        간호 · 간병통합서비스 병동 간호사의 조직몰입과 환자안전문화인식이 환자안전간호활동에 미치는 영향

        Purpose: The purpose of this study is to identify the effects of organizational commitment (OC) and perceived patient safety culture (PPSC) on patient safety nursing activities (PSNA) among nurses in comprehensive nursing care units. Methods: Participants were 173 nurses working at five general hospitals in Chungcheong area. Data were analyzed using descriptive statistics, x<sup>2</sup> test, t-test, ANOVA, Pearson’s correlation coefficient, and multiple regression analysis with SPSS/WIN 23.0 programs. Results: The mean scores of the OC and PPSC were 3.28±0.50 and 3.85±0.35, respectively. The mean score of PSNA was 4.55±0.41, and PSNA was significantly different by the experience of participating in hospital’s safety culture campaigns (t=2.70, p=.008). The results of the multiple regression analysis showed that ‘patient safety knowledge and attitudes’ (β=.27, p=.006) and ‘unpunished environment to error’ (β=.22, p=.004) as the sub-categories of PPSC were affecting factors on PSNA with an explanatory power of 26.0% (F=6.40, p<.001). Conclusion: The results of this study suggest that in order to promote PSNA among nurses in comprehensive nursing care units, it is necessary to develop a program to enhance patient safety-related knowledge and attitudes. In addition, the hospital's organizational efforts such as operating safety campaigns and creating an unpunished environment to error should be needed.

        자폐적 행동 양상을 보이는 아동과 어머니간의 애착 발달에 관한 분석적 연구

        This study was conducted to examine the feature of attachment and the main factors of attachment development between autistic child and his or her mother. Data was collected at Day Care Center for Children with Developmental Disorder in S National University Hospital through nurses's in S National University Hospital through nurses's participating observation, interview with the mothers, and review of nursing records, from April 2 to June 30, 1994. The subjects were all 6 children, one was a girl, the others were boys, and their mothers who participate at the sensory-integrative play for attachment promotion served at this center. Content analysis was done for data analysis. First, all data were categorized by main concepts, reciprocity and affect. Then these wee analyzd and discussed on the aspects of specificity, proximity, synchrony, joint attention, strong affect, emotional availability which were chosen as the components of attachment through literature review. The results of this study were summarized as follows : all subjects were underdeveloped, unstable and showed different level of mother-child attachment. But during the program their attachment behaviors improved little by little. The behaviors which represented specificity, proximity, and strong affect were increased considerably, on the other hand the behaviors which represented joint attention and emotional availability were scarce. The more mother and child had contact and interacted merrily, the more their attachment behaviors developed. Another important factors of attachment development in these cases were mother and child's each characteristics. Especially the child's autistic peculiarity, such as indifference, severe resistance, distracterbility, stereotype tendency, physical condition were major barriers of maintatining the mother-child interaction. Also mother's attitude to her child, raring skill and physical condition were influencing factors. A few mothers who were anxious and stubborn had much more difficulty to adjust to their children. The mother who had experienced happiness with her child, especially if the mother experienced more fun at the play by herself became more positive, active and flexible, and that mother-child interaction maintained much longer and improved. It was more critical to help them enjoy their interaction. Thus nurses had to plan and intervene more joyful and various play, and consider mother's need in addition to child's need. And The program should be strengthened to support the mother emotionally to maximize the therapeutic effect.

        자폐아동을 위한 모-아애착증진 프로그램의 효과

        자폐아동을 위해 개발된 모-아애착증진 프로그램의 효과를 평가하고자 참여군과 비참여군의 변화를 비교하였다. 대상자는 S대학교병원 애착증진 프로그램에 참여하는 자폐아동과 어머니 7쌍, 참여하지 않는 자폐아동과 어머니 4쌍이었다. 6개월 간격으로 2회, 비디오 촬영법 등을 통해 자료를 수집하였으며 Wilcoxon Rank(Sign) Sum Test로 모-아애착, 양육행위, 자폐적인 행동 특성의 변화를 비교 분석하였다. 그 결과 참여군의 애착행동의 변화는 같은 기간 비참여군의 애착행동의 변화보다 유의하게 컸으며 일치성을 제외한 근접성, 상호성, 정감성에서 양적으로나 질적으로 긍정적 변화를 보였다(p<.05). 접촉유지, 차별적, 애정적 행동이 크게 증가하였으나 공감적, 일치적, 합동주시 행동의 변화는 적었고 아동간에 개별차가 컸다. 아동의 성장발달을 위한 어머니의 양육행위도 참여군에서 크게 증가하였으나 그룹간 차이는 유의하지 않았다(p<.05). 애착행동의 증진은 또한 자폐적 행동 특성에 긍정적 영향을 미쳐 모-아애착 증진 프로그램이 자폐아동들의 사회적 행동의 증진 뿐만 아니라 문제 행동의 개선에도 유용한 조기 중재 프로그램으로서의 가능성을 보였다. 중심 단어 : 자폐아동ㆍ애착ㆍ모-아애착증진 프로그램ㆍ아동의 성장발달을 위한 양육행위. Objectives : This study was conducted to evaluate the effects of the attachment promotion program which was designed to promote attachment between autistic children and their mothers. The program was consisted of play activity centered mother-child interaction, educational acitivity for mothers by lecture, video-feedback session, and emotional supports. Methode : The subjects were consisted of 11 pairs of mother and child(7 for participant group : PG, 4 for non-participant group : NPG), who were 2-4 years-old autistic children. The data were collected by video-taping, behavioral observations, and analyzed by Wilcoxon Rank(Sign) Sum Test and Content Analysis. Results : The total attachment score increased from pre test to post-test of PG was significantly higher than that of NPG. The scores of attachment behaviors indicating proximity, mutuality and affect increased signiscantly(p<.05), while some behaviors indicating synchrony and joint-attention did not. Especially the affectionate behaviors of PG demonstrated the most notable changes and they became more active, voluntary and reciprocal. After the program, the mother's nurturing behavior of PG increased more than that of NPG, with no significant difference between two groups. The mother's behaviors to her child of PG became more child-centered, positively responsive, supportively expressive. As the attachment was promoted, there were also positive changes in the children's autistic behaviors with a great deal of individual differences. Conclusion : Our findings suggest that Mother-Child Attachment Promotion Program could be an effective early intervention model for autistic children.       • 자폐아 간호 : 사회적 발달증진을 위한 간호과정

