        Background/Aims: Westernization in many Asian countries have changed lifestyles and diets, so once rare diseases have now become prevalent. The aim of this study is to investigate the changing pattern of digestive and liver disease in Korea, from 1990 to 2006. Methods: We extracted data specific gastrointestina (GI) disease based on the International Classification of Diseases code from the in-patients records at the Kangnam St. Mary`s Hospital in 1990, 1996 and 2006. This hospital is a tertiary-care hospital in Seoul, Korea, which has a capacity of 800 beds. Results: The admission rate for GI disease increased between 1990 and 2006. Overall in-patients were 1,623 persons in 1990, 2,368 persons in 1996 and 4,166 persons in 2006. The mean age of in-patients increased as time went by. A stomach cancer was the most common diagnosis during all periods, but its prevalence has decreased. Colon cancer ranked the 7th in 1990, but markedly increased and now ranks the second. The third was a hepatocellular carcinoma. The bile duct and gallbladder cancer, pancreas and esophageal cancer ranking followed with little interval change. In cases of cancer patients, a regular admission dramatically increased for chemotherapy. The number of patients admitted with pre-malignant neoplasm of stomach and colon increased remarkably with the development of endoscopy. The liver transplant, inflammatory bowel discase, and reflux esophagitis emerged form mid-1990s with greater frequencies, yet much below the levels found in the West. The admission rate for peptic ulcer, especially ulcer bleeding remained relatively stable, despite a decreased rate for ulcer perforation. Liver cirrhosis, hepatitis, cholelithiasis with cholecystitis-cholangitis, appendicitis, hemorrhoid and gastritis all decreased. Conclusions: The stomach cancer is the leading cause of admission, despite a recent decline. Colon cancer showed a marked rise. The liver transplant, inflammatory bowel disease and reflux esophagitis were new diagnosis with an increased tendency.

        생쥐 배아의 체외배양시 저농도 산소 환경이 배아 발달에 미치는 영향

        N/A Objective: It is known that mouse embryos before implantation develop in a low oxygen environment of 3- 8% concentration and with antioxidant materials such as vitamins, antioxidant enzymes, ferrous binding proteins, and albumin in follicular and tubal fluids. However, the 20% oxygen culture condition with chemically defined media might be produce an abundance of ROS, and leads to developmental delay or developmental block in vitro. In this study, we attempt to elucidate the relationship between intracellular H2O2 production and embryo development in different oxygen culture conditions of mouse embryos. Methods: Prenuclear embryos from C57BL/CBA Fl hybrid and ICR mouse were cultured in incubators which provided 5% carbon dioxide, 20% oxygen and 5% carbon dioxide, 5% oxygen. Measurement of H2O2 level in a embryo was performed with DCHFDA(2,7 -dichlorodihydroflourescein diacetate)and analyzed with Quanti-cell 700, and the number of blastomeres was counted with DAPI( 4, 6'-diamidino-2-phenylindole). Results: Oxygen concentration of the culture medias was significantly higher in the 20% oxygen environment compared to that of 5% oxygen environment. Culture of mice embryos in high oxygen condition leads to high HO concentrations at 2 cell stage and developmental delay or 2-cell block regardless of the strain. But in a 5% oxygen environment, which is similar to in-vivo conditions HO production was suppressed continuously through out culture and development of embryos was definitely improved. Conclusion: These results suggest that there is a difference in the production of ROS or protective mechanism according to the mouse strains and stage of development, and it is thought that in-vitro culture in 5% oxygen environment provides stable in vivo equilibrium but in a 20% oxygen environment there is production of ROS which overcome the protective mechanism which leads to cellular damage and embryo developmental delay.

