        2급 응급구조사의 성인외상환자에 대한 처치의 적절성 평가

        In Korea, EMT have two classified subgroup of first and second degree : EMT-P, and EMT. For the beginning, authorization of the first EMT was made in this year through the EMT educational program. There are 8 items which EMT can perform the treatment without the supervision by a physician. But there was no previous report of pertinent assesment of treatment performed by the EMT. There upon authors have conducted review of assessment done by the EMT in EC setting, and proposed the following treatment application can be used by EMT at the field of accident. The study materials include adult trauma patient older than 15 years, and sustained trauma within 48 hours before visiting Ewha University DongDaeMoon Hospital Emergency Deparment. The duration of study was from Apr. 1 1995 to Aug. 31 1995. the study was conducted by 5 certified EMT. The EMT's performance was evaluated by the attending Emergency Medicine physician. The result were as followings ; 1. Among the total 5532 ED visiting patients during the study period, 1267 were adult trauma patients who recieved treatment within 48 hours of injury. 2. Among 1222 out of 1267 patients have completed performed survey. 3. Male to female ratio was 2.6 : 1, age distribution was from 15 to 87 years old and mean age was 34.6 years old. 4. The MOI showed : MVA (33.1%), blunt trauma (29.9%), slip down (16.8%), penetrating injury(15.4%), and miscellaneous (5.7 %), burn (3.1%), falling down injury(2.9 %), bite injury(1.1%) in order. 5. There were 437 cases of survey completed by the EMT. For assessment of mental status of 3 different method : AVPU, 5 scale mental status and GCS, were correlated with EM physician with 99.1%, 98.6%, 97.7 %, respectively. 6. For 320 out of 437 cases of survey completed, 319 patients recieved treatment by the EMT within 30 minute and 1 patient between 30 to 60 minute of an ED arrival. 7. For those 437 cases of survey completed, 266 patient (60.9%) had adequate treatment and 54 patients (12.4%) had inadequate treatment. 67 patient (15.3%) did not received treatment but it was adequate inway, and 50 patients (11.9%) did not received treatment even though patient required treatment. In cases of adult trauma patients, EMT's mental status assessment was correlated with EM physician's assessment. But treatment done by EMT without the supervision of EM physician was adequate only in 76.2 %. Therefore, continuous education and assessment of treatment performed by EMT in needed to improve field treatment by EMT future.

        This paper describes the steady-state performance of a self-excited wound type induction generator with stator and rotor windings connected in series along with excitation capacitors. For this purpose a mathematical model is developed by means of the well known generalized machine theory utilizing d-q axis orthogonal transformation. This model can be used to analyze short shunt and long shunt generators as well as shunt generator and the algorithm for analysis is presented. The characteristics of generators are compared one another by using parameters which are validated by Mostafa et at.

        낭종 적출후 악골 결손부 치유에 관한 동종골 이식 효과에 관한 연구

        This study was aimed to suggest to better treatment method of jaw cyst that the maximum diameter was wider than 3cm, using different treatment and clinical and radiographic result. We divided the 60 patients into three groups, group A(20 patients) were treated with cyst enucleation and Decalcified Freeze-Dried Allogeneic Bone(DFDB) graft, group B(20 patients) were treated with cyst enucleation and autogenous bone graft, group C(20 patients) were treated with only cyst enucleation. Each group was evaluated with panramic radiograph and clinical sign & symptom at pre-op and post-op(immediate, 6, 12, 24, 36 month). Bone density was evaluated with disital densitometer. The result was as follows : 1. Post-Op infection was higher in group C(4 pts.) than in group A(1 pt.) and B(1 pt.) 2. Post-Op gingival recession was higher in group C(3 Pts.) than in group A(1 pt.) and B(1 pt.) 3. Anatomic distortion was higher in group C(3 Pt.) than in group A(1 Pt.), and B(1 pt.) 4. reoperation was done in two patients who were in group C 5. There were donor site morbidity in two patients 6. there was no significant difference between group A and B in their bony density in their follow up period(p>0.05). 7. There were significant differences between group A, B and group C in their bony density until post-op 24 months but a little differences at post-op 36 months(P<0.01)

        This study was investigated on spark ignition engine performance in according to the variation of methanol-fuel introduction amounts with the two-systemic injection fuel supply method without the change of structure of the conventional spark ignition engine. The mass burned was to the fast combustion and engine torque is being increased in according to increased methanol-fuel instroduction amount. NOx and CO exhaust performance were much improved in according to increased methanol-fuel introduction amount but HC exhaust performance was decreased.

