Background/Aims Gastroesophageal reflux disease is a highly prevalent disease. Assessing treatment efficacy is critical in that clinical endpoints are properly evaluated. Clinical tools for symptoms severity assessment should be discriminative, predictive and evaluative. Methods In this study we compared a patient-oriented symptoms evaluation (ReQuest(TM)) vs a structured interview assessment initiated by a physician (sickness impact profile [SIP]). Both questionnaires were analyzed in a multidimensional space using latent factors. Five dimensions were found: 1 for the short ReQuest(TM) questionnaire and 4 for SIP. Results We included 1,522 women and 1,296 men; mean age was 36 ± 7 years, and mean body mass index was 26 ± 4. The score questionnaire assessment evaluation by physicians and patients did not correlate between them (between r = 0.03 and 0.26) except nausea and sleep disorder (r = 0.45 and 0.51) but both were sensitive enough to detect changes after treatment (P < 0.05). Medical specialty of the physician showed effect on the score of both, ReQuestTM and SIP evaluation. Questionnaire variance decomposition due to specialist was only 2% (P < 0.05). Conclusions While both evaluations are orthogonal (non-correlated), meaning patients and physicians measured diverse aspects of the same disease, they both were able to measure patient`s improvement with treatment. Surgical Management of an Infected Popliteal Artery Aneurysm

Infective aneurysms are rare due to the antimicrobial advances and the early treatment of systemic infections. They represent a diagnostic and therapeutic challenge. The treatment for these cases is generally characterised by excision and reconstruction using an autologous vein graft. We describe a case of a 66-year-old man who presented an 8 cm infected popliteal aneurysm where urgent surgical approach was performed. The vascular continuity was restored with a basilic vein. Clinical follow-up showed no signs of recurrent infection and patent bypass without any anastomotic pseudoaneurysm after a year.

NANOBAIN is the term used to refer to a new generation of advanced steels capable of producing by isothermaltransformation at low homologous temperatures, T/Tm~0.25 where Tm is the absolute melting temperature,a nanocrystalline microstructure, composed exclusively of two phases, thin plates of bainitic ferriteseparated by C enriched austenite. Such alloys are exclusively designed on the basis of bainitic transformationtheory and some physical metallurgy principles. In this work, by designing a new set of alloys capable ofproducing such microstructure, a further step toward the industrialization of NANOBAIN is taken. Some importantindustrial requirements, including circumventing the inclusion of expensive alloying elements and theneed for faster transformations, are also considered. For all the alloys, the experimental results validate thedesign procedure and they illustrate that the NANOBAIN concept is a step closer to industrialization, probingthat it is possible to obtain nanocristalline bainite in simpler alloy systems and in shorter times than thosereported previously.

This paper proposes a navigation method for manipulators which handle a surgical instrument or robotic platform introduced through a SILS multiport trocar. This method is based on a parallel position and force control which allow to the manipulator makes movements around the fulcrum point making the minimum force in the patient’s abdomen. In order to avoid the hysteresis effect due to the space between the instrument and the trocar when the instrument is inserted, a fuzzy gain scheduler has been designed. These algorithms have been implemented in a six degrees of freedom manipulator with a force sensor in the end effector. Finally, the experimental results are shown in order to demonstrate how it works in a real environment.
Pectin from Passion Fruit Fiber and Its Modification by Pectinmethylesterase

Passion fruit fiber pectin gels represent a new alternative pectin source with potential for food and non-food applications on a commercial scale. Pectic polysaccharides were extracted from passion fruit (Passiflora edulis) fiber using citric acid as a clean catalyst and autoclaved for 20 to 60 min at 121℃. The best condition of pectin yield with the highest molecular weight was obtained with 1.0% of citric acid (250 ㎎/g dry passion fruit fiber pectin) for 20 min of autoclaving. Spectroscopic analyses by Fourier transform infrared, enzymatic degradation reactions, and ion-exchange chromatography assays showed that passion fruit pectin extracted for 20 min was homogeneous high methoxylated pectin (70%). Gel permeation analysis confirmed that the pectin extract obtained by autoclaving by 20 min showed higher molecular weights than those autoclaved for 40 and 60 min. Passion fruit pectin extracted for 20 min was enzymatically modified with fungal pectinmethylesterase to create restructured gels. Short autoclave treatment (20 min) with citric acid as extractant resulted in a significant increase of gel strength, improving pectin extraction in terms of functionality. The treatment of solubilized material (pectic polysaccharides) in the presence of insoluble material (cellulose and hemicellulose) with pectinmethylesterase and calcium led to the creation of a stiffer passion fruit fiber pectin gel, while syneresis was not observed.

This paper discusses Neuroprosthetic applications for potential use by paralyzed patients or amputees. These systems require advanced processing of neural signals to drive the prosthetic devices using decoding algorithms. The possibility of predicting motor commands from neural signals are the core of neural prosthetic devices and along this line we show how it is possible to predict movement intentions as well as what subjects are seeing from the firing of population of neurons.

Background: Hyponatremia developing in hypothyroid patients has been encountered in clinical practice; however, its prevalencehas not been well established. Methods: Thirty patients diagnosed with differentiated thyroid cancer, rendered hypothyroid after surgery and levothyroxinewithdrawal, and who are for radioactive iodine (RAI) ablation were included. Serum sodium concentrations were measuredtwice, at the time of admission for RAI ablation, and before discharge after increased oral fluid intake. The outcome measureswere to determine the prevalence of hyponatremia among hypothyroid patients prior to RAI ablation and after oral hydrationpost-RAI, and to correlate the serum sodium levels pre-RAI and post-RAI with thyroid-stimulating hormone (TSH) concentrationand age. Results: Thirty patients were included, with ages from 23 to 65 years old (median, 40). Two patients (6.7%) were hyponatremicprior to RAI ablation, and eight patients (26.7%) had mild hyponatremia (130 to 134 mEq/L) after RAI and hydration. There wasno significant correlation between TSH levels and serum sodium levels prior to or after RAI. There was also no significant correlationbetween pre- and post-RAI sodium concentration and age. Conclusion: The prevalence of hyponatremia pre-RAI was 6.7%, and 26.7% post-RAI. No significant correlation was noted betweenTSH concentration and age on pre- or post-RAI sodium concentrations. Routine measurement of serum sodium post-RAI/isolation is still not advised. Measurement of sodium post-RAI may be considered in patients who are elderly, with comorbidconditions or on medications.
Towards a centralized route planning solution supporting time dependencies

Future route planning solutions are expected to provide significant Improvements in terms of route traversal times by accounting for both estimated and real-time traffic congestion states. Compared to current solutions, mostly based on static path segment costs, the main challenge is how to obtain reliable traffic congestion estimations for all the streets/avenues in a city for the different times of the day, for every day in a year. Modeling such a tremendous amount of data can be time-consuming and, additionally, centralized computation of optimal routes based on such time-dependencies has very high computational requirements. As a first approach to this problem, we propose a heuristic to reduce the modeling effort to a minimum while maintaing the benefits of time-dependent traffic congestion modeling. The effectiveness of this heuristic is assessed for the city of Valencia, Spain.
