        To evaluate the success rate, complication and restenosis rate of percutaneous transluminal coronary angioplasty (PTCA), we analized the result of PTCA for 153 lesions in 109 patients admitted to TaeJeon Eul Ji hospital between March 1993 and December 1994. The results were as follows ; 1) The PTCA was successful in 147 of 153 lesions, the success rate was 96%. The success rate was significantly higher in subgroups with type A (98%) than type B lesion (87%). 2) The PTCA was failed in 6 of 153 lesions (4%). The causes of PTCA failure included guidewire passage failure in 4, balloon passage failure in 1 and acute closure in 1 lesion. 3) The PTCA complications were death in 1 (0.9%), acute myocardial infarction in 1, ventricular tarchycardia in 2, cardiac tamponade in 1 and bleeding in 2 patients. 4) The follow up coronary angiography was performed in 83 of 153 (54%) for a mean follow up duration 7.1 months. The restenosis was observed 37 of 83 lesions, the restenosis rate was 45%. The angiographically restenosis rate was significantly higher in subgroup with over 60 years old (60%), male (50%), diabetes (67%), type B leion (59%), and over 10% residual stenosis (57%). I conclude that the PTCA is an effective and safe revascularization therapy that has a high success rate and low incidence of complications in selected patients with the coronary artery disease. The limitation of PTCA is the restenosis, so I should make a study of reducing the restenosis after PTCA.

        Temporal trends and in-hospital outcomes of primary percutaneous coronary intervention in nonagenarians with ST-segment elevation myocardial infarction

        Background/Aims: Data regarding the outcomes of primary percutaneous coronary intervention (PCI) for ST-segment elevation myocardial infarction (STEMI) in nonagenarians are very limited. The aim of the present study was to evaluate the temporal trends and in-hospital outcomes of primary PCI in nonagenarian STEMI patients. Methods: We retrospectively reviewed data from the Korea Acute Myocardial Infarction Registry (KAMIR) from November 2005 to January 2008, and from the Korea Working Group on Myocardial Infarction (KorMI) from February 2008 to May 2010. Results: During this period, the proportion of nonagenarians among STEMI patients more than doubled (0.59% in KAMIR vs. 1.35% in KorMI), and the rate of use of primary PCI also increased (from 62.5% in KAMIR to 81.0% in KorMI). We identified 84 eligible study patients for which the overall in-hospital mortality rate was 21.4% (25.0% in KAMIR vs. 20.3% in KorMI, p = 0.919). Multivariate analysis identified two independent predictors of in-hospital mortality, namely a final Thrombolysis in Myocardial Infarction (TIMI) flow < 3 (odds ratio [OR], 13.7; 95% confidence interval [CI], 3.2 to 59.0; p < 0.001) and cardiogenic shock during hospitalization (OR, 6.7; 95% CI, 1.5 to 30.3; p = 0.013). Conclusions: The number of nonagenarian STEMI patients who have undergone primary PCI has increased. Although a final TIMI flow < 3 and cardiogenic shock are independent predictors of in-hospital mortality, primary PCI can be performed with a high success rate and an acceptable in-hospital mortality rate.

        <P>Indium sulfide nanoparticle (NP)-embedded microporous carbons co-doped with S- and N-dopants are easily prepared by a direct carbonization of the as-prepared In(iii)-based metal-organic framework (In-MOF), [Et2NH2][In(tdc)2]·DEF, containing ditopic S-containing 2,5-thiophenedicarboxylate (tdc<SUP>2−</SUP>) bridging linkers as a potential source of S-dopant. The charge on the anionic framework of [In(tdc)2]<SUP>−</SUP> is balanced by Et2NH2<SUP>+</SUP>, which is also a potential N-dopant. Simultaneous embedding of In-based NPs, S-, and N-co-doping is achieved in a simple single step carbonization of In-MOF. Three porous carbon materials (PCMs), PCM-700, PCM-800, and PCM-900, are obtained from the carbonization of In-MOF at 700, 800, and 900 °C, respectively. The gas sorption analysis indicates them as good CO2 sorbents. The photocatalytic degradation of methyl orange by PCMs under visible light irradiation is also effectively operable owing to the photocatalytically active semiconducting indium sulfide NP with a small bandgap. The main component of indium sulfide NPs is revealed as In6S7 based on the powder X-ray diffraction pattern. Small amounts of metallic In and In2S3 are also observed. The specific capacitances of PCMs are also estimated from the galvanostatic charge/discharge curves. PCM-900 exhibits the highest gravimetric specific capacitance of 99.0 F g<SUP>−1</SUP> at a current density of 0.05 A g<SUP>−1</SUP>.</P>

