Epidermoid Cyst Arising from the Corpus Callosum

Most of intradural epidermoid cyst arise as slowly growing extraaxial lesions but purely intracerebral epidermoid cysts are rare. A 39-year-old female presented with a headache during several months. Brain computed tomography [CT] scan showed a mass lesion in the both frontal lobe with heterogenous density approximately $5{\times}5cm$ in size. Magnetic resonance imaging [MRI] revealed a mass of heterogenous signal intensity on T1, T2-weighted image and faint enhancement with gadolinium [Gd]. Through the both interhemispheric approach, mass was removed subtotally except the calcified portion tightly attached to the corpus callosum. The patient discharged without neurological deficit. The authors report a case of epidermoid cyst in the corpus callosum and discuss the pathogenesis of the intraparenchymal epidermoid cyst.
First Fatal Oseltamivir-Resistant 2009 Pandemic Influenza A (H1N1) Case in an Adult in Korea

It has been suggested that oseltamivir-resistant influenza viruses harboring the H274/275Y mutation are less virulent than are those that are oseltamivir-sensitive,and fatality attributed to infection with an oseltamivir-resistant virus is very rare. Here we report the first fatal adult case of oseltamivir-resistant 2009 pandemic influenza A (H1N1) in Korea. A 60-year-old Korean male who had hypertension, diabetes mellitus,chronic kidney disease, and dilated cardiomyopathy visited Chonnam National University Hospital because of a 7-day history of chest pain and dyspnea. The patient was at another clinic and had been medicated with oseltamivir (75 mg twice daily) beginning 7 days before admission. Empirical antibiotics were started on the first day of hospitalization. Reverse-transcriptase polymerase chain reaction for 2009 pandemic influenza A (H1N1) was reported to be positive, and a double dose of oseltamivir (150mg twice per day) was started on day four of hospitalization. However, the pneumonia worsened and the patient died, despite 3 days of high-dose antiviral therapy and 6 days of antibacterial therapy. An H275Y mutation was detected in the neuraminidase gene sequence. This case shows that oseltamivir resistance after short-term drug exposure is possible and can be fatal, emphasizing that early use of zanamivir should be considered in suspicious cases.

Using atomic layer deposition (ALD), RuO2 was deposited on mesoporous Al2O3 with a mean pore size of ~12 nm and particle diameter of ~1 mm. The entire internal structure of mesoporous Al2O3 was coated by RuO2 nanostructures using ALD, forming a mesoporous bead of RuO2/Al2O3. Substantially high CO oxidation activity at reaction temperatures below 100 C is also shown. There was a decrease in CO oxidation activity with time at 100 C, and post-annealing at 700 C fully recovered the catalytic activity of fresh RuO2/Al2O3. The RuO2/Al2O3 can be useful as an oxidation catalyst for eliminating harmful molecules.

Objective : The postoperative subsidence of anterior cervical interbody fusion for cervical degenerative diseases gives rise to segmental kyphotic collapse, screw loosening, and chronic neck pain. So, intraoperative custom-made polymethylmethacrylate [PMMA] C-cage has been developed to prevent subsidence following anterior cervical fusion. Methods : A total of patients who underwent anterior cervical interbody fusion with a intraoperative custom - made cervical cage filled with local bone and demineralized bone matrix [group A] were analyzed prospectively from June 2004 to June 2005. These were compared with 40 patients who were treated with iliac bone graft [group B]. We evaluated subsidence ratio, change of segmental angle, distraction length and segmental angle. Statistical analysis was performed using independent sample t-test and Pearson correlation coefficient. Results : Group A had a statistically significant decrease in subsidence ratio [$0.64{\pm}0.43%$, p=0.00]. distraction length [$2.42{\pm}1.25\;mm$, p=0.02], and follow angle change [$1.78{\pm}1.69^{\circ}$, p=0.01] as compared with Group B. However, there was no statistically significant difference in postoperative segmental angle change [p=0.66]. On the analysis of the correlation coefficient, the parameters showed no interrelationships in the group A. On the other hand, subsidence ratio was affected by distraction length in the group B [Pearson correlation=0.448]. Conclusion : This operative technique would be contributed for the reduction of a postoperative subsidence after the anterior cervical interbody fusion procedure for cervical disc disease with moderate to severe osteoporotic condition and segmental loss of lordosis.
Molten Carbonate Fuel Cell 용 Ni-Al anod 에서 미세구조가 creep 특성에 미치는 영향에 관한 연구

To improve the creep resistance of the anode for molten carbonate fuel cell, the fabrication processes of Ni anode with different Al contents were investigated, where four types of sintering condition have been examined at different temperatures and gas atmospheres. The effects of such different sintering conditions on the secondary phases, and the morphologies of secondary phase of the anode were examined. If Al is added over the solid solution limit, Ni₃Al or Al₂O₃ are formed as a secondary phase according to the sintering conditions and degrees of creep strain and conductivity are changed as results. The creep test and porosimeter analysis certificated that the Ni₃Al intermetallic compound precipitation and Al₂O₃ dispersion hardened structure had good creep resistances with suitable pore size distributions for anode, which resulted in the stable mechanical performances.

Non-malignant esophagorespiratory fistula is uncommon in adults. Among them, tuberculosis is the leading etiologic agent while traurnatic cases are increasing. Other less common causes are syphilis, fungal infection and esophageal diverticulum. We present a case of non-malignant esophagobronchial fistula through mid-esophageal diverticulum found in 50 year-old man who complained of chronic coughing, right lower chest pain and frequent episodes of paroxysmal coughing and choking sensation after food intake. Esophagography, fiberoptic esophagoscopy and bronchoscopy could visualise the fistulous tract through the mid-esophageal diverticulum. After surgical repair including fistulectomy, diverticulectomy with esophageal repair, and segmental lobectomy, he became well.

Background :All-trans retinoic acid (ATRA)/anthracycline chemotherapy is beneficial in newly diagnosed acute promyelocytic leukemia (APL); however, it is important to identify patients with high-risk disease to increase the cure rate. We investigated the outcome of ATRA/anthracycline chemotherapy and clinicobiological correlations of FLT3/ITD and NPM1 mutations in APL patients. Methods :Induction therapy included oral ATRA (45 mg/m^2/day) and idarubicin (12 mg/m^2/day, intravenous, on days 2, 4, and 6). Patients achieving complete remission (CR) received 3 courses of ATRA combined with reinforced consolidation therapy. Mutations were analyzed using GeneScan and polymerasae chain reaction assays of bone marrow samples obtained from patients at diagnosis. Results :Forty-five (84.9%) of 53 eligible patients achieved CR. The overall relapse rate was 8.9%, and the 3-year overall survival (OS) and leukemia-free survival (LFS) were 84.9±4.9% and 77.5±6.0%, respectively. The NPM1 mutation was not found in any patient, while the FLT3/ITD mutation was found in 10 (20.0%) patients. Of the FLT3/ITD+ patients, 80% belonged to the high-risk group, defined according to the presenting WBC and platelet counts. Among the patients who achieved CR, those who were FLT3/ITD+ had a higher relapse rate than those FLT3/ITD-. FLT3/ITD+ patients also had a significantly lower 3-year LFS than FLT3/ITD- patients. Multivariate analysis of the LFS showed that the FLT3/ITD mutation was independently associated with a shorter overall LFS, after adjusting for pretreatment risk stratification. Conclusion :This study investigated the clinical outcome of newly diagnosed APL patients treated with ATRA/anthracycline chemotherapy. Patients carrying the FLT3/ITD mutation had more aggressive clinical features and a poorer clinical outcome.
