      • 재발 또는 불응성 비호즈킨 림프종 환자에서 CDME 구제항암화학요법 후 고용량 항암화학요법 및 자가말초혈액 조혈모세포이식의 효과

        Background and objectives : The long-term survival in patients with non-Hodgkin's lymphoma (NHL) after conventional dose chemotherapy is about 35% and the rest of the patients tend to have relapse. So, in relapsed or refractory NHL, we compared the outcome of patients undergoing high-dose chemotherapy with autologous peripheral blood stem cell transplantation(APBSCT) with only salvage chemotherapy of cisplatin, dexamethasone, mitoxantrone, and etoposide(CDME). Materials and methods : From June 1993 to December 1999, 25 patients with relapsed or resistant NHL were treated with CDME regimen as salvage chemotherapy. Twelve patients were received four cycles of CDME chemotherapy, and 13 patients were received high-dose chemotherapy with APBSCT following two cycles of CDME chemotherapy. Results : The median follow-up duration was 12.8 months(range:4-68). The overall response rate was 41.7% (complete response rate 25%, partial response rate 16.7%) in 12 patients with CDME only. Thirteen patients who were treated with high-dose chemotherapy with APBSCT achieved 61.5% complete response rate and 15.4% partial response rate, with an overall response rate of 76.9%. The estimated 3-year progression-free survival rate was significantly higher among patients who received high-dose therapy than patients who received CDME only(41.5% vs 20.0%, p<0.05). And, 3-year overall survival rate was significantly higher among patients who received high-dose therapy(51.3% vs 25.0%, p <0.05). Conclusions : In relapsed or refractory NHL, CDME chemotherapy is an effective salvage chemotherapy and allow peripheral blood stem cell collection. Also, high-dose chemotherapy with APBSCT following CDME is superior to CDME salvage chemotherapy only.

      • 개에서 발생한 파열된 전이성 비장 혈관육종 1례

        Twelve-year-old, catrated male, weighting 8 kg, Miniature Schnauzer dog was referred to the Veterinary Medical Teaching Hospital of Chungnam National University with history of depression. Result of blood analysis revealed anemia, thrombocytopenia, azotemia, elevated ALP, ALT and CK. On the radiographic examination, splenomegaly and loss of abdominal serosal detail were observed. Abdominal ultrasonography showed rounded mass of spleen and liver, ascites were observed. Splenectomy was performed and found that splentic tail had been ruptured and two samples from nodules on liver were obtained for histopathology. The result of biopsy revealed that hemangiosarcoma of spleen metastasized to the liver. Carboplatin was chosen as the patient had congestive heart failture. During chemotherapy hemangiosarcoma was metastasized into peritoneum and abdominal cavity. Intraperitoneal chemotherapy was preceded at this moment. After 99 days of initial visit, patient was euthanized due to severe anemia and thrombocytopenia.

        Three-month-old, intact male, weighting 2.6 kg. Dachshund dog visited to the Veterinary Medical Teaching Hospotal of Chungnam National University with history of regurgitation. Result of blood analysis was in the normal range. On the radiographic examination, diffuse esophageal dilatation was observed. On the esophagram, esophageal dysmotility was observed. Acetylcholine receptor antibody titer was in the normal range. On the endoscopy, esophagitis was observed. Diagnosis was congenital idiopathic megaesophagus. This is the case report of congenital idiopathic megaesophagus.

