        Efficacy of Trimetazidine Dihydrochloride for Relieving Chronic Tinnitus: A Randomized Double-Blind Study

        Objectives: To evaluate the efficacy of trimetazidine dihydrochloride as a treatment for chronic tinnitus. Methods: A total of 97 chronic tinnitus patients were evaluated in this randomized, prospective, double-blind, placebo-controlled trial. After assessing for eligibility, 82 patients were randomly assigned into placebo or trimetazidine groups according to the medication. The trimetazidine group received 20×3 mg/day per oral trimetazidine dihydrochloride and the placebo group received 20×3 mg/day per oral placebo for 3 months. Tinnitus handicap inventory (THI), visual analogue scale (VAS) questionnaires and audiometric results were used to determine the effectiveness of trimetazidine treatment. Results: The study group comprised 82 tinnitus subjects, 42 (51%) of whom received trimetazidine dihydrochloride and 40 (49%) who received placebo. There was no significant difference between placebo and trimetazidine groups in THI grade and VAS (both pre- and posttreatment scores) (P>0.05) and no significant improvement was observed in subjective loudness score in either group (P>0.05). Additionally there was no significant difference between groups in pre- and posttreatment pure tone hearing thresholds at all measured frequencies (P>0.05). Conclusion: Trimetazidine dihydrochloride therapy was ineffective for relieving chronic tinnitus.

        Are There Relationships between Seminal Parameters and the Neutrophil-to-Lymphocyte Ratio or the Platelet-to-Lymphocyte Ratio?

        Purpose: The aim of this study was to evaluate the relationship of seminal parameters with the neutrophil-to-lymphocyte ratio (NLR) and the platelet-to-lymphocyte ratio (PLR), which are inflammatory markers, in men with an abnormal semen analysis and men with normozoospermia.Materials and Methods: Fifty-seven men with abnormal semen analyses and 59 men with normozoospermia were included in this cross-sectional study. A complete blood count was recorded, and the NLR and PLR were calculated from the hematologic parameters.Results: The NLR was 1.84±0.78 in the normozoospermic group and 1.80±0.75 in the abnormal semen analysis group. The PLR was 95.32±35.47 in the normozoospermic group and 93.57±28.09 in the abnormal semen analysis group. No significant differences were found between the normozoospermic and the abnormal semen analysis group in the NLR (p=0.828) or the PLR (p=0.768). In addition, no seminal parameters were correlated with the NLR or the PLR (p>0.05).Conclusions: We do not recommend using the NLR or the PLR as markers to screen for abnormal semen parameters or male infertility.

        Reconstruction of the Orbit With a Temporalis Muscle Flap After Orbital Exenteration

        Objectives. This study presents the role of the temporalis muscle flap in primary reconstruction after orbital exenteration. Methods. A retrospective nonrandomized study of orbital exenterations performed between 1990 and 2010 for malignant tumors of the skin, paranasal sinus, and nasal cavity is presented. Results. The study included 13 patients (nine men, four women; age range, 30–82 years) with paranasal sinus, nasal cavity, or skin carcinomas. Primary reconstruction of the cavity was performed in all patients after orbital exenteration. No visible defects in the muscle flap donor site were present. Local recurrences were readily followed up with nasal endoscopy, whereas radiology helped to diagnose intracranial involvement in three patients. Two patients died of systemic metastases and five died for other reasonsConclusion. The temporalis muscle flap is readily used to close the defect after orbital exenteration, and does not prevent the detection of recurrence.

