      • Energy-Efficient Design of an Ethyl Levulinate Reactive Distillation Process via a Thermally Coupled Distillation with External Heat Integration Arrangement

        <P>In this paper we describe an energy-efficient process design using a reactive distillation (RD) of the esterification reaction for the synthesis of ethyl levulinate I (LAEE). Two designs, a neat design and an excess design, were examined to find the best configuration for the process. In the neat design, there are two columns with equal molar feeds to produce LAEE. An additional column is required in the excess design to separate LAEE from the unreacted reactant (levulinic acid, LA). Later in the process, this unreacted LA is recycled back into the RD column. Compared to the neat design, the excess design showed superiority for the esterification process in terms of the total annual cost (TAC) and energy requirement. Therefore, three energy integration configurations, external heat integration (HI), thermally coupled distillation (TCD), and a combination of TCD and HI, were investigated in this excess design. A side reboiler was implemented in the water removal column to overcome the limitation of the temperature difference for heat transfer. The simulation results showed that the configuration of TCD with HI was the best one, which saved 32.9% and 10.5% of the energy and TAC, respectively, compared to the conventional RD with three columns without HI.</P>

        A Methodology for Optimizing Modular Design Considering Product End of Life Strategies

        Product modularity is a concept of clustering the independent components into logical units that are relatively independent to each other in functions. With the growing interest in environmental protection, the inclusion of product end of life issues during product design has been seen as a promising method to improve product utilization after its retirement. There are very few methodologies that deal with modularity at the conceptual design stage in which incorporate product end of life strategies and end of life cost in the literature. The lack of information during conceptual design will cause failure in overcoming drawbacks that will be apparent later in the detail design. The motivation of this study is to identify the relationship between product end of life strategies and modular design, and to formulate an optimization method to assist designers in designing products with appropriate end of life strategies to reduce environmental impact during product retirement. This paper proposes a practical method in optimizing modular design with different end of life strategies by using an Excel based approach that will be usable by designers.

        Verification of the Reliability and Validity of the Short Form 36 Scale in Indonesian Middle-aged and Older Adults

        Objectives: The Short Form 36 (SF-36) questionnaire is increasingly being used to measure health-related quality of life (HRQoL) in Indonesia. However, evidence that it is valid for use in Indonesian adults is lacking. This study assessed the validity and reliability of the SF-36 in Indonesian middle-aged and older adults. Methods: Adults aged 46-81 years (n=206) in Yogyakarta, Indonesia completed the SF-36, another measure of HRQoL (the EuroQoL visual analogue scale [EQ-VAS]), and measures assessing their demographic characteristics. Fifty-four percent (n=121) completed the SF-36 measure again 1 week later. Confirmatory factor analysis was conducted to confirm the factor structure of the SF-36. Internal consistency reliability was estimated using Cronbach’s alpha, and test-retest reliability was assessed using intraclass correlations. Convergent and discriminant validity were assessed by computing correlations among SF-36 subscales, between subscales and the 2 component scores, and between component scores and EQ-VAS scores. Results: Most scaling assumptions were met. The hypothetical factor structure fit the data poorly (root mean square error of approximation [RMSEA]=0.108) and modification was required for a good fit (RMSEA=0.060). Scores on all subscales demonstrated acceptable internal consistency (α>0.70) and test-retest reliability (r>0.70). Divergent validity was supported by weak to moderate inter-scale correlations (r=0.19 to 0.64). As expected, the 2 summary scores were moderately to strongly correlated with the EQ-VAS (r>0.60). Conclusions: The findings adequately support the use of SF-36 in Indonesian middle-aged and older adults, although the optimal algorithm for computing component scores in Indonesia warrants further investigation.

        Kahwa daun: traditional knowledge of a coffee leaf herbal tea from West Sumatera, Indonesia

        Background: Kahwa daun is a herbal tea made from coffee leaves produced by people in West Sumatera, Indonesia. It has an aroma, flavor, and appearance similar to coffee. There is no existing literature describing the traditional production method of kahwa daun. In this article, production techniques, moisture content, and the yield of the product are explored. Method: Descriptive qualitative research method was used to explore production techniques. Discussion, observation, in-depth interviews, and documentation were conducted in three districts in West Sumatera to collect primary data. Gravimetric method is used to determine the moisture content. Result: Processing of fresh coffee leaves into kahwa daun includes collection of raw materials, drying, packaging, and storage. Kahwa daun usually was made from robusta coffee leaves. The result showed that there were three main production techniques for kahwa daun in the province which include pendiangan (traditional drying above the cooking fire), smoking, and toasting in a handheld frame over a low heat. All techniques were different from the techniques usually used to produce herbal teas. The moisture content of kahwa daun was 3.6e7.6% (wb), and the yield of the product was 10e20%. Conclusion: Kahwa daun processing differs from both usual herbal tea production methods and commercial coffee leaf tea production. The efficacy and potential of kahwa daun as a health drink still require further research.

