        3개 대학병원의 주 진단 코딩사례 평가

        Background : Coding of principal diagnosis is essential component for producing reliable health statistics. We performed this study to evaluate the current practice of principal diagnoses determination and coding, and to give some basic data to improve coding of principal diagnosis. Method : Nineteen medical record administrators(MRAs) of 3 university hospitals participated in coding principal Dx. From August 1, 2001 to August 31, 2001. From each hospital, 10 medical records of patients with high frequency disease were selected randomly. Each 10 medical records were grouped into three(A,B,C). Then, these 30 medical records were given to each MRAs for coding. At the same time questionnaire was given to each of them. Questions were to prove how they decide and code the principal diagnosis among many current diagnoses ; how they decide and code the principal diagnosis when day see irrelevant diagnosis recorded as the principal diagnosis in medical record, when only tentative diagnoses were recorded without final diagnosis, and when different diagnoses were recorded in different sheets of same record. Agreement of coding among 3 hospitals were compared and survey results were analysed with SAS 6.12. Results : Agreement of coding was found in medical records 5-6 of each 10 medical records. Causes of disagreement were as follows. Difference of clinician’s opinion from each hospital; mixed use of guideline from KCD-3 and guideline from DRG; difference in 4th digit classification according to the absence of pathology report in the medical record; difference of abbreviations among hospitals. 57.9% of decided principal diagnosis after consulting to KCD-3 guideline. When there were difficulties in determining the principal diagnosis, 42.1% of MRAs decided principal diagnosis after discussion with the physician, 26.3% after discussion with fellow MRAs. Conclusion : There were differences in coding among hospitals. To minimize the difference, we suggest the development of disease-specific guidelines for coding in addition to the current general guideline such as KCD-3. To do this, Coding Clinic which can produce guidelines is needed.

      • A New Hybrid Cymbidium ‘Sunny Bell’ with Fragrant of Small Type

        A new hybrid, Cymbidium(C.) ‘Sunny Bell’ was developed as new hybrid by National Institute of Horticultural & Herbal Science, Rural Development Administration, Suwon, Korea in 2013. This hybrid came from the progenies crossed between C. karan and ebruenum in 2000. Finally, one hundred-one seedlings were obtained after planting and acclimatization in green house. In 2007, one line was selected by their performance such as flower color, leaf shape, flower stalk, and vigorous growth and named ‘Wongyuo F1-47. The line code 000390-46 had uniformity and excellent characteristics. The selected line after second characteristics test was named as ‘Sunny Bell’. This hybrid had light purple colored petal and lip (RP59B)and large sized flower(diameter 7.9cm). A ‘Sunny Bell’ had about 7.8 flowers per flower stalk and small sized flower and plant. General impression of petals and sepals showed some incurved shape. The attitude of peduncle was erect. Bloom was started from February under optimal culture condition

        A new hybrid, Cymbidium(C.) ‘Beauty smile’ was developed as new hybrid by National Institute of Horticultural & Herbal Science, Rural Development Administration, Suwon, Korea in 2013. This hybrid came from the progenies crossed between C. ‘2113’(Accession No.) and C. Lucky Rainbow ‘Randevous’ in 2000. Finally, eighty seven seedlings were obtained after planting and acclimatization in green house. In 2007, one line was selected by their performance such as flower color, leaf shape, flower stalk, and vigorous growth and named ‘Wongyuo F1-43. The line code 000550-78 had uniformity and excellent characteristics. The selected line after second characteristics test was named as ‘Beauty smile’. This hybrid had bright pink colored petal and lip (RP63D)and large sized flower(diameter 8.3cm). A ‘Beauty smile’ had about 13.3 flowers per flower stalk and medium sized flower and plant. General impression of petals and sepals showed some incurved shape. The attitude of peduncle was half-erect. Bloom was started from middle of November under optimal culture condition.

