급성백혈병의 화학요법에서 조혈간세포 보충요법

1. 목적 급성백혈병의 화학요법에 있어서 관해유도 요법시에 보다 더 강력한 항암화학요법을 시행하고 골수억제의 회복을 위하여 말초혈액에서 채취한 조혈모세포의 투여로 그 효과를 증대시키고자 시도하였다. 2. 방법 관해유도 화학요법후에 골수억제시기로부터 회복단계로서 백혈구감소증이 호전도면서 단핵세포가 증가될 때에 말초혈액에서 조혈간세포(stem cell)로 인정되고 있는 CD34+세포의 함유량이 1% 이상되는 시기를 찾아내고 그 때부터 평균 4차례의 단핵구를 분리채집을 하여 -74。C에 냉동보관하였다가 제2차 화학요법후에 해동시켜서 정맥주입하였다. 3. 결과 대상 급성백혈병환자는 6명에 23회의 조혈간세포를 함유한 단핵구 채집을 시행하였고 유효한 단핵구의 채집은 4례에서 가능하였고 2례에서는 유효한 채집이 못되었다. 채집효율은 39.0-74.5%로서 평균 49.8%였다. 채집 후에 혈액학적 변화는 혈색소의 약간 감소 (평균 1.34g/dl)와 혈소판의 감소 (평균20.25%)를 나타내었다. 해동 단핵구를 투여할시에 일과성인 불쾌감, 기침, 흉부압박감, 호흡곤란 등이 있었다. 4. 결론 자가골수이식에서 자기말초혈액조혈간세포이식으로 관심이 이행되는 현시점에 자가조혈간세포를 채집하여 수혈하는 기술이 확립되는 기초를 이루었다고 본다. 이번 대상들은 재발된 금성백혈병들에 시행하였으나, 향후에는 초발시에 제일차 관해유도 화학요법 과정에 시도해야할 것으로 생각한다. Object: Peripheral blood stem cells(PBSC) were collected after remission induction chemotherapy and reinfused after intensified chemotherapy in order to increase the chemotherapeutic efficacy. Method: Collection of mononuclear cells (MNC) was started when CD34+ cells above 1% and WBC above 1,000/ul with mononuclear cell percentage above 30%, the collection procedures were done 2-6 time in each patient of total 6 cases. The collected MNCs were stored in - 750C and reinfused after thawing in the water bath at 370C Results: Twenty three collection procedures from the 6 cases of acute leukemia had made effective MNC collection in 4 cases and mean collection efficiency was 49.8%(from 39.0% to 74.5%). Hematological changes after the procedure were mean reduction of hemoglobin 1.34g/dl and mean 20.25% of decreased platelet count. There were noticed transient symptoms of discomfort, coughing, chest tightness and dyspnea in association with the infusion of stored cells. Conclusion: The basic process for the peripheral blood stem cell transplantation (PBSCT) was accomplished with this supportive care for the consolidation chemotherapy of acute leukemia. Effective PBSCT would be warrented with more qualified process and high does chemotherapy. (Korean J Blood Transfus 6(2) : 41~47,1995)

A 6 years old Schunauzer dog (7.2kg, intact female) with history of hematuria for two days was referred to the Veterinary Medical Teaching Hospital of Chungnam National University. Complete blood cell count showed no significant abnormalities and serum chemistry profile showed increased phosphate profile. On urinanalysis, there was evidence of bacterial infection. On ultrasonographic evaluation, irregular renal contour and increased echogenecity, pelvic and uretral dilation, and thickening of urinary bladder wall were observed. Based on these finding, the dog was diagnosed as pyelonephritis. The dog was treated with antibiotics based on antimicrobial susceptibility test results, then successfully resolved after 8 weeks of treatment.

횡 요천골부 후피부판 (Transverse Lumbosacral Back Flap)을 이용한 천골부 욕창 치험례

Pressure sore is an area of ischemic tissue loss resulting from prolonged or often repeated pressure, usually over a bony prominence of body. Recently, the prevalence of pressure sore is major clinical problem, due to the increased number of patients with spinal cord injuries or longterm comatous patient after traffic, industrial accidents. The surgical principles in the treatment of pressure sores are total excision of the ulcer, scartissue, bursae, complete removal of all infected bone with recontouring of bony prominences to alleviate discrete pressure points and resurfacing and padding of the defects with healthy and well vascularized tissues. The sacral sore is the most common site for the development of pressure sore in Korea. We had experienced 14 cases who had received surgical treatments for sacral sores with transverse lumbosacral back flap. We obtained the following conclusions. Advantages 1)It is quick and easy procedure 2)The flap elevation itself is relatively bloodless 3)The donor defect is in a non-weight-bearing area 4)Muscle is not sacrified(possibly of importance in the ambulatory patient) 5)Surgical options are preserved in cases of recurrence(the flap itself may be reelevated and inset in some cases of recurrence) Disadvantages 1)It is lacking the bulky padding afforded by alternative musculocutaneous flaps 2)The vascularity of the flap tip is empirically less dependable than that of a musculocutaneous flap 3)The donor defect is large, cannot usually be closed primarily, and may prove of cosmetic concern

Image processing of real-time X-ray images of randomly-oriented and touching pistachio nuts for product inspection is considered. The image processing used to isolate individual nuts and an watershed transform algorithm are described. To improve the classification ratio compared with the previous research, some new features are introduced. Thus the classification ratio is much improved asa expected. The SAS classifier is used to classify good and bad nuts.

