Genus Penicillium comprising the most important and extensively studied fungi has beenwell-known as a rich source of secondary metabolites. Our study aimed to analyze andinvestigate biological activities, including in vitro anti-cancer, anti-inflammatory and anti-diabeticproperties, of metabolites from a marine-derived fungus belonging to P. levitum. Thechemical compounds in the culture broth of P. levitum strain N33.2 were extracted withethyl acetate. Followingly, chemical analysis of the extract leaded to the isolation of threeergostane-type steroid components, namely cerevisterol (1), ergosterol peroxide (2), and(3b,5a,22E)-ergosta-6,8(14),22-triene-3,5-diol (3). Among these, (3) was the most potent cytotoxicagainst human cancer cell lines Hep-G2, A549 and MCF-7 with IC50 values of 2.89,18.51, and 16.47 mg/mL, respectively, while the compound (1) showed no significant effectagainst tested cancer cells. Anti-inflammatory properties of purified compounds were evaluatedbased on NO-production in LPS-induced murine RAW264.7 macrophages. As a result,tested compounds performed diverse inhibitory effects on NO production by the macrophages,with the most significant inhibition rate of 81.37 ± 1.35% at 25 mg/mL by the compound(2). Interestingly, compounds (2) and (3) exhibited inhibitory activities againstpancreatic lipase and a-glucosidase enzymes in vitro assays. Our study brought out newdata concerning the chemical properties and biological activities of isolated steroids from aP. levitum fungus.
Estimation of flow residence time in a decay tank for a pool type research reactor using CFD

A decay tank shall be designed to provide enough flow residence time to ensure that the N-16 activity decreases before the coolant leaves the decay tank's shielding room. Therefore, estimation of the residence time in a decay tank is necessary before the conclusion of such a design. To estimate the flow residence time in a decay tank, the transport equation of a user-defined additional variable, which has a time unit and zero kinematic diffusivity, is solved using steady and unsteady assumptions. The minimum flow residence time is compared with that of an analytic method with an assumption of a uniform flow in a decay tank, and a particle tracking method. For the simulations, a commercially available CFD code, ANSYS-CFX, is used. When comparing the results, the unsteady simulation is seen to be much more efficient approach than steady simulation to obtain the minimum flow residence time of the decay tank.

15세기에 들어와서 동남아 지역 국가들의 발전이 시작되고, 정치가 안정되면서 지역뿐만 아니라 국제적으로도 무역 활동이 활발하게 이루어졌다. 당시 도자기 공예술은 한창 발전된 시기였다. 명나라의 해금 정책이 반포된 것은 베트남과 태국 도자기 수출 활동이 더 많아지게 되고 해로 실크로드가 발전하는 데에 기여한 요소이다. 본고는 15세기에 동남아 지역 도자기공예술 발전 상황을 소개하고 이를 통해 국제 무역 상황에 대해서도 살짝 소개하고자 한다.

We analyzed the clinical significance of pre-transplant International Prognostic Scoring System (IPSS) score and comorbidity in 68 patients who underwent allogeneic hematopoietic cell transplantation (HCT) for myelodysplastic syndrome (MDS) (n=48) or acute myeloid leukemia evolved from MDS (n=20) between December 1995 and January 2008 in a single institute. During a median follow-up period of 41.0 months (range, 3.2–132.0 months), 27 patients died, and 7 relapsed. The 5-year probabilities of overall survival (OS) and event-free survival (EFS) were 60.0 and 57.4%, respectively, and the 5-year cumulative incidences of non-relapse mortality (CINRM) and relapse were 32.7 and 9.9%, respectively. OS, EFS, and CINRM were significantly different according to pre-transplant IPSS score and presence of pre-transplant comorbidity, which were independent risk factors along with Karnofsky performance score in multivariate analyses. In conclusion, pre-transplant IPSS score and comorbidity may stratify the risk of post transplant outcomes in MDS.