        MicroRNA-10b Plays a Role in Bone Formation by Suppressing Interleukin-22 in Ankylosing Spondylitis

        Objective. The microRNA (miR)-10b is the T helper (Th) 17 cell specific in patients with ankylosing spondylitis (AS). The interleukin (IL)-22, which is closely related to Th17 cells, has been implicated in the regulation of new bone formation in experimental models. Therefore, the aim of this study was to evaluate whether miR-10b affects bone formation via the IL-22 pathway in AS. Methods. Primary CD4+ T cells from AS were purified and transfected with miR-10b, anti-miR-10b, or scramble. Cell-surface markers and cytokine expression were analyzed by flow cytometry and enzyme-linked immunosorbent assay. Primary bone-derived cells (BdCs) from the facet joints of the spine were isolated, then osteogenic differentiation of primary BdCs was performed. We assessed alkaline phosphatase (ALP) activity and staining of BdCs at early time points. Alizarin red S staining of BdCs was performed at late time points. Results. Overexpression of miR-10b reduced both IL-22 producing cell frequencies and cytokine production in T cells from the patients with AS. The IL-22 significantly increased ALP staining and bone mineralization. The ALP promotor activity of AS-BdCs was notably higher for the IL-22 concentration. The supernatants of the miR-10b overexpression group suppressed ALP activity on osteogenic progenitor cells from the facet joints of the spine in patients with AS. Conclusion. Our data suggest that miR-10b suppresses IL-22 production, which was involved in osteogenic proliferation in AS. Therefore, miR-10b might be a potential therapeutic candidate for regulation of new bone formation in patients with AS. (J Rheum Dis 2020;27:61-67)

        Effect of vitamin C on azoxymethane (AOM)/dextran sulfate sodium (DSS)-induced colitis-associated early colon cancer in mice

        BACKGROUD/OBJECTIVES: The objective of this study was to investigate the effects of vitamin C on inflammation, tumor development, and dysbiosis of intestinal microbiota in an azoxymethane (AOM)/dextran sulfate sodium (DSS)-induced inflammation-associated early colon cancer mouse model. MATERIALS/METHODS: Male BALB/c mice were injected intraperitoneally with AOM [10 ㎎/㎏ body weight (b.w)] and given two 7-d cycles of 2% DSS drinking water with a 14 d inter-cycle interval. Vitamin C (60 ㎎/㎏ b.w. and 120 ㎎/㎏ b.w.) was supplemented by gavage for 5 weeks starting 2 d after the AOM injection. RESULTS: The vitamin C treatment suppressed inflammatory morbidity, as reflected by disease activity index (DAI) in recovery phase and inhibited shortening of the colon, and reduced histological damage. In addition, vitamin C supplementation suppressed mRNA levels of pro-inflammatory mediators and cytokines, including cyclooxygenase-2, microsomal prostaglandin E synthase-2, tumor necrosis factor-α, Interleukin (IL)-1β, and IL-6, and reduced expression of the proliferation marker, proliferating cell nuclear antigen, compared to observations of AOM/DSS animals. Although the microbial composition did not differ significantly between the groups, administration of vitamin C improved the level of inflammation-related Lactococcus and JQ084893 to control levels. CONCLUSION: Vitamin C treatment provided moderate suppression of inflammation, proliferation, and certain inflammation-related dysbiosis in a murine model of colitis associated-early colon cancer. These findings support that vitamin C supplementation can benefit colonic health. Long-term clinical studies with various doses of vitamin C are warranted.

