        Clostridium difficile에 의한 설사의 예후인자

        배경 : Clostridium difficile에 의한 설사(Clostridium difficile associated diarrhea; CDAD)는 임상 경과가 다양하다. 최근에 유럽과 북미에서 C. difficile 감염의 발생률이 증가하고 고전적 치료에 잘 반응하지 않으며 이환률이 증가하였고, 이러한 원인이 새로운 균주의 탄생에 기인한다고 보고되었다. CDAD의 예후에 영향을 미치는 세균성 요인과 숙주 요인을 확인하기 위해 본 연구를 진행하였다. 재료 및 방법 : 2002년 8월부터 2003년 12월까지 CDAD가 진단된 20세 이상인 환자들을 대상으로 후향적 Cohort 연구를 하였다. 세균성 요인을 확인하기 위해 cdt A, cdtB, tcd A, tcd A rep 그리고 tcd B 유전자(binary toxin)를 확인하였다. 설사가 치료 시작 후 11일 이상 지속되거나, 2달 이내에 재발하거나, 수술 혹은 다른 시술이 필요한 경우, 사망한 경우 예후가 좋지 않은 것으로 정의하였다. 결과 : 총 115예에서 toxin을 생성하는 C. difficile가 동정되었으며, Toxin A와 toxin B 모두 양성인 균이 91예, toxin B만 양성인 균이 24예였다. Toxin A 생성 여부가 예후에 영향을 미치지 않았지만, 제산제를 사용한 환자에서 그렇지 않은 환자에 비해 toxin B만 양성인 균이 많이 동정되었다(P<0.05). 예후가 좋지 않은 경우는 39예(33.9%)였고 76예(66.1%)에서 예후는 양호하였다. 단변량 분석에서 70세 이상의 고령, 남성, 증상 발현 후 사용한 항생제의 개수 사용, 증상 발현 후 carbapenem, aminoglycoside, glycopeptide 사용, 당뇨 및 뇌졸중 병력이 있는 경우, 그리고 높은 Charlson index가 불량한 예후 인자로 확인되었다. 그러나 독립적인 예후 인자를 조사했을 때에는70세 이상의 고령(odds ratio=3.378, P=0.009), 증상 발현후 carbapenem 사용(odds ratio 7.210, P<0.001)이 예후에 중요한 영향을 미치는 요소로 확인되었다. 결론 : 70세 이상의 고령과 증상 발현 후 carbapenem 사용이 CDAD 독립적인 불량한 예후인자이다. Background : Clostridium difficile associated diarrhea (CDAD) has a wide range of clinical manifestations. The prognostic factors of CDAD are not fully understood. Materialsand Methods : A retrospective cohort study of 115 patients with CDAD from Aug. 2002 to Dec. 2003 was conducted to evaluate prognostic factors of CDAD. Bacteriologic factors were determined by detecting the binary toxin gene, tcd A, tcd A rep and tcd B gene. Poor prognosis was defined as diarrhea more than 10 days even with classic treatment, recurrence, death, and moribund discharge. Results : Approximately 79% of isolated strains were toxin A+/B+ strains and 21% were toxin A-/B+ strains. There was no difference in prognosis between toxin A+ and toxin A- strains. 39 (33.9%) cases showed poor prognosis and 76 (66.1%) cases showed good prognosis. Univariate analyses revealed that the poor prognostic factors were old age over 70 years old, male, the number of antibiotics used after onset of symptom, the administration of carbapenems, aminoglycosides, glycopeptides after onset of symptom, history of DM and stroke, and high Charlson comorbidity index. Multiple logistic regression analysis identified old age over 70 years old (odds ratio=3.378, P=0.009) and the administration of carbapenems after onset of symptom (odds ratio 7.210, P<0.001) as the independent poor prognostic factors. Conclusion : Old age over 70 and the administration of carbapenems after onset of symptom were the poor prognostic factors for CDAD caused by none-binary toxin producing strains.

