        광선각화증, 보웬병, 편평상피세포암에서 Ki-67, Cyclin A, p53, p16의 발현 양상

        Background: Actinic keratosis (AK) and bowen`s disease (BD) are pre-cancerous diseases, and are regarded as an early squamous cell carcinoma (SCC). AK and BD can be progressed into SCC. In this process, tumor suppressor and cell proliferative proteins may play important roles. Objective: To investigate the differences of expression patterns of the immunohistochemical (IHC) staining and useful markers for differential diagnosis in AK, BD and SCC. Methods: Biopsy had proven 17 cases of AK, 20 cases of BD and 17 cases of SCC, which were all selected. IHC staining for Ki-67 and cyclin-A, as cell proliferative markers, p53 and p16 as tumor suppressor markers, were performed. Labeling index (LI) and distribution pattern of IHC expressions were measured. Results: LI of Ki-67 in AK, BD and SCC were 30.6%, 60.2% and 54.8%, respectively. LI of cyclin-A in AK, BD and SCC were 9.2%, 24.4% and 24.1%, respectively. LI of p53 in AK, BD and SCC were 20.7%, 37.9%, and 39.9%, respectively. LI of p16 in AK, BD and SCC were 10.6%, 38.3% and 39.9%, respectively. Lower 1/3 was the most frequent distribution pattern in AK in all IHC stains, full thickness lower 2/3 were the most frequent distribution pattern in BD and SCC in all IHC stains. Conclusion: LI and distribution pattern of Ki-67, cyclin-A, and p16, as well as the distribution pattern of p53 may be useful markers to differentiate AK from BD and SCC. Higher degree and full-thickness distribution pattern IHC expressions in all stains may be helpful in the diagnosis of BD, rather than AK. (Korean J Dermatol 2012;50(4):290∼298)

      • 인체 대장암 및 자궁경부암에서 PCR-SSCP법을 이용한 Ki-ras 암유전자의 점돌연변이에 관한 연구

        인체 대장암 20례와 자궁경부암 10례의 암조직을 대상으로 c-Ki-ras 유전자 codon 12와 13에서의 점돌연변이를 알아보고자 polymerase chain reaction(PCR)과 single-strand conformation polymorphism(SSCP) 검사법을 시행하였다. 대장암의 경우 20례중 9례(45%)에서, 자궁경부암의 경우 10례중 1례(10%)에서 양성으로 나왔다. 대장암에서는 codon 12에서 GGT가 TGT로의 치환이 4례로서 가장 많았으며, AGT, CGT로의 치환이 각 1례였고 codon 13에서는 GGC가 GAC로의 치환이 2례, TGC로의 치환이 1례인 것으로 나타났다. 자궁경부암의 경우 codon 12에서 GGT가 AGT로의 치환이 1례인 것으로 나타났다. In an attempt to clarify the role of genetic alteration in the genesis of human colorectal and cervical cancers, tissue specimens from 20 patients with colorectal cancer and 10 patients with cervical cancer were examined for the presence of point mutation in K-ras2 exon 1 by single strand conformation polymorphism analysis of PCR product. Exon I of c-Ki-ras2 was amplified by polymerase chain reaction(PCR) and comparison was made between the normal and mutated genes by nondenaturing polyacrylamide gel electrophoresis(PAGE) of PCR product and nucleotide sequence analysis using asymmetric PCR with direct sequencing. Genomic DNA from white blood cells were used as normal control and those from A427 cell line were used as mutated control. 1.PCR product from A427 cell line showed a distinct migration shift pattern compared to the normal control in PAGE and the direct sequencing indicated that nucleotide sequence of codon 12 was mutated from GGT to GAT. In addition, there was a loss of normal allele in A427 cell line. 2.PCR product from 9 cases(45%) out of a 20 colon cancer patients showed migration shifts in PAGE, and all of these 9 patients invariably demonstrated mutation of ras gene, either in codon 12(6 cases:30%) or 13(3 cases:15%). The base substitutions in codon 12 were: from GGT to AGT(1), CGT(1), or TGT(4). Base changes in codon 13 were from GGC to TGC(1) or GAC(2). 3.PCR product from 1 cases(10%) out of a 10 cervical cancer patients showed migration shrifts in PAGE, Invariably demonstrated mutation of ras gene in codon 12. The base substitutions was from GGT to AGT(1) TQE incidence of point mutation of c-Ki-ras in colon cancer was high, however, it was low in cervical cancer, which showed the relation of ruts gene mutation with colon cancer. PCR -SSCP analysis is a simple, rapid and efficient method of detection of point mutation, especially when dealing with multiple samples.

      • Globalization of Korean Electrical Installations Standards and Codes Based on Comparison of IEC 60364 with NFPA

        Most of the laws and standards relevant to technology in Korea bear a resemblance to those in Japan. Electricity was not introduced to Korea until Lighting ceremony in Kyong-Bok palace observed in 1887. Since Korea was annexed to Japan in 1910 and our public works were under Japan control we have applied Japanese laws and regulations made by Chosun government-general. Independently from Japan the power electricity accepted power plants of USA, codes of NESC, ASME / ANSI, NEMA. Our generation and distribution system takes American style while our consumer side takes Japanese style. As global trade system was transferred GATT into WTO, the harmonization of technological standards, certification construction testing in the trend of world. This paper is based on WTO / TBT agreement coming in effect after 12th, April, 1979. For Korea the distribution system has a strong resemblance to American system so that unconvertible because IEC 60364 originated from Europe while NEC originated from USA. This papers deals with the fundamental elements of electrical safety system with comparing IEC 60364 with NEC. This paper considers how engineers should apply NFPA 70(NEC) and IEC based on the comparison of NEC to IEC in relation to electrical installations.

