        Towards automatic EEG cyclic alternating pattern analysis: a systematic review analysis: a systematic review

        This study conducted a systematic review to determine the feasibility of automatic Cyclic Alternating Pattern (CAP)analysis. Specifically, this review followed the 2020 Preferred Reporting Items for Systematic reviews and Meta-Analyses(PRISMA) guidelines to address the formulated research question: is automatic CAP analysis viable for clinical application?From the identified 1,280 articles, the review included 35 studies that proposed various methods for examining CAP,including the classification of A phase, their subtypes, or the CAP cycles. Three main trends were observed over timeregarding A phase classification, starting with mathematical models or features classified with a tuned threshold, followedby using conventional machine learning models and, recently, deep learning models. Regarding the CAP cycle detection,it was observed that most studies employed a finite state machine to implement the CAP scoring rules, which depended onan initial A phase classifier, stressing the importance of developing suitable A phase detection models. The assessment ofA-phase subtypes has proven challenging due to various approaches used in the state-of-the-art for their detection, rangingfrom multiclass models to creating a model for each subtype. The review provided a positive answer to the main researchquestion, concluding that automatic CAP analysis can be reliably performed. The main recommended research agendainvolves validating the proposed methodologies on larger datasets, including more subjects with sleep-related disorders,and providing the source code for independent confirmation.

        Spectrophotometric evaluation of restorative composite shades and their match with a classical shade guide

        Objectives The aim of this study was to verify the match between 5 shades of composites from different manufacturers with a shade guide and among the systems using a portable spectrophotometer. Materials and Methods Shade measurements were performed on specimens of Z350 XT (3M ESPE), Charisma Diamond (Heraeus Kulzer GmbH), Esthet X-HD (Dentsply Caulk), and Empress Direct (Ivoclar-Vivadent) for shades A1, A2, A3, B1, and C3 using a Vita Easyshade spectrophotometer (Vita Zahnfabrik) against a white background. Corresponding shades of Vitapan Classical (Vita Zahnfabrik) guide were measured likewise and shade variation (ΔE) was calculated based on International Commission on Illumination L*a*b* parameters. The ΔE of the composites in each shade was compared by one-way analysis of variance and Tukey's post hoc test (α = 0.05). Results All composites presented ΔE > 3.7 compared with the shade guide. Variation in shades A3, B1, and C3 was significantly different for all composites. ΔE of Z350 XT was significantly lower for A1 than for the other shades, whereas ΔE of Z350 XT and Charisma Diamond were significantly lower for A2 than for the other shades. Conclusions No composite shade matched with the shade guide. Equivalent shades of the restorative composite from different manufacturers may show clinically noticeable ΔE.         Validation and comparison of volume measurements using 1 multidetector computed tomography and 5 cone-beam computed tomography protocols: An in vitro study

        Purpose: The purpose of this study was to compare volume measurements obtained using 2 image software packages on Digital Imaging and Communications in Medicine (DICOM) images acquired from 1 multidetector computed tomography and 5 cone-beam computed tomography devices, using different protocols for physical volume measurements. Materials and Methods: Four pieces of bovine leg were prepared. Marrow was removed from 3 pieces, leaving cortical bone exposed. The resulting space of 1 piece was filled with water, another was filled with propylene glycol, and the third was left unfilled. The marrow in the fourth sample was left fully intact. Volume measurements were obtained after importing DICOM images into the Dolphin Imaging 11.95 and ITK-SNAP software programs. Data were analyzed using 3-way analysis of variance with a generalized linear model to determine the effects of voxel size, software, and content on percentage mean volume differences between tomographic protocols. A significance level of 0.05 was used. Results: The intraclass correlation coefficients for intraobserver and interobserver reliability were, respectively, 0.915 and 0.764 for the Dolphin software and 0.894 and 0.766 for the ITK-SNAP software. Three sources of statistically significant variation were identified: the interaction between software and content (P = 0.001), the main effect of content (P = 0.014), and the main effect of software (P = 0.001). Voxel size was not associated with statistically significant differences in volume measurements. Conclusion: Both content and software influenced the accuracy of volume measurements, especially when the content had gray values similar to those of the adjacent tissues.

        Application of Bonelike® as synthetic bone graft in orthopaedic and oral surgery in veterinary clinical cases.