        Effects of L-carnitine, Selenium-enriched Yeast, Jujube Fruit and Hwangto (Red Clay) Supplementation on Performance and Carcass Measurements of Finishing Pigs

        Fifty castrated crossbred (Landrace횞Yorkshire) pigs, weighing an average of 60.6짹3.1 kg were allotted to one of five treatments in a randomized block design to examine the effects of dietary inclusion of 0.1% L-carnitine (50 ppm carnitine), 0.1% selenium-enriched yeast (0.3 ppm selenium), 0.1% Jujube fruit or 0.1% Hwangto (Red clay) on pig performance and carcass quality. All diets were based on corn, wheat, soybean meal and wheat bran and were formulated to supply 13.8 MJ DE/kg. Dietary supplementation did not influence daily gain (p = 0.57), feed intake (p = 0.52), or feed conversion (p = 0.32). Digestibility of dry matter (p = 0.60), organic matter (p = 0.74), crude protein (p = 0.76), crude fibre (p = 0.70) and energy (p = 0.75) were also unaffected by inclusion of any of the additives. Tissue samples taken from the longissimus muscle showed that the levels of carnitine (p = 0.0001) and selenium (p = 0.0001) were significantly higher with dietary inclusion of carnitine or selenium-enriched yeast. Dietary treatment did not affect dressing percentage (p = 0.33), carcass lean yield (p = 0.99) or first, 10th and last rib midline backfat depth (p = 0.45, 0.82 and 0.47, respectively). Dietary treatment also did not affect the percentages of tenderloin (p = 0.37), bacon (p = 0.36), fat and bone (p = 0.56), picnic shoulder (p = 0.25), skirt (p = 0.80), fresh ham (p = 0.31) or ribs (p = 0.79). However, pigs fed the diet containing Jujube fruit had a higher percentage of Boston butt than pigs fed the carnitine or selenium supplemented diets (p = 0.01). Pigs fed added Hwangto had a higher (p = 0.04) percentage of loin compared with pigs fed supplementary selenium or Jujube fruit. Loin muscle from pigs fed carnitine had a significantly lower Hunter colour value for L (whiteness, p = 0.004) and a higher value for a* (redness; p = 0.069). The overall results indicate that supplementation with L-carnitine and selenium-enriched yeast can produce pork containing higher levels of carnitine and selenium, which could provide health benefits for consumers of pork without detrimental effects on pig performance.

Plant Sci. 94: 73-83. Pectin is one of the main components of plant cell walls, and its biosynthesis is controlled by pectin methylesterase (PME). Pectin methylesterase inhibitors (PMEIs) are key regulators of PME. We report here the cloning and characterization of a novel Solanum lycopersicum L. PMEI gene, SlPMEI. RT-PCR studies of leaf, seed, fruit, flower, and flower organs confirmed that SlPMEI is expressed specifically in pollen. Promoter analysis of SlPMEI revealed pollen-specific cis-acting elements (pollen lat52 and g10). In addition, SlPMEI is expressed independently of abiotic stress, pathogen exposure, and growth stage in tomato, and a histochemical analysis of promoter activity revealed pollen-specific expression in both Arabidopsis and tomato. Under the microscope, we observed pollen-specific GUS expression in the stamen of transgenic tomato plant. These results indicate that the promoter of SlPMEI has strong pollen-specific activity, and could therefore be useful for development of industrially and agronomically important transgenic plants. </P>

        ON JACOBSON AND NIL RADICALS RELATED TO POLYNOMIAL RINGS

        This note is concerned with examining nilradicals and Jacobson radicals of polynomial rings when related factor rings are Armendariz. Especially we elaborate upon a well-known structural property of Armendariz rings, bringing into focus the Armendariz property of factor rings by Jacobson radicals. We show that J(R[x]) = J(R)[x] if and only if J(R) is nil when a given ring R is Armendariz, where J(A) means the Jacobson radical of a ring A. A ring will be called feckly Armendariz if the factor ring by the Jacobson radical is an Armendariz ring. It is shown that the polynomial ring over an Armendariz ring is feckly Armendariz, in spite of Armendariz rings being not feckly Armendariz in general. It is also shown that the feckly Armendariz property does not go up to polynomial rings.