        Types of schools can be classified into 3 type-bureaucratic, communal, political types. But, in reality, the 3 thpes of schools are usually mixed in a school. People concerned in education can be classified into 3 kinds-people planning and managing the education in general(ministry of education, provincial office of education), people teaching students in each level of schools(teachers), people benefited by the edcation (students, parents, inhabitants). Generally speaking, the first group has so called the formal views on it, and the third group has the popular views on it. The current situation of our education in Korea is a confused, unstable situation in which the two educational views (mechanical view, organic view) confronting and conflicting each other and the two educational functions (essential function, instrumental function) confronting and conflicing each other are flowing together in a mixed state. Such situation is influencing each school in which the three kinds of people concerned in our school education insist so strongly their opinions and rights on their own respectively. Such situation requires a multi-dimensional approach in our school management, Such multi-dimensional approach comprises the essential dimension and the instrumental dimensions, and the intrinsic and extrinsic dimensions. Such approach expects in our school educaion l) the cultural relevance 2) the political correspondence 3) the economic efficiency and 4) the educational effectiveness. A school managar in such situation requires l) to establish the self-identity of his own, 2) to exersise the autonomy and control in a reasonable way in his school and 3) to put into practice the essential social behavior norms in the school life. 1) The self-identity can be constructed as such total, comprehensive answers from the following questions. a) Who am I? What am I? b) What am I to do? What am I to be? c) What do they expect me to do? What do they expect me to be? 2) In a school organization, the internal order is the order which the autonomy of its members pursue and the external order is the order which the control of its members pursue. The order which both autonomy and control produce in a school organization is productive in that both contribute to achieving the objectives which the school organization pursue. 3) Whether the situation of a school is bureaucratic, communal or political, the people concerned in the school education have to keep up with such social behavior norms as the following. a) Law-abiding Our democracy is a system based on the "law-abiding." It is the first essential step we all have to take. If we had some unsatisfaction with the current law, first of all, we should try to amend the law, not to violate it. b) Accountability Each of us should discharge one's accountability in a school to which he belongs. First of all, each teacher have to teach his students so sincerely, and each student is required to study as well as he can. c) Dialogue Dialogue means a talking-with each other who has an opponent opinion about a problem. If two persons (groups) have an opponent opinion, they should keep on talking with each other leading to a compromise, harmony and a mutual agreement. Our democracy is based on the mutual agreement. Dialogue makes away with the self-righteousness. d) Open-heartednenss In our democracy we protect one's privacy of an individual from being known. But we hope to make everything public in a public organization. As the proverb goes, "The outside of an absolutism is clean but its inside is unclean. The outside of the democracy is unclean, but its inside is clean." In a public organization, its business including monetary and personnel affairs, has to be open to the public at any time, in any case, if necessary. e) Mutual restraint and cooperation Our democracy never admits one's self-righteousness. The theologian Nieber said, "Democracy is possible because we have the ability to go to the righteousness, and it is necessary because we have the desposition to go to the unrighteousness." Our democracu provides us with the cooperation as well as the check and balance. As we see in the above paragraph, the "law-abiding and accountability" will be more emphatic in a bureaucratic situation of a school, the "dialogue and open-heartedness will be more necessary in a communal situation of a school, and the "mutual restraint and cooperation" will be more requisite in a political situation of a school.

      • 철골 기둥-보 접합부의 구조적 거동에 관한 실험적 연구

        One of the most influential elements is the moment resisting beam-column connection on the view of the behavior and the cost of multistory steel building frames. A majority of these connections are column flange connections attached to beam frames. This is so called strong-axis connection. Another type of moment resisting connection commonly found in building frames is the web connection. This is so called weak-axis connection. In this connection, the beams are attached to the plane of the column web perpendicularly. The action of the bending moment in the beam tends to bend the column about its weak axis. In this study, some of the fundamental behaviors of beam-column connections will be examined through changing the connection details as well as compared with the previous connection details. The purpose of this study is to develop the details in the beam-column connection in weak axis for middle & low-rise steel construction system.

        Whirl Fire와 Pool Fire의 화염 거동에 관한 연구

        4-panel of Im height and 45cm width were fixed on the 40cmx40cm bottom plate and the opening of the panel comer was 5cm. Diameter of stainless vessel is 10em and its height is 2cm and it located at the center of the bottom plate. 78mL liquid fuel was filled in the vessel and its depth was lcm. Flame temperature was measured with K type thermocouple, and radiation heat of flame was measured with heat flux meter. Flame height and its behavior was visualized with video camera. and mass burning rate was measured by fuel combustion time. According to the development of fire, flame swirling was begin. From the experiment the mass burning rate was larger and the height of flame was higher than the usual pool fire flame. Flame temperature and heat flux also increased far more than the pool fire. Consequently the swirling air flow through the openings between the panel and thermal buoyance contribute to increase of heat release rate, flame length and mass burning rate.

