        New bimaxillary orthognathic surgery planning and model surgery based on the concept of six degrees of freedom

        The aim of this paper was to propose a new method of bimaxillary orthognathic surgery planning and model surgery based on the concept of 6 degrees of freedom (DOF). A 22-year-old man with Class III malocclusion was referred to our clinic with complaints of facial deformity and chewing difficulty. To correct a prognathic mandible, facial asymmetry, flat occlusal plane angle, labioversion of the maxillary central incisors, and concavity of the facial profile, bimaxillary orthognathic surgery was planned. After preoperative orthodontic treatment, surgical planning based on the concept of 6 DOF was performed on a surgical treatment objective drawing, and a Jeon's model surgery chart (JMSC) was prepared. Model surgery was performed with Jeon's orthognathic surgery simulator (JOSS) using the JMSC, and an interim wafer was fabricated. Le Fort I osteotomy, bilateral sagittal split ramus osteotomy, and malar augmentation were performed. The patient received lateral cephalometric and posteroanterior cephalometric analysis in postretention for 1 year. The follow-up results were determined to be satisfactory, and skeletal relapse did not occur after 1.5 years of surgery. When maxillary and mandibular models are considered as rigid bodies, and their state of motion is described in a quantitative manner based on 6 DOF, sharing of exact information on locational movement in 3-dimensional space is possible. The use of JMSC and JOSS will actualize accurate communication and performance of model surgery among clinicians based on objective measurements.

        급성 심근경색 후 심인성쇽의 발생에 관여하는 위험인자

        N/A Objectives: Cardiogenic shock resulting from acute myocardial infarction is a serious complication with high mortality. The early identification of patients at high risk of developing post-infarction cardiogenic shock might allow early intervention in an attempt to prevent cardiogenic shock and to reduce the mortality due to cardiogenic shock. The aim of the present study was to examine the risk factors of inhospital development of cardiogenic shock among patients with acute myocardial infarction. Methods: We studied 152patients with acute myocardial infarction who were admitted to Kang-Nam St. Mary's hospital within 24hours after the onset of chest pain and did not have cardiogenic shack on admission between March 1991 and May 1994. Clinical data of these patients were analyzed. Results: Of 152patients, 17(11.1%) developed cardiogenic shock during their hospital stay. Cardiogenic shock developed in 53% of cases more than 24hours after admission. 82.4% of patients with cardiogenic shock died whereas a 6.7% in-hospital mortality was found among patients without cardiogenic shock. Multivariate regression analysis that controlled for variables affecting incidence of postinfarction cardiogenic shock showed that independent risk factors for in-hospital cardiogenic shock were history of myocardial infarction (adjusted relative odds[RO]=5.294, 95% confidence interval[CI]=2.149 to 13.041); heart failure on admission (RO=3.344, 95% CI=1.738 to 6.432); hyperglycemia (>180mg/dl) in non-diabetic patients (RO=3.270, 95% CI=1,590 to 6.727); age over 70 year old (RO=2.912, 95% CI= 1.816 to 4.668); ST deviation over 4mm (RO=2.417, 95% CI=1.225 to 4.767); peak LDH level greater than 1600U/ml (RO=1.154, 95% CI=1.080 to 1.233). Patients with one independent risk factor had an estimated probability of 10.5% for developing inhospital cardiogenic shock; patients with two independent risk factors, 48.5%, patients with three risk factors, 65.0% patients with four risk factors, 65.7% patients with five risk factors, 67.2%. Conclusion: Of post-infarction cardiogenic shock during admission, 53% developed more than 24hours after admission. The more independent risk factors on admission for inhospital cardiogenic shock patients with acute myocardial infarction had, the more likely in-hospital cardiogenic shock developed.

      • 유한차분법에서 열손실 정확도에 미치는 Node 개수의 영향

        The effect of the number of nodes on the heat loss from a rectangular fin for a finite difference method is studied. There are two wasys for selecting nodes for the upper half fin in this finite difference method. In the first place, all the △x are te same and all the △y are the same for the entire upper half fin. Incremental length of x(i.e. △x)is divided by two near the fin tip while all the △y are the same for another way. The results show that 1) About 30 nodes are enough to obtain the satisfactory exact anlysis(relative error< 5%) on the heat loss for a given range of Biot number in case of short fin (i.e. L≤2). 2Under usual circumstances (Bi<0.1), the relative error of heat loss between using 30 nodes and 90 nodes is within 4% for given range of non-dimensional fin length. 3)The relative error of the calculated heat loss (the number of node=90) as compared to the expected exact heat loss is less then 1.5% for Bi=0.1 and L=10 while that is over 13% for Bi=1.0 and L=10.