        Background: Diabetic nephropathy is found in 20%-40% of diabetic patients who are mostly type 2 diabetes. Research fi nding out the effectiveness of angiotensin-converting enzyme inhibitor (ACEI) for reducing microalbuminuria incident in type 2 diabetic patient with normoalbuminuria have been published. This evidence based case report (EBCR) will show the search and critical appraisal of those studies. Methods: The search of evidence was done in Pubmed and Cochrane Library database using keywords. After applying inclusion and exclusion criteria, fi ltering doubles, reading abstract and looking for full text availability: there were two remaining journals which were appraised based on standardized British Medical Journal criteria of validity, importance, and applicability. Results: Of the two meta-analysis, Article written by Hirst JA et al. is valid, important, and applicative for patient`s case. Meanwhile, article written by Vejakama P et al. is valid, but not important and applicative because of studies` heternogenity Conclusions: ACEI reduced microalbuminuria incident by 16% signifi cantly over placebo for type 2 diabetic patients with normoalbuminuria.

        Background: Based on program evaluation conducted at a hypertension primary health care, we found that the major problem of this clinic was lack of medication and control compliance of the patients. The chosen solution to solve this problem was developing an education-oriented control book for hypertensive patients in primary health care. Methods: This qualitative study lasted for nine days in a hypertension primary health care clinic in Central Jakarta, Indonesia. It consisted of six stages: the project design, development of control book, development of communicative educational messages, pretesting, revision, and evaluation by health professionals. Health staffs working at the clinic participated in fi lling the ten-question questionnaire of evaluation for previous control book and education-oriented control book. The mean scores were compared using t-test or Mann-Whitney U test. Discrimination was validated by the area under the curve (AUC) of receiver operating characteristic (ROC) curve analysis. Results: The education-oriented control book has 20 pages, 20. 5 cm X 14. 5 cm in size. The second page (after the title page) until the sixth page contains communicative educational messages. The seventh-the nineteenth page consist of patient`s health care records and the last page was for blood pressure chart. From the analysis, we found total mean scores for previous control book and education-oriented control book were 29. 2 ± 1. 64 and 34. 2 ± 1. 64 (p<0. 001). ROC curve analysis showed statistically signifi - cant power of discrimination in education-oriented control book (AUC = 0. 881). Conclusions: The education-oriented control book presented a signifi cant discrimination in our population and has potential for clinical applications as a part of health promotion media in hypertension primary health services.

        Plantwide design for high-purity formic acid reactive distillation process with dividing wall column and external heat integration arrangements

        We assessed eight configurations by implementing a dividing wall column (DWC) arrangement and an external heat integration (HI) arrangement for the reduction of energy consumption in the high-purity formic acid (FA) production process. At first, a patented high-purity FA production configuration was adopted and several main process variables were optimized. The optimal configuration was considered the base case for further investigation. The DWC arrangement was applied in the base case configuration to overcome the remixing phenomenon. Next, the external HI arrangement was implemented in those configurations. The simulation results showed that the non-reactive upper DWC between columns C2 and C3 with the HI configuration was the best configuration that provided 46.9% energy saving compared to base case configuration.

        Protective Effects of Anthocyanin Extract from Purple Sweet Potato (Ipomoea batatas L.) on Blood MDA Levels, Liver and Renal Activity, and Blood Pressure of Hyperglycemic Rats

        There has been a dynamic progression in the study of purple sweet potatoes, particularly in regard to their antioxidant compounds, such as anthocyanins. Antioxidants can reduce oxidative stress due to hyperglycemia, therefore research into the protective effects of hyperglycemia is essential. This study was conducted to investigate the protective effects of anthocyanin extracts from purple sweet potatoes on blood malondialdehyde (MDA) levels, liver and renal activity, and blood pressure in hyperglycemic rats. Anthocyanin from purple sweet potato (APSP) was extracted with ethanol-citric acid 3% solvent. Twenty-four rats were split into four experimental groups: (i) healthy rats; (ii) hyperglycemic rats without anthocyanin treatment; (iii) hyperglycemic rats treated with APSP extract at a dose of 50 mg/kg; and (iv) hyperglycemic rats treated with APSP extract at a dose of 100 mg/kg. Rats received treatment for 35 days. The results showed that consumption of APSP significantly reduced levels of MDA in the blood, and liver and renal systems. APSP could reduce the urea and creatinine levels, which are indicative of improved renal function. In addition, APSP could decrease serum glutamate oxalacetate transaminase and serum glutamate pyruvate transaminase levels, indicative of protective activity of the extract on liver, and decrease systolic blood pressure. Accordingly, it was concluded that APSP could be developed as a functional food for treatment of diabetes.

        Evaluation of the Tax Allowance Facility in the Indonesian Fish Processing Industry

        This study aims to analyze the achievement of policy objectives in terms of effectiveness, similarity and accuracy of income tax incentives in the form of tax allowances for the Indonesian fish processing industry. This research was conducted with a qualitative approach and qualitative data collection techniques through literature study and field study. The results show that although the tax allowance facility procedure has met the principle of simplicity, the facility has not effectively increased investment because it is still a relatively small industry that uses it. The fiscal cost burden still incurred by the taxpayer that makes this policy inefficient. This facility is also not appropriate for the fish processing industry because the allowance can only be utilized by large-scale industries, while small- and medium- scale fish processing industries cannot utilize the tax allowance facility.