The aim of this study is to search an effective method for implementing the 7th mathematics curriculum. From this year the 7th mathematics curriculum is being implenented responding to the national demand. It aims to enlarge 'mathematical power'. Also it focuses on a stagewise differentiated curriculum and balancing of mathematics teaming contents. But, reviewing the 7th mathematics curriculum and the mathematics textbook, it does not show the effective implementation method for the stagewise-differentiated curriculum. Thus in this thesis, it aims to develop the supplemental and enriched learning materials for implementing the stagewise-differentiated curriculum effectively. Achieving the purpose, this thesis reviews the 7th mathematics curriculum and suggest constructivism and realistic mathematics education as fundamental theories for developing the supplemental and enriched learning materials. And it shows a process of developing the supplemental and enriched mathematics teaming materials for the 2nd graders in primary school. Therefore it suggests a mathematics learning model for differentiated individual learning based on problem based learning and conflict teaching-learning and shows performed experimental lessons. According to the result, it concludes that attitude of students toward the lesson was voluntary, that presentations of them were active and that they accepted the lesson interesting. But the lack of a proper learing time is problematic. Nonetheless teachers could perform differentiated mathematics lesson using the supplemental and enriched learning materials, with their will and effort.

In order to obtain the relationship between Lactobacillus colony count tests and Snyder tests with D.M.F.S. score. 51 dental students in K.P.N.U. were tested and examined. Thereafter the data from these tests were analysed and evaluated comparatively. The obtained results were as follows. There was a significant correlation coefficiency between Lactobacillus counts and Snyder tests. (p < 0.05) The Snyder tests were somewhat superior to Lactobacillus counts. There was no statistical difference among volumes of saliva samples used in the Snyder test. (p > 0.05) The Snyder test was positive in 52.9 % and Lactobacillus count was positive in 39.2% of total tested persons.

트래드밀 운동프로그램 적용이 성인비만자의 심폐기능과 혈청지질에 미치는 영향

The purpose of this study was to investigate the effect of treadmill exercise programs on cardiopulmonary functions and serum lipids in adult obesity men. The subjects consisted of 33 obese adults between 30 and 70 years old and all of them had no other complications. Subjects participated in treadmill exercise programs for 12 weeks according to the exercise prescriptions. They started to exercise for 20 minutes per day, six times a week at 50% of maximum O_2 consumption (V˙O_2max), and later exercised for 50 minutes at 80% of V˙O_2max, which was the maximum exercise intensity of the program (Exercise intensity has been increased gradually). The changes of body composition, serum lipids, and cardiopulmonary functions in the subjects before and after the treadmill exercise program have been measured. The results are as follows. 1. Body weight, percent body fat, and fat body weight were reduced(p<0.05, p<0.01), and body fluid were increased after exercise programs(p<0.01). The group below 50 years old and above 25% of body fat showed more decrease in body weight and percent body fat, and more increase than that above 50 years old below 25% of body fat. 2. Resting heart rate, systolic and diastolic blood pressure were significantly reduced after exercise programs(p<0.05, p<0.01), but these parameters decreased approximately same for the groups of ages and percent body fat. 3. Vital capacity, forced expiratory volume for 1 second percent and maximal voluntary ventilation were increased after exercise programs. The group below 50 years old and above 25% of body fat showed more increase than that above 50 years old and below 25% of body fat. 4. Total cholesterol, LDL cholesterol, total cholesterol/HDL cholesterol rate and triglycerides were reduced (p<.05, p<0.01), and HDL cholesterol levles were increased after exercise programs. The group below 50 years old and above 25% of body fat showed more decrease in total cholesterol, LDL cholesterol, total cholesterol/HDL cholesterol rate and triglycerides, and more increase than that above 50 years old and below 25% of body fat. 5. Heart rate increase during exercise were reduced after exercise programs. The group below 50 years old and above 25% of body fat showed more decrease than that above 50 years old and below 25% of body fat. 6. Blood pressure increase during exercise were reduced after exercise programs. The group above 25% of body fat showed more decrease than that below 25% of body fat. 7. Oxygen uptake of during exercise were increased after exercise programs. 8. Maximal oxygen uptake were significantly increased after exercise programs (p<0.01). 9. The regression equations between heart rate, systolic blood pressure, and oxygen uptake were obtained. These correlation coefficients of the post-programs showed higher that those of pre-programs. In this study, the treadmill exercise programs suitable for the obese was developed and applying this programs weight, percent of body fat, body fat weight, and serum lipids levels ere reduced. Thus cardiopulmonary functions was improved. The group below 50 years old and above 25% of body fat showed more decrease than that above 50 years old and below 25% of body fat. It is to be noted that both systolic blood pressure and oxygen uptake can be estimated by heart rate only using the regression equations.

A link level handoff scheme is proposed in this paper. The scheme reduce traffic losses during handoff. in wireless LAN based Home networking. The scheme utilizes buffer and image queue in the device derive, and it is able to recover traffic that would otherwise be lost during handoff. Simulation results show that the proposed scheme can eliminate or significantly reduce the packet losses in UDP traffic, and help preserve througput for TCP traffic when the re-transmission time out is large. To achieve maximum benefit, we propose to apply the forwarding scheme adaptively depending on the traffic type.