        Autologous bone remains the gold standard grafting substrate for bone fusions used for small gaps and critical defects. However, significant morbidity is associated with the harvesting of autologous bone grafts and, for that reason, alternative bone graft substitutes have been developed. In the present case series, a glass-reinforced hydroxyapatite synthetic bone substitute, with osteoinductive and osteoconductive proprieties, was applied. This synthetic bone substitute comprises the incorporation of P2O5-CaO glass-based system within a hydroxyapatite matrix, moulded into spherical pellets with 250-500 μm of diameter. A total of 14 veterinary clinical cases of appendicular bone defects and maxillary / mandibular bone defects are described. In all clinical cases, the synthetic bone substitute was used to fill bone defects, enhancing bone regeneration and complementing the recommended surgical techniques. Results demonstrated that it is an appropriate synthetic bone graft available to be used in veterinary patients. It functioned as a space filler in association with standard orthopaedic and odontological procedures of stabilization, promoting a faster bone fusion without any local or systemic adverse reactions. This procedure improves the animals’ quality of life, decreasing pain and post-operative recovery period, as well as increasing bone stability improving positive clinical outcomes.

        Dietary chia (Salvia hispanica L.) improves the nutritional quality of broiler meat

        Objective: The current study was conducted to evaluate the quality and profile of fatty acid in the breast and thigh, and the performance of broilers fed diets containing seed or oil of chia (Salvia hispanica L.) as a replacement for soybean, in the rearing period from 29 to 42 days of age. Methods: On the 29th day of age, 120 broilers were distributed in four treatments evaluated in five replicates of six birds. The grain or oil of soybean was respectively replaced on a weight-to-weight basis in the formulation by the seed or oil of chia, constituting the experimental diets. The roasted whole soybean and chia seed were included in the feed at 16.4%, whereas the soybean and chia oils were included at 2.5%. Results: The dietary chia oil increased the lipid peroxidation in the thigh meat, and the dietary chia seed increased the cooking loss of the thigh. However, for the other physicochemical parameters evaluated and for the proximate composition of the breast and thigh, in general, the inclusion of chia seed or oil in the diet provided similar or better results than those observed when the diets contained soybean oil or roasted whole soybean. With regard to the fatty acid profile and associated parameters, dietary chia increased the concentrations of α-linolenic, eicosapentaenoic, and docosahexaenoic acids and reduced the Σω-6:Σω-3 ratio and the atherogenicity and thrombogenicity indices of the broiler meat. However, the dietary chia seed worsened the feed conversion ratio. Conclusion: Diet containing 2.5% chia oil supplied to broilers during the period from 29 to 42 days of age improves the feed conversion ratio, increases the deposition of the ω-3 fatty acids in the breast and thigh, in addition to reducing the Σω-6:Σω-3 ratio and the atherogenicity and thrombogenicity indices, thereby resulting in meat with higher nutritional quality.

        Hypoalbuminemia as a risk factor for thromboembolic events in inflammatory bowel disease inpatients

        Background/Aims: Inflammatory bowel disease (IBD) are chronic entities characterized by local and systemic inflammationand may be associated with thrombosis. The aim of this study was to identify the prevalence of thromboembolic events (TEE)in hospitalized IBD patients and identify risk factors for their occurrence. Methods: This retrospective, single-center study includedpatients treated at a Brazilian IBD referral unit between 2004 and 2014. Patients hospitalized for more than 48 hoursdue to active IBD and who did not receive prophylaxis for TEE during hospitalization were included. Patients were allocated to2 groups: those with TEE up to 30 days or at the time of hospitalization (TEE-group) and patients without TEE (control-group). Clinical and laboratory characteristics were evaluated. Results: Of 53 patients evaluated, 69,8% with Crohn’s disease (CD) and30.2% with ulcerative colitis (UC). The prevalence of TEE 30 days before or during hospitalization was 15.1%, with 10.8% in CDand 25% in UC. In the TEE group, mean serum albumin was 2.06 g/dL versus 3.30 g/dL in the control group. Patients with albuminlevels below 2.95 g/dL (43.18%) had a higher risk of developing TEE (relative risk, 1.72; 95% confidence interval, 1.17–2.53)(P<0.001). Conclusions: Albumin levels were significantly lower in patients with TEE, and hypoalbuminemia was considered arisk factor for the development of TEE in this population. (Intest Res 2019;17:63-69)

        Effect of a professional soccer match in skin temperature of the lower limbs : a case study

