        The objective of this study was to investigate the managing realities of EVD and provide a descriptive survey with the attempt to obtain basic data needed for the standardization of EVD management. For that purpose, 220 questionnaire copies were distributed to those nurses who were working with intensive care units at university hospitals, general hospitals, semi-general hospitals in Seoul, Kyongki-de and local areas. Of 158 returned questionnaire copies, 139 were used for analysis. Data were collected during the term of academic seminars of Neurosurgery Nursing Committee and KICA (Korea Intensive Care Association), ranging from Oct. 26 through 27. 2000. The tools utilized a structured questionnaire form consisting of 21 questions on general characteristics, the level of understanding, the level of work performance, standard management guidelines, etc. The data collected were analysed with frequency, percentage, mean vague, chi -square test using the SPSS 10.0 program Findings were revealed, as follows: For managing the prevention of EVD infection, aseptic technique was found to be of the most importance (48.9%),'the technique should be carried out at an operation room'(92.8%), but it was actually performed at intensive care units (over 51.5%). 'The term of antibiotics use should be less than 7 days' (81.8%), while it was actually executed for less than 7 days (49.5%). 'The proper time of quittance from it should be within less than 7 days'(74.1%) but actual quittance was executed only by 51.1%. 'Set exchange is needed' by 54.0% but actual exchange was implemented merely by 28.1%. 'The subject of management should be nurses' was reflected by 40% but actually 52.0% were carried out. As a result of hating analysed the understanding of the management of EVD by general characteristics, a significant difference was found in the necessity for set exchange by hospitals, in the time of quittance of a catheter, and in ablution items by work experience. The result of hating analysed the work performance levels of EVD management by general characteristics revealed that a significant difference in administration facilities of medicine by the classification of hospitals, especially the use of antibiotics and the quitting time of a catheter by the classification of intensive care units, and in the use of antibiotics and ablution items by the standard management guidelines.

        Purpose: This study was to identify the meaning of the lived experiences, to describe of the meaning structures and to develop the strategies of nursing intervention centering to these meanings of the smoking cessation. Method: This study was derived from a phenomenological analytic method suggested by Giorgi. The participants in this study were five adults who had the previous experience of smoking cessation. The data were collected from September of 2001 to April of 2002 through systemic interviews and participatory observations. Average of five interviews were performed, and each interview lasted an hour and half. Result: The meaning of smoking cessation was categorized with nine components. That is (1) obstinacy of the habit of smoking : difficulty of endurance, succumb to temptation of smoking, repetition of smoking and smoking cessation, habit-forming. (2) Bring about a symptom of improving : took place headache, expectoration of sputum, sense of instability. (3) Waver in worthy ; doubts about smoking cessation, ridiculed smoking cessation. (4) Be narrowed social life ; become estranged from friends. (5) Futility ; unnecessary, harm. (6) Self-repression ; occurred indomitable mind, strong will, endurance. (7) Gratification ; self-admiration, receive praise from family. (8) Delightfulness : clean in body and clothes, be disgusted with the foul order of smoking. (9) Improvement in welfare ; a clear mind and good memory, improve in health, have a good appetite. Conclusion: The nursing intervention must be focused on these concerns to accomplish successful smoking cessation program.

      • N-Substituted anthranilate 유도체의 합성

        In order to search of new drug effect, ten N-Substituted anthranilate derivatives were synthesized. Methyl anthranilate was synthesized by the reaction of methanol in the presence of conc. sulfuric acid and 5-hydroxyethylanthranilate was synthesized by the reac-tion of ethanol saturated with hydrogen chloride gas. According to Schotten-Baumann method, these acid amides could be synthesized by the reaction of methylanthranilate or 5-hydroxyethylanthranilate with acid chloride (CICO-?? : x=H, p-chloro, o-chloro, p-nitro, m-nitro)in the presence of 10% sodium hydroxide solution. The structure of these compounds was identified by IR and NMR Spectra.

        노인층과 청ㆍ장년층 뇌졸중 환자가족의 건강상태ㆍ부담감 및 삶의 질과의 관계

        The purpose of this study is to investigate the correlation among the stroke patient family's upon the oomparative appreciation of the adult stroke patient's family and elderly stroke patient's family For this purpose, data were collected from the family care-givers for two groups of stroke Patients under sixty years old and over sixty years of age, admitted at K Hospital and H Hospital in Seoul. The instruments for this research are based on the tool for measuring physital health and Young-hee(1992), the tool fort measuring the sense of burden by S대, Mee-hae and Oh, Ga-sil(1993), and the tool for the quality of life by Noh, Yoo-ja(1988). The sampling for this study was done from December, 2000 until February, 2001. Questionnaire data were drawn up by personal interviews added by the staff nurses. The analysis of collected data are based on general characteristics calculated at the rate of 100 percent of the average, t-test, ANOVA(some difference on a level with p<.05 being subsquently confirmed by DMR) for Health Status, Burden, Quality of Life and Pearson Corretation to verify the hypothetical correlation among the subjects. The results of this study are as follows: 1. In the adult stroke patient family, the factors influencing the physical health proved to be age, present occupation and family-formation. Here, the factors influencing psychological health turned out to be age, matrimonial status, present occupation and family-formation. In the elderly stroke patient family, the factors influencing physical health proved to be age, gender, final academic status, matrimonial status, present occupation, and relation with the patient. Here, the factors influencing the psychological health were age, final academic status, matrimonial status, present occupation, relation with the patient and family-formation. In the former case, the influencing factors upon the burden were matrimonial status, relation with the patient and family-formation. In the latter case, the influences upon the burden were age, gender, final academic status, matrimonial status, present occupation and relation with the patient. In the former case, the influences on the quality of life were gender, and economic situation. In the later case, the influencing factors on the quality of life were age, final academic status, matrimonial status, present occupation, and relation with the patient. 2. The rate of the physical condition in the former case turned out to be 2.83, and the psychological condition 2.37. The physical condition of the latter case was 2.76, and the psychological condition 2.46. The rate of the burden in the former case was 3.14, and that of the latter case was 3.04. The rate of quality of life in the former case proved to be 2.46, and that of the latter case 2.55. 4. The rate of correlation between the burden and the Quality of life appealed to be the of correlation between the psychological health and the burden of a similar (r= -.565). The rate of correlation between the physical health and the psychological health proved to be a moderate correlation (r= .372). The rate of correlation between physical health and the burden turned out to be a low counter-correlation (r= -.276). According to this study, there proved to be a very close correlation the stroke Patient family's health, the burden and quality of life. Thus, it would be necessary to find out various nursing interventions in order to mitigate the stroke patient family's burden in the process of caring for the patients.