        Changes in plasma lipoxin A4, resolvins and CD59 levels after ischemic and traumatic brain injuries in rats

        Ischemic and traumatic brain injuries are the major acute central nervous system disorders that need to be adequately diagnosed and treated. To find biomarkers for these acute brain injuries, plasma levels of some specialized pro-resolving mediators (SPMs, i.e., lipoxin A4 [LXA4], resolvin [Rv] E1, RvE2, RvD1 and RvD2), CD59 and interleukin (IL)-6 were measured at 0, 6, 24, 72, and 168 h after global cerebral ischemic (GCI) and traumatic brain injuries (TBI) in rats. Plasma LXA4 levels tended to increase at 24 and 72 h after GCI. Plasma RvE1, RvE2, RvD1, and RvD2 levels showed a biphasic response to GCI; a significant decrease at 6 h with a return to the levels of the sham group at 24 h, and again a decrease at 72 h. Plasma CD59 levels increased at 6 and 24 h post-GCI, and returned to basal levels at 72 h post-GCI. For TBI, plasma LXA4 levels tended to decrease, while RvE1, RvE2, RvD1, and RvD2 showed barely significant changes. Plasma IL-6 levels were significantly increased after GCI and TBI, but with different time courses. These results show that plasma LXA4, RvE1, RvE2, RvD1, RvD2, and CD59 levels display differential responses to GCI and TBI, and need to be evaluated for their usefulness as biomarkers.

        <P>14-3-3 proteins act as adapters that exert their function by interacting with their various protein partners. 14-3-3 proteins have been implicated in a variety of human diseases including neurodegenerative diseases. 14-3-3 proteins have recently been reported to be abundant in the neurofibrillary tangles (NFTs) observed inside the neurons of brains affected by Alzheimer’s disease (AD). These NFTs are mainly constituted of phosphorylated Tau protein, a microtubule-associated protein known to bind 14-3-3. Despite this indication of 14-3-3 protein involvement in the AD pathogenesis, the role of 14-3-3 in the Tauopathy remains to be clarified. In the present study, we shed light on the role of 14-3-3 proteins in the molecular pathways leading to Tauopathies. Overexpression of the 14-3-3σ isoform resulted in a disruption of the tubulin cytoskeleton and prevented neuritic outgrowth in neurons. NMR studies validated the phosphorylated residues pSer214 and pSer324 in Tau as the 2 primary sites for 14-3-3 binding, with the crystal structure of 14-3-3σ in complex with Tau-pSer214 and Tau-pSer324 revealing the molecular details of the interaction.         자가치아이식술에 관한 임상적 및 방사선학적 연구

        Autogenous tooth transplantation, or autotransplantation, is the surgical movement of a tooth from one location in the mouth to another in the same individual. Tooth transplantation offers one of the fastest and most economically feasible means of replacing a missing tooth. Despite the increasing use of osseointegrated implants in patients with missing teeth, their use is contraindicated in growing patients. Transplantation of available teeth remains a suitable choice for replacing missing units in young patients. Once thought to be experimental, autotransplantation has achieved high success rates and is an excellent option for tooth replacement. Successful transplantation depends on specific requirements of the Patients. the donor tooth. the recipient site, assessment of patients, and surgical procedure. The aim of this paper is to underline the evidence based principles for successful autotranbplantation and report clinical and radiographic results related with various conditions in three cases.

