Background: Shared decision making using patient decision aids (PtDAs) was established over a decade ago, but few studies have evaluated its efficacy in Asian countries. We therefore evaluated the application of PtDAs in a decision conflict between two muscle relaxant reversal agents, neostigmine and sugammadex, and sequentially analyzed the regional differences and operating room turnover rates.Methods: This multicenter, outcome-assessor-blind, randomized controlled trial included 3,132 surgical patients from two medical centers admitted between March 2020 and August 2020. The patients were randomly divided into the classical and PtDA groups for pre-anesthesia consultations. Their clinicodemographic characteristics were analyzed to identify variables influencing the choice of reversal agent. On the day of the pre-anesthesia consultation, the patients completed the four SURE scale (sure of myself, understand information, risk-benefit ratio, encouragement) screening items. The operating turnover rates were also evaluated using anesthesia records. Results: Compared with the classical group, the PtDA group felt more confident about receiving sufficient medical information (P < 0.001), felt better informed about the advantages and disadvantages of the medications (P < 0.001), exhibited a superior understanding of the benefits and risks of their options (P < 0.001), and felt surer about their choice (P < 0.001). Moreover, the PtDA group had a significantly greater tendency to choose sugammadex over neostigmine (P < 0.001). Conclusions: PtDA interventions in pre-anesthesia consultations provided surgical patients with clear knowledge and better support. PtDAs should be made available in other medical fields to enhance shared clinical decision-making.
A Novel Single-Phase Soft-Switching Unipolar PWM Inverter

A novel single-phase soft-switching unipolar PWM inverter (SSUPI) using a simple auxiliary resonant unit is proposed. All main switches of high-frequency arm operate at zero-voltage-switching (ZVS) turn-on, while the auxiliary switches operate at zero-current-switching (ZCS) turn-off. All main switches of low-frequency arm operate at 60 ㎐ to greatly reduce switching losses. As a result, commutations of the main power devices occur with low losses. Consequently, higher efficiency is achieved. Moreover, a unipolar PWM scheme is used to reduce the output voltage harmonics. Some experimental results of the proposed SSUPI, rated 500 W and operated at 40 ㎑, are presented for demonstration.
A Novel Three-Phase Voltage-Doubler Soft-Switching Active Filter

A novel three-phase voltage-doubler soft-switching active filter (VDSSAF) comprising four main switches and four auxiliary switches is proposed in this paper. All main switches in the proposed VBSSAF operate at zero-voltage-switching (ZVS) turn on, while the auxiliary switches operate at zero-current-switching (ZCS) turn off. The reduction in the number of switches can also increase the reliability, since the control circuits are thereby reduced to two sets. The proposed VBSSAF can compensate the current harmonics, improve the power factor and balance the unbalanced nonlinear load. Moreover, a critical value of the capacitor voltage magnitude on the DC side and a critical value of the reactor inductance on the AC side for successful harmonic current tracking are also derived for design applications. Some experimental results of the VBSSAF, rated 400 W and operated at 40 ㎑, are provided to demonstrate the performance.

Endothelial dysfunction is an early indicator of cardiovascular diseases. Increased stimulation of tumor necrosis factor-a (TNF-a) triggers the inflammatory mediator secretion of endothelial cells, leading to atherosclerotic risk. In this study, we investigated whether sulforaphane (SFN) affected the expression of intracellular adhesion molecule-1 (ICAM-1) in TNF-a-induced ECV 304 endothelial cells. Our data showed that SFN attenuated TNF-a-induced expression of ICAM-1 in ECV 304 cells. Pretreatment of ECV 304 cells with SFN inhibited dose-dependently the secretion of proinflammatory cytokines, such as interleukin (IL)-1b, IL-6, and IL-8. SFN inhibited TNF-a-induced nuclear factor-jB (NF-jB) DNA binding activity. Furthermore, SFN decreased TNF-a-mediated phosphorylation of IjB kinase (IKK) and IjBa, Rho A, ROCK, ERK1/2, and plasminogen activator inhibitor-1 (PAI-1) levels. Collectively, SFN inhibited the NF-jB DNA binding activity and downregulated the TNF-a-mediated induction of ICAM-1 in endothelial cells by inhibiting the Rho A/ROCK/NF-jB signaling pathway, suggesting the beneficial effects of SFN on suppression of inflammation within the atherosclerotic lesion.