        악관절 융기 증강술을 통한 습관성 악관절 탈구환자의 치험례

        Recurrent mandibular dislocation is frequent morbidity of temporomandibular joint relatively. There are many etiologic causes in TMJ disorders but, difficult to find obvious one. Various treatment methods have been utilized for a mandibular dislocation. It is categorized into two groups broadly - nonsurgical or surgical methods. The basic rationale of the surgical method is to allow free movement of the condyle by reducing height of articular eminence or to limit anterior excessive movement of the condyle by increasing height of articular eminence or soft tissue anchoring procedure. In this case, 69 year-old woman was treated by augmentation of the articular eminence with mandibular symphysial bone graft leading to osteosynthesis without difficulty. As a result, favorable postoperative outcome was obtained functionally without any complication or recurrence.

      • TiAIN을 코팅한 WC공구의 절삭성능에 관한 연구

        Thin film of TiAlN was deposited onto ISO P20 tungsten carbide cutting insert tip substrate specimen using the FVAS (filtered vacuum arc system) at the substrate temperature of 80℃. Cutting and wear tests have been performed to the TiAlN coated and uncoated WC specimen, respectively under the same cutting conditions for the comparison. Cutting forces and tool wear of coated and uncoated carbide cutting tools were investigated by means of cutting length and cutting forces. Also tool wear and tool life were analyzed using the 3ch tool dynamometer amp lifier and oscilloscope. In this study cutting tests for the cutting performance of TiAlN coated WC-Co P20 to the steel alloy of SNCN420 have been curried out. Through the comparisons of cutting forces during the cutting tests and tool wear, we got the conclusions as follows.; ① In usual cutting conditions, main cutting force component showed higher value than the thrust force normal to the cutting direction especially cutting forces in the of early stage on cutting tests. But, after loosing the protective hard thin film on substrate or insert tip, the thrust force normal to the cutting direction showed much higher than main cutting force component. ② Within the range of 0.3㎜ build up edge in P20 insert tip, TiAlN coated tip was evaluated more stable in flank wear and less in crater wear than the uncoated specimen. ③ Through the wear growth curve of the specimen, tool life differences of uncoated and TiAlN coated tip at high speed (V=250[m/min]) was higher than the low speed(V=200[m/min]) case, compared to the average but the tool wear was almost same in both specimen for the case of normal wear occurred.

        주의력 결핍 과잉 운동장애에 대한 Methylphenidate의 임상 효과

        The objectives of this study were to evaluate the effect of methylphenidate on attention deficit hyperactivity disorder, using child attention problem checklist to 42(male : 29, female : 13) attention deficit hyperactivity disorder patients from March 1992 to February 1993. The results were as follows : There was significant improvement on symptoms in the scores rated by teachers in 7th & 28th day after methylphenidate administration compared to baseline score(p<0.05) and 28th day score showed significant improvement compared to 7th day scores(p<0.05). There was significant improvement on symptoms in the scores rated by parents in 28th day after methylphenidate administration compared to baseline score(p<0.05) and 28 th day score showed significant improvement compared to 7th day score. In single administration of methylphenidate in Sunday morning, there was significant improvement on symptoms in morning score compared to afternoon score(p<0.05). In the administration of methylphenidate of same dosage, the 7th day rating score of teacher showed significant improvement on symptoms compared to the Sunday morning rating score of parents(p<0.05) and the 28th day comparision was also showed signigicant improvement on symptoms in the scores rated by teachers compared to the scores rated by parents(p<0.05).