        Characterization of Ni-W Solid Solution Alloy Powders and Sintered Compacts Synthesized via Mechanically Activated Hydrogen Reduction of NiO-WO3 Mixtures

        Blended nickel oxide - tungsten oxide powders corresponding to the compositions of 70 wt% nickel - 30 wt%tungsten were mechanically alloyed (MA’d) for different durations such as 0 h, 6 h, 12 h and 24 h and reduced/alloyed at 550 °C for 1 h followed by 600 °C for 0.5 h under hydrogen (H2) atmosphere. H2 reduction of the MA’d fine oxide powders resulted in the fabrication of nanocrystalline Ni(W) solid solution alloy powders, whereas a mixture of Ni and WO2 powders were obtained via hydrogen reduction of asblended oxide mixtures, which revealed the activation of the reduction process by MA. Obtained powders were sintered at 1300 °C for 1 h under H2 and Ar gas flowing conditions. X-ray diffraction patterns taken from the sintered samples revealed the presence of the Ni(W) solid solution phase for all samples, whereas the presence of elemental W phase was observed in the sintered as-blended and reduced powders. The lowest relative density value of 92.04% and microhardness value of 1.27 GPa were measured for the sintered as-blended and reduced powders, which increased to between 97.62% and 98.72% and 2.19 GPa and 2.23 GPa, respectively, with the applied MA.

        Superoxide Anion Production by the Spermatozoa of Men with Varicocele: Relationship with Varicocele Grade and Semen Parameters

        Purpose: To investigate the pathophysiological role of superoxide anion and total reactive oxygen species (ROS) production by the spermatozoa of men with varicocele and its relationship with varicocele grade and semen parameters. Materials and Methods: This prospective study included 34 men with grade II–III varicocele, regardless of their fertility status. The control group consisted of 13 healthy men. Semen characteristics were examined according to the 2010 World Health Organization criteria. The swim-up method was used for sperm preparation. Total ROS and superoxide anion production was assayed by luminol- and lucigenin-dependent chemiluminescence (CL), respectively. Results: The men with varicocele had significantly higher total ROS and superoxide anion levels than the healthy control subjects (2.9±0.4 relative light unit (RLU) vs. 2.4±0.1 RLU, p=0.001 for luminol-dependent CL and 2.8±0.4 RLU vs. 2.3±0.2 RLU, p=0.002 for lucigenin-dependent CL). Cases of grade III varicocele had significantly higher superoxide anion and total ROS levels than grade II cases and control subjects (p<0.001). Superoxide anion and total ROS levels were negatively correlated with all semen parameters. Conclusions: The superoxide anion levels produced by spermatozoa were significantly higher in varicocele patients than in control subjects. ROS production was related to increased varicocele grade, impaired semen concentration, and abnormal morphology in men with varicocele. Our findings suggest that superoxide anion overproduction may be an important step in the cascade of ROS-related damage to spermatozoa, resulting in impaired semen parameters in patients with varicocele.

        Synthesis of W-3 wt% Mn-2 wt% VC Composites by High Energy Milling and Sintering

        A W-3 wt% Mn matrix alloy reinforced with 2 wt% of 24 h pre-milled VC particles were synthesized via high energy milling for 3 h, 6 h, 12 h, 24 h and then sintered at 1300 °C. The W-3 wt% Mn alloy was also prepared by high energy milling of a powder blend of W-3 wt% Mn for 6 h. Effects of milling duration as well as Mn and VC addition on the microstructural and mechanical properties of the W-3 wt% Mn-2 wt%VC composite powders and sintered samples were investigated. Microstructural characterizations of the composite powders and sintered samples were carried out via SEM and XRD analyses. Density measurements and hardness measurements of the sintered samples were also carried out. While the relative density increased from 92.4% for the sintered W-3 wt% Mn matrix to 97.8% for the sintered W-3 wt% Mn-2wt% VC composite milled for 24 h, there was an increase of ~85% in hardness values for the W-3 wt%Mn-2 wt% VC samples milled for 24 h.

