        직장 카르시노이드 종양에서 Ki-67 발현의 예후적 중요성

        Background/Aims: Rectal carcinoid tumors can be resected with endoscopy, and it is important to assess their prognostic factors. We evaluated the potential of Ki-67 expression as a prognostic factor in rectal carcinoid tumors. Methods: We retrospectively reviewed the medical records of 37 patients with rectal carcinoid tumors who got endoscopic resection from January 2001 to January 2011 at Inje University Seoul Paik Hospital. We analyzed their endoscopic and histologic findings, Ki-67 expression, clinical outcome, and prognosis. Results: The mean age (±SD) of the patients was 56.3±10.7 years, and the male : female ratio was 3.6:1. The mean tumor size was 0.5±0.4 cm, 33 patients showed grade 1 tumors (89.2%) and the average Ki-67 expression was 0.7±1.2%. Thirty five patients underwent endoscopic mucosal resection, and two required endoscopic submucosal dissection. Eight patients had positive margins after resection, but no cases of lymphovascular invasion were identified. The median follow-up duration was 21.4±25.4 months, and no recurrences were observed. Conclusions: In low grade rectal carcinoid tumors which are lack of central depression on colonoscopy, the expression of a molecular marker of malignant potential, Ki-67, was low. Therefore, endoscopic resection seemed to be a safe and effective treatment for these tumors.

        The impact of pathologic differentiation (well/ poorly) and the degree of Ki-67 index in patients with metastatic WHO grade 3 GEP-NECs.

        <P> e15686 </P><P> Background: Herein, we investigated the impact of pathologic differentiation (well or poorly differentiated) in metastatic grade 3 GEP-NEC patients receiving etoposide and platinum-based therapy. Simultaneously, we evaluated a more exact Ki67 index cut-off point to select patients with grade 3 GEP-NEC who might benefit from etoposide plus platinum (EP)-based therapy. Methods: Among patients pathologically diagnosed with metastatic grade 3 GEP-NECs at Samsung Medical Center between June 2013 and March 2016, 31 GEP-NEC patients receiving etoposide and platinum-based therapy were included in this study. Results: Primary sites included 13 foregut-derived GEP-NECs [stomach (n = 4), duodenum (n = 4), and pancreas (n = 5)] and 2 hindgut-derived GEP-NECs of the rectum. Sixteen unclassified GEP-NECs originated from 7 gall-bladder (GB), 6 liver and 3 unknown primary sites. According to pathologic differentiation, 14 patients had well differentiated and 17 had poorly differentiated grade 3 GEP-NECs. Between well differentiated and poorly differentiated grade 3 GEP-NECs, there was a significant difference in the distribution of Ki67 index. There was no significant difference of treatment efficacy between well and poorly differentiated grade 3 GEP-NECs (RR; 35.7% vs. 41.2%, p = 0.525). Tumor response to EP occurred in 5 of 7 patients with Ki67 > 60% and 7 of 24 with Ki67≤60%, which was significantly different (RR; 71.4% vs. 29.2%, P = 0.043). There was no significant difference in PFS according to pathologic differentiation (well differentiated vs. poorly differentiated) and Ki67 index ( > 60% vs ≤60%). Conclusions: Grade 3 GEP-NECs could be morphologically classified into well and poorly differentiated NETs. Additionally, among grade 3 GEP-NECs, there was a significant difference in ranges of Ki67 index between well and poorly differentiated NECs. Higher levels ( > 60%) of Ki67 index might be a predictive marker for efficacy of EP as a standard regimen in grade 3 GEP-NECs. </P>

