The purpose of this research was to understand the structure of the lived experience of a child terminally ill with cancer. The research question was "What is the structure of the experience of a child terminally ill with Cancer?" The sample consisted of 17 children admitted to the Cancer units of two university hospitals in Seoul. The unstructured interview were carried out from January, 1992 through December, 1992. They were audio -recorded and analyzed using Van Kaan's method. There are different views on the causing factors of Cancer. Irregular daily habits, fatigue, weak, physical condition, disease, anger, tension, guilt feeling, air pollution and unhealthy diet are considered to be related factors. The reaction to Cancer can be varied : fear, despair, avoidance, acceptance, apathy, grief, anger, anxiety over medical expense. Over a period of illness, these children experience side effects of medicine, acute pain, loss of appetite, withdrawal of activities, feeling of loneliness or isolation, loss of interest in communication, search for a religion and support from religious leaders. The children with Cancer have observed various changes in family relationships. The expressed desires of these children include health, happy family, possession of desired things, religious expectations, being useful to other people. The nurse, by providing empathetic support, should be a person with whom childrens can express their feelings and share their experiences.
The Role of Customer Values in Increasing Tourist Satisfaction in Gianyar Regency, Bali, Indonesia

Customer value has long been believed to be a direct trigger for increased tourist satisfaction, but as a mediating variable, it still needs to be proven further. This paper aims to examine the causal relationship between research variables, as well as to examine the role of customer value as a mediating variable in the relationship between service quality, brand quality, tourism products, customer value, and tourist satisfaction with tourists’ objects. The population is tourists who visit tourist sites/destinations in the Gianyar Regency on the island of Bali, Indonesia, and the sample size is 270 respondents, selected through random sampling. Structural equation modeling (SEM), a multivariate statistical analysis technique, is used to analyze the causal relationships between variables. The results show that the model is accepted, and customer value is proven to be a positive mediating variable. The results also show that service quality, brand quality, and tourism products have an effect on customer value. This provides insight into the practical implications for tourism managers to increase the brand quality of tourist attractions as well as increase the professionalism and quality of tour guide services. This, in turn, will increase customer value and increase tourist satisfaction.

<P>Surprisingly high monomer selectivity was demonstrated in competitive radical addition with two kinds of methacrylates carrying sodium and ammonium cation. Crucial is size-specific recognition by a lariat crown ether embedded close to the reactive halide in a designer template initiator. Especially, a combination with an active ruthenium catalyst led to outstanding selectivity at low temperature. RIKEN Structural Genomics Beamlines have been constructed for the crystallographic analysis in the structural genomics research at synchrotron radiation facility SPring-8. Synchrotron radiation accelerates the crystallographic analysis of protein structure. The target of the research and development is focused on the automatic beamline operation to maximize beamline efficiency. We are developing the sample management system, which is composed of the sample auto-changer and the database system, for high-throughput data collection. The sample management system and the beamline operating system make it possible to execute automatic data collection without any operators. The beamlines will be ready for user operation in autumn 2002. The concept of automatic beamline operation and the present status of RIKEN Structural Genomics Beamlines will be presented.

Purpose: Postoperative pancreatic fistula is a serious and fatal complication of gastrectomy for gastric cancer. Blunt trauma to the parenchyma of the pancreas can result from an assistant's forceps compressing and retracting the pancreas, which in turn may result in pancreatic juice leakage. However, no published studies have focused on blunt trauma to the pancreas during laparoscopic surgery. Our aim was to investigate the relationship between compression of the pancreas and pancreatic juice leakage in a swine model. Materials and Methods: Three female pigs were used in this study. The pancreas was gently compressed dorsally for 15 minutes laparoscopically with gauze grasped with forceps. Pancreatic juice leakage was visualized by fluorescence imaging after topical administration of chymotrypsin-activatable fluorophore in real time. Amylase concentrations in ascites collected at specified times was measured. In addition, pancreatic tissue was fixed with formalin, and the histology of the compressed sites was evaluated. Results: Fluorescence imaging enabled visualization of pancreatic juice leaking into ascites around the pancreas. Median concentrations of pancreatic amylase in ascites increased from 46 U/L preoperatively to 12,509 U/L 4 hours after compression. Histological examination of tissues obtained 4 hours after compression revealed necrotic pancreatic acinar cells extending from the surface to deep within the pancreas and infiltration of inflammatory cells. Conclusions: Pancreatic compression by the assistant's forceps can contribute to pancreatic juice leakage. These findings will help to improve the procedure for lymph node dissection around the pancreas during laparoscopic gastrectomy.
Capsule endoscopy in inflammatory bowel disease: when and how

Capsule endoscopy (CE) is emerging as an important investigation in inflammatory bowel disease (IBD); common types include the standard small bowel CE and colon CE. More recently, the pan-enteric CE was developed to assess the large and small bowel in patients with Crohn’s disease (CD). Emerging indications include noninvasive assessment for mucosal healing (both in the small bowel and the colon) and detection of postoperative recurrence in patients with CD. Given the increasing adoption, several CE scoring systems have been specifically developed for IBD. The greatest concern with performing CE, particularly in CD, is capsule retention, but this can be overcome by performing cross-sectional imaging such as magnetic resonance enterography and using patency capsules before performing the procedure. The development of software for automated detection of mucosal abnormalities typically seen in IBD may further increase its adoption. (Intest Res 2020;18:265-274)
