        Delayed Vascular Claudication Following Diagnostic Cerebral Angiography - A Rare Complication of the AngioSeal Arteriotomy Closure Device

        With rapidly increasing numbers of neuroendovascular procedures performedannually in recent years, use of arterial closure devices after femoral arteryaccess has been exceedingly common secondary to reduced time tohemostasis, decreased patient discomfort, earlier mobilization, and shortenedhospital stay. Although uncommon, use of these devices can leadto a different spectrum of complications, as compared to manual compression. Ischemic symptoms following the use of these devices can have unexpectedclinical sequelae and can occur in a delayed fashion. Awarenessand recognition of such complications is important with the dramaticallyincreased use of these devices in recent years. We report on a case ofdelayed vascular complication manifesting as vascular claudication followinguse of the AngioSeal closure device.

        Production responses of Holstein dairy cows when fed supplemental fat containing saturated free fatty acids: a meta-analys

        Objective: A meta-analysis was conducted to evaluate the effects of supplemental fat containing saturated free fatty acids (FA) on milk performance of Holstein dairy cows. Methods: A database was developed from 21 studies published between 1991 and 2016 that included 502 dairy cows and a total of 29 to 30 comparisons between dietary treatment and control without fat supplementation. Only saturated free FA (>80% of total FA) was considered as the supplemental fat. Concentration of the supplemental fat was not higher than 3.5% of diet dry matter (DM). Dairy cows were offered total mixed ration, and fed individually. Statistical analysis was conducted using random- or mixed-effects models with Metafor package in R. Results: Sub-group analysis showed that there were no differences in studies between randomized block design and Latin square/crossover design for dry matter intake (DMI) and milk production responses to the supplemental fat (all response variables, p≥0.344). The supplemental fat across all studies improved milk yield, milk fat concentration and yield, and milk protein yield by 1.684 kg/d (p<0.001), 0.095 percent unit (p = 0.003), 0.072 kg/d (p<0.001), and 0.036 kg/d (p<0.001), respectively, but tended to decrease milk protein concentration (mean difference = –0.022 percent unit; p = 0.063) while DMI (mean difference = 0.061 kg/d; p = 0.768) remained unchanged. The assessment of heterogeneity suggested that no substantial heterogeneity occurred among all studies for DMI and milk production responses to the supplemental fat (all response variables, I2≤24.1%; p≥0.166). Conclusion: The effects of saturated free FA were quantitatively evaluated. Higher milk production and yields of milk fat and protein, with DMI remaining unchanged, indicated that saturated free FA, supplemented at ≤3.5% dietary DM from commercially available fat sources, likely improved the efficiency of milk production. Nevertheless, more studies are needed to assess the variation of production responses to different saturated free FA, either C16:0 or C18:0 alone, or in combination with potentially optimal ratio, when supplemented in dairy cow diets.

        Denial-of-Service (DoS) attack is one type of epidemic in an internetworking infrastructure that demands an effective detection method. This study presents an algorithm that is modeled from our analysis of results of Activity Profiling and Change Point Monitoring algorithms in detecting five DoS attacks - ICMP Echo Flood DDoS, Ping of Death, UDP Flood, TCP SYN and DDoS. Here, we named the algorithm Exemplar. Additionally, a packet sniffer was developed as a component of Exemplar. Exemplar showed better performance in terms of the number of attacks detected, detection probability and average detection speed.

