        특이한 악관절 강직증 : 진성 악관절 강직증과 상악골과 하악골 사이의 골성 유합

        TMJ ankylosis is classified with true and false type. A true ankylosis is defined as any condition that produces fibrous or bony adhesions between the articular surfaces of the temporomandibular joint. The main causes of true ankylosis are trauma or infection. A false ankylosis results from pathologic conditions outside the joint that limit mobility of the mandible such as myogenic disorders, coronoid impingement or rare direct bony fusion between maxilla and mandible. The treatment of choice of TMJ ankylosis is surgical intervention. We experienced the male patient with complete mouth opening limitation since 45 years before. This patient has true TMJ ankylosis and rare bony synostosis between maxilla and mandible in the right posterior region. We performed surgical intervention and had a favorable result.

      • 일령에 따른 닭 태자 샘위의 조직학적 변화에 관한 연구

        닭 태자의 샘위에서 일령에 따른 크기, 조직학적 변화를 관찰하기 위하여 부란 10일부터 부화직후에 걸쳐 10개군으로 구별하고 일반 조직학적 관찰을 실시하였던 바 다음과 같은 결과를 얻었다. 샘위는 전 연령에 걸쳐 긴 방추형으로 복강의 상부에서 관찰되었으며 크기는 부란 10일경에 직경 1.70±0.05mm, 길이 2.40±0.08mm로 관찰된 후 일령에 따라 증가하여 부화직후에는 직경 6.61±0.77mm, 길이 14.21±1.03mm로 관찰되었다. 조직학적으로 닭 태자의 샘위의 상피는 부란 초기 거짓중층원주상피로 구성되었으며 이후 단층원주상피로 분화되고 발생초기 상피의 함입에 의하여 proventricular gland가 형성되며 복합관상샘으로 분화되었다. 또한 점막상피는 다시 분화되어 부란 후기에 gastric gland를 형성하였다. To investigate, histological changes in the proventriculus of the chicken embryos with incubation periods. Samples were taken from 10 groups(10 days of incubation to hatching), and stained with H-E. The findings were as follows. The proventriculus of chicken embryos were observed by long spindle shaped in upper abdominal cavity and the size were detected that 1.70 ± 0.05mm in diameter and 2.40 ± 0.08mm in length at 10 days of incubation, after that the size were increased with incubation periods and in hatching, the size were reached at 6.61±0.77mm in diameter and 14.21±1.03mm in length. Histologically, the epithelium of the proventriculus were pseudostratified columnar epithelium in early incubation periods and after that these epithelia differentiated to simple columnar epithelium. The proventricular glands were made by invagination of surface epithelium in early incubation periods and after that these glands were differentiated to compound tubular gland type. In late incubation periods, the surface epithelium were differentiated twice and made gastric glands. The tunica muscularis and serosa were relatively well developed throughout all incubation periods but muscularis mucosa were observed after hatching.

      • 烏貝散이 HCI-aspirin 으로 유발된 십이지장궤양에 미치는 조직학적 영향

        오패산이 HCI-aspirin tm으로 유발된 rat 십이지장궤양에 미치는 조직학적 영향을 관찰하기 위하여, 오패산을 1,3 및 5일 동안 경구투여하고 십이지장의 조직학적 변화를 관찰하였던 바 다음과 같은 결과를 얻었다. 실험 전 기간동안 대조군에서는 십이지장 융모의 위축과 붕괴가 관찰되었으며, 조직학적으로는 점막상피의 변성 및 탈락이 관찰되었고 점막고유층에서는 염증세포의 침윤(infilteration)의 소견이 관찰된 반면 오패산 투여군의 투여 1일에서는 대조군과 유사하게 관찰되었으나, 투여 3일후부터 점막상피의 첨부에서 경미한 카타르성 물질이 관찰된 이외에 정상군과 유사한 소견을 나타내었다. 이상에서 오패산이 십이지장궤양의 치유를 촉진시키는 것으로 관찰되었다. In order to study, the histological effects of Opeaesan on the duodenal ulcer induced by HCI-aspirin tm in rate, experiments were done by oral administration and observed histological profiles of the ulcerative lesions. Ulcerative lesions were unmerously detected in control groups especially in pylorus-duodenal junctions. Histologically, very severe injury to duodenal epithelium wer observed in control groups and these injuries were increased with time intervals. But in the Opaesan-administration groups, no gross lesion of ulcer were demonstrated and histologically minor injury to the mucosal epithelium were observed. Among the Opaesan treatment groups, most slight injuries to duodenal mucosa were detected in 5 days after administration when the histological profiles were similar to that of normal groups. According to these result, it is suggested that Opaesan should be used in treatment of the duodenal ulcer but the funcitonal mechanisms were unknown.