      • Protein Disulfide Isomerase Inhibition Synergistically Enhances the Efficacy of Sorafenib for Hepatocellular Carcinoma

        Aims: Sorafenib is the only approved targeted drug for hepatocellular carcinoma (HCC), but its effect on patients’ survival gain is limited and varies over a wide range depending on patho-genetic conditions. Thus, enhancing the efficacy of sorafenib and finding a reliable predictive biomarker are crucial to achieve efficient control of HCCs. Methods: In this study, we employed a systems approach by combining transcriptome analysis of the mRNA changes in HCC cell lines in response to sorafenib with network analysis to investigate the action and resistance mechanism of sorafenib. Results: Gene list functional enrichment analysis and gene set enrichment analysis (GSEA) revealed that proteotoxic stress and apoptosis modules are activated in the presence of sorafenib. Further analysis of the endoplasmic reticulum (ER) stress network model combined with in vitro experiments showed that introducing an additional stress by treating the orally active protein disulfide isomerase (PDI) inhibitor (PACMA 31) can synergistically increase the efficacy of sorafenib in vitro and in vivo, which was confirmed using a mouse xenograft model. We also found that HCC patients with high PDI expression show resistance to sorafenib and poor clinical outcomes, compared to the low PDI expression group. Conclusions: These results suggest that PDI is a promising therapeutic target for enhancing the efficacy of sorafenib and can also be a biomarker for predicting sorafenib responsiveness.

      • EDN1 Expression as a Novel Biomarker for Predicting Sorafenib Responsiveness in Patients with Hepatocellular Carcinoma

        Aims: Predictive biomarkers for sorafenib in hepatocellular carcinoma (HCC) are still not available, despite the modest benefit of sorafenib. We present a novel and sensitive biomarker that can predict the responsiveness to sorafenib. Methods: A candidate biomarker was found through differential mRNA expression analysis between sorafenib-sensitive and resistant cell lines from The Cancer Cell Line Encyclopedia (CCLE), a public data base. It was tested with in vitro shRNA experiments for its effect to sorafenib-resistance. Consecutive patients with HCC who had undergone sorafenib treatment were included from a prospective cohort and the test set (n=48) and the validation set (n=46) were established. Specimens were archived before sorafenib treatment and a candidate marker was evaluated by the quantitative digital analysis algorithm for immunohistochemistry with regard to their association with response assessment by mRECIST, time to progression (TTP) and overall survival (OS). Further, we performed additional validation study using serum samples before sorafenib treatment obtained from different set of HCC patients (n=40). Results: Endothelin-1 (EDN1, ET-1) was the only differentially expressed molecule between sorafenib-sensitive and resistant cell lines. Knock-down of EDN1 in HCC cell line increased sorafenib sensitivity. In test set, low EDN1 expression group showed significantly better response to sorafenib (non-progressive disease) compared to high EDN1 expression group (72.7% versus 2.7%, respectively; P<0.001). With the same criteria for the validation set, EDN1 expression maintained predictability for sorafenib responsiveness. Survival analysis showed that high EDN1 expression was an independent prognostic factor for poor OS (hazard ratio [HR〕, 2.374; 95% confidence interval [CI〕, 1.051-5.360; P=0.037) and short TTP (HR, 1.907; 95% CI, 1.085-3.350; P=0.025) after sorafenib treatment. We confirmed our results in serum samples of another set. Conclusions: EDN1 expression can distinguish responders to sorafenib and can be a useful predictive biomarker for sorafenib in HCC.

        Purification and Characterization of Hemolymph Origin Silk Gland Proteins of Wax moth, Galleria mellonella

        Hemolymph origin silk gland protein (HOSGP) was identified from the silk gland of wax moth, Galleria mellonella and characterized using antibodies against larval hemolymph protein and silk gland. Hemolymph and silk gland extracts were electrophoresed and western-blotted with each developmental stage. HOSGPs were present in the silk gland. To confirm the presence of HOSGP in other tissues, western-blotting was performed with extracts of Malpighian tubule, midgut and fat body. HOSGP was also present in the fat body. HOSGP was pre-sent in large amounts in the silk gland of the last instar larvae and wandering stages when silk is actively produced. HOSGP was partially purified by KBr density gradient ultracentrifugation and prep-cell, and characterized.