        Synthesis and Importance of Bulky Aromatic Cap of Novel SAHA Analogs for HDAC Inhibition and Anticancer Activity

        On the basis of potent HDAC-inhibitory activity and anticancer activity of SAHA, novel SAHA derivatives 3a-d and 7 with a bulky cap such as p-dimethylaminophenyl, 4-phenylaminophenyl, 4-phenyloxyphenyl, 9H-fluorenyl or naphthalenyl ring were synthesized starting from the corresponding aryl amines or naphthalenyl acetic acid using an EDC-mediated amide coupling reaction in the presence of HOBt followed by a nucleophilic addition-elimination reaction with hydroxylamine. Compounds 3b, 3c and 3d showed more potent inhibitory activity on total HDACs (14~27-fold), HDAC1 (8~15-fold), HDAC2 (1.3~25-fold) and HDAC7 (1~3-fold) and more potent anticancer activity (2~22-fold) against MCF-7, MDA-MB-231, MCF-7/Dox, MCF-7/Tam, SK-OV-3, LNCaP and PC3 human cancer cell lines than SAHA.

        공요 자기 문화재의 디지털화 보호 관리체계 구축 방안 -블록체인을 중심으로-

        도자 문화재 실물과 대응 관계를 이루는 문화재 정보 데이터(사진, 3D 모델 등)에서 블록체인 활용은 도자 문화재의 보존, 소유권 확인, 도난 방지, 감정, 손상 방지 등과 관련된 문제를 효과적으로 해결할 수 있다. 이에 본 연구에서는 블록체인 기술을 활용해 쓰촨(四川) 공요 자기 문화재의 디지털 아이덴티티 확립 및 보호 관리체계 방안을 제시하고자 하였다. 이를 통해 전통 도자 문화유산의 디지털 보호 관리 체계의 신뢰성 향상과 도자예술의 보존과 계승에 필요한 정보를 제공하고자 하였다. 연구방법으로는 석 · 박사학위논문, 정기간행물, 단행본, 인터넷사이트 검색 등을 바탕으로 문헌조사를 하였고, 블록체인 기술을 통해 공요 자기 문화재의 디지털 아이덴티티와 관리 시스템 구축을 하였다. 본 연구에서는 3D 스캔 기술을 통해 3D 모델을 만든 후에 초분광 카메라로 수집 및 분석을 진행하고 표면 재질과 무늬를 기록하였으며, 기초 서술 데이터를 완성하였다. 이를 통해 공요 자기 문화재의 블록체인 시스템을 구축하고, 도자 문화재의 디지털 아이덴티티 정보를 블록체인 시스템에 입력함으로써 대응관계를 형성하였다. 이와 같이 본 연구에서는 블록체인 기술을 통해 공요 자기 문화재의 디지털 아이덴티티와 관리 시스템을 구축함으로써 도자 문화재의 과학적인 디지털화 체계를 마련하였다. 본 연구에서 구축한 공요 자기 문화재의 디지털화 보호 관리 체계가 널리 활용되기 위해서는 블록체인 기술의 구조적인 문제점에 대한 지속적인 관심과 논의가 이루어져야 할 것이다. The use of blockchain in the cultural asset information data(photo, 3D model, etc.) of having a corresponding relationship with the real ceramic cultural assets can effectively solve the problems related to preservation, ownership confirmation, theft prevention, valuation, damage prevention and others in ceramic cultural assets. Accordingly, the purpose of this study was to suggest a method of a digital identity establishment & security management system in Gongyo(邛窑) ceramic cultural assets in Sichuan(四川) with the application of blockchain technology. Through this, the aim was to offer information necessary for improving reliability of the digital security management system in traditional ceramic cultural heritage and for preserving and succeeding to ceramic art. A research method was carried out a documentary survey based on master doctorate dissertations, periodic publications, book forms, Internet Site Search. The blockchain technology led to constructing the digital identity & management system in Gongyo(邛窑) ceramic cultural assets. This study was progressed the collection and analysis using a hyperspectral camera after making 3D model through 3D scan technology, was recorded the surface materials and patterns, and was completed the basic description data. This led to establishing the blockchain system of Gongyo(邛窑) ceramic cultural assets and to forming a corresponding relationship by inputting the digital identity information on ceramic cultural properties in blockchain system. In this way, this study built the digital identity & management system in Gongyo(邛窑) ceramic cultural assets through the blockchain technology, thereby having arranged the scientific digitalization system in ceramic cultural assets. To be widely used the digitalization security management system in Gongyo(邛窑) ceramic cultural assets that this study constructed, the continuous interest and discussion will need to be made about a structural problem of blockchain technology.