        A stress analysis on a strip with a circular hole was carried out by the computer assisted photoelastic experimental method. A stress concentration phenomenon around the hole of a strip under axial loading was carefully investigated through this study. The result shows that it well agrees with the results of finite element method and photoelasticity. We suggest that computer assisted photoelasticity experiment is very efficient method for data analysis compared to conventional experimental stress analysis method.

        上口脣 缺損의 再建을 爲한 Estlander-Abbe氏 皮膚瓣 使用例

        Innumerable procedures have been devised for the repair of upper lip defects. The ideal repair should give a normal-looking not too tight lip with a vermillion border, an adequate sulcus, good sensation and good muscle tone and also. It must also be simple to perform under local anesthesia. If the lesions are small, immediate closure after excision can be made. Replacement of larger losses of upper lip has to brought tissues to fill the defects. In the majority of methods, pedicle from adjacent, neighboring or distant parts is used. Also among them, a portion of one lip had frequantly been used to replace losses in the other lip by rotating a flap. In the moderate to larger sized lateral defects of the upper lip, the Estlander Abbe's operation is the most useful procedure.

        The influence of seawater corrosion on fatigue life at low stress level appears to be larger than that at high stress level about M.E.F. dual phase steel. Especially, the variation-range between high stress level and low stress level is large in the sea bottom corrosion of 250 hours and 1000 hours about S-N curves. According to corrosion time and corrosion conditon the amount of reduction of fatigue limit and fatigue life is varied. Fatigue limit and fatigue life for sea surface corrosion condition is lower than those for sea bottom corrosion condition and the influence of sea surface corrosion of fatigue life is large. It is appeared the relation between fatigue length and the number of cycle is depended on stress level, corrosion time and corrosion condition and corrosion pits occurred by seawater corrosion are become the cause of micro-crack. The variation-range of the crack propagation rate for low stress level is larger than that for high stress level. Portions of these fracture surface don't occur plastic deformation by S.E.M observation. However, these are found quasi-cleavage fracture that occur seperate fracture.

      • 雜犬에서 Isoflurane 全身痲醉時 Labetalol과 Sodium Nitroprusside가 心血力學에 미치는 影響

        Many studies have been performing to attenuate hemodynamic responses in related to anesthesia of hypertensive patients because increased blood pressure and heart rate may be even fatal. The author performed this experimental study to evaluate the hemodynamic effect of labetalol which has dual site of action(non-specific beta and postsynaptic alpha 1 receptor blockade) and SNP on cardiovascular system under the 1 % isoflurane anesthesia in 14 mongrel dogs. Heart rate(HR), systolic arterial blood pressure(SAP); diastolic arterial blood pressure(DAP), mean arterial blood pressure(MAP), central venous pressure(CVP), pulmonary arterial pressure (PAP), pulmonary capillary wedge pressure(PCWP), and cardiac output(CO) were measured after labetalol 0.08mg/kg, 0.24mg/kg, and 0.72mg/kg intravenous bolus injection and SNP 0.7mg /kg/min, 2.1mg/kg/min, and 6.3mg/kg/min intravenous dripping. Besides stroke volume (SV), cardicac index(CI), left ventricular stroke work index(LVSWI), systemic vascular resistance (SVR) and pulmonary vascular resistance(PVR) were calculated. The results were summerized as follows 1. SAP, DAP and MAP were decreaed significantly in both groups. HR were decreased significantly in labetalol group but there was no change in SNP group. 2. PCWP showed a tendency to increase in labetalol group but to decrease in SNP group, PAP showed no change in both groups. 3. CO and CI were decreased significantly but SV was not changed in labetalol group. There were no changes in SNP group. LVSWI showed a tendency to decrease in both groups. 4. CVP, SVR and PVR were not changed in both groups. The author conclude both labetalol and SNP are effective to attenuate hemodynamic responses, in hypertensive patients. But labetalol is preferable for hypertensive patient with coronary heart. disease because it has dual blocking action.