        "빈 현대"의 데카당스 극복

        In dieser Arbeit wird versucht, die hthetischen Werke van Schnitzler, Beer-Hohann, Andrian, Hohannsthal und Altenberg im Kontext der europaischen kadence-Literatur umfassend darzustellen. Zuerst wird gezeigt, wie der norddeutsche Naturalismus von Hauptmann, die skandinavische Modeme von Ibsen, Jakobsen und Maeterlinck sowie der franzosische Impressionismus und Symbolismus von Huysmus, P. Bourget und M. Barres in der Wiener Modeme rezipiert wurden. Der die Sachenzustiinde der Welt realistisch abbildende Naturalismus wurde in Wien positiv als eine Darstellung des ganzen Lebens gewertet, dann die die geheimen existentiellen hgste der modemen Menschen psychoanalytisch darstellende Literatur der skandinavischen Modeme als faszinierendes Weuland der Seele entdeckt, und schlieBlich die kein Sinngefige mehr zeigende morbide Welt des franzosischen Impressionismus und Symbolismus als Diagnose der problematischen Situationen der modemen Zeit empfunden. Es wurde jedoch gleichzeitig davor gewamt, daB die Decadence-Literatur den Verfall verkliiren konne. Man distanzierte sich von dem dilettantischen Asthetizismus und wollte ihn dwch die ethische Haltung zugunsten eines gesunden Lebens ubenvinden. Es folgt dam in dieser Hinsicht die Analyse der Literatur der Wiener Modeme. In den Dramen Schnitzlers zeigt sich, daB die hthetisch lebenden Figuren nicht in der Lage sind, einen echten Dialog miteinander zu fihren. Die psychoanalytische Herangehensweise IaBt uns die kritische Sicht des Autors auf die Krise der Modeme vermuten. Auch in Beer-Hofmanns Novelle Der Tod Geogs kommt ein iisthetisch lebender Mann vor, der kaum eine Kommunikation mit anderen Menschen f&rt. Seine subjektiven Wahmehmungen erweitem sich zu einer Phantasiewelt. In seiner Phantasie erlebt er jedoch den Tod seiner imaginierten Frau und dann den realen Tod des Freundes Georg. Diese Erfahrungen mit dem Tod als ontologische Bedingung des Menschen lassen ihn die Wichtigkeit einer Gemeinschafi, m der auch er gehort, erkennen. In Andnans Edhlung Der Garten der Erkemblis lebt der junge Erwin von der Sehnsucht nach der Erkenntnis gewisser Wahrheiten getrieben. Er ahnt die Wahrheiten in allen schonen Erscheinungen des Lebens wie Kunst, Religion, Nab, Wissenschafi usw. Er gelangt dann zur Erkenntnis, dass das Leben, das in ihm selbst liegt, die einzige Wahrheit ist. Und er hort im Traum alle Menschen ihn bei seinem Namen den und stirbt. In den lyrischen Dramen Hofmansthals widerfahrt haufig der iisthetisch lebenden Hauptfigur plotzlich der Tod als unvermeidliches Schicksal. Im Angesicht des Todes erkennt sie die Notwendigkeit, ihre Identitat in der Gesellschafi neu zu definieren. In den Prosaskizzen Altenbergs interessiert sich der iisthetisch denkende Eaihler als Personifikation des Autors selbst fk das Seelenleben von normalen Menschen. Die Menschen leben alle von der unstillbar aufsteigenden Sehnsucht nach einer Verbindung mit ihren geliebten Mitmenschen getrieben. Diese Sehnsucht ist meiner Ansicht nach in der Lage, die Menschen zu einer Gemeinschaft zu binden. Zusammenfassend liiat sich sagen, dal3 die Literaturen der Wiener Modeme nicht der europaischen Decadence verfielen, sondem sich kritisch davon distanzierten.

