        Effect of Naturally Fermented Wheat Straw Based Complete Feeds on the Growth of Buffalo Calves

        A 152 day trial was conducted to see the effect of feeding naturally fermented wheat straw (FWS) with either energy, protein or energy protein supplements on the growth of buffalo calves. Twenty four male buffalo calves (10-12 months old) divided in 6 equal groups were individually offered FWS as sole roughage along with either conventional concentrate mixture (conc), maize grains (M), solvent extracted mustard cake (DMC), M-DMC mixture (50:50), deoiled rice bran (DRB) or uromol bran mixture (UBM) in 70:30 ratio. The digestibility of nutrients, nitrogen retention and nutritive value was maximum in FWS:UBM followed by FWS:DMC and FWS:Conc groups. Almost, all the blood parameters were observed well within the normal range except that of blood urea (FWS:UBM) and creatinine (FWS:DMC and FWS:DRB). The dietary combination in which FWS was supplemented with only conventional protein supplement like DMC proved to be highly efficient as far as live weight gain was concerned. FWS supplemented with energy-protein combination i.e. MDMC could also be used as complete feed for growing calves in comparison to conventional feeding system.

        In-sacco Degradability of Dietary Combinations Formulated with Naturally Fermented Wheat Straw as Sole Roughage

        Twelve dietary combinations were prepared using 70 parts of fermented wheat straw (FWS) as the sole roughage supplemented with 30 parts of either the low protein concentrate mixture (Conc.-I), high protein concentrate mixture (conc.-II), maize grains (M), solvent extracted mustard cake (DMC), deoiled rice bran (DRB), uromol bran mixture (UBM), deep stacked poultry litter (DSPL), dried poultry droppings (DPD), M-DMC mixture (50:50), M-UBM mixture (50:50), M-DPD mixture (50:50) or M-UBM-DPD mixture (50:25:25) and evaluated by in-sacco technique. The above dietary combinations were also evaluated by changing the roughage to concentrate ratio to 60:40. The digestion kinetics for DM and CP revealed that FWS:DPD had the highest, whereas, the FWS:M-DMC had the lowest rapidly soluble fraction. The potentially degradable fraction was found to be maximum in FWS:M and minimum in FWS:DPD dietary combinations. The higher degradation rate of FWS:DRB and FWS:UBM combinations was responsible for their significantly (p<0.05) higher effective degradability as compared to other combinations. The highest undegradable fraction noted in FWS:M-UBM-DPD followed by FWS:DMC was responsible for high rumen fill values. The FWS:DRB, FWS:UBM and FWS:DPD combinations had higher potential for DM intake. The dietary combination with higher concentrate level (60:40) was responsible for higher potentially degradable fraction, which was degraded at a faster rate resulting in significantly higher effective degradability as compared to the corresponding dietary combination with low concentrate level (70:30). The low undegradable fraction in the high concentrate diet was responsible for low rumen fill values, which predicted of high potential for DM intake. Out of 24 dietary combinations, FWS with either of UBM, DRB, DMC, Maize, M-DMC or DPD in 70:30 ratio supplemented with minerals and vitamin A in comparison to conventional feeding practice (roughage and concentrate mixture) could be exploited as complete feed for different categories of ruminants.

        In vitro Evaluation of Phalaris minor Seeds as Livestock Feed

        The nutritional worth of Phalaris minor seeds was assessed in comparison to conventional cereal grains like maize and wheat. P. minor seeds had higher total ash and cell wall constituents as compared to wheat and maize grains. The CP content of P. minor was comparable to wheat grains but higher than maize grains. The in vitro studies revealed that the net gas production and availability of ME from P. minor was comparable to that of maize but the digestibility of nutrients was significantly (p<0.05) lower than that of conventional cereal grains. The digestion kinetic parameters for DM and CP revealed that P. minor had the highest (p<0.05) soluble fraction (a) followed by wheat and maize. Reverse trend was observed for insoluble but potentially degradable fraction (b). The effective and true DM and CP degradability was significantly (p<0.05) higher in wheat grains followed by that in P. minor and maize grains. The digestibility of OM and NDF was not affected by replacing cereal grains in concentrate mixture with P. minor seeds up to 75 per cent level. But the availability of ME from concentrate mixtures was comparable to control only up to 50% level of replacement. Replacement of cereal grains with P. minor did not affect the rapidly soluble fraction and insoluble but potentially degradable fraction of concentrate mixture containing P. minor up to 75 per cent, but it was depressed significantly at 100% replacement level. The effective and true degradability of DM of concentrate mixtures containing P. minor from 50 to 100 per cent was comparable to that of conventional concentrate mixture (CCM). The wheat based concentrate mixtures showed higher net gas production (208 vs. 201 ml/g DM/24 h), digestibility of nutrients and ME availability (9.64 vs. 9.54 MJ/kg DM) as compared to maize based concentrate mixture. The wheat based concentrate mixture had significantly (p<0.05) higher rumen undegradable fraction and effective degradability. The data conclusively revealed that conventional cereal grains could be replaced with P.minor seeds up to 75 per cent without affecting the availability of nutrients.