        Background: Numerous studies have investigated the potentialrelationship of human leukocyte antigen (HLA)-G 14bp insertion/deletion (INS/DEL) polymorphism with autoimmune disease (AID). However, results from published data were inconclusive. Objectives: Our aim in this study was to determine whether the 14bp INS/DEL polymorphism in the HLA-G gene contributes to the risk of AID. Methods: A systemic literature search of PubMed and Embase databases was conducted to identify eligible studies investigating the association of HLA-G 14bp INS/DEL polymorphism with AID. The final analysis included 13 publications with a total of 6462 individuals. Results: Overall, no significant association between HLA-G 14-bp INS/DEL polymorphism and overall AID was detected in all comparison models. Further subgroup analyses based on AID types and ethnicity showed no significant association. Conclusion: Our results suggest that the HLA-G 14bp INS/DEL polymorphism might not be related to the development of AID. Further studies with larger sample size are warranted to validate our finding.

      • 소아에서 발생한 골수이형성 증후군 2례

        Myelodysplastic syndrome(MDS) is a group of acquired conditions characterized by progressive bone marrow failure associated with normocellular or hypercellular bone marrow that cannot be attributed to nutritional deficiency, chronic infection, or other chronic systemic illness, and cannot be reversed by the successful treatment of those conditions. Whereas the cases of secondary MDS occur at all ages, primary MDS is essentially a disease of elderly, though no age group is exempt and predomonant in men. The MDS is fundamentally clonal disorders at the level of the haemopoietic totipotent stem cell. Clinically, they are characterized by varing degrees of peripheral cytopenia(s) with morphological and functional abnormalities of blood elements. We have experienced two unrelated pediatric cases, who showed pancytopenia on admission and the characteristic marrow findings of MDS. The first case, a 12 months-old girl, showed increased reticulin with abnormal localization of immature precursor cells(ALIP) in her bone marrow, in addition to dysplastic features such as bi-and multi-nucleation of late normoblasts and internuclear bridging. She also showed 22% of blasts in her bone marrow cells thus classified in the category of RAEB-t after FAB classification. Also found were a chromosomal abnormality namely 46 XX t(14q21q), which was an unusual one. Her family brought her home and, 4 month later, she deceased. The second case, a 12 year-old boy, showed pancytopenia with Pseudo-Pelger-Huet anomaly in neutrophils but no blasts in the peripheral blood. Bone marrow of this case also showed the picture of dyserythropoiesis with binuclearity, internuclear bridging, nuclear fragments, multinuclearity of late normoblasts, in addition to megaloblastoid change in the normoblasts and increased mitosis in granulocytic series. No chromosomal abnormality was found. Classified as RA after FAB classification of MDS. Showed good response to conservative management for anemia for more than one year.

        심인성쇼크로 관상동맥 중재술 중 대동맥 내 풍선펌프 사용 시 임상 경과

        Background/Aims: The mortality of hospitalized patients undergoing treatment with an intra-aortic balloon pump (IABP) due to cardiogenic shock is well known as quite high. The aim of this study was to evaluate the outcome of percutaneous coronary intervention (PCI) with an IABP in patients with acute coronary syndrome (ACS) and cardiogenic shock and identify the predictors of in-hospital mortality. Methods: 134 patients who underwent PCI with IABP due to ACS complicated by cardiogenic shock were consecutively enrolled. Outcomes were obtained and analyzed during hospitalization and after 1 year. Results: The incidence of all-cause mortality was 35.8% (in-hospital mortality, 34.3%, 1-year mortality, 1.5%). The nonsurvival group exhibited higher peak levels of creatine kinase MB, lower ejection fractions, and higher incidences of ST elevation myocardial infarction, ventricular arrhythmia, and use of an assistive device than did the survival group. Aging (hazard ratio 2.839, 95% confidence interval 1.408-5.723, p = 0.004), the use of a temporary pacemaker (2.035, 1.114-3.720, 0.021), the use of a mechanical ventilator (4.376, 1.852-10.341, 0.001), and the performance of cardiopulmonary resuscitation (CPR) (2.219, 1.017-4.839, 0.045) were independent predictors for in-hospital mortality. However, out-of-hospital mortality among in-hospital survivors was not affected by predictors of in-hospital mortality. Conclusions: The incidence of in-hospital mortality was high, as expected in patients undergoing PCI with IABP due to ACS with cardiogenic shock. Aging, CPR, and additional procedures such as pacemaker use and mechanical ventilation were predictors of in-hospital mortality. However, the patients who were successfully discharged after the complex procedure showed acceptable 1-year outcomes. (Korean J Med 2015,89:186-191)

