Arrhythmogenic right ventricular dysplasia/cardiomyopathy (ARVD/C) is predominantly an inherited cardiomyopathy with typical histopathological characteristics of fibro-fatty infiltration mainly involving the right ventricular (RV) inflow tract, RV outflow tract, and RV apex in the majority of patients. The above pathologic evolution frequently brings patients with ARVD/C to medical attention owing to the manifestation of syncope, sudden cardiac death (SCD), ventricular arrhythmogenesis, or heart failure. To prevent future or recurrent SCD, an implantable cardiac defibrillator (ICD) is highly desirable in patients with ARVD/C who had experienced unexplained syncope, hemodynamically intolerable ventricular tachycardia (VT), ventricular fibrillation, and/or aborted SCD. Notably, the management of frequent ventricular tachyarrhythmias in ARVD/C is challenging, and the use of antiarrhythmic drugs could be unsatisfactory or limited by the unfavorable side effects. Therefore, radiofrequency catheter ablation (RFCA) has been implemented to treat the drug-refractory VT in ARVD/C for decades. However, the initial understanding of the link between fibro-fatty pathogenesis and ventricular arrhythmogenesis in ARVD/C is scarce, the efficacy and prognosis of endocardial RFCA alone were limited and disappointing. The electrophysiologists had broken through this frontier after better illustration of epicardial substrates and broadly application of epicardial approaches in ARVD/C. In recent works of literature, the application of epicardial ablation also successfully results in higher procedural success and decreases VT recurrences in patients with ARVD/C who are refractory to the endocardial approach during long-term follow-up. In this article, we review the important evolution on the delineation of arrhythmogenic substrates, ablation strategies, and ablation outcome of VT in patients with ARVD/C.

Objective: Hypoactivity in the reward system among patients with attention deficit hyperactivity disorder (ADHD) is a well-known phenomenon. Whether the activity in the reward pathway is related to harm avoidance, such as in sensitivity to punishment, is unclear. Evidence regarding the potential difference between ADHD patients and controls in terms of this association is scarce. Methods: Event-related functional magnetic resonance imaging was conducted on subjects performing the Iowa gambling test. Fourteen adults with ADHD and 14 controls were enrolled in the study. Results: Harm avoidance was found to be positively correlated with the activities of the bilateral orbitofrontal cortex and right insula in individuals with ADHD. A group difference was also confirmed. Conclusion: Understanding the roles of harm avoidance and brain activation during risk tasks is important.

Objective: Bipolar disorder (BD) is characterized by the poor sleep quality. Whether the striatal dopamine transporter (DAT) availability is related to sleep quality among patients with BD is unclear. Methods: Fifty-three euthymic patients with BD (24 BD-I and 29 BD-II) and sixty-eight healthy controls were enrolled. The Chinese Version of the Pittsburgh Sleep Quality Index (PSQI) was used, and the availability of DAT was assessed by single-photon emission computed tomography (SPECT) using [99mTc] TRODAT-1. Results: The sleep disturbance component of the PSQI was significantly associated with the level of DAT availability among patients with BD. Conclusion: The striatal dopaminergic activity that contributes to resilience to adversity was associated with sleep pattern among patients with BD.

Objective: Weight gain is an important risk factor for morbidity and mortality among patients with schizophrenia. We speculated that positive symptoms, related to dopaminergic hyperactivity and altered mesolimbic function, are associated with weight gain. Methods: Twenty-two antipsychotic-naïve, first-episode patients with schizophrenia were enrolled. The Positive and Negative Syndrome Scale was completed at enrollment and follow-up. Body mass index (BMI) was also measured. Results: The increase in BMI, after 6.04 ± 2.16 years of follow-up, was associated with positive symptoms, but not negative symptoms, before treatment with antipsychotics in antipsychotic-naïve patients with schizophrenia. Conclusion: This finding implied that dopaminergic hyperactivity could be an important factor to predict the treatment outcome. Body weight control is important for the health management of patients with schizophrenia with more severe positive symptoms.

The layered coding structure of scalable video coding (SVC) with adaptive inter-layer prediction causes noticeable computational complexity increments when compared to existing video coding standards. To lighten the computational complexity of SVC, we present a fast algorithm to speed up the inter-mode decision process. The proposed algorithm terminates inter-mode decision early in the enhancement layers by estimating the rate-distortion (RD) cost from the macroblocks of the base layer and the enhancement layer in temporal, spatial, and inter-layer directions. Moreover, a search range decision algorithm is also proposed in this paper to further increase the motion estimation speed by using the motion vector information from temporal, spatial, or inter-layer domains. Simulation results show that the proposed algorithm can determine the best mode and provide more efficient total coding time saving with very slight RD performance degradation for spatial and quality scalabilities.

The new emergent paradigm of urban development theory that is based on complexity sciences allows us to understand and analyse cities in a newway. Theoretically, complexity sciences enable us to depict the fundamental characteristics of urban development,including nonlinearity, self-organization, and emergence. Empirically, the agent-based modelling (ABM) approach can help us to conduct simulations of complex systems,including cities, in an effective way. In the present paper, we demonstrate a computer simulation of urban growth based on the spatial garbage can model represented in an ABM framework. In the simulation, we treated the city as an open system in that the fundamental elements of the system flow in and out of the system over time. We then computed over time the levels of entropy as a measurement of the degree of structural order of the systems, namely, decision and spatial structures. The results showed that these entropies decreased over time, indicating that the city self-organizes itself reminiscent of a dissipative structure.

If machining abnormalities, such as build-up-edge and cutter breakage, occur during machining, serious damage on the workpiecesurface and deterioration of production efficiency will happen, and cause additional energy consumption for rework and increase themanufacturing cost. To minimize the effects, an on-line diagnosis method with use of Fast Fourier Transform and algorithm of shorttimesignal variation analysis was developed to analyze the vibration signals to quickly detect the two abnormalities in this study. Inaddition, the real-time machining information from CNC controller was extracted through a bi-lateral communication module toprevent misdiagnosis. The control commands were automatically generated by the proposed system and directly sent to the CNCcontroller to stop the machine for cutter replacement. Without complex computation, the system can detect the occurrence of BUEor cutter breakage within 1 second and complete machine control within 3 seconds. The system can instantly transmit and save thereal machining information and diagnosis/control results to the remote central monitoring platform for further process improvement. With use of TCP/IP communication protocol the central monitoring platform can remote log into the on-site monitoring computer todirectly operate the diagnosis/control system. Experimental results showed the feasibility and effectiveness of the proposed system.

We describe the case of a patient with severe hypertensive left ventricular hypertrophy and sustained hemodynamically unstableventricular tachycardia (VT). Entrainment was demonstrated in the electrophysiological study. Activation mapping and pacemappingidentified the location of the intramural reentrant VT with the exit site close to the epicardium. Differentiating arrhythmogenic right ventricular dysplasia/cardiomyopathy (ARVD/C) from other cardiomyopathies is clinically important but challenging. Although the modified Task Force Criteria can facilitate diagnosis of ARVD/C according to clinical manifestations, histopathological examination plays a pivotal role in excluding other diseases that can mimic ARVD/C. Here, we report a patient with amyloidosis that initially presented similarly to ARVD/C. The diagnosis was confirmed by endomyocardial biopsy, and catheter ablation eliminated the ventricular tachyarrhythmias through an epicardial approach.