        Higher Binding Affinity and In Vitro Potency of Reslizumab for Interleukin-5 Compared With Mepolizumab

        Reslizumab and mepolizumab are recently approved monoclonal antibodies for the treatment of severe (uncontrolled) eosinophilic asthma. Both are effective in neutralizing the function of interleukin-5 (IL-5). This study is the first to compare the binding affinity and in vitro potency of both antibodies in head-to-head assays. Two assays assessed binding affinity (using the equilibrium dissociation constant [KD]) of each drug for human IL-5. In the Biacore surface plasmon resonance assay, the association constant (kon) values for human IL-5 for reslizumab and mepolizumab were 3.93 × 106 and 1.83 × 105, respectively. The dissociation constant (koff) values were 4.29 × 10−4 and 2.14 × 10−4, respectively. Calculated KD values for human IL-5 for reslizumab and mepolizumab were 109 and 1,170 pM, respectively, representing an approximately 11-fold stronger binding affinity with reslizumab. In the Kinetic Exclusion Assay, the kon values for human IL-5 for reslizumab and mepolizumab were 3.17 × 106 and 1.32 × 105, respectively. The koff values were 1.36 × 10−5 and 1.48 × 10−5, respectively. Measured KD values for human IL-5 for reslizumab and mepolizumab were 4.3 and 112 pM, respectively, representing an approximately 26-fold stronger binding affinity for reslizumab. A human-IL-5-dependent cell proliferation assay was developed to assess in vitro potency, based on a human cell line selected for enhanced surface expression of IL-5 receptor-alpha and consistent proliferation response to IL-5. The concentration at which 50% inhibition occurred (IC50) was determined for both antibodies. Reslizumab and mepolizumab inhibited IL-5-dependent cell proliferation, with IC50 values of approximately 91.1 and 286.5 pM, respectively, representing on average 3.1-fold higher potency with reslizumab. In conclusion, comparative assays show that reslizumab has higher affinity binding for and in vitro potency against human IL-5 compared with mepolizumab. However, these results do not take into consideration the different methods of administration of reslizumab and mepolizumab.

        Impact of Childhood Exposure to Intimate Partner Violence and Other Adversities

        Childhood exposure to intimate partner violence (IPV) and other adversities has been shown to have adverse effects on health and well-being not only in childhood but also throughout the lifespan. This paper focuses on the prevalence of childhood adversities including exposure to IPV. The intersection of adverse childhood experiences and IPV victimization/perpetration in adulthood is also explored. The neurobiology of trauma is discussed and by understanding the impact of trauma on health, it is hoped that enhancement of resilience is possible. Based on the identification of protective factors at the individual, familial, and community level, examples of interventions that encourage safe, stable, and nurturing relationships between parents and children are described.

        Microarray Analysis of Oral Mucosal Epithelial Cell Sheet

        Corneal epithelium regeneration using autologous oral mucosal epithelial cell sheet is a successful new approach in corneal therapy. In the present study, gene expression profiling was performed to characterize engi-neered cell sheets. Cell sheets were obtained by culturing isolated rabbit oral mucosal epithelial cells on a thermo-responsive cultureware (UpCell®, CellSeed Inc. Japan). H&E staining of cell sheets showed a multistratified epi-thelium, similar to corneal epithelium. DeltaN-p63 stained positive in the basal cells, indicating that cell sheets have renewal capacity. Microarray analysis of these cell sheets showed that only 160 genes out of 43,000 rabbit probes listed on the microarray chip were identified. We first identified the extracellular matrix group of genes and found that matrix metalloproteinase MMP-1, MMP-3 and MMP-12, known to promote angiogenesis, were down regu-lated, while MMP-13 and collagen type VIII alpha 1 (COL8A1), proteins involved in wound healing, were up reg-ulated. Tissue inhibitors of metalloproteinase TIMP-1 and TIMP-3, anti-angiogenic factors, were also identified. Gap junction protein A7 (GJA7 or Connexin 45) was found up regulated, indicating that cell sheets have developed well preserved cell-cell interactions. Alcohol dehydrogenase 5 (ADH class III) and aldehyde dehydrogenase (ALDH1A1), involved in protecting the cornea against oxidative stress induced by UV radiation, were also found up regulated. In conclusion, microarray analysis has led us to identify new target molecules and their subsequent biochemical analysis indicated how the composite cell sheets are advantageous to the original isolated cells in terms of the integrity and potency of corneal epithelial grafts without any scaffolds.