<P><B>Abstract</B></P><P><B>Background:</B></P><P>To determine whether the effectiveness and safety of fixed-dose combinations (FDCs) of amlodipine orotate/valsartan (AML/VAL) 5/160 mg are noninferior to those of valsartan/hydrochlorothiazide (VAL/HCTZ) 160/12.5 mg in hypertensive patients with inadequate response to valsartan 160 mg monotherapy.</P><P><B>Methods:</B></P><P>This 8-week, active-controlled, parallel-group, fixed-dose, multicenter, double-blind randomized controlled, and noninferiority trial was conducted at 17 cardiovascular centers in the Republic of Korea. Eligible patients had mean sitting diastolic blood pressure (msDBP) ≥90 mm Hg despite monotherapy with valsartan 160 mg for 4 weeks. Patients were randomly assigned to treatment with AML/VAL 5/160 mg FDC (AML/VAL) group or VAL/HCTZ 160/12.5 mg FDC (VAL/HCTZ) group once daily for 8 weeks. A total of 238 patients were enrolled (AML/VAL group, n = 121; VAL/HCTZ group, n = 117), of whom 228 completed the study.</P><P><B>Results:</B></P><P>At 8 weeks after randomization, msDBP was significantly decreased in both groups (−9.44 ± 0.69 mm Hg in the AML/VAL group and −7.47 ± 0.71 mm Hg in the VAL/HCTZ group, both <I>P</I> < .001 vs baseline). Between group difference was −1.96 ± 1.00 mm Hg, indicating that AML/VAL 5/160 mg FDC was not inferior to VAL/HCTZ 160/12.5 mg FDC at primary efficacy endpoint. Control rate of BP defined as the percentage of patients achieving mean sitting SBP (msSBP) <140 mm Hg or msDBP <90 mm Hg (target BP) from baseline to week 8 was significantly higher in the AML/VAL group than that in the VAL/HCTZ group (84.3% [n = 102] in the AML/VAL group vs 71.3% [n = 82] in the VAL/HCTZ group, <I>P</I> = .016). At 8 weeks after randomization, mean uric acid level was significantly increased in the VAL/HCTZ group compared to that at baseline (0.64 ± 0.08 mg/dL; <I>P</I> < .001). However, it was slightly decreased from baseline in the AML/VAL group (−0.12 ± 0.08 mg/dL; <I>P</I> = .085). The intergroup difference was significant (<I>P</I> < .001).</P><P><B>Conclusion:</B></P><P>The effectiveness and safety AML/VAL 5/160 mg FDC are noninferior to those of VAL/HCTZ 160/12.5 mg FDC in patients with hypertension inadequately controlled by valsartan 160 mg monotherapy.</P>
Eosinophilia in Pleural Effusions: a Speculative Negative Predictor for Malignancy

Background: Eosinophilic pleural effusion (EPE) is an eosinophil count more than 10% on cytology of pleural samples. Recently, it was reported that malignancy had been the most prevalent cause inducing EPE. Therefore, we conducted an analysis on the prevalence and etiology of EPE and investigated the relationship between EPE and malignancy. Materials and Methods: Data for pleural cell differential count from patients receiving thoracentesis during the period from January 2008 to December 2013 were compared with clinical data and established diagnosis of patients obtained via electronic chart review. Results: A total of 6,801 requests of pleural cytology from 3,942 patients with pleural effusion who had received thoracentesis were available at Far Eastern Memorial Hospital from 2008 to 2013, and of these subjects, 115 (2.9%) were found to have EPE. The most frequent cause of EPE was malignancy (33.0%, n=38), followed by parapneumonic effusions (27.8%, n=32), tuberculosis pleuritis (13.9%, n=16), transudate effusions (12.2%, n=14) and the presence of blood or air in pleural space (10.4%, n=12). Additionally, an inverse relationship of eosinophilia in pleural fluid was identified in patients with malignancy and EPE. The cut-off eosinophil count in pleural fluid was 15% for the most accurate discrimination between malignancy and benign disorders in patients with EPE. At the cut-off level, the sensitivity and specificity were 65.8% and 67.5%, respectively. Conclusions: Pleural fluid eosinophilia was a speculative negative predictor for malignancy, despite the fact that cancers, including lung cancers and metastatic cancers to lung, were the most leading cause of pleural fluid eosinophilia. An inverse correlation was observed between the pleural eosinophil percentage and the likelihood of malignancy in patients with EPE.