        Background: Rheumatoid arthritis (RA) is a systemic infi ammatory disease and mainly affects in the synovial joints. However, little information is available regarding the etiological mechanisms on the RA and natural compounds reducing RA symptom. Here, we investigated whether kaempferol, a natural compound abundantly found in editable plants, could target kinase domain of fi broblast growth factor (FGF) receptor (FGF-R), that is a well-known receptor tyrosine kinase implicated in RA pathogenesis. Methods: RA and osteoarthritis (OA) synovium tissues were used for immunohistofi uorescence assay and immunohistochemical staining. Mouse embryonic fi broblasts were used to investigate FGF-mediated cell migration study. Autoimmune arthritiswas induced by type II collagen (CII) immunization in DBA/1J mice. Kaempferol (2mg/ kg) was administered orally three times per week. The frequencies of IL-17-producing CD4+ T (Th17) cells and CD4+ CD25+ Foxp3+ regulatory T (Treg) cells were analyzed by fi ow cytometry. Results: Phosphorylation of the FGFR3 and RSK2 was highly detected in RA tissues compared with OA. KI-67, a cell proliferation marker, was higher in RA than that of OA. Cell migration induced by FGF was mediated through RSK2, and kaempferol inhibited FGF-induced cell migration by suppression of RSK2-mediated activities of the AP-1 and NF-κB and gene expression of COX-2. We found that active RSK2 positive cells were extensively co-stained with CD68 in RA synovium. Kaempferol inhibited FGF-induced synovial cell migration through RSK2-mediated signaling pathway. Kaempferol treatment suppressed arthritis severity in mice with autoimmune arthritis through suppression of Th17 and reciprocal induction of Treg cells in vivo. Infi ammation and joint destruction were signifi cantly suppressed in the joints of kaempferol- treated mice. Conclusions: Dual targeting of kaempferol on FGFR and RSK2 could be a novel therapeutic strategy in RA.

        BMB Reports : Connexin32 inhibits gastric carcinogenesis through cell cycle arrest and altered expression of p21^Cip1 and p27^Kip1

        Gap junctions and their structural proteins, connexins (Cxs), have been implicated in carcinogenesis. To explore the involvement of Cx32 in gastric carcinogenesis, immunochemical analysis of Cx32 and proliferation marker Ki67 using tissue-microarrayed human gastric cancer and normal tissues was performed. In addition, after Cx32 overexpression in the human gastric cancer cell line AGS, cell proliferation, cell cycle analyses, and p21^Cip1 and p27^Kip1 expression levels were examined by bromodeoxyuridine assay, flow cytometry, real-time RT-PCR, and western blotting. Immunohistochemical study noted a strong inverse correlation between Cx32 and Ki67 expression pattern as well as their location. In vitro, overexpression of Cx32 in AGS cells inhibited cell proliferation significantly. G1 arrest, up-regulation of cell cycle-regulatory proteins p21^Cip1 and p27^Kip1 was also found at both mRNA and protein levels. Taken together, Cx32 plays some roles in gastric cancer development by inhibiting gastric cancer cell proliferation through cell cycle arrest and cell cycle regulatory proteins.         전치부 반대교합아동의 발음장애에 관한 음향적 연구

        It has widely known for speech problem in patients with malocclusion,but there have been insufficient studies on degrees of serverity.Patients with openbite and CIⅢ malocclusion usually have speech problem.It has widly known that fricative /s/ is pronunciated most abnomally in CIⅢ malocclusion than any other consonant.Therefore 20 childen with anterior crossbite selected for control group.7 sounds such as /사(sa),서(sδ),소(so),수(su),스(sω),시(si),세(se)/ were recorded,and the formants and formants ratio were measured by using Sensimetric Speech Station,which is speech analyzing program in IBM pc. The results were as follows: 1.In anterior crossbite group,F1 of all 7 sounds were significantly higher than normal anterior relationship group(P<0.05). 2.There were no significant difference in F2 between two groups except/소(so),수(su)/(P>0.05). 3.In anterior crossbite group,F2/F1 ratio of all 7 sounds were significantly smaller than normal anterior relationship group(P<0.05). 4.There were no significant difference in accordance with tongue position(P>0.05).