      • 등온 사각빔에서 혼합대류 열전달

        Mixed convection heat transfer is investigated by numerical solution of Navier-Stokes and energy equations for and isothermal rectangular beam. The solutions have been obtained for aspect ratios of beam, W=0.3~3, Reynolds numbers, Re=50~150 and Grashof numbers, Gr=0~5×10⁴. The results indicate that as Re increases, the mean back surface Nusselt number decreases to Re=120 for W=0.3 and Re=95 for W=1, 3 and then increases. Independent of aspect ratios of the beam, the mean beam Nusselt number at Gr=10⁴ shows similar value in the region of mixed convection. The mean beam Nusselt number distribution for W=0.3 shows the highest value at Gr/Re²≤1.0 and that for W=3 at Gr/Re²〉1.0. Comparison between the experimental and numerical results shows good agreement.

        計裝化衝擊試驗에서 打擊速度와 溫度의 變化가 豫크랙을 揷入한 衝擊試驗片의 動的破壞靭性에 미치는 影響을 考察한 結果 다음을 알 수 있었다. 豫크랙을 揷入한 試驗片이 V-노치 試驗片에 比하여 實驗値의 分散度가 적으며 0℃以下의 溫度領域에서 動的破壞靭性의 有效性이 있었다. 또한 打擊速度와 實驗溫度의 影響은 溫度가 速度보다 훨씬 上廻하는 傾向이었다. The effect of various impact velocity and temperature on dynamic fracture toughness is tested with precracked Charpy specimen, and conclude following results. Precracked specimen has less dispersion of test results than V-notched specimen, unstable crack initiation load didn't agreed to maximum load at room temperature but at 0℃ below, thus, the availability of dynamic fracture toughness is relatively high. Comparing V-notched specimen, the precracked Charpy specimen represented lower shelf of temperature different. But, at this time, the absorbed energy is estimated equivalently. With the decrease of temperature, the reduction of fracture toughness was obvious, but the effect of impact velocity didn't appear.

        Sleeve 혈관문합술에 대한 조직학적연구

        During the past decade, microsurgery has become clinical routine and resulted in many technical innovations. Inspite of this, microsurgery still entails certain disadvantages; it is time consuming, technically demanding and sometimes results in distressful failure. The sleeve microanastomosis, proximal vessel end is inserted into distal vessel using only two extraluminal sutures, is easier and less time consuming than conventional end to end anastomosis. However, there is a few reports about the histological study to support the beneficial technique of the anastomosis. An animal study was carried out by doing 25 sleeve anastomoses of carotid arteries of white stratin rats and subsequent histological observations in different time interval in order to clarify the modesty of repair in anastomosed vessels. The following are noted as results: Patency rate of the anastomosis was 84%. No aneurysm was found at any site of the anastomosis. The narrowing of lumen of the inserted proximal vessel, the formation of thrombosis & fibrin clot in foci at proximal vessel end, and the loss of intima at the site of anastomosis were noticed in 3 days following the anastomosis. Still narrowing of the lumen of inserted vessel, the formation of thrombosis & fibrin clot in foci at proximal vessel end were persisted with focal necrosis of the media at the stitch area of dista lvessel in one week following anastomosis. Disappearance of thrombosis & fibrin clot on proximal vessel end complete restoration of intimal continuity with slight atrophy of media on distal vessel were noted in 3 week following anastomosis. Neoitima formation at anastomosis site with moderate atrophy of media on distal vessel were noted in 5 week following anastomosis. Subintimal hyperplasia was not observed except in one case of medial necrosis due to wide adventitial stripping in 8 week following anastomosis.