        The Implication of Cardiac Injury Score on In-hospital Mortality of Coronavirus Disease 2019

        Backgrounds: The severe acute respiratory syndrome-coronavirus 2 (SARS-CoV-2) has spread worldwide. Cardiac injury after SARS-CoV-2 infection is a major concern. The present study investigated impact of the biomarkers indicating cardiac injury in coronavirus disease 2019 (COVID-19) on patients' outcomes. Methods: This study enrolled patients who were confirmed to have COVID-19 and admitted at a tertiary university referral hospital between February 19, 2020 and March 15, 2020. Cardiac injury was defined as an abnormality in one of the following result markers: 1) myocardial damage marker (creatine kinase-MB or troponin-I), 2) heart failure marker (N-terminal-pro B-type natriuretic peptide), and 3) electrical abnormality marker (electrocardiography). The relationship between each cardiac injury marker and mortality was evaluated. Survival analysis of mortality according to the scoring by numbers of cardiac injury markers was also performed. Results: A total of 38 patients with COVID-19 were enrolled. Twenty-two patients (57.9%) had at least one of cardiac injury markers. The patients with cardiac injuries were older (69.6 ± 14.9 vs. 58.6 ± 13.9 years old, P = 0.026), and were more male (59.1% vs. 18.8%, P = 0.013). They showed lower initial oxygen saturation (92.8 vs. 97.1%, P = 0.002) and a trend toward higher mortality (27.3 vs. 6.3%, P = 0.099). The increased number of cardiac injury markers was significantly related to a higher incidence of in-hospital mortality which was also evidenced by Kaplan-Meier survival analysis (P = 0.008). Conclusion: The increased number of cardiac injury markers is related to in-hospital mortality in patients with COVID-19.

        TiO₂와 Al₂O₃의 기상 VOCs 흡착 특성 평가 및 다양한 구조체로의 성형을 통한 열적 내구성 확보에 관한 연구

        본 연구에서는 금속산화물 2종, TiO2 분말과 Al₂O₃ 분말을 이용하여 건식 조건에서의 기상 VOCs 흡착 성능을 평가하였으며, BET분석과 암모니아 in-situ FT-IR 분석을 통해 비표면적, 표면 산점을 분석하고 성능과의 상관성을 평가하였다. 그 결과 TiO₂ 분말, Al₂O₃ 분말은 각각 317.6 ㎡ g<SUP>-1</SUP>, 64 ㎡ g<SUP>-1</SUP>의 비표면적을 갖으며, TiO₂ 분말의 경우 표면에 다수의 산점이 관찰되었다. 두 금속 산화물 분말을 이용하여 기상 VOCs 흡착 성능을 평가한 결과, 비표면적이 크고 다수의 산점을 보유한 TiO₂ 분말이 비교적 우수한 흡착 성능을 나타내었다. 특히 비표면적이 흡착성능에 직접적인 영향을 미치는 것으로 판단되며, 산점에 의한 영향에 대해서는 추가적인 연구가 요구된다. 우수한 흡착 성능을 나타낸 TiO₂를 기반으로 honeycomb, hollow fiber, disc의 성형체로 제조 한 결과, 분말보다 흡착 성능은 낮았으나 적용성 측면에서 유리하며 제조공정의 특성상 우수한 열적 내구성을 갖는 polymeric disc 흡착제의 경우, 수회의 고온 탈착공정 후에도 흡착 성능을 안정적으로 유지함을 확인하였다. In this study, the adsorption performance of vapor phase VOCs under dry conditions was evaluated by using two metal oxides, TiO₂ powder and Al₂O₃ powder. BET analysis and ammonia in-situ FT-IR analysis were used to analyze specific surface area and surface acid site. As a result, TiO₂ powder and Al₂O₃ powder had a specific surface area of 317.6 ㎡ g<SUP>-1</SUP> and 64 ㎡ g<SUP>-1</SUP>, respectively. In the case of TiO₂ powder, many acid sites were observed on the surface. As a result of evaluating the vapor phase VOCs adsorption performance using two metal oxide powders, TiO₂ powder having a relatively large specific surface area and a large number of acid sites exhibited relatively good adsorption performance. In particular, it is considered that the specific surface area directly affects the adsorption performance, and further study on the effect of the acid site is required. Based on the TiO₂ exhibited excellent adsorption performance, it manufactured into various forms of honeycomb, hollow fiber and disc. As a result, the adsorption performance was lower than that of the powder, but it is advantageous in view of applicability. In addition, it was confirmed that the disc adsorbent having excellent thermal durability due to the characteristics of the manufacturing process stably maintains adsorption performance even at a high temperature desorption process several times.

