      • S-580 : Vitamin D and carotid subclinical atherosclerosis in SLE

        Vitamin D and carotid subclinical atherosclerosis in SLE Chang-Bum Bae, Ju-Yang Jung, Bo-ram Go, Hyun-Ah Kim, Chang-Hee Suh Department of Rheumatology, Ajou University School of Medicine, Suwon, Korea Introduction Atherosclerosis develops earlier in systemic lupus erythematosus (SLE) and is the important cause of mortality. It has been suggested that vitamin D deficiency contributed cardiovascular disease and immune dysregulation in SLE. Methods One-hundred two female patients with SLE and 52 normal controls (NC) were recruited. The carotid artery intima-media thickness (cIMT) and plague were assessed by B-mode ultrasound at the carotid artery level. Vitamin 25(OH)D3 levels were checked by immunoradiometric assay using the Bio-Line 25(OH)D3-Ria CT kit (Bio-Line S.A., Belgium). Disease activity markers were checked at the time of enrollment. Results The cIMT of SLE is 0.41±0.08mm, which is higher than NC (0.37±0.11 mm, p=0.012). Also, carotid artery plague index (PI) of SLE is higher than NC (0.68±1.39 vs 0.26±0.87, p=0.026). The cIMT was correlated with age (r=0.442, p<0.001), body mass index (r=0.246, p=0.013), waist-hip ratio (r=0.245, p=0.013), SLE disease activity index (r=0.239, p=0.016), taking aspirin (r=0.244, p=0.013), and the carotid artery PI was correlated with renal involvement (r=0.224, p=0.023) in patients with SLE. Vitamin 25(OH)D3 was not showed correlation with cIMT and carotid artery PI. Also, it was not correlated with disease activity markers. Conclusion In patients with SLE, the risk of cardiovascular disease measured by cIMT and carotid artery plaque was higher than NC, which may be derived from systemic inflammation. However, vitamin D levels were not showed significant correlations with carotid subclinical atherosclerosis. It may be not suitable to assess vitamin D as marker of disease activity or subclinical atherosclerosis.

        In vivo study for the hemostatic efficacy and foreign body reaction of a new powder-type polysaccharide hemostatic agent

        Purpose: Various hemostatic agents have been introduced in therapy as postoperative bleeding is a poor prognostic factor for postoperative outcomes. These products can be divided into those that directly promote the hemostatic cascade and those that physically form a barrier by absorbing blood. The latter, powder-type hemostatic agents have the advantages of being inexpensive and more absorbable with less foreign body reactions (FBRs) and are applicable to a relatively wide area. This study was conducted to verify the safety and efficacy of a newly invented polysaccharide product (OOZFIX, Theracion Biomedical), which improves blood absorption and hemostatic effects. Methods: Two separate animal experiments were performed. The first evaluated FBRs histologically at 3 days, 2 weeks, and 4 weeks, after implantation of OOZFIX in rats, and the second compared hemostatic performance of OOZFIX and Arista AH (Bard) in the porcine liver punch biopsy model. Results: We found minimal FBRs in the 3-day group and no reactions in both the 2-week and 4-week groups after implantation of hemostatic agents. The time to hemostasis of OOZFIX was not significantly different from that of Arista AH (median [interquartile range]: 9 [6–10] minutes vs. 8 [6–10] minutes, respectively; P = 0.522). When comparing the serial bleeding grade tendency, there was no statistical difference between OOZFIX and Arista AH (P = 0.656). Conclusion: OOZFIX caused a minimal FBR that disappeared within 2 weeks in vivo, and its hemostatic performance was comparable with that of an existing agent, Arista AH. Further clinical studies are required in the future.

        사이버대학의 교수자 지원체제의 중요도와 실행도에 대한 조사 연구

        정보통신기술의 발달로 인해 급속히 확장되고 있는 사이버대학의 성패를 좌우할 수 있는 대표적 요인으로 질 관리가 중시되고 있다. 사이버대학에서 e-Learning을 실행에 옮기는 e-Learning 교수자들은 질 관리의 핵심 요소이다. 이에 본 연구는 문헌 연구를 통해 e-Learning 교수자의 역할과 그를 수행하기 위한 사이버대학의 교수자 지원체제의 구성요소를 도출하였다. 이를 활용하여 9개 사이버대학의 교수자를 대상으로 설문조사를 실시, 교수자 지원체제에 대한 중요도와 실행도를 분석하여 사이버대학의 질적 향상을 위한 교수자 지원체제의 방향과 지침을 제공하였다. 연구 결과 교수자 지원체제의 중요도는 모든 영역에서 높게 나타났고 실행도는 학습자 평가 지원 영역이 가장 높게, 보상 시스템 지원 영역이 가장 낮은 실행도를 나타냈다. 모든 영역에서 중요도와 실행도는 유의미한 차이를 보였으며 ‘중요도-실행도 분석’ 결과 보상 시스템 지원, 교수자 연수 지원, 코스 운영 지원 영역이 집중적으로 관리되어야 하는 영역으로 나타났다. 집중 관리 항목으로는 교수자 연수 지원 영역에서 e-Learning 및 사이버대학의 이해, 교수 설계, 효과적인 사이버 강의 기법, 상호작용 촉진법 이해, 사이버 수업 운영 항목이 나타났다. 코스 운영 지원 영역에서는 학습자 질의 응답과 사무 보조, 진도 및 학습 관리를 담당하는 조교 지원 항목이, 보상 시스템 지원 영역에서는 보상과 인센티브 항목이 나타났다. The quality management is emphasized for the success of cyber university which is expanded rapidly through applying the information technology. For the cyber university the e-Learning instructor who implements the e-Learning is the core factor of the quality management. The objective of this study is to provide the direction and guideline for the instructor support service for the quality improvement of the cyber university. The role of e-Learning instructor and the elements of the cyber university instructor support service supporting this role are clarified through pre-study. The importance, presence of the cyber university instructor support service and the differences between importance and presence of the cyber university instructor support service are surveyed and analyzed through collecting the answers of prepared questionnaire. The results of this study are summarized as follows: Firstly, the importance of the cyber university instructor support service is highly expressed at all aspects; the most important elements are evaluation and the interaction is lowest evaluated. Secondly, the survey result of the presence of cyber university instructor support service can be summarized by; the highest presence is evaluation and the lowest is reward system. Thirdly, the importance is highly evaluated than the presence on every element of the cyber university instructor support service and meaningful differences are surveyed. The importance is recognized comparatively highly recognized than the presence. Fourthly, the analyzing result of importance-presence analysis matrix shows that reward system, instructor training and course are the most important elements.

