억제대 적용지침 개발을 통한 불필요한 억제대 적용율 감소활동

문제: 억제대 적용에 대한구체적 적용지침 없이 의례적으로 억제대를사용함으로 인해 환자들의 신체적 또는 정신적 안전을 위협할 수 있다. 목적: 완전 진정상태 이거나 근력이 약하여 발관능력이 없는 환자에 대한 예방적 적용, 바빠서 환자를볼수 없 는경우, 간호사본인의 판단이 없이 타인의 요구에 따르는등의 불필요한 억제대 적용이 증가하는 원인을 파악하여 중환자실의 특수성에 맞는 억제대 적용및 제거 지침을 마련하고자 한다. 의료기관: 인천광역시에 소재한 의과대학 소속병원의 집중치료실 개선효과: 억제대 적용에 대한중환자실 간호사에게 미치는 영향을 보면 태도 정도 p=0.09(p<0.1), 올바른 수행 능력은 p= 0.005 (p <0.0히로통계적으로유의한것으로 나타났다. 지식 정도 p=0.172(p<0.05) 통계적으로 유의 하지는 않지만 모든항목이 개선 활동전 에 비해 2,3차 개선 활동후에 향상한 것으로 나타났다. 개선활동전에 8.1%였던 간호 기록, 0.7%였던 간호중재, 0%였던 의사처방율이 3차 개선 활동 후 3가지 모두 100% 달성되었다. 이는 억제대 적용율 갑소 및 부적절한 억제대 적용율은 유의하게 감소하였다. 교훈: 중환자실 간호사의 억제대 적용에 대한태도, 지식, 올바른 간호 수행 능력은 지속적으로 재평가 되어지고, 교육내용은좀더 나은 간호제공을 위하여 개발 되어져야 할 것이다. 억제대 적용 기준과 제거 기준을 Check List 하여 객관성의 유지를 위한 개선 활동은계속적으로 Feedback 되어야 할 것이다.

Familial adenomatous polyposis (FAP) is an autosomal dominant syndrome, typically characterized by multiple colorectal adenomas and increased incidence of colorectal carcinomas if it is not treated. Moreover, a variety of extracolonic manifestation are seen. The prevalence of thyroid tumors developing in patients with FAP is about 1∼2%. Generally, papillary thyroid cancer indicates low mortality but ,however, as people get older, the cancer becomes more aggressive. So, the operation is required. Recently we experienced a case of FAP, presenting with papillary thyroid carcinoma, and reported with a brief review of literatures.

The bioequivalence of two nicotine patches was evaluated in 16 normal male volunteers (age 21∼27 yrs) following single transdermal application. Test product was "Nicostop patch" made by Sam Yang Co. and reference product was "Nicotinell TTS patch" made by Korean Searle Ciba-Geigy Co. After nicotine patches were applied onto the inside of the forearm, blood was taken at predetermined time intervals and the nicotine concentration in plasma was determined with a sensitive GC method using NPD detector. AUC and C_max the differences in AUC and C_max between two products were 5.47% and 2.70%, respectively. The powers (1-β) for AUC and C_max were >90% and 88.76%, respectively. Detectable differences(△) and confidence intervals were all less than 20%. All of these parameters met the criteria of KFDA for bioequivalence, indicating that "Nicostop patch" is bioequivalent to "Nicotinell TTS patch".

This study intended to estimate information costs at primary care level and to investigate the factors that would impact on the costs. The object of this study was the outpatients of the 8 clinics selected by random sampling in Kwan-ak Gu, Jong-no Gu, and Kang-nam Gu. This study employed the research method to interview with 164 outpatients of these clinics mentioned above. This study was undergone with hypothesis that the information searching activity in medical sector would include the purchasing activity. Therefore, the cost of medical shopping-around was regarded as also information costs. But, Owing to the technical reason, this study did not include the time cost, the provisional cost that would be spent for the delayed pain(the case of not being cured in exact time), and the cost calculated by valuing the pain itself. Thus, including shopping-around cost, the estimate of information cost was 45,787 won. According to the results, the information cost will rise as the disease-experience is less, symptoms of the disease is more serious, client-agent relationship is weaker, and usual health status is worse.