        자궁경부암에서 종양진행과 E-cadherin/α-catenin 발현이상에 관한 연구

        목적 : E-cadherins은 상피특이적이고 칼슘 의존적 세포간 부착에 관여하는 막간 당단백질군에 속한다. E-cadherin의 기능은 그것과 세포질에서 연결되는 α-catenin에 의해 조절된다. α-catenin은 actin과 E-cadherin을 연결시킨다. 이 연구는 E-cadherin a/α-catenin의 발현이 양성자궁질환, 상피내암, 침윤암에서 어떤 차이를 보이는지 알아보는 것이다. 연구 방법 : 89개의 조직표본을 펀치생검, 원추 절제술, 자궁적출술에 의해 얻었다. 절편은 FIGO staging에 따라 양성 경부, 상피내암, 침윤암으로 분류되었다. E-cadherin/α-catenin의 발현은 monoclonal antibody로 면역조직화학 염색을 하여 조사하였다. 세 군간의 E-cadherin/α-catenin의 발현양상을 조사했다. 통계적 의미를 조사하기 위해 x² test를 이용했다. 결과 : E-cadherin에서는 정상염색(normal), 낮은 양성(low-positive), 이종(異種, heterogeneous)염색, 음성(negative) 염색 결과가 양성 질환인 경우 각각 19, 1, 0, 2예, 상피내암인 경우 12, 5, 3, 4예, 침윤암인 경우 10, 8, 20, 5예를보였다. α-catenin에서도 양성 질환인 경우 각각 18, 1, 1, 2예, 상피내암인 경우 9, 9, 4, 2예, 침윤암인 경우 8, 8, 19, 8예를 보였다. 조직검사와 E-cadherin과 α-catenin의 발현강도와 분포사이에 통계적으로 의미있는 상관관계 (χ² test, p<0.0001)를 보였다. 즉 양성질환 보다 상피내암, 침윤암일 수록 두 물질의 비정상적인 발현양상을 관찰할 수 있었고, E-cadherin/α-catenin의 발현도 변화와 조직학적 진단간에 상관관계가 있었다. 결론 : 이 결과에 따라 저자는 자궁경부암에서 종양진행에 따라 E-cadherin/α-catenin의 발현에 있어서 분포와 강도가 변화함을 알게 되었다. Objective : E-cadherins are a family of transmembrane glycoproteins involved in epithelial specific and calcium-dependent intercellular adhesion. E-cadherin function is regulated by its associated cytoplasmic protein, α-catenin, which links E-cadherin to the actin-based cytoskeleton. This study is to determine what lack and differrence in expression of E-cadherin and α-catenin between benign, CIS, and invasive carcinoma of uterine cervix. Methods : 89 tissue specimens were obtained by punch biopsy, conization, and hysterectomies. Sections were classified according to FIGO staging as benign cervix, carcinoma in situ, invasive cervical carcinoma. The expression of E-cadherin and α-catenin was examined immunohistochemically with monoclonal antibodies. What degree in expression of E-cadherin and α-catenin in three group was investigated. The χ² test was used to assess the statistical significance of this study. Results : The correlation between the altered expression of E-cadherin/α-catenin and the histopathologic diagnosis was statistically significant. Conclusion : According to this result, we found that the distribution and intensity in expression of E-cadherin and α-catenin is altered during tumor progression of cervical carcinoma.

      • 철 결핍성빈혈이 혈청 지질 및 렙틴농도에 미치는 영향

        The purpose of this study was to investigates the changes in serum lipid and leptin of iron deficiency anemia female gymnasts who excessive training and weight control. The mean age of subject was 17.6 years old. Their mean height was 158.4±4.76cm and mean weight 48.7±4.70kg. Mean body fat percent was 21.2±3.01%, and body mass index was 19.5±1.30 kg/m^2. The mean value of energy and iron intake were 1367.2±360.9 kcal(74±38.0% of the Recommended Dietary Allowance) and 7.1±2.7 mg(38.6±15.8% of RDA), The rate of te 3 major nutrition (P : F : C) was 63 : 26 : 11%. It showed that high fat intake rate. The hemtological status showed hemoglobin(2.3±1.0g/dl), Ferritin(46.7±32.l ng/ml), sTfR(2.8±1.4 mg/L) was low, but in a normal range. Mean serum triglyceride concentration and LDL-cholesterol, Atherogenic index was normal rage, but iron deficient group was higher than normal group(P<.05). Mean Insuin concentration was significantly lower than normal group(P<.05).
