An application of VR is extended from simple visualization to provision of means of information acquisition, basis of decision making and entertainment using HCI (Human Computer Interaction). In this thesis, we developed a non-immersive VR system based on 3D modeling data using Java 3D API (Application Programming Interface) and it can play an important role in effective decision making through indirect experience of non-existing objects. Using proposed system, even non-expert can estimate the final result of product before pilot-production or mock-up manufactured. It also can used as selection tool for option or color which meet user's preferences or advertisement purpose of individual enterprise.

Methods: A total of 143 treatment-naive patients who had been treated with telbivudine for at least 6 months were consecutively enrolled. We determined the cumulative rates of virologic response (<12 IU/ml by PCR assay), serologic response, biochemical response and monitored any side effects. Results: The median age and follow-up duration were 50.7 years and 16.4 (range 6-24 months) months, respectively. Sixty seven patients (46.9%) were positive for HBeAg and the median serum HBV DNA level was 7.23 (range 4.31-8.26) log IU/ml. In HBeAg positive group, the cumulative probabilities of HBV DNA undetectability were 14.9% , 24.6%, 34.4% and 26.3% at 6, 12, 18 and 24 months, respectively. Partial virologic response at 6 months occurred in 82.1% of patients and inadequate virologic response at 6 months in 3.0%. During the follow-up period, 8 patients achieved HBeAg loss and 3 patients achieved HBeAg seroconversion. Meanwhile, in HBeAg-negative group, the cumulative rates of virologic response were 64.5%, 86.4%, 82.1% and 81.3%, respectively and the cumulative rates of biochemical response were 89.5%, 98.3%, 100% and 100%, respectively. Among the all patients, there were 42 cases of virologic breakthrough and 21 cases of genotypic resistance and virologic breakthrough. There were 46 (32.2%) cases of CK elevation and 2 (1.4%) cases of myopathy. However, there was no clinically significant safety issue. Conclusions: In Korean treatment-naive HBeAg-negative CHB patients, telbivudine therapy induced favorable virologic, serologic and biochemical responses.

Background: Transient elastography (TE), a noninvasivetool that measures liver stiffness, has been evaluated in metaanalyses for effectiveness in assessing liver fibrosis in European populations with chronic hepatitis C (CHC).However, these data cannot be extrapolated to populations in Asian countries, where chronic hepatitis B (CHB) is more prevalent. In this study, we performed a meta-analysis to assess the overall performance of TE for assessing liver fibrosis in patients with CHB. Methods: Studies from the literature and international conference abstracts which enrolled only patients with CHB or performed a subgroup analysis of such patients were enrolled. Combined effects were calculated using area under the receiver operating characteristic curves (AUROC) and diagnostic accuracy values of each study. Result: A total of 18 studies comprising 2,772 patients were analyzed. The mean AUROCs for the diagnosis of significant fibrosis (F2), severe fibrosis (F3), and cirrhosis (F4) were 0.859 (95% confidence interval [CI], 0.857?0.860), 0.887 (95% CI, 0.886?0.887), and 0.929 (95% CI, 0.928?0.929), respectively. The estimated cutoff for F2 was 7.9 (range, 6.1?11.8) kPa, with a sensitivity of 74.3% and specificity of 78.3%. For F3, the cutoff value was determined to be 8.8 (range, 8.1?9.7) kPa, with a sensitivity of 74.0% and specificity of 63.8%. The cutoff value for F4 was 11.7 (range, 7.3?17.5) kPa, with a sensitivity of 84.6% and specificity of 81.5%. Conclusion: TE can be performed with good diagnostic accuracy for quantifying liver fibrosis in patients with CHB.

Aims: To further observe and identify the effect of nonalcoholic fatty liver disease (NAFLD) on the response to tenofovir disoproxil fumarate (TDF) treatment in patients with chronic hepatitis B (CHB). Methods: From 2012-2014, consecutive antiviral-naïve CHB patients who started TDF and underwent controlled attenuated parameter (CAP) measurement were enrolled from two tertiary medical center. Based on CAP score, patients were divided into groups with NAFLD (CAP score ≥ 250) and non-NALFD (CAP score < 250). The demographic, anthropometric and clinical data was investigated at baseline, 24wk, 48wk, and 96wk. Results: A total of 159 patients (mean age 47.23±11.61 years, 90 (56.6%) males) were involved in the study, with overall percentages of NAFLD as 35.8% (n=57). The two groups were not significantly different in terms of age, sex, biochemical parameters and the status of compensated liver cirrhosis. Moreover, HBV DNA level and the status of HBeAg positive were equally distributed in both two groups. However, the NAFLD group showed a higher level of BMI compared with non-NAFLD group (p < 0.05). Although the HBV DNA undetectable rate increased gradually after treatment with TDF in both two groups, non-NAFLD group showed a higher virological response rate compared to NAFLD group (54.4% vs. 41.1%, 80.6% vs. 69.6%, 95% vs. 72.7%; week 24, week 48, week 96, respectively). However, there was significant difference between two groups only at week 96 (p<0.001). The rate of ALT normalization was higher in non-NAFLD group throughout the whole time spot, but reached no statistical significance. Multivariate analysis indicated that a higher HBV DNA level was associated with early virological non-response and that hepatic steatosis may influence long-term virological non-response. Conclusions: NAFLD significantly affected long-term virological response in CHB patients treated with TDF. Further investigation is needed in the future, which calls for a specific antiviral strategy in CHB patients with NAFLD.
보급형 3D프린터를 이용한 인체 모형 뼈 팬텀 제작의 기초연구 : Femur 대상으로 적층형 출력 방식 이용