        Novel two-dimensional In<sub>2</sub>O<sub>3</sub> nanodiscs for high-rate performance of solid-state symmetric supercapacitors

        <P><B>Abstract</B></P> <P>In this study, we report a two-dimensional In<SUB>2</SUB>O<SUB>3</SUB> nanodiscs for solid-state symmetric supercapacitors (SSCs) synthesized by one-step hydrothermal method. It exhibits high specific capacitance of 622 F g<SUP>−1</SUP> at 3 A g<SUP>−1</SUP> and 412 F g<SUP>−1</SUP> at 7 A g<SUP>−1</SUP> based on individual electrodes. Further, the SSCs demonstrate a wide potential window of 1.0 V, excellent specific capacitance of 89.7 F g<SUP>−1</SUP> at a current density of 1 A g<SUP>−1</SUP>, a high energy density of 12.5 W h kg<SUP>−1</SUP> at a power density of 500 W kg<SUP>−1</SUP>, and an outstanding stability with ∼97% rate retention after 10,000 cycles.         Four different methods. Clark area routing method. SCS TR-55, Nakayasu synthetic hint hydrograph, and kajiyama formula were used to calculate the desi후 flood for Deok Dong reservoir watershed located in Kyung Ju. The results obtained using Clark area routing method were determined as the design flood for the watershed. The recurrence interval of 500 years was selected for the watershed. The design runoff hydrographs were routed through reservoir using the Puls method. The actual storm events which were relatively large in their magnitudes were also analyzed. The outflow calculated in this study showed considerable differences from results available in the literatures. The several errors that can not be ignored found in the literatures were indicated and explained to provide more accurate results for the watershed. Some basic informations useful for the prevention of disaster due to heavy storms were proposed.

        Modeling of asymmetric degradation based on a?non-uniform electric field and temperature in amorphous In–Ga–Zn–O thin film transistors

        <P>We propose a new local degradation model based on a non-uniform increase in donor-like traps (DLTs) determined by distributions of an electric field and?measured device temperature in amorphous In–Ga–Zn–O (a-IGZO) thin film transistors (TFTs). A systematic investigation of the degradation model reveals that vertical field-dependent DLTs are essential for modeling of measured asymmetric electrical characteristics between the?source and drain after positive gate and drain bias stressing. An increased temperature due to self-heating is found to play a role in intensifying the asymmetric degradation. From the individual simulation of measured transfer curves at different stress times, the?model parameters and an asymmetry index as a function of stress time are extracted. It is expected that this novel methodology will?provide new insight into asymmetric degradation and?be utilized to predict the influence of electric field and heat on?degradation under various bias-stress conditions in a-IGZO TFTs.</P>
