최근 근치적 절제가 불가능한 국소진행성 비소세포 폐암에서 화학요법을 방사선요법과 동시에 투여하는 치료방법에 대한 연구가 활발이 이루어지고 있으며 이러한 치료과정 중 생긴 여러 부작용에 대한 연구도 이루어지고 있지만 아직까지 기관폐루가 생겼다고 보고는 없다. 따라서 치료과정 중 기관폐루가 의심되는 소견이 있을 경우 이에 대한 조기 진단과 치료를 통하여 부작용에 따른 사망률을 최소화하여야 하겠다. The incidence of lung cancer is increasing steadily and various methods have been attempted to enhance the cure rate of locally advanced non small cell lung cancer, which is hardly removable surgically, but the results are not satisfactory yet. Recently, research is being made actively on the simultaneous application of chemotherapy and radiotherapy for locally advanced non small cell lung cancer that cannot be radically resected and the we experienced a case of patient diagnosed with bronchopulmonary fistula during the treatment process, and therefore report it here.

Addition of EI systems to the synchronization networks is being conducted as the digital transmission in Korea is changing from TI to EI. In this study, the quality of the synchronization signal of DOTS's of each toll office which is under the early stage of transition to the EI synchronization networks is measured and analyzed. In particular, the purpose of this study is to find out how to speed up the deployment of the EI synchronization networks by comparing the quality of the TI and EI synchronization signals. According to the results, the TI and EI synchronization signals distributed from KRF directly have no difference in quality. In addition, the synchronization signal redistributed from DOTS which is phase-locked on KRF and the synchronization signal from KRF directly have no difference in quality. Hence, it is concluded that there will be no problems in quality of synchronization resulting from the gradual transition to El while making the most of the current TI facilities.
국내 대학병원에서 정주용 Ciprofloxacin 사용의 적정성 평가

목적 : 국내 병원에서의 정주 ciprofloxacin 사용의 적정성을 평가하고자 하였다. 재료 및 방법 : 2003년 9월 25일부터 8개의 대학병원에서 정주 ciprofloxacin을 받았던 290명의 연속적으로 모집한 성인환자를 대상으로 약제사용 평가(DUE)를 전향적으로 수행하였다. 이 연구에 사용된 DUE 기준은 미국병원약사회의 기준을 국내 실정에 맡게 수정하여 사용하였으며 1) 사용의 정당성에 관한 항목, 2) 사용 시 준수되거나 시행되어야 할 사항에 대한 항목, 3) 부작용에 대한 항목, 4) 치료 효과에 대한 항목으로 나누어 조사하였다. 결과 : 환자의 평균 나이는 54세였다. 33예(26.9%)에서만 약제 사용의 적정성 측면에서 기준을 만족하였고 91예(73.1%)에서는 사용이 적절하지 않았다. 정주 cipro-floxacin의 부적절한 사용의 흔한 원인은 수술 전후의 예방약 사용, 비뇨기계 처치 전의 예방적 사용, 열성 질환에 대한 경험적 사용 등의 순이었다. 사용 시 준수사항 의 측면에서는 사용 전 배양검사(73.8%)와 사용 중 배양검사(68.6%)가 낮은 수행 정도를 보였으나 그 외는 비교적 높은 적정성(82.4-100%)을 보여주었다. 정주 ciprofloxacin사용의 합병증은 드물었다. 치료 효과의 면에서 치료반응은 93.3%였다. 결론 : 약제 사용의 정당성 기준에 따른 정주 cipro-floxacin의 부적절한 사용은 연구에 포함된 대학병원에서 매우 흔하였다(73.1%). 그러므로 적절한 항생제 사용에 대한 교육과 제도적 장치가 매우 필요한 것으로 판단되었다. Background : To evaluate the appropriateness of intravenous (IV) ciprofloxacin usage in university hospitals in Korea. Materials and Methods : We prospectively evaluated the appropriateness of drug usage in 290 consecutive adult patients who received IV ciprofloxacin in eight teaching hospitals during the period from September 25, 2003 to November 15, 2003. Drug use evaluation (DUE) in this study was based on the modified standard by the American Society of Hospital Pharmacy with regard to the justification of drug use, critical and process indications, complications, and outcome measures. Results : Average age of the patients was 54 years. The use of IV ciprofloxacin was appropriate only in 33 cases (26.9%). Common reasons for inappropriate use of IV ciprofloxacin include perioperative uses or prophylactic injection before urologic procedures, and empirical therapy for febrile episodes. Most of the "critical indications" and "process indications" showed high rate of appropriateness (82.4-100%). However, the rate of taking culture and sensitivity test before (73.8%) and during ciprofloxacin use (68.6%) were low. Complications with ciprofloxacin use were rare. In 93.3% of cases, ciprofloxacin treatment was clinically effective. Conclusion : Use of intravenous ciprofloxacin is frequently inappropriate in tertiary care hospitals in Korea. Implementation of education program and antibiotic control system is critical in improving the appropriateness of antimicrobial use.

We investigated the distribution of the marine cladoceran resting eggs in the intertidal sediments of Gomso Bay, Korea and compared the data with those in the subtidal sediments. The abundance of the eggs in the top 6cm of the sediments was higher than that in the other depths. The abundances of the eggs in seaward intertidal sediments below mean sea level (MSL) were higher than those in the sediments over MSL, but were not significantly different from those in the subtidal sediments. The distribution of the resting eggs of cladocerans in the intertidal sediments was affected by the grain-size and moisture content of sediments. The results of the present study suggest that the intertidal sediments as well as the subtidal sediments are a potential egg bank which plays important roles in population dynamics of zooplankton in coastal waters, in particular, recruitment of eggs into plankton.
Resting Eggs of Copepods in the Intertidal Sediments of Gomso Bay, the West Coast of Korea

To Investigate the distribution of resting eggs at the intertidal zone, Gomso Bay, samples were taken from the top of the sediments to 10 cm depth at four sites using a cylindrical corer in February, 1997. Additional samples were also taken from one station at subtidal zone to compare the distributions between the inter- and subtidal zones. The resting eggs of few copepods, Acartia pacifica, Centropages abdominalis, Calanopia thompsoni, and Tortanus forcipatus were studied. Mostly, the abundance of the resting eggs in the lower intertidal zone was greater than that in the upper intertidal zone, but was not significantly different from that in the subtidal zone. The abundance of the resting eggs in the intertidal sediments was related with the grain-size and moisture content of sediments. Intertidal sediments are potential egg banks like subtidal sediments.
Supersonically spray-coated copper meshes as textured surfaces for pool boiling

<P><B>Abstract</B></P> <P>Pool boiling is a process through which heat is removed upon the vaporization of a coolant fluid surrounding a heated surface and is often applied for cooling high-performance computing systems and nuclear reactors. Increasing the surface-to-volume ratio in confined spaces enhances this cooling method. Here, we introduce textured copper pillars with various geometric arrangements and study their effects on the pool-boiling performance. Frustum pyramids were formed by supersonic spraying copper microparticles through a wire mesh to form pillars of various sizes. We identified an optimal pyramid-base size of 0.91 mm on each side corresponding to the maximum heat transfer coefficient, critical heat flux, boiling heat transfer, and cross-flow coolant velocity over the pyramids. Maximum bubble nucleation was also achieved using this specific geometric arrangement.