        Vibrio cholerae is mainly known to cause gastrointestinal infection after seawater exposure or raw seafood intake. It is rarely reported to cause cellulitis or sepsis, but threre has been no known case after acupuncture. Herein, We report a 56-year-old cirrhotic patient of non-O1, non-O139 Vibrio cholerae septicemia caused by cellulitis of both lower extremities after acupuncture.

      • STZ 유발 당뇨쥐에서 시간에 따른 생리적 항산화계의 변화

        The purpose of this study was to investigate change of physiological antioxidative system according to the time in streptozotocin-induced diabetic rats. Sprague-Dawley male rats weighing 150±10gm were randomly assigned to one normal and five STZ-induced diabetic groups. Diabetic groups were classified to experimental period. Diabetes was experimentally induced by intravenous injection of 55 mg/kg of body weight of STZ in citrate buffer(PH 4.3) after 6 weeks feeding of three experimental diets. Animals were sacrificed at 0, 3, 6, 12, 18th days of diabetic states. 1. Activity of superoxide dismutase(SOD) in liver was increased in that of diabetic mellitus(DM) groups at 3th day but that of DM groups was not significant from 6th day according to period. Glutathion peroxidase(GPX) was significantly decreased in DM groups from 6th day 2. Reduced glutathione(GSH) contents in liver significantly decreased in diabetic groups from 3rd day compared with those of DM groups according to the period. Oxidized glutathione(GSSG) was higher from 6th day. GSH/GSSG ratio was significantly lower than that of normal group from 3rd day to all experimental period 3. Contents of vitamin E in liver of DM groups were significantly decreased compared with that of normal group from 6th day. 4. Lipid peroxide(LPO) contents in liver of DM groups were significantly increased compared with that of normal group from 3rd day. The present results indicate that STZ-induced diabetic rats were reduced by antioxidative defense system and taken by peroxidate damage in tissue compared with normal group from 3rd day or 6rd day after injection STZ. It lended to acceleration all diabetic groups but no significance according to the experimental time.

      • 근적외분광분석법을 이용한 참기름 정량 분석 및 진위 판별

        In this study, sesame oil have been analyzed by near infrared spectroscopy (NIRS). Sesame oil was mixed with the other edible oil. NIR transmittance spectra of sesame oil mixtures were acquired by using a dip probe. Partial least squares regression (PLSR) was applied to develop a calibration model over the spectral range 1100-1750nm. The calibration model predicted the content of sesame oil for validation set with a standard errors of prediction (SEP) of 0.3871%. Soft Independent Modeling of Class Analogy (SIMCA) was used to identify sesame oil and adulteration. This model identified the sesame oil and adulteration for validation set with 100% accuracy. This study showed that the rapid and non-destructive the determination of the quantitative analysis of sesame oil, the identification of sesame oil and adulteration was successfully performed by portable NIR system.

      • 에어로빅 운동이 비만 여중생의 복부비만과 심폐기능 및 인슐린에 미치는 영향

        The purpose of this study was to analyze the effects of aerobic exercise on abdominal obesity, cardiopulmonary function and serum insulin in obese middle school girls participated in aerobic exercise for 20 weeks. The conclusions are as follows ; 1. Total body fat, percent body fat, body mass index, waist circumference and waist-hip ratio were significantly decreased, but lean body mass was significantly increased in the changes of body composition factors. 2. Systolic blood pressure, diastolic blood pressure were significantly decreased, but maximal oxygen consumption was significantly increased in the changes of cardiopulmonary function factors. 3. Total cholesterol, triglyceride, low-density lipoprotein cholesterol and insulin were significantly decreased, but high-density lipoprotein cholesterol was significantly increased in the changes of serum lipid and insulin factors. In conclusions, aerobic exercise may be very effective to decrease the obesity and geriatric diseases.

      • EGFR 돌연변이와 ROS1 전위를 동시에 가진 폐선암 환자의 Erlotinib 치료 1예

        The rearrangement of c-ros oncogene 1 (ROS1) has been recently identified as an important molecular target in non small cell lung cancer (NSCLC). ROS1 rearrangement and epidermal growth factor receptor (EGFR) mutation were mutually exclusive each other in previous studies, and the clinical implication of co-existence of the two genetic alterations has not been determined. We report a case of 46-year-old female never-smoker NSCLC patient whose tumor harbored ROS1 rearrangement and EGFR mutation concomitantly. She had undergone curative surgery for stage IIIA NSCLC, and the recurrence in left pleura and brain occurred at 2 years after the surgery. She received several lines of chemotherapy including docetaxel plus carboplatin, erlotinib, pemetrexed, and gemcitabine. Erlotinib therapy showed a favorable treatment response with progression-free survival of 9.5 months and partial response of tumor on radiologic evaluations. This case represents a successful erlotinib treatment in a NSCLC patient with concurrent ROS1 rearrangement and EGFR mutation.