        조선시대와 대한제국기 國璽의 변화상

        'Guksae' means state seal. It is the seal of ruler of a country and symbol of the country. The term'guksae' has been used in Korea since the period of the Three States and the concept hasn't changed. Some records in Joseon era indicates that guise was installed and reverberated from'Ming' and'Ching' and some diplomatic documents in relation with China had cases that used it. In this paper I collected seal shape of Joseon guksae and readjusted relevant documents letters and type style and period of use. Moreover I briefly reviewed about making of guksae due to changes of diplomatic relationship with Japan during Gabogyeongjang period and the status of empire during Korean Empire period with change pattern of guksae. 'Joseon-gukwang-ji-in (Joseon King's Seal)' with probation was the first guksae that received from Ming in June 1401. Including this Joseon Dynasty received guksae from Ming and Ching for three times each. I found out about Joseon era's guksae by arranging reverberation period relevant documents jewel documents type style and period of use of each guksae. The three guksae received from Ming had almost similar forms and the guksaes received from Ching had changes of pattern with Chinese letter and Manchurian letter. The guksae received in November 1637 had'Joseon-gukwang-ji-in' engraved with Manchurian letter and the second one received in 1653 had Chinese letter and Manchurian letter in one seal. Meanwhile I found out from'Yeongjodae' record that Joseon made and used a seal before receiving the first guksae from Ming. The jewel letter was'Joseon-wang-bo (Joseon King's Jewel)' and it was recorded in'Gang-sun-yong-gyo-ji' that is possessed by'Gyujanggak' and with information about its size type style and spot. The guksae was made during'Taejo' era when no precedent security case of Goryeo'S guksae so this information could show the history of Goryeo in actual object. Around Gabogyeongjang period Joseon ended the relationship to serve China and abolished guksae installation system but made its own guksae and used it. Moreover Joseon's relationship with Japan was also changed so the existing custom to use'Wi-jeong-yi-deok' in the country's book was abolished but new guksae was made and used. Total number of guksae became with nine subjects. There were some changes from the jewel letter in guksae reverberated in existing supremacy ; the letter'in (seal)' was changed to letter'bo (jewel)' and the installation name was changed from'Joseon-guk-wang (King of Joseon)' to'Dae Joseon (Great Joseon)' as the country name. The handle had'guyu (turtle)' shape as previous one. Gojong made a new guksae which was suitable for the empire when he established Korean Empire. Three types of guksae including'Dae-han-guk-sae (Korean guksae)','Hwangje-ji-sae (Emperor's guksae') and'Hwangje-ji-bo (Emperor's jewel)'. There are some changes from the guksaes received from Ming and Ching. First the handles were changed from'Guyu (turtle)' to'Yongyu (dragon)' shape. The jewel letter was changed from'in (seal)' to'guksae (state seal)' and'bo (jewel)'. The type style was also changed from complex'Cheobjeon' to simple'Sajeon' style. The changed guksae reflected the position which was changed from a feudal country to an empire. Moreover some internal palace diplomatic document implied that a guksae called'Hwangje-eo-sae (Emperor' seal)' was existed although it was not in records.

        Shaphylococcus enterotoxin B와 lipopolysaccharide를 작용시킨 사람 섬유아 세포에서 생성된 Transforming Growth Factor-β₁의 정량적 분석