        Gintonin facilitates catecholamine secretion from the perfused adrenal medulla

        The present study was designed to investigate the characteristics of gintonin, one of components isolated from Korean Ginseng on secretion of catecholamines (CA) from the isolated perfused model of rat adrenal gland and to clarify its mechanism of action. Gintonin (1 to $30{\mu}g/ml$), perfused into an adrenal vein, markedly increased the CA secretion from the perfused rat adrenal medulla in a dose-dependent fashion. The gintonin-evoked CA secretion was greatly inhibited in the presence of chlorisondamine ($1{\mu}M$, an autonomic ganglionic bloker), pirenzepine ($2{\mu}M$, a muscarinic $M_1$ receptor antagonist), Ki14625 ($10{\mu}M$, an $LPA_{1/3}$ receptor antagonist), amiloride (1 mM, an inhibitor of $Na^+/Ca^{2+}$ exchanger), a nicardipine ($1{\mu}M$, a voltage-dependent $Ca^{2+}$ channel blocker), TMB-8 ($1{\mu}M$, an intracellular $Ca^{2+}$ antagonist), and perfusion of $Ca^{2+}$-free Krebs solution with 5mM EGTA (a $Ca^{2+}$chelater), while was not affected by sodium nitroprusside ($100{\mu}M$, a nitrosovasodialtor). Interestingly, LPA ($0.3{\sim}3{\mu}M$, an LPA receptor agonist) also dose-dependently enhanced the CA secretion from the adrenal medulla, but this facilitatory effect of LPA was greatly inhibited in the presence of Ki 14625 ($10{\mu}M$). Moreover, acetylcholine (AC)-evoked CA secretion was greatly potentiated during the perfusion of gintonin ($3{\mu}g/ml$). Taken together, these results demonstrate the first evidence that gintonin increases the CA secretion from the perfused rat adrenal medulla in a dose-dependent fashion. This facilitatory effect of gintonin seems to be associated with activation of LPA- and cholinergic-receptors, which are relevant to the cytoplasmic $Ca^{2+}$ increase by stimulation of the $Ca^{2+}$ influx as well as by the inhibition of $Ca^{2+}$ uptake into the cytoplasmic $Ca^{2+}$ stores, without the increased nitric oxide (NO). Based on these results, it is thought that gintonin, one of ginseng components, can elevate the CA secretion from adrenal medulla by regulating the $Ca^{2+}$ mobilization for exocytosis, suggesting facilitation of cardiovascular system. Also, these findings show that gintonin might be at least one of ginseng-induced hypertensive components.

        In the present study, we aimed. to consider the name of twelve main Meridian in order to understand the concept of the Meridian system and to applicate acupuncture theory in the clinical field properly and fundamentally. We referred to several literatures on the matter and we found out that the name of present twelve Meridian was not settled until the period of Yellow Emperor's Classic. In the time of Yellow Emperor's Classic, the two theory of Internal Organ and Meridian system were combined and added new theory of 'WoonGi'. The general rule of combination is that the component of name of Meridian consist of 'Hand' or 'Foot', 'three Eum and Yang' and internal organ. The internal organ coupled with the root ki or middle ki of 'three Eum and Yang' and the 'Hand' or 'Foot' depend on the location of internal organ that belongs to meridian. The name of Twelve Meridian is a kind of system formed to analyze the two concepts organically and dynamically. One is material internal organ and the other is immaterial meridian system.

From experimentally carried out "Sports events Choosing System" not as usual be inforced "Total practical Skill System", Following Positive effects and sujestions were Presented; 1. "Sports events choosing system" gave birth to more students' interesting and more voluntary participation to the sports events they chose, and sports skill and instruction level was elevated due to charge of expert according to each sports events. This system would be managed continuously hereafter for it related to life sports. 2. This System needs to closed cooperations between College and Community, for the facilities of community are utilized by college. 3. To complete the plan for activation of college Physical Education, additionally, the successional study ; namely "autonomous extracurricular sports activity" should be excuted hereafter.
