      • Cultural immersion in the education of healthcare professionals: a systematic review

        PurposeWith the ever-changing cultural makeup of society, the ability to deliver culturally appropriate healthcare is essential. An educational method aimed at increasing cultural knowledge and sensitivity in the education of healthcare professionals is cultural immersion, which creates opportunities for transformational learning through direct interactions with culturally diverse populations. The purpose of this systematic review was to examine the qualitative effects of cultural immersion experiences on graduate-level healthcare professional students. MethodsA search of the CINAHL (Cumulative Index to Nursing and Allied Health Literature) and ERIC (Education Resources Information Center) databases was performed, utilizing search terms including cultural immersion, cultural sensitivity, educational outcomes, and healthcare professionals. The search was limited to publications within the last 10 years. The articles were screened according to title, abstract, and full-text following the application of inclusion/exclusion criteria. Themes identified within each article were collected and categorized, using a qualitative methodology, into 5 overarching domains to assess the educational experiences. Studies were scored for quality using the qualitative portion of the McGill Mixed Methods Appraisal Tool-2011. ResultsNine studies incorporating a total of 94 participants with experiences in 14 culturally diverse environments revealing 47 individually identified themes were included in the review. The results indicated that all cultural immersion experiences stimulated increased cultural awareness and sensitivity. ConclusionCultural immersion experiences produced a positive, multi-domain effect on cultural learning in students of the health professions. The results of this review provide support for implementing cultural immersion experiences into the education of healthcare professionals with the goal of increasing cultural sensitivity.

      • USE OF FLOW-THROUGH SYSTEMS TO STUDY ANAEROBIC MICROBIAL PROCESSES WITHIN COASTAL LAGOON SEDIMENT

        ABSTRACTExperimental continuous flow-through systems were used to investigate the effect of continuous nutrient inputs into sediments collected from a brackish coastal lagoon. Mixed anoxic surface sediments were percolated (40 ml·h-¹) in reactors with selected substrates (sucrose, acetate or trimethylamine) that enter the anaerobic food chain at different levels. After two months of continuous flow conditions, the number and activity of fermentative, denitrifying, sulfate-reducing and methanogenic bacteria were determined. Sulfate-reduction was the dominant terminal process in natural sediment and remained so in the different reactors, exhibiting rates one to three and one to two orders of magnitude greater than denitrification and methanogenesis rates, respectively. Sulfate-reduction was limited by electron donor availability in natural sediment. Continuous supply of sucrose allowed Sulfate-reduction as high as observed when acetate was a suitable energy source for sulfate-reducers. Denitrification was markedly limited by the low availability of nitrate. Methane production was essentially derived from trimethylamine, whereas acetate and hydrogen were primarily metabolized by sulfate-reducers. Maximal Sulfate-reduction resulted in methane production and denitrification markedly reduced, and the concomitant and maximal activities of sulfate-reducers and fermenters led to the near exclusion of methanogens and denitrifiers in the mediation of carbon flow.

      • Spray Characterization of Gas-Centered Swirl Coaxial Injectors U sing an Optical Probe

        In order to investigate spray characteristics of gas-centered swirl coaxial injectors, a phase detection optical probe is employed to obtain the spatial evolution of the drop size and velocity. From the study on the optical probe responses under various impact angles, it is demonstrated that the drop size and velocity can be measured with an uncertainty less than 15% when the probe axis remains within about ±15° of the drop velocity direction. This typical uncertainty is in good agreement with a previous study. It is also shown that the drop sizes measured by the optical probe are in accord with those evaluated by image processing techniques. Finally, the experiments with the optical probe are performed in dense sprays, as it were, in the near field of gas-centered swirl coaxial injectors. Some experimental results are presented and discussed to be of help to understanding of spray characteristics of the injectors.

