Uterine leiomyoma is one of the most common gynecological tumor, whereas acute torsion of the uterine leiomyoma is very rare. We report a case of subserosal leiomyoma that was first detected by ultrasonography, and further confirmed as torsion of subserosal leiomyoma by laparoscopic surgery. Uterine leiomyoma is a benign subclinical gynecological tumor occurring frequently in women of childbearing age. The symptoms include menorrhagia, abnormal vaginal bleeding, dysmenorrhea, dyspareunia, pelvic pain, and urination frequency. The infrequent acute complications include thromboembolism, acute torsion, renal failure, acute pain due to secondary degeneration, mesenteric vein thrombosis, and intestinal gangrene. They are very rare complications that their exact incidence is not known. This paper reports the case in which the uterus was normal, but the subserosal leiomyoma was twisted to cause acute abdominal pain. Due to the rarity and difficulty of preoperative diagnosis by ultrasonography( US), computed tomography(CT) scan and magnetic resonance imaging(MRI), the diagnosis is established by operative findings. Furthermore, if the diagnosis is not made promptly, ischemic gangrene and peritonitis can occur. Thus, the torsion of subserosal leiomyoma should be diagnosed and treated by a surgical approach immediately upon suspicion. With a review of related literature, we discuss a case of torsion of subserosal leiomyoma, where the patient was misdiagnosed as secondary degeneration of uterine leiomyoma at our hospital prior to the operation.
A comparison of two- and three-port laparoscopic myomectomy

목적: 2 years ago, FDA announced its safety communication in respect to laparoscopic power morcellation used for hysterectomy or myomectomy in women with uterine fibroids that it poses a risk of spreading unsuspected cancerous tissue. In awareness of that recommendation, we aimed to evaluate the surgical outcomes of two-port laparoscopic myomectomy removing uterine fibroids within confined tissue-retrieval bag manually in comparison of conventional laparoscopic myomectomy undergoing power morcellation. 방법: Design: Retrospective chart review study Setting: Tertiary teaching university hospital Patients: We examined 300 women who underwent myomectomy for symptomatic uterine fibroids from January 2010 to December 2015. 결과: 2 study group showed no statistically significant difference in terms of operation time including elapsed time for fibroid morcellation and major surgical complications. 결론: We analyzed the feasibility and safety of two-port laparoscopic myomectomy without performing power morcellation in terms of surgical outcomes and operating times. It might be a safe treatment option for power morcellation having benefits of minimally invasive surgery at the same time.

Primary vaginal cancer combined with uterine prolapse is very rare. We present a case of 80-year-old postmenopausal women complaints of something coming out per vagina for the past 20 years, along with blood stained discharge, foul odor leukorrhea, and severe pelvic pain for the last 3 months. A 4×5 cm ulcer was present on middle third of vaginal wall with marked edema and ulceration of surrounding tissue. The prolapse was reduced under intravenous sedation in operating room. On gynecologic examination, uterus was normal in size, no adnexal mass was examined, and both parametrium were thickened. Papanicolaou smear was normal. Biopsy of the ulcer at vaginal wall revealed invasive squamous cell carcinoma of vagina. MRI of abdomen and pelvis showed left hydronephrosis and liver metastasis. PET/CT revealed metastasis to lung, liver and iliac bone. She died from progression of disease one month after diagnosis.

Adenoid basal carcinoma of the uterine cervix is uncommon neoplasia mostly occurring in postmenopausal women. It has excellent prognosis and a favorable clinical course. In addition, adenoid basal carcinoma is differentiated from adenoid cystic carcinoma by histologic and cellular morphologies, and immunohistochemistry. In this paper, we present the case of a 22 year old Korean female. She initially had a high-grade squamous intraepithelial lesion (HSIL) on Pap smear and a subsequent cervical loop electrosurgical excision procedure (LEEP) specimen revealing adenoid basal carcinoma. The lesion showed the histologic characteristics of adenoid basal carcinoma. Because of the lesion`s low potential for recurrence and metastasis, the young primipara had a conization procedure performed and has been under close observation.

A Rare Case of an Autoamputated Ovary with Mature Cystic Teratoma

Autoamputated ovary with mature cystic teratoma (MCT) is a rarely reported gynecologic entity with an unknown prevalence. A 34-year-old woman referred to our clinic for presumed left ovarian tumor. Pelvic examination, ultrasonography and computed tomography scan revealed a 5-cm, cystic ovarian mass with calcification and fat component, and tumor markers were as follows, cancer antigen (CA) 125; 10.4 U/mL, CA19-9; 2 U/mL. Laparoscopy was performed. The mass was identified in the left adnexal region without any ligamentous or direct connection with the pelvic organs. The right ovary was normal. However, the left ovary and the tube could not be identified in its proper anatomical location. The mass was successfully removed with sharp and blunt dissection. A review of histopathologic study revealed a MCT. The patient became pregnant within seven months and gave birth to a healthy baby by cesarean section. We present a rare case of an autoamputated ovary with MCT. (J Menopausal Med 2017;23:74-76)
Case Reports : A case of Lipoleiomyoma in utero

Lipoleiomyoma of the uterus is known to be extremely rare and it is suspected to be a variation of leiomyoma. Thus, the typical signs and symptoms of lipoleiomyoma are quite similar to that of leiomyoma, such as a palpable mass, hypermenorrhea and pelvic pain. Due to its fatty content, lipoleiomyoma also can be easily mistaken for benign cystic ovarian teratoma. We report here on a case of lipoleiomyoma of the uterus in a 75-year-old woman with a palpable pelvic mass and profuse uterine bleeding. The preoperative presumptive diagnosis was made by assessing the computed tomography (CT) images, and then the lipoleiomyoma was pathologically confirmed after the operation.
