Staphylococcus lugdunensis is part of the normal human skin flora and has the ability to establish primary infection in deep skin and vascular infections. A 41-year-old male presented with brittle, thick, crusted right thumb nail with yellow to brown discoloration and erythematous edema around the nail fold. We identified S. lugdunensis by bacterial culture for the pus. The lesion was improved after 2 months treatment of oral cefditoren and clarithromycin. We report a case of onychia and paronychia caused by S. lugdunensis. (Korean J Dermatol 2012;50(1):70∼72)

The superficial variant of morphea is a recently described as an entity that differs in its clinical and histologic presentation from classic morphea. The condition is characterized by minimal to no induration and hypopigmented or hyperpigmented patches, without associated symptoms, systemic involvement, contractures, or atrophy. Histologically, it shows thickened collagen bundles, which restricts to the superficial dermis, sparing the deep dermis and without associated evidence of lichen sclerosus et atrophicus. Diffuse dermal angiomatosis is an acquired, benign vascular proliferation with poorly circumscribed, violaceous, and livedoid plaques with frequent ulceration. Histologically, there is diffuse and interstitial proliferation of CD31- positive endothelial cells and myofibroblasts within the dermis. Endothelial atypia, mitoses, and vasculitis are lacking. Here we describe a 52-year-old male who presented with erythematous and purpuric patches on the trunk and lower extremities for 2 years. Histologic examination demonstrated a dense proliferation of endothelial cells in the papillary and reticular dermis. Small vascular spaces and extravasated erythrocytes were noted. Immunohistochemical staining using anti-CD31 antibody was positive. (Korean J Dermatol 2013;51(2):144∼147)

Herpes zoster is an infective skin disease that is encountered commonly in dermatologic field. The best known complication of herpes zoster is postherpetic neuralgia (PHN). Also, many patients with herpes zoster experience neuropathic itch with or without pain. Those postherpetic itches (PHI) are rare, but some patients injure themselves by scratching skin with severe itching sensation, and with the loss of the protective sensation. So, PHI should be considered when evaluating a patient after an occurance of herpes zoster. Gabapentin has been successfully used to treat PHI in some cases. We report a case of postherpetic itch that has been successfully treated with gabapentin, as an unusual clinical presentation. (Korean J Dermatol 2012;50(4):337∼340)