        Characterization Investigations of 2 wt% LaB6 Particulate-Reinforced Al-7 wt% Si Matrix Composites Fabricated by Powder Metallurgy Methods

        In this study, first of all, sub-micron LaB6reinforcing particles were synthesized in the size range of 100–500 nm viamechanochemical synthesis and leaching steps. Afterwards, Al-7 wt% Si matrix alloy and 2 wt% laboratory synthesizedLaB6powders were mechanically alloyed (MA’d) at different durations to constitute Al-7 wt% Si/2 wt% LaB6. Followedby compaction under a pressure of 450 MPa in a uniaxial cold press, samples were sintered at 570 °C for 2 h in flowing Argas atmosphere. Microstructural characterizations of the powders and sintered samples were performed using X-Ray Diffractometry(XRD) and Scanning Electron Microscopy (SEM) techniques. Effects of both MA (0, 1, 4 and 8 h) and LaB6reinforcement (0 and 2 wt%) on the mechanical properties of the composites were determined in terms of microhardness anddry sliding tests. Both MA process and LaB6reinforcement were found to make positive contribution to the hardness andwear resistance values. As-blended and sintered Al-7 wt% Si matrix alloy had a hardness value of 0.43 GPa. On the otherhand, 8 h MA’d and sintered Al-7 wt% Si/2 wt% LaB6composite had the highest hardness (0.97 GPa) and the 4 h MA’d onehad the lowest wear rate (3.25 × 10−6 mm3/N mm) among all the sintered samples. The hardness and wear resistance valueswere increased by approximately 2 times in the 4 h MA’d and sintered Al-7 wt% Si-2 wt% LaB6composite compared to its1 h MA’d counterpart.

        Axon Count and Sympathetic Skin Responses in Lumbosacral Radiculopathy

        Background and Purpose Electrodiagnostic studies can be used to confirm the diagnosis oflumbosacral radiculopathies, but more sensitive diagnostic methods are often needed to measure the ensuing motor neuronal loss and sympathetic failure. Methods Twenty-six patients with lumbar radiculopathy and 30 controls were investigatedusing nerve conduction studies, motor unit number estimation (MUNE), testing of the sympathetic skin response (SSR), quantitative electromyography (QEMG), and magnetic resonancemyelography (MRM). Results Using QEMG as the gold standard, the sensitivity and specificity of MUNE for the abductor hallucis longus muscle were 71.4% and 70%, respectively. While they were 75% and 68.8%, respectively, when used MRM as gold standard. The sensitivity and specificity ofMUNE for the extensor digitorum brevis muscle were 100% and 84.1%, respectively, when theperoneal motor amplitude as the gold standard. The SSR latency was slightly longer in the patients than in the controls. Conclusions MUNE is a simple and sensitive test for evaluating autonomic function and for diagnosing lumbosacral radiculopathy in patients. MUNE could be used routinely as a guide for the rehabilitation of patients with radiculopathies. SSR measurements may reveal subtle sympathetic abnormalities in patients with lumbosacral radiculopathy.

        Medium and Long-Term Data from a Series of 96 Endoscopic Transsphenoidal Surgeries for Cushing Disease

        Objective : Postoperative data on Cushing’s disease (CD) are equivocal in the literature. These discrepancies may be attributed to different series with different criteria for remission and variable follow-up durations. Additional data from experienced centers may address these discrepancies. In this study, we present the results obtained from 96 endoscopic transsphenoidal surgeries (ETSSs) for CD conducted in a well-experienced center. Methods : Pre- and postoperative data of 96 ETSS in 87 patients with CD were included. All cases were handled by the same neurosurgical team between 2014 and 2022. We obtained data on remission status 3−6 months postoperatively (medium-term) and during the latest follow-up (long-term). Additionally, magnetic resonance imaging (MRI) and pathology results were obtained for each case. Results : The mean follow-up duration was 39.5±3.2 months. Medium and long-term remission rates were 77% and 82%, respectively. When only first-time operations were considered, the medium- and long-term remission rates were 78% and 82%, respectively. The recurrence rate in this series was 2.5%. Patients who showed remission between 3−6 months had higher longterm remission rates than did those without initial remission. Tumors >2 cm and extended tumor invasion of the cavernous sinus (Knosp 4) were associated with lower postoperative remission rates. Conclusion : Adenoma size and the presence/absence of cavernous sinus invasion on preopera-tive MRI may predict long-term postoperative remission. A tumor size of 2 cm may be a supporting criterion for predicting remission in Knosp 4 tumors. Further studies with larger patient populations are necessary to support this finding.
