Number of Metastatic Lymph Nodes and Ratio of Metastatic Lymph Nodes to Total Number of Retrieved Lymph Nodes Are Risk Factors for Recurrence in Patients With Clinically Node Negative Papillary Thyroid Carcinoma

        Chuan-Ming Zheng,지용배,송창면,Ming-Hua Ge,태경 대한이비인후과학회 2018 Clinical and Experimental Otorhinolaryngology Vol.11 No.1

Objectives. The number of metastatic lymph nodes (LNs) and the ratio between the number of metastatic LNs and the total number of retrieved LNs (the LN ratio [LNR]) have been proposed as risk factors for recurrence of papillary thyroid carcinoma (PTC). However, the significance of the number of LNs and the LNR in patients with clinically node negative PTC has not been clearly determined. The purpose of this study is to evaluate their significance. Methods. We retrospectively analyzed 382 patients with PTC who had undergone total thyroidectomy with prophylactic central neck dissection (CND) between January 2000 and December 2010. We excluded patients with lobectomy, concurrent lateral compartment neck dissection, a follow-up period less than at least 2 years, number of harvested central LNs less than or equal to one, clinically positive LN, distant metastasis, recurrent cancer or other types of malignancy. The correlations between recurrence and various clinicopathologic characteristics including tumor size, extrathyroidal extension (ETE), stage, number of metastatic central LNs, and the LNR were investigated. Results. After a mean follow-up period of 82.2±26.4 months, recurrence occurred in 14 patients (3.7%). Tumor size ≥20 mm, maximal ETE, presence of central LN metastasis, number of metastatic LNs ≥2, and LNR ≥0.31 correlated with recurrence in the univariate analysis. However, tumor size ≥20 mm, maximal ETE, number of metastatic LNs ≥2, and LNR ≥0.31 were significantly associated with recurrence in the multivariate analysis (hazard ratio=6.61, 7.17, 3.43, and 11.23, respectively). Conclusion. The LNR and the number of metastatic LNs are independent prognostic risk factors for recurrence in patients with clinically node negative PTC, and these factors can be used to guide postoperative adjuvant therapy and follow-up strategy after prophylactic CND.

Output Tracking Predictive Control of Networked Systems with Two-channel Random Communication Constraints

        Chuan-Dong Bai,Tong Mu,Zhong-Hua Pang,Jian Sun,Guo-Ping Liu 제어·로봇·시스템학회 2023 International Journal of Control, Automation, and Vol.21 No.2

In this paper, the output tracking control problem is investigated for a networked control system with twochannel random network delays and packet dropouts as well as stochastic noise. To actively compensate for these random communication constraints in the feedback and forward channels, a novel networked predictive control (NPC) method is proposed based on the input-output difference equation model, where the two-channel communication constraints are handled separately according to their different features. Furthermore, different from the existing NPC methods based on round-trip time delays, actual control inputs rather than the predicted ones are employed to generate future control commands. Then a delay-independent closed-loop stability condition is obtained, and a condition to guarantee a zero steady-state output tracking error is derived. Also, theoretical analysis shows that the proposed NPC method can achieve the same output tracking performance as the corresponding local control system. Finally, the effectiveness of the proposed method is evaluated by simulation and experimental results.

Experimental investigation on the subsurface stress distributions in specimens with different strengths after ultrasonic impact treatment

        Chuan Liu,Jia-bin Shen,Chang-hua Lin,Jian-fei Wang,Jian-xin Wang 대한기계학회 2021 JOURNAL OF MECHANICAL SCIENCE AND TECHNOLOGY Vol.35 No.5

Ultrasonic impact treatment (UIT) was carried out on three specimens with different strengths and, to measure the subsurface stress distribution of the specimens, the layer-by-layer removal x-ray diffraction method was applied. The stress redistribution due to material removal was corrected by finite element analysis. The effect of material strength on the after-UIT subsurface stress was investigated. The results show that the UIT can cause a compressive stress layer with a depth of over 2.5 mm for specimens with different strengths, while the material strength has no significant effect on the depth of UIT-induced stress. The UIT-induced peak compressive stresses in the three specimens exceed the material yield strength, appearing at the depth of 0.2 mm beneath the surface and increasing with the increase of material yield strength. The ratio of UIT-induced peak compressive stress magnitude to material yield strength decreases with the increase of material yield strength.