        The Clinical Correlations between Diabetes, Cigarette Smoking and Obesity on Intervertebral Degenerative Disc Disease of the Lumbar Spine

        Study Design: Retrospective analysis of a nationwide private insurance database. Chi-square analysis and linear regression models were utilized for outcome measures. Purpose: The purpose of this study was to investigate any relationship between lumbar degenerative disc disease, diabetes, obesity and smoking tobacco. Overview of Literature: Diabetes, obesity, and smoking tobacco are comorbid conditions known to individually have effect on degenerative disc disease. Most studies have only been on a small populous scale. No study has yet to investigate the combination of these conditions within a large patient cohort nor have they reviewed the combination of these conditions on degenerative disc disease. Methods: A retrospective analysis of insurance billing codes within the nationwide Humana insurance database was performed, using PearlDiver software (PearlDiver, Inc., Fort Wayne, IN, USA), to identify trends among patients diagnosed with lumbar disc degenerative disease with and without the associated comorbidities of obesity, diabetes, and/or smoking tobacco. Patients billed for a comorbidity diagnosis on the same patient record as the lumbar disc degenerative disease diagnosis were compared over time to patients billed for lumbar disc degenerative disease without a comorbidity. There were no sources of funding for this manuscript and no conflicts of interest. Results: The total number and prevalence of patients (per 10,000) within the database diagnosed with lumbar disc degenerative disease increased by 241.4% and 130.3%, respectively. The subsets of patients within this population who were concurrently diagnosed with either obesity, diabetes, tobacco use, or a combination thereof, was significantly higher than patients diagnosed with lumbar disc degenerative disease alone (p <0.05 for all). The number of patients diagnosed with lumbar disc degenerative disease and smoking rose significantly more than patients diagnosed with lumbar disc degenerative disease and either diabetes or obesity (p <0.05). The number of patients diagnosed with lumbar disc degenerative disease, smoking and obesity rose significantly more than the number of patients diagnosed with lumbar disc degenerative disease and any other comorbidity alone or combination of comorbidities (p <0.05). Conclusions: Diabetes, obesity and cigarette smoking each are significantly associated with an increased diagnosis of lumbar degenerative disc disease. The combination of smoking and obesity had a synergistic effect on increased rates of lumbar degenerative disc disease. Patient education and preventative care is a vital goal in prevention of degenerative disc disease within the general population.

        Isolated Spontaneous Primary Tubercular Erector Spinae Abscess: A Case Report and Review of Literature

        Isolated spontaneous primary tubercular erector spinae abscess in an immunocompetent patient is very rare. Here, we report such a case of 21-year-old female, which was successfully managed with timely diagnosis and intervention. Isolated primary tubercular abscess of erector spinae is a rare differential diagnosis of low back pain; however, it must be suspected in an endemic region for tuberculosis, especially when raised erythrocytic sedimentation rate and C-reactive protein are present. Excision along with anti tubercular therapy proved to be a successful strategy in our patient.

        Thoracic metastasis in advanced ovarian cancer: comparison between computed tomography and video-assisted thoracic surgery

        Objective: To determine which computed tomography (CT) imaging features predict pleural malignancy in patients with advanced epithelial ovarian carcinoma (EOC) using video-assisted thoracic surgery (VATS), pathology, and cytology findings as the reference standard. Methods: This retrospective study included 44 patients with International Federation of Obstetrics and Gynecology (FIGO) stage III or IV primary or recurrent EOC who had chest CT ≤30 days before VATS. Two radiologists independently reviewed the CT studies and recorded the presence and size of pleural effusions and of ascites; pleural nodules, thickening, enhancement, subdiaphragmatic tumour deposits and supradiaphragmatic, mediastinal, hilar, and retroperitoneal adenopathy; and peritoneal seeding. VATS, pathology, and cytology findings constituted the reference standard. Results: In 26/44 (59%) patients, pleural biopsies were malignant. Only the size of left-sided pleural effusion (reader 1: rho= -0.39, p=0.01; reader 2: rho=-0.37, p=0.01) and presence of ascites (reader 1: rho=-0.33, p=0.03; reader 2: rho=-0.35, p=0.03) were significantly associated with solid pleural metastasis. Pleural fluid cytology was malignant in 26/35 (74%) patients. Only the presence (p=0.03 for both readers) and size (reader 1: rho=0.34, p=0.04; reader 2: rho=0.33, p=0.06) of right-sided pleural effusion were associated with malignant pleural effusion. Interobserver agreement was substantial (kappa=0.78) for effusion size and moderate (kappa=0.46) for presence of solid pleural disease. No other CT features were associated with malignancy at biopsy or cytology. Conclusion: In patients with advanced EOC, ascites and left-sided pleural effusion size were associated with solid pleural metastasis, while the presence and size of right-sided effusion were associated with malignant pleural effusion. No other CT features evaluated were associated with pleural malignancy.         Incidental benefits after fecal microbiota transplant for ulcerative colitis

