase in population III, IV and V after 48 hours of culture. 3. DBcAMP (0.5mM) increased alkaline phosphatase in population II, III and V and DBcAMP (0.5mM) plus PGE_2 (500ng/ml) increased alkaline phosphatase in population II, III, IV and V after 24 hours of culture. 4. Theophylline (1mM) increased alkaline phosphatase in population IV and V, and theophylline (1mM) plus PGE_2 (500ng/ml) increased alkaline phosphatase in population III, IV and V after 24 hours of culture.

      • 도시지역 지하수 오염취약성 평가방법의 적용성 연구

        According to favorable water quality, groundwater is used as subsitutive water resources for surfare water. However, groundwater is contaminated more and more due to industrialization and population increase. This study assesses groundwater vulnerability to contaminants in 12 dongs (administrative districe) of Changwon city using with DRASTIC. DRASTIC was originally applied to the situations that contamination sources are on ground surface, and contaminants flow into groundwater with infiltration of rainfall. In addition to DRASTIC factors, groundwater contamination in urban area is also related with overpumping and with lowering of water level. Correlation coefficient between mumimum values of DRASTIC index and unfitness rate to drinking water quality for 10 dongs is low as 0.40. On the contrary, the correlation coefficient between minimum values of DRASTIC index and groundwater discharge rate and that between minimum values of DRASTIC index and well distribution density are 0.70 and 0.87, respectively. Thus, to evaluate the potential of groundwater contamination in urban areas, it is necessary to consider other factors such as groundwater abstraction rate and well density per unit area with ratings and weights as well as existing six DRASTIC factors.

      • 고속철도 건설과 관련하여 지하수위 분포를 이용한 지하수환경 변화 분석

        This study is aimed for estimating change of groundwater environment in relation to constructing Honam express railway in Cheonan-Iksan section. This study was conducted by measuring depth to water and by calculating groundwater level anomaly. Depth to water ranges from 0.03 to 21.1 m and ranges from 0.17 to 227.95 m mean sea level. Distributions of depth to water, of groundwater elevation and of groundwater level anomaly were analyzed to estimate water-level change due to express railway construction. High water-level anomaly section is found in Jangjae(Cheonan)-Daejeon-Iksan section with anomaly index 85.39. High water-level change is anticipated in 22.22-24.19 km, 25.1-25.53 km, 25.53-26.46 km, 26.46-27.77 km, 28.7-29.29 km, 30.18-30.26 km, and 30.26-32.4 km sections from the start of Jangjae(Cheonan).

        FORMOCRESOL을 이용한 치수절단술시 FORMALDEHYDE의 전신적 흡수 및 급성독성에 관한 연구

        Formaldehyde that is active constituent in formocresol used in pulpotomy is known as toxic agent that causes systemic toxicity. To determine the fate of the ^14C-formaldehyde which was absorbed following its application to pulpotomy sites, by-pass of posterior facial vein and cannulation of external carotid artery were established with 20G-polyethylene tube. Blood samples (0.2ml) were collected in glass syringe before the pulpotomy and then at 0, 3, 5, 10, 20, 30, 45, 60, 90 and 120 minutes thereafter. Plasma was separated after centrifugation at 3000 x g for 15 min and aliquots of plasma were counted by liquid scintillation counter. After liver, kidney, lung ,heart, spleen, muscle, and alveolar socket were removed and solubilized with tissue solubilizer, the radioactivities in solutions were counted by liquid scintillation counter. The acute toxicity of formocresol was determined by BUN, GOT and OCT assay in blood samples collected 0, 2, 4, 6, 9 and 12 hours by means of cardiac puncture. The results were as follows ; 1. The radioactivity of ^14C-formaldehyde reached maximal value at 3-5 minutes and decreased thereafter. 2. About 8% of ^14C-formaldehyde placed in the pulpotomy site was actually absorbed systemically until 2 hours. 3. About 50% of ^14C-formaldehyde absorbed actually was distributed within 20 minutes. 4. The radioactivity of organ per gram was highest in the spleen and lowest in the muscle. 5. Total radioactivity of organ was highest in the liver and lowest in the heart. 6. There were no significant differences between control and experimental groups in BUN, GOT and OCT assay.