Establishment of Tripterygium wilfordii Hook. f. Hairy Root Culture and Optimization of Its Culture Conditions for the Production of Triptolide and Wilforine

( Chuan Shu Zhu ),( Guo Qeng Miao ),( Jia Guo ),( Yan Bo Huo ),( Xing Zhang ),( Jia Hua Xie ),( Jun Tao Feng ) 한국미생물 · 생명공학회 2014 Journal of microbiology and biotechnology Vol.24 No.6

In order to solve the shortage of natural Tripterygium wilfordii Hook. f. plant resource for the production of the important secondary metabolites triptolide and wilforine, hairy roots were induced from its root calli by Agrobacterium rhizogenes. Induced hairy roots not only could be maintained and grown well in hormone-free half-strength Murashige and Skoog medium but also could produce sufficient amounts of both triptolide and wilforine. Although hairy roots produced approximately 15% less triptolide than adventitious roots and 10% less wilforine than naturally grown roots, they could grow fast and could be a suitable system for producing both secondary metabolites compared with other tissues. Addition of 50 μM methyl jasmonate (MeJA) could slightly affect hairy root growth, but dramatically stimulated the production of both triptolide and wilforine, whereas 50 μM salicylic acid had no apparent effect on hairy root growth with slightly stimulatory effects on the production of both secondary metabolites. Addition of precursor nicotinic acid, isoleucine, or aspartic acid at the concentration of 500 μM had varying effects on hairy root growth, but none of them had stimulatory effects on triptolide production, and only the former two had slightly beneficial effects on wilforine production. The majority of triptolide produced was secreted into the medium, whereas most of the produced wilforine was retained inside of hairy roots. Our studies provide a promising way to produce triptolide and wilforine in T. wilfordii hairy root cultures combined with MeJA treatment.

      • Analysis of Different Ways of Drainage for Obstructive Jaundice Caused by Hilar Cholangiocarcinoma

        Xu, Chuan,Lv, Peng-Hua,Huang, Xin-En,Wang, Shu-Xiang,Sun, Ling,Wang, Fu-An Asian Pacific Journal of Cancer Prevention 2014 Asian Pacific journal of cancer prevention Vol.15 No.14

Objective: To evaluate the prognosis of different ways of drainage for patients with obstructive jaundice caused by hilar cholangiocarcinoma. Materials and Methods: During the period of January 2006- March 2012, percutaneous transhepatic catheter drainage (PTCD)/ percutaneous transhepatic biliary stenting (PTBS) were performed for 89 patients. According to percutaneous transhepatic cholangiography (PTC), external drainage was selected if the region of obstruction could not be passed by guide wire or a metallic stent was inserted if it could. External drainage was the first choice if infection was diagnosed before the procedure, and a metallic stent was inserted in one week after the infection was under control. Selection by new infections, the degree of bilirubin decrease, the change of ALT, the time of recurrence of obstruction, and the survival time of patients as the parameters was conducted to evaluate the methods of different interventional treatments regarding prognosis of patients with hilar obstruction caused by hilar cholangiocarcinoma. Results: PTCD was conducted in 6 patients and PTBS in 7 (p<0.05). Reduction of bilirubin levels and ALT levels was obvious after the procedures (p<0.05). The average survival time with PTCD was 161 days and with PTBS was 243 days (p<0.05). Conclusions: With both drainage procedures for obstructive jaundice caused by hilar cholangiocarcinoma improvement in liver function was obvious. PTBS was found to be better than PTCD for prolonging the patient survival.