        Nickel-Titanium file과 Stainless steel file을 이용한 근관형성시 컴퓨터 단층촬영사진상의 근관형태 변화에 관한 연구

        The goals of root canal instrumentation are complete debridement of pulp tissue, removal of microbes and affected dentin, and proper cleaning and shaping of the root canal space before obturation. Instrumentation with stainless steel files has been shown to produce undesirable results in canals, regardless of the improved technique or modified file type used. Nickel-Titanium(Ni-Ti) alloy has been shown to be exceptionally elastic, having a lower bending moment and lower permanent set after torsion, compared with similar guage stainless steel. The purpose of this study was to evaluate the change of root canal prepared by Ni-Ti rotary and stainless steel instruments. Thirty-four single rooted teeth of similar shape and canal size were divided into three groups. The teeth were canned by computed tomography before instrumentation. In group 1, canals were instrumented using a step-back technique with K-file. In group 2, canals were prepared with K-flex file using the same technique as group 1. Group 3 was prepared with nickel-titanium(Ni-Ti) rotary instrument using a manufacture's instruction. Instrumented teeth were again scanned using computed tomography, and reformated images of the uninstrumented canals were compared with images of the instrumented canals. K-flex file and Ni-Ti file caused significantly less canal transportation than K-file in the 8mm root canal section from the apex(p<0.05). K-flex file and Ni-Ti file produced more centered canal preparation than K-file in the 2mm section(p<0.05). Ni-Ti file maintained more precisely the center of the canal than K-flex file in the 10mm section(p<0.05). There was no difference in the removed volume of canals among each groups.

      • 배기계에서 비정상 가스의 소음특성에 관한 해석

        일반적으로 자동차의 소음은 주로 엔진에서 발생되어서 공기를 통해 전달되는 소음과 흡·배기계를 구성하는 tube 또는 Chamber를 통하여 전달되는 소음으로 구성된다. (1) 차량소음의 주요 소음원은 흡기계의 흡기소음과 배기계의 배기소음을 들 수 있으며, 소음중 배기소음은 도시소음의 주된 요인이며 차량의 승차감을 저해하는 간접적인 원인 중의 하나이다. 따라서 일반적인 자동차에는 반드시 소음기 즉, 머플러를 부착하여 소음을 어느 정도 이하로 감소시키게 되어 있다.(2.3) 전통적으로 소음기에 대한 연구는 주로 음향필터이론에 바탕을 두고 진행되어왔으나 음향필터이론은 배기소음기의 소음감쇄작용에 대한 체계적인 이론적 설명을 제시하지는 못했다. 음향에너지의 소산(Dissipation)을 고려하지 않고 음향이론상에서 배압을 예측하는 것은 불가능하다.(4) 그로므로 본 연구에서는 내연기관의 다양한 배기시스템에 대하여, 비선형 작용을 고려하므로서 1차원 비정상 등엔트로피 분석을 이용한 특성법(5)을 사용하였으며, 이러한 분석법에 배기시스템내에서 유동현상의 완전한 처리를 위하여 준정적 유동 접근법에 근거한 경계모델을 포함시켰다. 또한 이론의 타당성을 검증하기 위하여 최근까지 연구되어져온 블로오등을 사용한 시험결과는 달리 로터리 밸브 시뮬레이터를 사용하여 압력-시간 파형을 측정하였으며 측정된 결과와 계산된 결과를 비교하여 맥동 압력파 전파 특성을 확인하였다. 본 연구의 목적은 엔진 성능과 관계된 비정상 가스유동의 거동특성을 유한진폭과 이론을 사용하여 성공적으로 예측하는 방법을 제시하는데 있으며, 각 머플러에 대한 소음감쇠특성 및 소음기의 일반적인 특성을 분석하는데 있다. This thesis describes a theoretical and experimental investigation of the pressure propagation characteristics of some basic internal combustion engine exhaust systems. The mufflers of internal combustion engine have been applied to control the exhaust noise for many years. The greatest reduction in engine noise is generally achieved through proper muffler selection. Although proper muffler selection may seem relatively simple, a numbers of considerations must be made. Additional considerations in the reduction of exhaust system noise include proper selection of piping lengths and diameters, proper mounting of exhaust system components, and proper positioning of the exhaust outlet. In the present work, account is taken of the nonlinear behavior. For sections of parallel pipe, an one-dimensional unsteady homentropic analysis is employed, and a numerical solution is obtained with the aid of a digital computer, using the concept of characteristics. This analysis is than combined with boundary models, based on a quasi-steady flow approach, to give a complete treatment of the flow behavior in the exhaust system

        The speed characteristics of self-excited single-phase induction Generator using Three-Phase Squirrel-Case Induction Machine which has only two series capacitors with respect to the load are analyzed in this paper. The symmetrical component method and the condition of equivalent circuit are applied to develop an equivalent model of this generator. The speed characteristics of this generator are computed by using this model and compared with those of the self-regulated generator.

        A stripline holder is desinged and fabricated in order to measure complex permittivity (??) and permeability (??) simultaneously over a broad band. ?? and ?? are computed from S - parameter (S₁₁ and S₂₁) measurements made on a stripline sample holder loaded with the material under test. Measured ?? and ?? data for teflon and polyethylene are relatively accurate over the 0.5 GHz - 5.5 GHz band.

        The column for Steel Framed Reinforced Concrete Structure (SFRCS) and the column for Reinforced Concrete Structure (RCS) could be the most common building structure. The increasing of the need for massive space hasaffected the size of building components for supporting the massive structure. However, the changing of components size makes inefficient space of building. Hence, to meet the need for acquiring efficient space comparing the budget and cost the new structure method, Concrete Filled Tube Steel (CFT), was developed. CFT is the structure for which steel tube instead of other materials such as wood for holding concrete is used. The most benefit of this one is to help in reducing the size of the building components and local buckling because of tube steel holding concrete. For this reason, this research will examine the probability of applying CFT on construction sites by using the concrete (800kg/cm2) especially for CFT through the data from the real size mock-up.