      • 실내무선환경하의 광대역 CDMA 시스템의 성능해석에 관한 연구

        현재 폭발적인 잠재력을 가지고 있는 무선 LAN이나 Bluetooth와 같은 시스템의 실내 무선채널 환경에서는 각종 전자기기의 급속한 발전으로 인공잡음(전력선,마이크로웨이브 오븐, PC실내 기자재 및 기기 등)의 발생이 크게 증가하고 있으며, 이러한 인공잡음은 실내 무선 채널 환경을 더욱 악화시키고 있다. 이러한 인공 잡음의 통계적 특성은 기존에 논의되었던 가우스 잡음과 상당한 차이를 보이는 임펄스 형태의 잡음 분포를 가지는 것으로 알려져 있다. 따라서 본 논문에서는 다중경로 페이딩에 임펄스 잡음이 부가되는 최악의 실내 무선채널 환경에서 광대역 DS-CDMWBPSK 시스템의 BER 성능을 이론적으로 유도하고, 시뮬레이션을 통해서 유도된 이론식이 합당함을 보였다. A variety of man-made noises, that is, noise from powerline, home appliances such as microwave oven and office appliances such as PC, has remarkably been increased under indoor wireless channel in the systems, such as wireless LAN or Bluetooth having explosive potential market. These man-made noises deteriorate indoor wireless channel performance. It is known that statistical characteristics of these noise have the noise distribution of impulsive type which greatly differ from the Gaussian noise. Therefore, it is expected that impulsive noise is added to the fading channel in indoor wireless communication systems. In this paper, It has theoretically derived the BER performance from an asynchronous DS-CDMA/BPSK system under impulsive noise added to the signal going through multipath fading, and has been proved to be reasonable the derived theoretical equation through computer simulation at indoor wireless environments.

      • 심실전도장애환자에서 위상각 분석의 의의

        It is known that the normal His-Purkinje system provides for nearly synchronous activation of the right and left ventricles. When this His-Purkinje conduction is abnormal, the resulting sequence of the both ventricular contraction should be changed. These abnormal mechanical consequences resulting from abnormal conduction system have been difficult to demonstrate because these are very rapid and complex. We evaluate the phase angle analysis using radionuclide gated blood pool scan(GBP) as a quantitative method which can determine the asynchronous contraction of left ventricule in patients with complete left bundle branch block(C-LBBB, 20 cases) and complete right bundle branch block(C-RBBB, 15 cases). The results as follows : 1) The ejection fractions, peak ejection rates, in GBP were significantly decraesed in patients with C-LBBB compared with controls(10 cases, 70± 4.2 vs 44± 13.9%, 3.51± 0.62 vs 2.48± 0.98, p<0.05 by t-test respectively). The peak filling rates were also decreased in patients with C-LBBB(3.38± 0.92 vs 1.76± 0.71, 1A.05). 2) In the phase angle analysis, the standard deviations and the width of half maximum of phase angle were significantly increased in patients with C-LBBB(8.6± 1.8 vs 20.6± 18.1, 16.0± 3.9 vs 22.5± 9.2, p<0.05 by t-test respectively). And the ranges of phase angle were also increased in C-LBBB(51.3± 5.4 vs 95.7± 31.7, p<0.05). 3) But there were no significant differences in GBP and phase angle analysis between patients with C-RBBB and controls. As the results above, it can be concluded that the phase angle analysis can provide an accurate noninvasive method to detect the asynchronous contraction in patients with abnormal intraventricular electrical activation.

This sijo was written when the author was in the position called jangrungcharnbong, and it has been evaluated as an excellent example of yeongun-gasa and gihang-gasa.

        반도체 제조공정의 폐수슬러지로 합성된 Hydroxyapatite를 이용한 인산이온의 흡착

        The hydroxyapatite(HAp) for the present study was prepared with the wastewater sludge from semiconductor fabrication process and it was crystallized in an electric furnace for 30 min at 900℃. The adsorption characteristics of HAp for phosphate ion in aqueous solution has been investigated. The adsorbed ratio of phosphate ion for HAp were investigated according to the reaction time, amount of HAp, concentration of standard solution, pH of solution, and influence of concemitant ions. The amount of adsorbed phosphate ion decreased with the increase of pH due to the mutual electrostatic repulsion between adsorbed phosphate ions and competitive absorption between phosphate ion and OH- ion in aqueous solution. The maxium amount of the adsorption equilibrium for phosphate ion was about 24 ㎎/g of HAp. The HAp would likely to be a possible absorbent for the removal of phosphate ion in the waste water.

        This paper is to seek the wisdom about the direction and response of the welfare policy in Korea by examining closely the change of the English welfare policy in the past as history teaches the wisdom. The contents of this paper are to compare the ideologies, organizations, programs and finances of welfare in England before and after the economic crisis. First of all I can discover the distinguished changes in ideologise, organizations and programs of welfare and discover the constant and incremental change in terms of finances. Especially It is in contrast to the intention of the Thatcher government that it shows us constant and incremental change in the financial aspect. The case of the change of the welfare policy in England is not of general application in our welfare policy. However, There is any inkling. The economic crisis in England is attributable to the expanding welfare expenditures but our economic crisis has its origin in political corruption, undeveloped monetary situation and wasteful importation of foreign capital of the financial cliques, I think. Therefore, the method to deal successfully with the economic crisis is differs from country to country. We ought to not cutback the welfare expenditures. If not so, our level of welfare will go down more and more. We have to change the welfare policy drastically to emancipate the poverty class through the increase of welfare expenditures.