      • Comparison of Infection between Internal-External and External Percutaneous Transhepatic Biliary Drainage in Treating Patients with Malignant Obstructive Jaundice

        Xu, Chuan,Huang, Xin-En,Wang, Shu-Xiang,Lv, Peng-Hua,Sun, Ling,Wang, Fu-An Asian Pacific Journal of Cancer Prevention 2015 Asian Pacific journal of cancer prevention Vol.16 No.6

Purpose: Percutaneous transhepatic biliary drainage (PTBD) is a form of palliative care for patients with malignant obstructive jaundice. We here compared the infection incidence between internal-external and external drainage for patients with malignant obstructive jaundice. Methods: Patients with malignant obstructive jaundice without infection before surgery receiving internal-external or external drainage from January 2008 to July 2014 were recruited. According to percutaneous transhepatic cholangiography (PTC), if the guide wire could pass through the occlusion and enter the duodenum, we recommended internal-external drainage, and external drainage biliary drainage was set up if the occlusion was not crossed. All patients with infection after procedure received a cultivation of blood and a bile bacteriological test. Results: Among 110 patients with malignant obstructive jaundice, 22 (52.4%) were diagnosed with infection after the procedure in the internal-external drainage group, whereas 19 (27.9%) patients were so affected in the external drainage group, the difference being significant (p<0.05). In 8 patients (36.3%) in the internal-external group infection was controlled, as compared to 12 (63.1%) in the external group (p< 0.05). The mortality rate for patients with infection not controlled in internal-external group in one month was 42.8%, while this rate in external group was 28.6% (p< 0.05). Conclusion: External drainage is a good choice, which could significantly reduce the chance of biliary infection caused by bacteria, and decrease the mortality rate at one month and improve the long-term prognosis.

Modeling strength of high-performance concrete using genetic operation trees with pruning techniques

Chien-Hua Peng,I-Cheng Yeh,Li-Chuan Lien 사단법인 한국계산역학회 2009 Computers and Concrete, An International Journal Vol.6 No.3

Regression analysis (RA) can establish an explicit formula to predict the strength of High-Performance Concrete (HPC); however, the accuracy of the formula is poor. Back-Propagation Networks (BPNs) can establish a highly accurate model to predict the strength of HPC, but cannot generate an explicit formula. Genetic Operation Trees (GOTs) can establish an explicit formula to predict the strength of HPC that achieves a level of accuracy in between the two aforementioned approaches. Although GOT can produce an explicit formula but the formula is often too complicated so that unable to explain the substantial meaning of the formula. This study developed a Backward Pruning Technique (BPT) to simplify the complexity of GOT formula by replacing each variable of the tip node of operation tree with the median of the variable in the training dataset belonging to the node, and then pruning the node with the most accurate test dataset. Such pruning reduces formula complexity while maintaining the accuracy. 404 experimental datasets were used to compare accuracy and complexity of three model building techniques, RA, BPN and GOT. Results show that the pruned GOT can generate simple and accurate formula for predicting the strength of HPC.

      • Internal-External Percutaneous Transhepatic Biliary Drainage for Patients with Malignant Obstructive Jaundice

        Xu, Chuan,Lv, Peng-Hua,Huang, Xin-En,Sun, Ling,Wang, Shu-Xiang,Wang, Fu-An Asian Pacific Journal of Cancer Prevention 2014 Asian Pacific journal of cancer prevention Vol.15 No.21

Purpose: To evaluate the effect of internal-external percutaneous transhepatic biliary drainage (IEPTBD) for patients with malignant obstructive jaundice. Methods: During the period of January 2008 and July 2013, internal-external drainage was performed in 42 patients with malignant obstructive jaundice. During the procedure, if the guide wire could pass through the occlusion and into the duodenum, IEPTBD was performed. External drainage biliary catheter was placed if the occlusion was not crossed. Newly onset of infection, degree of bilirubin decrease and the survival time of patients were selected as parameters to evaluate the effect of IEPTBD. Results: Twenty newly onset of infection were recorded after procedure and new infectious rate was 47.6%. Sixteen patients with infection (3 before, 13 after drainage) were uncontrolled after procedure, 12 of them (3 before, 9 after drainage) died within 1 month. The mean TBIL levels declined from 299.53 umol/L before drainage to 257.62 umol/L after drainage, while uninfected group decline from 274.86 umol/L to 132.34 umol/Lp (P < 0.5). The median survival time for uninfected group was 107 days, and for infection group was 43 days (P < 0.05). Conclusions: The IEPTBD drainage may increase the chance of biliary infection, reduce bile drainage efficiency and decrease the long-term prognosis, and the external drainage is a better choice for patients with malignant obstructive jaundice need to biliary drainage.