        Gut dysbiosis can result in several diseases, including infections (Clostridium difficile infection and infectious gastroenteritis), autoimmune diseases (inflammatory bowel disease, diabetes, and allergic disorders), behavioral disorders and other conditions like metabolic syndrome and functional gastrointestinal disorders. Amongst various therapies targeting gut microbiome, fecal microbiota transplantation (FMT) is becoming a focus in the public media and peer reviewed literature. We have been using FMT for induction of remission in patients with moderate to severe active ulcerative colitis (UC) and also for subsequent maintenance of remission. Four cases reported incidental benefits while being treated with FMT for UC. These included weight loss (n = 1), improvement in hair loss (n = 1), amelioration of axial arthritis (n = 1) and improvement in allergic rhinitis (n = 1), thereby suggesting potential clinical applications of FMT in treating extraintestinal diseases associated with gut dysbiosis.       • Survival of Triple Negative versus Triple Positive Breast Cancers: Comparison and Contrast

        Background: Triple negative (TN) and triple positive (TP) breast cancers both are aggressive types but TN generally has a shorter survival. Objectives: To compare the clinical characteristics and treatment outcomes for patients with TN versus TP breast cancer and to assess various prognostic factors affecting overall survival. Materials and Methods: A retrospective audit of 85 breast cancer patients was conducted in the Department of Radiation Oncology and Medical Oncology on patients from 2006 to 2013 for whom IHC for ER, PgR and Her-2 neu were available. The patients were stratified into: ER-, PR- and Her-2 neu- (Arm A, n=47) and ER+, PgR+ and Her-2 neu+ (Arm B, n=38). Results: TN subtype had higher numbers of premenopausal and advanced stage patients as compared to TP subtype. The locoregional recurrence (LRR) and distant metastatic rate was also higher in TN subtype but there was no definite pattern in both the arms. Among the prognostic factors, patients with premenopausal status and advanced stage in TN breast cancer had inferior survival (P=0.07) whereas for those with postmenopausal status and early stage there was no survival difference between the two arms. Conclusions: TN subtype tends to be more aggressive in terms of younger age and advanced stage at presentation, higher tumour grade, LRR and metastasis, suggesting need for future research efforts on providing aggressive treatment to these patients. We could attribute better outcome for TP subtype to receptor positivity enabling role of hormonal treatment and targeted therapy, although less number of patients received targeted therapy.

        Histopathological changes in gall bladder mucosa in relation to the number, and size of gallstones, and analysis of the findings in the context of age distribution of the patients: A perspective

        Backgrounds/Aims: To study histopathological changes in gall bladder mucosa in cholelithiasis patients, and analyse their relation to the number and size of gallstones. These findings were evaluated in the context of age distribution of the study population. Methods: One hundred cases of cholecystectomy were part of the study, which was conducted in collaboration with the pathology department. The time period of the study was January 2020 to June 2021. Results: Maximum cases had multiple stones (69.0%), while one third cases (31.0%) had solitary stones. While initial univariate analysis showed age (odds ratio [OR], 6.882; p = 0.043), gallstone number (OR, 9.1; p = 0.050), gallstone size (OR, 17.111; p = 0.013), and duration of symptom (OR, 34.125; p = 0.001) to be significant risk factors associated with gallbladder carcinoma, multivariate analysis found none of these variables to be significant. However, conditional multivariate analysis for the duration of symptom (p = 0.008; OR, 21.118) yielded significant p- value. With histopathological diagnoses, 5% of cases had gallbladder cancer. Conclusions: This study shed light on the rising incidence of cholelithiasis in the young population and the high rate of gallbladder carcinoma in Punjab, India. Although gall stone characteristics (size, number) and patient age appeared to be significant risk factors when their individual relation with gallbladder carcinoma was studied, multivariate analysis, could not prove that. Conditional multivariate analysis showed the duration of symptom to be the only significant risk factor associated with gallbladder carcinoma. Further research with larger sample size is needed to study the rising incidence of gallbladder carcinoma, and the risk factors associated with it.
