        Correcting Postural Deviations: An Evidence- Supported Shift in Paradigm

        Somatic syndromes of the spinal region may develop secondary to postural dysfunction. Traditionally, emphasis has been placed on correcting postural asymmetries by attempting to change muscle-tendon unit length. The effectiveness of this approach in providing prolonged muscle-tendon unit length changes and optimal postural outcomes has met with controversy. The time has come for a redirection of goals in conservative care for postural dysfunction of the spinal regions. An emphasis needs to be placed on continuous, self-awareness training of postural correction that will facilitate lasting neuromuscular adaptations. The purpose of this review is to rationalize the need to veer from exercises attempting to adjust the muscle-tendon unit length and recognize the evidence-supported impact of simple self-correction strategies meant to facilitate neuromuscular adjustments and correct postural asymmetries.

        A Comparison of the Immediate Effect of Swedish Massage vs Passive Stretching on Range of Motion in Overhead Athletes

        [OBJECTIVES] To compare the clinical outcome and effectiveness of Swedish massage and a traditional passive stretching program, on the shoulder of ROM in overhead athletes. [PARTICIPANTS] A total of 10 overhead throwing overhead male athletes (age= 23.5 ± 1.5yrs; height= 186.69 ± 6.35 cm; mass= 91.86 ± 12.48 kg). [METHODS] Participants were randomly assigned to either Swedish massage or a traditional passive static stretching program for 1 session of 5 minutes. Baseline and post-intervention shoulder ROM to end range in the flexed, external rotation, internal rotation, horizontal adduction and abduction positions. The Pearson chi-square test was applied to compare the differences of shoulder ROM of both therapeutic variables. Recorded outcome measures on the participants’ shoulder ROM (flexion with scapula fixed, horizontal adduction, horizontal abduction, internal rotational & external rotation) pre- & post-interventions. RESULTS Results indicate that participants in the Swedish massage therapeutic intervention group had significant improvements in their internal rotation post-treatment (M=5.20, SD= 1.304, p=.038) and the traditional passive stretching protocol group had significant improvements in shoulder external rotation post-treatment (M=11.20, SD=2.68, p=.016). [CONCLUSION] Swedish massage had better effects in four out of the five shoulder motions measured compared to passive static stretching. Educating overhead athletes on the potential beneficial effects of massage therapy and its role in maintaining shoulder range of motion.

        Lumbar Disc Injury: Mechanisms and Intervention Options

        Low back pain is prevalent in sport and the lumbar disc is a pain generator. Conservative care of these physically demanding patients is not consistent as outcome data has not established optimal strategies. Investigating disc injury mechanisms using experimental models will contribute to the creation of treatment strategies that will take into effect movements to avoid and minimize to reduce unwanted strain on the injured tissues throughout the recovery process. Additionally, how disc tissue acquires nutrition would help provide insight into activities that may facilitate the ability of disc tissue to sustain health and promote optimal healing. Rotational forces and compression in trunk-flexed positions may inflict injury to the annulus and endplate structures. Activity to facilitate nutrients through the endplate into the nucleus of the disc and restriction of exasperating movements may be effective in returning athletes to their desired levels of performance efficiently. The purpose of this review is to inquire possible disc injury mechanisms and how they occur in sport. It will also address recommendations for treatment interventions based on disc healing and metabolism evidence.

        The Effect of a Short Foot Eccentric Endurance Exercise on Foot Center of Pressure: A Pilot study

        PURPOSE: This study aimed to investigate the effect of a 5-week short foot eccentric endurance training program on the foot center of pressure measures in healthy university graduate students. METHODS: Eleven healthy students between the ages of 18 and 35 years (n=11, 3 males, 8 females; mean age 26.5±2.5 years; BMI 23.9±2.6 kg/㎡) were included in the study. The participants volunteered in a 5-week progressive eccentric contraction short foot exercise program performed three times per week. The main outcome pressure measurements pre- and post-intervention in static double leg stance were measured using a MatScan<sup>®</sup> Pressure Mapping System. RESULTS: Statistically significant differences were found in the base of support (p=.002, ES=2.59), time to boundary (p=.024, ES=1.63), and overall minima measures (p=.05, ES=1.36) pre- and post-intervention. CONCLUSIONS: A 5-week progressive eccentric contraction short foot exercise program significantly decreased the center of pressure excursion measures, demonstrating improved postural control.

      • 오버헤드 선수를 위한 어깨 안정화 훈련: 운동 차례 (Review)

        Context: Glenohumeral joint instability, primarily anteriorly, is a common problem that can predispose the shoulder to conditions such as secondary and internal impingement. These conditions are experienced frequently with overhead athletes and can both negatively impact their performance and cost significant time away from sport. Objectives: The purpose of this review is to introduce specific types of strengthening and sensorimotor control exercises in a predetermined sequence that are effective in improving dynamic stability to the scapulothoracic articulation and glenohumeral joint. Data Sources: PubMed and Google Scholar databases were searched without limitations from inception until December 2021 for interventions relating to glenohumeral joint stability exercises. Findings: Eccentric strengthening, closed kinetic chain, balance, plyometric and perturbation training all contribute to improvement in joint dynamic stability. Conclusions: The specific types of strengthening and sensorimotor control exercises introduced in this review are effective in providing dynamic stability to the glenohumeral joint. These exercises can be used in the prevention and treatment of conditions encountered by athletes in all sports involving overhead movements.

        Molecular and Cellular Crosstalk between Bone and Brain: Accessing Bidirectional Neural and Musculoskeletal Signaling during Aging and Disease

        Molecular omics technologies, including proteomics, have enabled the elucidation of key signaling pathways that mediate bidirectional communication between the brain and bone tissues. Here we provide a brief summary of the clinical and molecular evidence of the need to study the bone–brain axis of cross-tissue cellular communication. Clear clinical and molecular evidence suggests biological interactions and similarities between bone and brain cells. Here we review the current mass spectrometric techniques for studying brain and bone diseases with an emphasis on neurodegenerative diseases and osteoarthritis/osteoporosis, respectively. Further study of the bone–brain axis on a molecular level and evaluation of the role of proteins, neuropeptides, osteokines, and hormones in molecular pathways linked to bone and brain diseases is critically needed. The use of mass spectrometry and other omics technologies to analyze these cross-tissue signaling events and interactions will help us better understand disease progression and comorbidities and potentially identify new pathways and targets for therapeutic interventions. Proteomic measurements are particularly favorable for investigating the role of signaling and secreted and circulating analytes and identifying molecular and metabolic pathways implicated in age-related diseases.

        Problem Orientated Learning in Technical and Vocational Education

        진보일로에 있는 기술적 경제적 변화에 따라 작업현장에서 적응능력을 기르는 것이 직업교육의 중요한 목표 중 하나이다. 이 목표에서 도달은 問題中心學習을 통해 가능할 것으로 여겨진다. 이를 위해서 기술분야 직업에서 대두될 수 있는 문제해결방법의 원칙을 서술하였다. 다음으로 問題解決思考의 促進을 위한 수업 방법을 나타내는 도표(Table 1)를 제시하였다. 이 도표는 문제해결의 단계(5단계)적인 방법과 문제해결을 위해 敎師가 학생들에게 줄 수 있는 네 가지 종류의 助言(hints)으로 구성되어 있다.