        This study was conducted to put an appropriate caring focus of nursing for autistic children by reviewing literature. Autistic children have lots of social disturbances. These social impairments are basic barriers to living with other people and adjusting themselves to circumstances. In this respect, nurses should help autistic children effectively by applying nursing process. First, social skills and pathological behaviors have to be measured qualitatively and quantitatively. Second the clear nusring goal has to be set, then the plan for structuring routine and special activity, play priority, nursing delivery consistency has to be established. Also nurses have to plan for selection of toys, participation of family members. Third, effective intervention for these children has to be given with some consideration. It needs natural and slowly progressive approach according to optimal level of each child. Finally, nurses have to evaluate progress and feedback regulary with associate nurses and family. Promotion of social development needs further effort and creativity in development of assessment tool and natural and therapeutic play. Especially nurses must try to help promote early social development and expand nursing role and function at community and home care setting, as well as clinical nursing.

        병리검사 처방오류 개선 시스템 구축

        문제: 조직병리검사의 처방이 복잡하고 전문화됨에 따라 오류처방이 빈번하게 발생되고 이를 해결하기위한 프로세스가 복잡하여 부서간의 갈등과 업무의 가중, 또한 병원 수입 감소가 초래되었다. 목적: 발생된 오류처방의 유형과 발생율, 오류처방을 해결하기 위한 프로세스를 파악하여 이에 대한 문제점을 개선하여 업무의 효율성과 관련부서간의 부가업무 감소와 갈등해소, 더불어 병원수익을 향상시키는 것에 목표를 두고 활동하였다. 의료기관: 서울시에 소재한 종합병원 질 향상 활동: 조직병리검사 처방의 오류처방 발생 원인과 유형, 발생율을 조사하여 원인을 파악하고 개선을 위한 프로세스를 구축하여 질 향상을 도모하였다. 개선효과: 수술실에서 의뢰한 외과 입원환자의 일반조직병리검사 오류율은 개선 전 7.8%에서 개선 후 3.5%, 동결절편조직검사 오류율은 개선 전 28.6%에서 개선 후 0%로 이전보다 모두 개선되었다. 또한 수술실 간호사의 업무 만족도는 일반조직병리검사의 처방시 95%, 동결절편조직검사 처방시 90%로 높은 만족도를 보였다. 단 병리과의 경우 업무가 증가됨에 따라 초과근무시간이 증가하였다.

      • 정신간호 실습학생의 환자와의 치료적 관계에 있어 性과 관련된 문제점에 대한 연구

        The purpose of this study was to determine the prevalence of sexual harassment of nursing students from patients in the psychiatric unit and to get basic data for efficient guidline to deal with those problematic situation. The subjects of this study were 215 nursing students selected from 2 junior colleges and 2 nursing colleges. The data were collected by means of a self-reported questionnare from Sep. 22 to Dec. 30, 1994. Data analysis included percentage, X^(2)-test, Pearson Correlation. The results were as follows; A total of about 68% students had experiences of being harassed by sexual behavior from patients. The most frequent means of harassment was physical touch, sexual innuendo, etc. There were no significant correlation between the occurrence of sexual harassments and characteristics of respondents, clinical situation. Most of students with this experiences had negative feelings and appraisal of the patient. Especially about 40% students coped with those situations ineffectively. These results suggest that the students should learn to get more practical and concrete coping skill for sexual harassments through clinical conference.

        A study on the standard of nursing practices. In order for new nurses to increase rapid adaptation and realistic nursing practice abilities in a new clinical setting, it is essential that there be a congruence with contents and methods of nursing practice education between the nursing education agency and clinical nursing practice managers. Therefore, a research on standard nursing practice for new nurses was conducted in order to induce common understanding about nursing practice between instructors of nursing college and head nurses of hospital, before opening 500∼700 bed sized hospital which used clinical practice setting for this nursing college and main working field after graduation. To achieve the above research purposes, nursing practice education materials for new nurses of two affiliated hospitals and four 500∼700 bed sized hospitals were collected. And then the contents of 6 materials were categorized according to theme, afterwards new contents of education materials were selected through discussions among the 5 researchers. Ultimately, the standard of nursing practices for new nurses was developed in hospital with 500∼700 beds.