본 연구에서는 보급형 3D 프린터를 이용하여 인체 Femur와 유사한 HU 값을 가진 팬텀을 제작하여 기존 돼지 뼈를 대체할 수 있는지 분석하고자 하였다. 인체 Femur의 HU 값을 알아보기 위해 연령별 총 372명의 데이터를 분석하였다. 보급형 3D 프린터를 이용하여 PLA-Cu 20%를 이용하여 인체 뼈 모형 팬텀을 제작하여 CT 검사하였다. 돼지 뼈는 생후 6개월 된 돼지로 도축된 지 2일이 지난 뼈를 이용하였다. 검사결과 내부 채움 80%로 제작한 3D 프린팅 팬텀이 인체의 모든 데이터와 유사한 값이 나타났고(p<0.05) 돼지뼈와는 차이가 있었다(p>0.05). 또한 연령대별 Femur의 HU 값의 경우 연령대가 증가할수록 HU의 값은 줄어드는 것으로 확인 되었다(p<0.05). 3D 프린팅과 HU 값은 적층 높이에 대해서는 약한 음의 상관성을 확인 하였지만 내부 채움에서는 182.13±1.290으로 강한 양의 상관성(R2=0.996)을 확인하였다(p<0.05). 결론적으로 3D 프린팅을 이용한 인체 모형 팬텀이 기존 돼지뼈 팬텀에 비해 인체와 유사한 정도의 HU 값을 나타낼 수 있음을 확인할 수 있었으며 이에 본 연구가 3D 프린터를 이용한 인체 모형 팬텀의 제작에 기초자료를 제공할 수 있을 것이라 사료된다. The purpose of this study was to create a phantom with a HU value similar to that of the human Femur using a 3D printer to replace the existing pig bone. A total of 372 people were analyzed to determine the HU value of human Femur. Using a 3D printer, a human bone model phantom was fabricated using PLA-Cu 20% and subjected to CT examination. Pig bones were 6 months old pigs, and bones 2 days after slaughter were used. As a result of the examination, the 3D printing phantom made with 80% of the internal filling showed a similar value to all data of the human body (p<0.05), and there was a difference from the pig bone (p>0.05). In addition, in the case of the HU value of Femur by age group, it was confirmed that the value of HU decreased as the age group increased (p<0.05). 3D printing and HU values confirmed a weak negative correlation with respect to the stacking height, but confirmed a strong positive correlation (R2 = 0.996) with 182.13±1.290 in the inner filling (p<0.05). In conclusion, it was confirmed that the human body model phantom using 3D printing can exhibit a similar level of HU value to the human body compared to the existing pig bone phantom, and this study will provide basic data for the production of a human body model phantom using a 3D printer.

75세 남자환자가 3개월 전부터 지속된 복부 불편감과 식욕 저하로 병원을 방문하였고, 복부전산화단층촬영을 시행한 결과 췌장 발육 부전증과 췌장 두부의 종괴, 복막의 비후가 관찰되었다. 자기공명담췌관촬영술과 내시경역행담췌관 조영술에서 주 췌관은 정상이었으나 췌부와 미부의 췌관 및 산토리니관은 관찰되지 않았다. 양전자방출단층촬영에서 복막과 폐의 암전이 소견이 나타났으며, 복수천자에서 얻은 세포검사에서 선암종이 확인되어 타 장기의 전이가 진행된췌장암을 진단하였다. 이에 저자는 췌장암이 동반된 배측 췌장 발육 부전증 1예를 보고하는 바이다. Agenesis of dorsal pancreas (ADP) is an extremely rare congenital anomaly defined as an absence of the dorsal ductal system resulting from failure in the embryologic development of the pancreatic dorsal bud. Most of ADP patients are asymptomatic but some of them suffer recurrent pancreatitis and diabetes. Few number of pancreatic adenocarcinoma in association with ADP has been published previously in other countries. There was no such case reported in Korea. <P>Poly(vinylidene fluoride) (PVDF)/graphene oxide (GO) nanocomposite fibers were dry-jet wet spun at the GO concentrations of 0, 1, and 2 wt % with respect to the polymer. The as-spun fibers were drawn in the draw ratio (DR) range of 2-6.5, and the correlation between the PVDF chain conformation and the mechanical properties of the fibers upon drawing has been studied by two-dimensional correlation spectroscopy of Fourier-transformed infrared, wide-angle X-ray diffraction, differential scanning calorimetry, and tensile testing. The PVDF/GO nanocomposite fibers exhibited that the mobile PVDF crystals due to the conformational defects and kinks were nucleated because of the polar interaction between PVDF chains and functional groups of GO, whereas the control PVDF fiber showed the conventional conversion of crystal polymorphs (α and γ phases to β phase). As a result, the nanocomposite fiber showed dramatically improved toughness (enhanced by 1123% at a DR of 2 and 120% at a DR of 6.5) as compared to that of the control fiber. Furthermore, the tensile strength and modulus of the PVDF/GO (2 wt %) fiber were 394 MPa and 4.6 GPa, respectively, whereas those of the control PVDF fiber were 295 MPa and 3.9 GPa, respectively.</P> [FIG OMISSION]</BR>