        Manipulation of Tissue Energy Metabolism in Meat-Producing Ruminants - Review -

        Skeletal muscle is of major economic importance since it is finally converted to meat for consumers. The increase in meat production with low costs of production may be achieved by optimizing muscle growth, whereas a high meat quality requires, among other factors, the optimization of intramuscular glycogen and fat stores. Thus, research in energy metabolism aims at controling muscle metabolism, but also liver and adipose tissue metabolism in order to optimize energy partitioning in favour of muscles. Liver is characterized by high anabolic and catabolic rates. Metabolic enzymes are regulated by nutrients through short-term regulation of their activities and long-term regulation of expression of their genes. Consequences of liver metabolic regulation on energy supply to muscles may affect protein deposition (and hence growth) as well as intramuscular energy stores. Adipose tissues are important body reserves of triglycerides, which result from the balance between lipogenesis and lipolysis. Both processes depend on the feeding level and on the nature of nutrients, which indirectly affect energy delivery to muscles. In muscles, the regulation of rate-limiting nutrient transporters, of metabolic enzyme activities and of ATP production, as well as the interactions between nutrients affect free energy availability for muscle growth and modify muscle metabolic characteristics which determine meat quality. The growth of tissues and organs, the number and the characteristics of muscle fibers depend, for a great part, on early events during the fetal life. They include variations in quantitative and qualitative nutrient supply to the fetus, and hence in maternal nutrition. During the postnatal life, muscle growth and characteristics are affected by the age and the genetic type of the animals, the feeding level and the diet composition. The latter determines the nature of available nutrients and the rate of nutrient delivery to tissues, thereby regulating metabolism. Physical activity at pasture also favours the orientation of muscle metabolism, towards the oxidative type. Consequently, breeding systems may be of a great importance during the postnatal life. Research is now directed towards the determination of individual tissue and organ energy requirements, a better knowledge of nutrient partitioning between and within organs and tissues. The discovery of new molecules (e. g. leptin), of new molecular mechanisms and of more powerful techniques (DNA chips) will help to achieve these objectives. The integration of the different levels of knowledge will finally allow scientists to formulate new types of diets adapted to sustain a production of high quality meat with lower costs of production.

        UNESCO Woman Diver, Ms. Hee Choon Becomes Human Spirit beyond the Jeju Tragedy : Hee Choon Oh’s Testimony at the UNESCO, Peace Island Network in Paris and Paris Arirang

        Ms. Hee Choon Oh was a Jeju Haenyeo(woman diver) and one of the few surviving victims of the Jeju 4.3 (1948). As a Haenyeo: Jeju society was very poor at the time during the Japanese colonial period and the division of the two Korea, especially after the Korean War, so she had to work regardless of gender. It was hard to go into the sea, but it was the only thing to support my family. In retrospect, Going out to work in the sea was like a life-threatening adventure every day. While there were many emergency moments, She becomes one with the sea and forgot all the pain. After having spent a lot of time together with the sea, I had to stop working as a Haenyeo because it was hard to take care of nine children after having heart surgery. I was able to receive Honorary Haenyeo through continuous exchanges and dedication to the society. As a victim of the Jeju 4.3: I had no choice but to explain it why I had to do work as a Haenyeo and to understand my life. I am a woman who chose the job of Haenyeo, but I have lived through the pain of the dark modern and contemporary history of Korea. The unfair one-year prison life that I wanted to hide even from my children hurt me all my life. Over the past 70 years, sharing pain with the sea, neighbors, and family, serving the region, and hopefully waiting for a better society and justice to come. and finally It was not until 2019, 70 years after 1948, that I was officially acquitted by a Korean court. I regained my honor as a Haenyeo and was able to get rid of my deep sadness

        PAIN-T A Tool for Patient Self-Assessment of Pain

        Pain can provide important information to be used in treating illness. But since there is no medical device that can directly measure a patient’s pain, physicians and medical staff must rely on patients themselves to indicate the location and intensity of their pain. This task is typically accomplished using instruments that facilitate patients expressing their pain in terms of abstract schema that correlate to medical formalisms for quantifying pain. This paper examines the preliminary development and testing of PAIN-T, a novel system utilizing successful elements from existing pain assessment approaches along with the unique capabilities of multitouch tablet computers to allow patients to easily and accurately asses their own pain with a minimum of instruction and assistance.

      • Variation in mass and nutrient concentration of leaf litter across years and sites in a northern hardwood forest

        <P> Leaf litterfall represents an important nutrient flux in forests, but separating leaves by species and collecting fresh litter annually for nutrient analysis is time-consuming and expensive. To quantify the sources of variation in litterfall nutrient estimates and guide optimal allocation of research effort, we analyzed nutrient concentration (5 years) and mass (6 years) of leaf litter for nine tree species in 13 northern hardwood sites. Coefficients of variation (CVs) in nutrient concentration were higher across sites than over time within sites for most elements; phosphorus was especially variable across sites (56% CV). Thus, to estimate litterfall nutrient fluxes accurately in forests of this type, nutrient analyses should be site-specific as well as species-specific but may not need to be repeated annually (CVs over time averaged 17% for calcium, 21% for magnesium, 28% for potassium, and 32% for phosphorus concentration). Total leaf litterfall mass varied considerably from year to year, ranging from 234 to 370 g·m<SUP>-2</SUP> averaged over 13 sites. We recommend that litter collectors be elevated above the ground to avoid oversampling during extreme wind events. Use of species-specific allometric equations, or even basal area, to estimate the species composition of total litter mass may obviate the need to sort litter by species. </P>
