      • Lack of Any Relationship of Stomach Cancer Incidence and Mortality with Development in Asia

        Background: The aim of this study was to evaluate the incidence and mortality of stomach cancer, and its relationship with the Human Development Index (HDI) and its components in Asia in 2012. Materials and Methods: This ecological study wa conducted based on GLOBOCAN project of WHO for Asian countries. We assessed the correlations between standardized incidence rates (SIR) and standardized mortality rates (SMR) of stomach Cancer with HDI and its components using SPSS18. Results: A total of 696,231 cases (68.7% in males and 31.3% in females, ratio of 2.19:1) and 524,465 deaths (67.1% in men and 33.0% in women, ratio 2.03:1) were included in 2012. Five countries with the highest SIR of stomach cancer were Republic Korea, Mongolia, Japan, China and Tajikistan. Five countries with the highest SMR of stomach cancer were Mongolia, Tajikistan, Kyrgyzstan, Kazakhstan and China. Correlation between HDI and SIR was 0.241 (p = 0.106), in men 0.236 (p = 0.114) and in women -0.250 (p = 0.094). Also between HDI and SMR -0.250 (p = 0.871) in men -0.018 (p = 0.903) and in women -0.014 (p = 0.927). Conclusions: No significant correlation was observed between the SIR of stomach cancer, and the HDI and its dimensions, such as life expectancy at birth, mean years of schooling, and income level of the population.

        Defecographic Findings in Patients with Severe Idiopathic Chronic Constipation

        Background/Aims: Chronic constipation is a common gastrointestinal disorder diagnosed using Rome III criteria. Defecography is a radiographic method used to identify anatomic abnormalities of anorectum. The present study aimed to evaluate the defecographic findings in patients with severe idiopathic chronic constipation. Methods: One hundred patients, who complained of severe idiopathic chronic constipation with abnormal balloon expulsion test, underwent defecography after injection of barium. An analysis of radiographs was performed by an expert radiologist for the diagnosis of descending perineum syndrome, rectocele, enterocele, rectal ulcer, rectal prolapse, fecal residue of post defecation, and etc. Then, they were compared between the two sexes. Results: Normal defecography was only observed in two participants. Descending perineum syndrome was the most common abnor-mality (73.3%). The results showed that rectocele (80.8%) and descending perineum syndrome (69.2%) were most frequent in women. In males, descending perineum syndrome and rectal prolapse were more prevalent (87% and 43.5%, respectively). Compared with men, rectocele and rectal ulcer were more frequently observed in women (p<0.001, and p=0.04, respectively), while men were more affected by descending perineum syndrome (p=0.04). In total, women had a greater incidence of abnormal defecographic find-ings compared with men (p=0.02). Conclusions: Defecography can be performed to detect anatomic abnormalities in patients with severe idiopathic chronic con-stipation and abnormal balloon expulsion test. This technique can assist physicians in making the most suitable decision for surgical procedure. (Korean J Gastroenterol 2017;70:39-43)

        Right Sensory Alien Hand Phenomenon from a Left Pontine Hemorrhage

        BackgroundaaAcute onset of a sensory alien hand phenomenon has been observed only from a supratentorial lesion involving the non-dominant hand, mostly from a right posterior cerebral artery infarction. A single acute vascular lesion resulting in a dominant hand sensory alien hand syndrome has not been previously documented. Case ReportaaA 78-year old right-handed woman exhibited right sensory alien hand phenomenon from a left pontine hemorrhage. Disturbance of proprioceptive input and visuospatial perception are likely to play a role in manifesting the sign. ConclusionsaaDominant-hand sensory alien hand phenomenon may occur in an acute setting from a left pontine hemorrhage.

        Optimum Design of Integer and Fractional-Order PID Controllers for Boost Converter Using SPEA Look-up Tables

        This paper presents a method of designing optimal integer- and fractional-order proportional-integral-derivative (FOPID) controllers for a boost converter to gain a set of favorable characteristics at various operating points. A Pareto-based multi-objective optimization approach called strength Pareto evolutionary algorithm (SPEA) is used to obtain fast and low overshoot start-up and dynamic responses and switching stability. The optimization approach generates a set of optimal gains called Pareto set, which corresponds to a Pareto front. The Pareto front is a set of optimal results for objective functions. These results provide designers with a trade-off look-up table, in which they can easily choose any of the optimal gains based on design requirements. The SPEA also overcomes the difficulties of tuning the FOPID controller, which is an extension to the classic integer-order PID controllers and potentially promises better results. The proposed optimized FOPID controller provides an excellent start-up response and the desired dynamic response. This paper presents a detailed comparison of the optimum integer- and the fractional-order PID controllers. Extensive simulation and experimental results prove the superiority of the proposed design methodology to achieve a wide set of desired technical goals.