        TGF-β1 is a potent chemotactic factor for inflammatory cells and fibroblasts. It also stimulates the celluar source and components of extracellular matrix and the production of proteinase inhibitors. Collectively, these biologic activities lead to the accumulation and stabilization of the nascent matrix, which is vital to infection control. The objective of this study is to investigate production of TGF-β in vitro fibroblast culture in the presence of Staphylococcus enterotoxin B(SEB) and/or lipopolysaccharide(LPS) and to elucidate the role of TGF-β1 which may be responsible for infection control. The fibroblasts were originated from gingiva and facial dermis in 26 year-old male patient. In the presence of LPS(0.01㎍, 0.1㎍, 1.0㎍), SEB(0.01㎍, 0.l㎍, 1.0㎍) respectively, cells(5×103ml) were cultivated in vitro. At 1, 3, and 5 days after incubation, cells were counted. Also, cells(2.5×105ml) were cultivated in EMEM with LPS(0.01, 0.1 and 1.0㎍), SEB(0.01, 0.1 and 1.0㎍) respectively and LPS(0.1㎍) and SEB (0.1㎍) in combination for 24,48, and 72 hours respectively. Culture supernatants were harvested at 1, 2, and 3 days after incubation period and triplicate culture supernatants were pooled and TGF-β1 was assayed in duplicate. The results were as follows. 1.In gingival fibroblast induced with SEB and LPS respectively or in combination, the suppression of cell Proliferation occurred very significantly since 3 days after incubation, compared with the control and the production of TGF-β1 occurred very significantly at 1 day after incubation, compared with the control. 2.In facial dermal fibroblast induced with SEB and LPS respectively or in combination. the suppression of cell proliferation occurred very significantly at 1 day after incubation, compared with the control. In SEB exposure, the production of TGF-β1 was decreased very significantly at 1 day after incubation, compared with the control. However, in LPS, SEB and LPS exposure, the production of TGF-β1 was increased very significantly at 1 day after incubation, compared with the control. In conclusion, the concentration of bacterial toxins and the incubation period correlated with cell proliferation and production of TGF-β1 very significantly. The gingival and facial dermal fibroblasts have different phenotype each other The orchestrated understanding of fibroblast proliferation and TGF-β1 production play an important part in host defense against the bacterial Infection and may prevent tissue necrosis such as necrotizing fasciitis and life-threatening syndrome such as multiple organ failure.

        제주시 일부 초, 중, 고등학생과 대학생들의 생체전기 저항법에 의한 체성분 조사

        Purpose : obesity is a frequent nutritional disorder in children and adolescent and its prevalence is increasing in Korea. The aim of this study was to measure body composition using bioelectical impedence in primaty, middle, high and college students in Cheju. Methods : We measured height, weight, and body composition(Fat %, WHR, Fat feee mass) by InBody 3.0 in 1,431 students aged 10-25 years. We also examined degree of obesity comparing standard weight for height, body mass index, and precent of body fat. Results : Fat free mass increased by aging in both sex, higher in male. Fat mass was highest in Primary school in male(12.09±4.86kg), college in female(16.03±4.22kg). Percent of body fat was highest in Primary student(21.64±8.02%), decreased middle(19.96±7.26%), high school(15.07±5.13%), and increased in college student(17.33±5.21%) in male, however, lowest in primary student(22.78±6.10%), increased by aging (middle;25.38±6.19%, high;27.61±5.27%, college;28.27±4.49%) in female. Percent of body fat was significantly higher in female except in primary school age. The prevalence of obesity based on fat % was highest in primary school student(primary, middle, high, college student; 33.3%, 23.3%, 7.6%, 8.2%) in male(≥25%), and highest in college student(11.1%, 26.6%, 32.3%, 38.6%) in female(≥30%), whereness, based on a standard weight for height(≥20%), 13.16%, 10.84%, 6.90%, 10.10% in male, 4.44%, 7.21%, 12.11%, 9.59% in female. Conclusion : It was necessary to evaluate obesity degree by body fat percentage, especially in adolescent and young female