        The purpose of this study is to analyze the relationship between lower limbs skin temperature (Tsk) recorded by infrared thermography and creatine kinase (CK) response following an official soccer game. A pro-fessional soccer player, from the first division Brazilian League, defender, 27 years and 183 cm of height was analyzed. Measures of Tsk with a thermal camera and blood CK concentrations were obtained 24 hr be-fore (24H-PRE), 24 (24H-POST) and 48 hr (48H-POST) after an official match. The results showed that CK values were 193 U/L 24 hr before, ris-ing to 1,083 U/L 24 hr after the game and 414 U/L 48 hr after the game. Anterior thigh Tsk was 31.5ºC, 33.8ºC, and 31.8ºC in the moments 24H-PRE, 24H-POST, and 48H-POST, respectively. Similarly, anterior leg presented temperature of 31.2ºC, 33.3ºC, and 31.5ºC at the same moments. Qualita-tive analysis of the thermograms showed that whole lower limbs are much warmer 24 hr after the match and certain areas as the right anteri-or thigh, the left anterior leg, both anterior ankles, and both posterior thighs have not fully recovered their initial Tsk 48 hr after the match. The results of this study indicate that participation in a professional soccer match can lead to significant increases in Tsk values measured by IRT 24 hr after the match. Considering this variable as an indicator of muscu-lar damage, it could help in the process of training control, being part of an injury prevention program in professional soccer clubs.

        High prevalence of musculoskeletal pain in individuals with severe obesity: sites, intensity, and associated factors

        Background: Musculoskeletal pain is associated with obesity; however, information on factors associated with pain in adults with obesity and severe obesity is limited. The purpose of this study was to assess the prevalence of musculoskeletal pain by site and intensity of pain and associated factors in individuals with severe obesity (body mass index ≥ 35.0 kg/m<sup>2</sup>). Methods: Baseline data from the DieTBra Trial study evaluating pain symptoms in nine body regions over the last seven days using the Nordic Questionnaire on Musculoskeletal Symptoms and Numerical Pain Scale. The variables analyzed using multiple Poisson regression with hierarchical analysis were: sociodemographic, lifestyle, food consumption, clinical, and anthropometric, and the outcome was moderate and intense pain. Results: In 150 participants, there was a high prevalence of ankle and foot pain (68.7%), lower back pain (62.7%), pain in the knees (53.3%) and upper back pain (52.0%), with a predominance of intense pain. Factors associated with pain according to specific sites were: type 2 diabetes with hand/wrist pain; sedentary time with hip pain; insomnia with pain in the hip and knee; edema in the lower limbs with pain in the lower back and ankles/feet; degree of obesity with ankle/foot pain; and percentage of total fat with ankle/foot pain. Conclusions: There was a high prevalence of pain and intense pain in individuals with severe obesity and an association with clinical variables, the degree of obesity, and sedentary lifestyle.

        Cytomegalovirus Infection after Renal Transplantation: Occurrence, Clinical Features, and the Cutoff for Antigenemia in a University Hospital in Brazil

        Background: Cytomegalovirus (CMV) is the main infectious agent causative of morbidity and mortality in transplant recipients. This study aimed to describe the occurrence and clinical features of CMV infection, and the optimum antigenemia assay cutoff associated with symptomatic infection. Materials and Methods: This was a cohort study that investigated 87 patients undergoing renal transplantation. The patients were monitored with the CMV antigenemia assay performed weekly for the first 3 months post-transplantation and subsequently, when CMV infection was suspected clinically. Results: CMV infection was observed in 63.2% (55/87) of the recipients during the follow-up. Of the 65 episodes observed, 75% (49/65) occurred until 100 days after transplantation (D+100) and 25% (16/65) after D+100 with a median of 60 days. CMV infection was associated with age of the transplant recipients (P = 0.001) and use of deceased donor organ (P = 0.009). There were asymptomatic (34%) and symptomatic (66%) episodes of CMV infection, in which diarrhea was the most common symptom (22.6%), followed by elevated creatinine levels (14.5%), fever (12.9%) and leukopenia (10.5%). The optimum cutoff point associated with symptomatic infection was 5 positive cells/200,000 leukocytes (area under the curve = 0.87, positive predictive value = 88% and negative predictive value= 71%). Conclusion: The high occurrence and the risk factors for CMV infection such as the age of recipients, the number of positive cells in the antigenemia assay, and use of a deceased donor organ should be considered for appropriate monitoring and management of kidney recipients during the post-transplant period.