        Endpoint Perfect Tracking Control of Robots - A Robust Non Inversion-Based Approach

        This paper presents a simple and robust non inversion-based perfect tracking control (RNIBPTC) strategy for robot manipulators. The proposed approach is capable to eliminate the environmental problems arising from classic feedforward control design and so guarantees an appropriate level of robustness of control system to uncertainties including external disturbances, un-modeled dynamics, friction force and variation of payload. Extensive simulation results performed using a two degree-of-freedom actuated elbow robot prove the effectiveness of the proposed approach. The results are also compared to those obtained from Internal Model Control approach. Using free model of system in control law design is a considerable point in the field of robot manipulator control.

        Role of Urinary Levels of Endothelin-1, Monocyte Chemotactic Peptide-1, and N-Acetyl Glucosaminidase in Predicting the Severity of Obstruction in Hydronephrotic Neonates

        Purpose: Antenatal hydronephrosis (AH) is found in 0.5%–1% of neonates. The aim ofthe study was to assess the urinary concentrations of 3 biomarkers, endothelin-1 (ET-1),monocyte chemotactic peptide-1 (MCP-1), and N-acetyl-glucosaminidase (NAG) in severelyhydronephrotic neonates. Materials and Methods: Neonates with a history of prenatal hydronephrosis were enrolledin the prospective study in 2 groups. Group 1 included neonates with severe formsof obstruction requiring surgical intervention and group 2 included neonates with milderforms of obstruction without any functional impairment. Fresh voided urinary levelsof ET-1, MCP-1, and NAG were measured and their ratios to urinary Cr were calculated. Results: Fourty-two neonates were enrolled into the 2 groups: group 1, 24 patients (21male, 3 female); group 2, 18 neonates (16 male, 2 female). There were no statisticallysignificant differences between urinary ET-1, NAG, MCP-1 values, and ET-1/Cr andNAG/Cr ratios in groups 1 and 2. The urinary MCP-1/Cr ratio was significantly higherin group 1 than in group 2. For comparison of groups 1 and 2, the cut-off values weremeasured as 0.5709 ng/mg (sensitivity, 75%; specificity, 67%; positive predictive value[PPV], 71%; negative predictive value [NPV], 71%), 0.927 ng/mg (sensitivity, 77%; specificity,72%; PPV, 77%; NPV, 72%), and 1.1913 IU/mg (sensitivity, 62%; specificity,67%; PPV, 68%; NPV, 60%) for ET-1/Cr, MCP-1/Cr, and NAG/Cr ratios, respectively. Conclusions: The urinary MCP-1/Cr ratio is significantly elevated in neonates with severeobstruction requiring surgical intervention. Based upon these results, urinaryMCP-1/Cr may be useful in identification of severe obstructive hydronephrosis inneonates.

        Optimal Controller Design for Single-Phase PFC Rectifiers Using SPEA Multi-Objective Optimization

        In this paper a new method for the design of a simple PI controller is presented and it has been applied in the control of a Boost based PFC rectifier. The Strength Pareto evolutionary algorithm, which is based on the Pareto Optimality concept, used in Game theory literature is implemented as a multi-objective optimization approach to gain a good transient response and a high quality input current. In the proposed method, the input current harmonics and the dynamic response have been assumed as objective functions, while the PI controller's gains of the PFC rectifier (Kpi, Tpi) are design variables. The proposed algorithm generates a set of optimal gains called a Pareto Set corresponding to a Pareto Front, which is a set of optimal results for the objective functions. All of the Pareto Front points are optimum, but according to the design priority objective function, each one can be selected. Simulation and experimental results are presented to prove the superiority of the proposed design methodology over other methods.

        SECOND-ORDER SLIDING-MODE CONTROL FOR A PRESSURIZED WATER NUCLEAR REACTOR CONSIDERING THE XENON CONCENTRATION FEEDBACK

        This paper presents findings on the second-order sliding-mode controller for a nuclear research reactor. Sliding-mode controllers for nuclear reactors have been used for some time, but higher-order sliding-mode controllers have the added advantage of reduced chattering. The nonlinear model of Pakistan Research Reactor-1 has been used for higherorder sliding-mode controller design and performance evaluation. The reactor core is simulated based on point kinetics equations and one delayed neutron groups. The model assumes feedback from lumped fuel and coolant temperatures. The effect of xenon concentration is also considered. The employed method is easy to implement in practical applications, and the second-order sliding-mode control exhibits the desired dynamic properties during the entire output-tracking process. Simulation results are presented to demonstrate the effectiveness of the proposed controller in terms of performance, robustness, and stability