        Most Technological laws of Korea are almost identical to those of Japan. Electricity was not introduced to Korea until lighting ceremony in Kyong-Bok palace observed in 1887. Since Korea was annexed to Japan in 1910, Korea have applied Japanese law and regulations made by Chosun government-general. And public works were under Japan control. Korea accepted adopted the power plants of America, codes of NESC, ASME / ANSI, MEMA while Japan accepted or adopted only IEC. Our generation distribution system takes American style while our consumer side takes Japanese style. As global trade system was transferred GATT into WTO, the technological standards, certification, construction and testing are in harmony internationally. The WTO / TBT agreement came in effect after 12th, April, 1979. The sections in the agreement require the members to exchange their own standards and codes for the harmonized ones by degree. The Korean distribution system has a strong resemblance to American system so that the internal engineers are confused in application of the relevant standards. IEC60364 and NEC are technologically similar but practically unconvertible because both have their different originality, that is to say Europe and USA. This paper deals with the fundamental elements of electrical safety system on review of IEC 60364 and NEC. This paper considers how engineers should apply IEC60364 and NFPA 70(NEC) based on all-the-way review of them.

      • Duplication or deletion of chromosome 17p11.2-p12 in CMT1 and HNPP peripheral neuropathy patients

        CMT와 HNPP 말초신경증은 가장 흔하게 발병하는 운동 및 감각 말초신경계의 선천성 질환이다. CMT1과 HNPP의 유전적 원인으로는 염색체 17p11.2-p12의 1.5 Mb 지역의 중복과 결실이 각각 50%이상과 70% 이상을 차지하는 것으로 알려졌다. 본 연구에서 한국인 CMT1 및 HNPP 환자를 대상으로 중복 및 결실을 조사하였는데, 31 CMT1 가족 중 13 가족에서 중복을 보였으며 (42%), 24 HNPP 가족 중 19 가족에서 결실을 보였다 (79%). 17p11.2-p12 중복을 보이는 CMT1A 환자가 HNPP 결실환자보다 발병 연령, 발병 기간, 근육 이양성 및 족 기형성에서 훨씬 심한 증상을 보였다. Charcot-Marie-Tooth disease (CMT) and hereditary neuropathy with liability to pressure palsies (HNPP) are most common inherited motor and sensory peripheral neuropathies. The majority of CMT1 patients have a 1.5 Mb duplication in chromosome 17p11.2 while most HNPP patients have a deletion of the same region. In the analysis of Korean CMT1 and HNPP patients, each HNPP deletion and CMT1A duplication was observed from 79% (19 families) among 24 HNPP families and 42% (13 families) among 31 CMT1 families. CMT1A duplication patients showed more severe clinical symptoms than HNPP deletion patients, when we examined oneset age, disease duration, muscular atrophy, foot deformity and scoliosis.

        Pediatric infratentorial ependymoma: prognostic significance of anaplastic histology.

        <P>Pediatric infratentorial ependymomas are difficult to cure. Despite the availability of advanced therapeutic modalities for brain tumors, total surgical resection remains the most important prognostic factor. Recently, histological grade emerged as an independent prognostic factor for intracranial ependymoma. We retrospectively reviewed the treatment outcome of 33 pediatric patients with infratentorial ependymoma. Progression-free survival (PFS) and overall survival (OS) rates were calculated and relevant prognostic factors were analyzed. Fourteen patients (42%) were under the age of 3 at diagnosis. Gross total resection was achieved in 16 patients (49%). Anaplastic histology was found in 13 patients (39%). Adjuvant therapies were delayed until progression in 12 patients (36%). Actuarial PFS rates were 64% in the first year and 29% in the fifth year. Actuarial OS rates were 91% in the first year and 71% in the fifth year. On univariate analysis, brainstem invasion (P = 0.047), anaplastic histology (P = 0.004), higher mitotic count (P = 0.001), and higher Ki-67 index (P = 0.004) were significantly related to a shorter PFS. Gross total resection (P = 0.029) and a greater age at diagnosis (P = 0.033) were significantly related to a longer PFS. On multivariate analysis, anaplastic histology alone was significantly related to a shorter PFS (P = 0.023). Gross total resection (P = 0.039) was significantly related to a longer overall survival (OS) on multivariate analysis. Anaplastic histology and gross total resection were the most important clinical factors affecting PFS and OS, respectively. Anaplastic histology, mitotic count, and Ki-67 index can be used as universal and easily available prognostic parameters in infratentorial ependymomas.</P>

        일부 농촌 지역의 취학전 어린이 시력에 관한 연구

        We performed ophthalmic examinations for 569 children in cheong yang(3-6 years of age), during May, 2002. 78 children were shown to have either visual acuity of less than 0.5 or anisometropia of over 2 lines. Detailed ophthalmic examinations were conducted on those 78 children. The 69 children(l2.1%) had ametropic eyes, 14(2.5%) amblyopia, and 3(0.5%) strabismus. Among the types of ametropia, Myopic astigmatism 21(30.4%) was the most common type. Astigmatism was combined with all other types in 56(81.2%) children. Among the types of amblyopia, visual deprivation amblyopia was 8(57.1%) and anisometropic amblyopia was 6(42.9%). In cases of strabismus, exotropia was 3. The early detection and adequate care of eye problems for preschool children are important to prevent the permanent visual disability. It is necessary to make institutional devices for preschool vision screening and participation rate for this screening will be more increased by efforts of constitutional support and parental cooperation.