        사천(四川) 공요(邛窑) 자기의 디지털화 보호에 관한 연구

        정보기술이 널리 응용되면서 각국 디지털박물관 프로젝트가 잇따라 진행되면서 문화재 분야에서 ‘디지털화’, ‘정보화’ 열풍이 불고 있다. 문물(文物) 디지털화, 문서 디지털화 등이 박물관학, 예술학 등 학계의 연구핵심이 되면서 디지털 인문, 디지털 고고학, 디지털 관리 · 보호, 디지털 보존 등의 연구도 활발해지고 있다. 본 연구에서는 사천(四川) 공요(邛窑) 자기 디지털 보호를 위해 정밀도가 높고 반사광이 강한 도자기 유물의 스캔과 3D 재건 기술을 제시하였다. 이를 통해 전통문화 유산의 보호와 계승에 이바지하고, 도자예술의 디지털화에 필요한 기초자료를 제공하고자 하였다. 이러한 연구목적을 달성하기 위해 연구방법으로는 석 · 박사학위논문, 정기간행물, 단행본을 포함한 도서 · 출판물, 관련 인터넷사이트 검색 등 문헌조사를 바탕으로 하였고. 공요(邛窑) 자기의 디지털 보호 방법으로 3D 스캐닝으로 디지털 자료를 수집하고, 여러 가지 조명 환경을 조성한 다음 ICP 알고리즘을 통한 데이터를 처리하여 3D 모형을 만들고, 데이터베이스를 구축하였다. 본 연구에서 제시한 사천(四川) 공요(邛窑) 자기의 디지털화 전략이 활성화되기 위해서는 정보 가시화를 활용하여 디지털화 공요(邛窑) 자기에 대한 정보 이해 및 인지 능력을 높이고, 데이터베이스로 공요(邛窑) 자기 정보 가시화의 설계방법에 대한 체계화가 요구된다. 또한 사천(四川) 공요(邛窑) 자기의 디지털화 사업의 연속성과 지속적인 발전을 위해서는 장기적인 계획 하에 지속적인 콘텐츠 개발이 이루어져야 할 것이다. 이와 더불어 다른 데이터베이스와의 통합을 위한 표준화와 3D시뮬레이션 영상 개발의 구체적인 계획이 필요하다. As information technology has widely been applied, digital museum projects are under way one after another in each country, and there is a craze for “digitization” and “informatization” in the area of cultural assets. The digitization of civilization and document digitization become critical research themes in academic circles including museology and science of arts, and a lot of research efforts are channeled into digital humanities, digital archeology, digital management/protection and digital preservation, etc. In this study, scanning and 3D reconstruction technique for ceramic relics that feature high precision and excellent light reflex were presented to ensure the protection of Sacheon Gongyo ceramics digitization. This study purports to make a contribution to the protection and transmission of cultural heritage and to provide some information on the digitization of ceramic arts. As for the research method, related literatures that included theses, dissertations, periodicals, books and publications were analyzed, and related Internet sites were retrieved. Regarding a way of digital protection for Gongyo ceramics, digital materials were collected by 3D scanning, and different lighting environments were prepared. And then a database was set up after the data were processed by ICP algorithm to make a 3D model. To accelerate the vitalization of the digitization strategy for Sacheon Gongyo ceramics presented in this study, information visualization should be used to improve people’s understanding and awareness of information on digitized Gongyo ceramics, and the systemization of design methods for information visualization for Gongyo ceramics through database is required. To guarantee the continuity and sustained development of a digitization project for Sacheon Gongyo ceramics, long-term plans should be drawn up to keep developing contents. Besides, standardization for integration with other databases and specific plans for the development of 3D simulation videos are both necessary.
