There has been a significant increase in the use of mushrooms for therapeutic and medicinal purposes, in particular, use of the species Agaricus blazei Murrill, a basidiomycota of Brazilian origin. The objective of this study was to identify scientific evidence regarding the influence of A. blazei Murrill on the immune system. We undertook an integrative review of indexed publications published between 2000 and 2009, using the following question as a guideline: “What evidence can be found in the literature regarding the influence of A. blazei Murrill on the immune system?” Fourteen studies verified that there is in vitro and in vivo research demonstrating this mushroom's influence on the immune system. All research was characterized as evidence level 7 (preclinical study [animals/in vitro]). The research shows that A. blazei Murrill functions through bioactive compounds via mechanisms that are not yet entirely clear, although it has been shown that they promote action on the innate and adaptive immunological response, activation of the complement system, and synthesis of pro- and anti-inflammatory cytokines and even aid in diapedesis. Despite broad scientific evidence demonstrating relevant immunomodulatory properties of A. blazei Murrill, randomized clinical trials with human subjects are still needed in order for the mushroom to be put into clinical practice.

Purpose: The purpose of the present study was to examine the effects of the area of the base of support formed by the human body on the maximal voluntary isometric contraction of upper limb muscles. Methods: The study was conducted with 20 normal adults. To identify changes in the base of support, the maximal voluntary isometric contraction of the biceps muscle was measured in a standing position, a sitting position, and a lying position for each subject. The sizes of the base of support formed in the standing, sitting, and lying positions were set to 1, 2, and 3 respectively, based on the sizes, to analyze the correlations. The maximal voluntary isometric contraction of the biceps muscle was measured using surface electromyograms (EMGs) (Noraxon DTS, Germany). Results: The results showed negative correlations in which, as the size of the base of support increased, the maximal voluntary isometric contraction of the biceps muscle decreased. Conclusion: Changes in the base of support of the body affect the maximal voluntary isometric contraction of the upper limbs. Therefore, when resistance exercises are applied for muscle strengthening, the positions should be changed considering the changes in muscle activity according to those positions. In addition, when EMGs are used to measure the maximal voluntary isometric contraction, the measurements should be conducted in the same positions, considering muscle activity that changes according to the base of support and positions, for data quantification.
척주측만증 환자의 만곡과 후쿠다 검사의 상관성에 관한 연구

PURPOSE: The Fukuda test can be used at home and in school to diagnose scoliosis at an early stage and prevent serious curvature of the spine. This study aimed to use the Fukuda test to detect scoliosis. An additional aim was to invoke the national interest in imbalanced postures and habits, which result in scoliosis, by providing data obtained in periodic assessments. METHODS: The study consisted of 35 idiopathic scoliosis patients (22 in right lumbar spinal region and 13 in the left lumbar spinal region). The distance of displacement and angle of displacement were measured following the Fukuda test. A correlation analysis was then used to examine the distance of displacement and angles of displacement and rotation with regard to the direction of the curve in scoliosis. RESULTS: There was a significant negative correlation (p<0.00) between the direction of the curve in scoliosis and the angle of displacement, but there was no correlation between the cobb``s angle and distance of displacement or between the cobb``s angle and angle of rotation. CONCLUSION: The Fukuda test did not capture changes in spinal curvature such as the cobb``s angle, or subsequent changes in the muscles. Thus, the Fukuda test is not suited to examining the direction or status of the thoracic curve in scoliosis patients. Simple methods to objectively measure scoliosis need to be developed.

Background and Objectives: Bilateral microphones with contralateral routing of signal (BiCROS) hearing aid is an option for hearing rehabilitation in individuals with asymmetric sensorineural hearing loss (ASNHL). The clinical factors influencing the trial and purchase of BiCROS were investigated. Subjects and Methods: We reviewed the medical records of 78 patients with ASNHL who were recommended to use BiCROS and analyzed the demographic and audiological factors influencing the trial and purchase of BiCROS. Results: Among the 78 patients, 52 (66.7%) availed of the free BiCROS trial and 21 (26.9%) purchased BiCROS. The mean pure tone audiometry (PTA) air conduction (AC) threshold of the better- and worse-hearing ears were 44.2±12.8 dB and 90.7±22.5 dB HL, respectively. The decision for trial or purchase of BiCROS was not influenced by age, sex, duration of hearing loss of the worse-hearing ear, or PTA AC threshold or speech discrimination score of both ears. The first and third quartiles of the PTA AC thresholds for the better-hearing ear of BiCROS buyers were 38.75 dB and 53.75 dB HL, respectively. The counterpart values for the worse-hearing ear were 72.50 dB and 118.75 dB HL, respectively. Conclusions: The clinical factors analyzed in this study were found to be irrelevant to the trial and purchase of BiCROS in patients with ASNHL. Nevertheless, the distribution range of the auditory thresholds of the subjects using BiCROS can be a useful basis for the counseling of patients with ASNHL and selection of candidates for BiCROS use.
FCP 14 : Screening of psoriatic arthritis in Korean psoriasis patients using PASE questionnaire

Background: Psoriatic arthritis (PsA) is a chronic seronegative oligoarthritis causing irreversible joint damage. Early recognition of PsA in patients with psoriasis is important to prevent physical disability and deformity. However, diagnosing PsA pose a distinct challenge for most dermatologists in the context of a busy clinic. Objectives: The aim of this study was to validate Psoriatic Arthritis Screening Evaluation (PACE) questionnaire in detecting PsA in Korean patients with psoriasis. Methods: A total 148 patients with a diagnosis of psoriasis, but not previously diagnosed with PsA were administered with PACE questionnaire prospectively. All patients underwent radiologic and laboratory examinations. In addition, they were clinically evaluated by a rheumatologist. Results: Eighteen patients (12.1%) were diagnosed with PsA meeting the Classification Criteria for Psoriatic Arthritis (CASPAR) criteria. The sum of PACE score in patients with PsA showed signifcant difference compared with those without PsA. Receiver operator curves showed an area under the curve of 0.82 (95% CI 0.72, 0.92) for the total score. A cut-off value ≥36 showed sensitivity of 77.8%, and specificity of 82.3% for the diagnosis of PsA. Conclusion: The PACE questionnaire is simple and convenient screening tool for detecting PsA in Korean dermatologic clinic. Dermatologists should consider PASE questionnaire for psoriasis patients attending clinics, as it proved robust for the early identification of PsA.

Background: Mohs micrographic surgery(MMS) is the most widely used treatment option for non-melanoma skin cancer(NMSC) because of very high success rate of complete excision. However, even MMS cannot guarantee a 100% chance of cure and there is risk of recurrence after MMS. Objectives: To analyze clinical and histologic data of cases of recurred NMSC after MMS and to understand characteristic features of recurred cases. Methods: Total of 17 patients, including 9 with basal cell carcinoma(BCC) and 8 with squamous cell carcinoma(SCC), recurred after MMS during 15 year period at Pusan National University Hospital. Previous treatment history, location of lesion, clinical and histologic subtype and data related to MMS were investigated. Results: Five patients with BCC and 4 with SCC had previous treatment history such as laser ablation and incomplete excision. In BCC group, 4 were located on periorbital area and 6 had aggressive histologic type. The average number of MMS stage, tumor size and length of surgical margin in study groups was more larger than that of total patient group treated with MMS at our hospital. The mean interval between operation and recurrence was 52 months in BCC group and 9 months in SCC groups. Conclusion: Based on our study, particular attention should be drawn to NMSC patients with previous treatment history, specific anatomical location such as periorbital area, and large tumor size to reduce the risk of recurrence after MMS.
