      • Internet Of Things As A Solution Enabler In Health Sector

        India is a country where vast medical needs are not met. Internet of Things (IoT) has created a new horizon in health care sector which specifically aims at connecting the world via smart objects. IoT enabled devices are used in various industries such as Envi-ronmental Monitoring, Building and Home Automation, Medical and Healthcare Systems, Infrastructure Management, Transport Systems, Energy Management, Industrial Applica-tions, etc. Healthcare industry is one of the biggest industries that provides health care products and incorporates the delivery of services to patients. These services are supported by electronic processs called as E-health. Using IoT devices remote health monitoring and notification can be done easily as well as quickly in case of emergency. Smart TV as a platform for E-Health Services can also be called as “Health-TV” which is one of the technology that provides IoT based e-Health Solutions to the patients. ICT in-dustry mainly focuses on the easy access of remote health services such as medication reminder, health monitoring and health coaching at home. It also helps in independent living style for the patient like senior citizen, managing and accessing health services in a specific condition from home via Health TV. “Smart infusion pumps” is another technol-ogy which is used for treating patients more carefully and accurately. It collects data and then aggregates in a database which contains the records of the administration in the central server. They are basically network devices which connect hospitals and clinics through wireless local area network.”Smart-o-meter” is a Web-based patient-centered decision support system for BP control using an iterative, user-centered design process so that it meets standards of feasibility and acceptability for patient navigators and partici-pants. The outcome or suggestion of this intensive research:  The government should implement the health care technological solutions, as pri-vate organizations can’t reach maximum masses of India.  A shift from traditional methods to the conventional methods of health care prac-tices in India is required so as to build a better environment for the people to live in.  As many Indians are still hesitant of relying on internet based technology, espe-cially IoT (fact from our survey over respondent in city PUNE, INDIA), a proper aware-ness session and demo session should be conducted in leading hospitals and institutes.

        Internet of Things is an emerging trend that will take existing health care sector to a new dimension. IOT based e-health products like Smart Infusion Pumps, Smart Monitoring, & Health TV will provide the edge over the existing traditional methods used in healthcare sector. The e–Health solutions provided through IoT devices are more accurate and accountable in context of IoT landscape, which provides various challenges along with exciting opportunities in the industry. Healthcare sector is not about just affordability, access & quality but also about financial & social aspects. Therefore, different Business Models have been developed by different enterprises for Transforming Health systems so as to create real time use and impact for the users especially patients, their families & communities where they belongs the centre of care. The main purpose behind every business model is to generate money in terms of profit. It is nothing but an organized structure where a company knows about its key partners, customers, resources, channels & revenue as well as cost structure. This in turns gives a picture about what an enterprise is, what are its strategies, its competitors etc. Better model showcase the enterprise their possibilities, competencies & responsiveness with the customers as well as clients.

        Clinical efficacy of 0.75% ropivacaine vs. 2% lignocaine hydrochloride with adrenaline (1:80,000) in patients undergoing removal of bilateral maxillary third molars: a randomized controlled trial

        Background: Lignocaine with adrenaline is routinely used as a local anesthetic for dental procedures. Adrenaline was added to increase the duration of anesthesia. However, epinephrine containing a local anesthetic solution is not recommended in conditions such as advanced cardiovascular diseases and hyperthyroidism. Recently, ropivacaine has gained popularity as a long-acting anesthetic with superior outcomes. The goal of this study was to assess and compare the effectiveness of 0.75% ropivacaine alone and 2% lignocaine with adrenaline (1:80,000) in the removal of bilateral maxillary wisdom teeth using the posterior superior alveolar nerve block technique. Methods: This was a single-blind, randomized, split-mouth, prospective study assessing 15 systemically sound outpatients who needed bilateral removal of maxillary third molars. We randomly allocated the sides and sequences of ropivacaine and lignocaine with adrenaline administration. We evaluated the efficacy of both anesthetics with regard to the onset of anesthesia, intensity of pain, variation in heart rate, and blood pressure. Results: The onset of anesthesia was faster with lignocaine (138 s) than with ropivacaine (168 s), with insignificant differences (p = 0.001). There was no need for additional local anesthetics in the ropivacaine group, while in the lignocaine with adrenaline group, 2 (13.3%) patients required additional anesthesia. Adequate intraoperative anesthesia was provided by ropivacaine and lignocaine solutions. No significant difference was observed in the perioperative variation in blood pressure and heart rate. Conclusion: Ropivacaine (0.75%) is a safe and an adrenaline-free local anesthetic option for posterior superior alveolar nerve block, which provides adequate intraoperative anesthesia and a stable hemodynamic profile for the removal of the maxillary third molar.

        Polyostotic Spinal Paget’s With Lumbar Kyphotic Deformity: A Case Report And Literature Review

        Study Design: Case report Objectives: Paget’s disease is a metabolic disorder of the skeletal system involving the spine commonly. Prevalence of Paget’s is uncommon in Southern Asia when compared to the West. The disease can be divided into three major phases namely lytic, mixed and a sclerotic phase. They have 2 types of clinical presentation- polyostotic or monostotic type. Blood and radiological investigations help in diagnosis. The mainstay of management is pharmacotherapy, rarely requiring surgery. Materials and Methods: A 45 years old male, presented with back pain and claudication, initially mistreated to be a case of Pott’s spine. After investigating, he was operated because of worsening symptoms and conformation of diagnosis. Retrospective analysis of operation theatre and out patient records was done, compiled and presented. Results: Confirmatory diagnosis of Paget’s disease was done based on histopathology. A whole body radionucleotide scan was done to rule out any other skeletal lesion. He was started on medical management for Paget’s. He improved symptomatically, during the last follow up he was asymptomatic with no clinical disability and radiological improvement of lesion. Conclusions: Atypical presentation of Paget’s with a polyostotic spinal involvement in a South Asian is rare. High suspicion and prompt investigations help in early diagnosis and treatment. Pharmacotherapy is the mainstay of treatment with surgery when indicated.

        Double-String Model for Auditory Transduction of Drosophila

        The Drosophila auditory system consists of four large basal segments: the arista, the funiculus, the pedicel, and the scape. When an acoustic stimulus is applied to the arista and the funiculus their mechanical vibrations are transmitted to chordotonal neurons in Johnston's organ where mechanoelectric transduction arises. We study the mechanotransduction mechanism in the Drosophila auditory system by using a laser Doppler vibrometer (LDV) and extracellular electrophysiology. We nd that large and small peaks appear alternatively and that the antenna vibration is asymmetric depending on whether the pedicel and the scape are fixed. Interestingly, we find that this asymmetric vibration accompanies the alternating neural peak structure. Here, we propose a mathematical model to explain the alternating peak structure by using a model consisting of two opposing neurons that are modeled as strings. Generally, strings have tension only when they are elongated. This property allows the alternating neural peaks for asymmetric antenna motion.