        제주도 고등학교 학생들의 비만 정도, 심혈관 위험인자, 소아시 비만과의 관련성

        Background: Recently, adolescent obesity was increased and tended to become obese adults, facing increased risk for diabetes, cardiovascular disease. Even before reaching adulthood, adolescent obesity already was experiencing hypertension, dyslipidemia, and diabetes mellitus. The purpose of this study was to find prevalence of obesity of high school students in Jeju island and association between obesity and cardiovascular risk factors and association between childhood obesity and adolescent obesity. Method: The study subjects were 6,064 students of twenty-two high school in Jeju island and health survey data was gathered from high school health examination data and hospital health examination data from Aug. 1999 to Dec. 1999. Adolescent obesity was evaluated by ideal body weight method in 1998 and body mass index. Cardiovascular risk factors were checked total cholesterol, systolic and diastolic blood pressure, fasting blood sugar. In 1,534 students, obesity at primary school is checked by primary and middle school health record. Results: Prevalence of obesity in Jeju island high school students is 12.6% in male students, 13.3% in female students by ideal body weight method and 8.7% in male students, 8.5% in female students by body mass index. Prevalence of obesity of male students in city area and district area of Jeju island were each 12.9%, 10.3% and that of male students in city area was significantly higher. Prevalence of obesity of female students in city area and district area of Jeju island were each 12.9%, 14.2% and not significantly different. Prevalence of obesity of male students in general and commercial school group were each 13.9%, 10.3% and that of general school group was significantly higher. In female students, Prevalence of obesity general and commercial school group was not significantly different. Total cholesterol, systolic and diastolic blood pressure were significantly associated with degree of obesity in male and female students(P<05) and fasting blood sugar was only significantly associated with degree of obesity in male students(P<05). In 1,514 students checked by health record, Odds ratio of obesity at primary and middle school that at high school were 12.96~22.77 and was significantly associated. Conclusion: Prevalence of obesity of high school students in Jeju island is very high and adolescent obsity was significantly associated with cardiovascular risk factors. Mostly childhood obesity tended to become adolescent obsity and program of control of obesity at childhood and adolescence is required.

      • 소아에서 발생한 골수이형성 증후군 2례

        Myelodysplastic syndrome(MDS) is a group of acquired conditions characterized by progressive bone marrow failure associated with normocellular or hypercellular bone marrow that cannot be attributed to nutritional deficiency, chronic infection, or other chronic systemic illness, and cannot be reversed by the successful treatment of those conditions. Whereas the cases of secondary MDS occur at all ages, primary MDS is essentially a disease of elderly, though no age group is exempt and predomonant in men. The MDS is fundamentally clonal disorders at the level of the haemopoietic totipotent stem cell. Clinically, they are characterized by varing degrees of peripheral cytopenia(s) with morphological and functional abnormalities of blood elements. We have experienced two unrelated pediatric cases, who showed pancytopenia on admission and the characteristic marrow findings of MDS. The first case, a 12 months-old girl, showed increased reticulin with abnormal localization of immature precursor cells(ALIP) in her bone marrow, in addition to dysplastic features such as bi-and multi-nucleation of late normoblasts and internuclear bridging. She also showed 22% of blasts in her bone marrow cells thus classified in the category of RAEB-t after FAB classification. Also found were a chromosomal abnormality namely 46 XX t(14q21q), which was an unusual one. Her family brought her home and, 4 month later, she deceased. The second case, a 12 year-old boy, showed pancytopenia with Pseudo-Pelger-Huet anomaly in neutrophils but no blasts in the peripheral blood. Bone marrow of this case also showed the picture of dyserythropoiesis with binuclearity, internuclear bridging, nuclear fragments, multinuclearity of late normoblasts, in addition to megaloblastoid change in the normoblasts and increased mitosis in granulocytic series. No chromosomal abnormality was found. Classified as RA after FAB classification of MDS. Showed good response to conservative management for anemia for more than one year.

        스티렌 폭로 근로자들의 기중 및 혈중 스티렌과 요중 만델산의 관계 분석

        The concentration of styrene in air in blood and mandelic acid in urine were checked for the 60 workers with normal liver function, exposed to styrene. Styrene in air were sampled with personal air sampler at least 4 hours and analyzed by gas chromatography. Blood and spot urine were collected at the end of shift with a vacuum tube and a polythylene bottle and analyzed by gas chromatography and high performance liquid chromatography. Means of air and blood styrene and urine mandelic acid were 8.16 ppm(geometric mean), 0.199 mg/L, and 0.519 g/g creatinine, respectively. The concentration of styrene in air and mandelic acid in urine were high in the FRP factories and low in polymerization factory. Styrene in blood showed large difference by the working process. Styrene in air showed a good correlation with mandelic acid in urine(r=0.6369) and styrene in blood(r=0.6371). The mandelic acid in urine and styrene in blood corresponded to exposure of 50 ppm styrene were 0.890 g/g creatinine and 0.434 mg/L. However, hippuric acid in urine did not show any correlation with styrene in air. Urine mandelic acid excretion expected ratio showed a tendency to decrease according to obesity index and to increase with alcohol consumption.