Great lateral forces arise commonly on retaining wall, piers and bulwark in harbours, bridge abutments and tower structure. Earth pressure dynamic forces(impact of ships), or wind pressure may be named as typical causes for these forces. This paper based on a series of model tests was performed to proposal the decision method of a allow lateral resistance of single pile under lateral load. Model tests were performed in changing of relative density. The model piles were made of copper tube, and the homogeneous model grounds were used air-dried fine sand enough to get through t.4 sieve. A summary test results were analyzed through the experiment as follows. The allow lateral resistance of the proposal method was larger than existing method as safety factor was applied two, that of the existing method was larger than proposal method as safety factor was applied three. But The allow lateral resistance was same to proposal method and existing method as safety factor was applied 2.5.

본고는 궁중정재 중 놀이성과 유희성을 갖고 있는 포구락과 매우 흡사하지만 현재 시행되고 있지 않고 기록만 남아 있는 보상무에 관해 연구한 것으로 연구 결과는 다음과 같다. 첫째로 창사 부분에서 큰 변화는 무자년(1828)때 창작되어진 창사가 1931년 이왕직아악부 때까지 그대로 불려 졌는데, 기축년(1829) 때만 잠시 다른 창사가 불려졌다. 두 번째 춤 구성에 있어서는 각종 `정재무도홀기`와 `이왕직아악부홀기`를 중심으로 1929년과 1931년의 대형 변화를 통한 춤의 변화양상을 살펴보았는데, 춤 구성에는 여러 변화가 있었고, 춤사위에 있어서는 시대의 흐름에도 불구하고 변화가 없었다. 세 번째 보상무의 반주음악은 의궤와 홀기에서 여러 가지 악곡명으로 나타나는 특징이 있다. 이것은 속칭 아명(雅名)이라고 하며, 실제 연주된 곡은 아명 밑에 세주(細註)로 쓰여진 곡을 연주하였다. 여기서 특이한 점은 보통의 정재들은 향당교주(鄕唐交奏)로 연주하는데, 보상무의 1901년 `신축진찬의궤`와 `신축진연의궤`는 원무곡(原舞曲)이라는 것을 연주하였다고 기록하고 있다는 점이다. This current of the times has influenced Korean traditional dance and it activates the effort to discover original motions in the field of Korean traditional dance. Because of the importance of the work, this paper is going to show the research of the unique feature of Posangmu, which was made according to Pogurak in the 23 kinds of Chongjae that appeared newly during the time when Prince Hyomyong (1809-1830) was the right power of representation attorney and exists only in record today. In Kungjung Chongjae performance the same dance works are performed now as in old times. It shows our traditional dances are well preserved through repeated performance with the dance works which stand for Kungjung Chongjae. If we find out forgotten traditional dances and revive them, we will be able to build the frame of dance studies and to help Kungjung Chongjae`s development. Posangmu is different from controlled Chongjae which was performed in front of the ruling class such as king and queen. Moreover it gives the audience extempore amusement and glee. So Posangmu needs to be studied with Pogoorak together and needs to be developed as a new type of Chongjae.
Pseudomonas syringae pv. phaseolicola로부터 제한효소의 분리정제 및 특성

Pseudomonas syringae pv. phaseolicola로 부터 새로운 제한효소 Psy1을 분리하여 그 촉매적 특성에 관하여 조사하였다. 효소의 정체를 위하여 70g의 균체를 이용하였고, 300watt에서 6분간(2분간 3번씩) 열을 위헤서 초음파 마쇄하였다. 이것을 streptomycin sulfate 침전, ammonium sulfate 침전, phosphocellulose pll, DEAE-cellulose, hydroxyapatite, Sephadex G-100 column chromatography를 통해 정제하여 순수한 단백질을 얻었다. 그 단백실 subunit의 분자량은 0.1%의 SDS를 포함하는 polyacrylamide gel에서 전기 영동한 결과를 토대로 분석한 결과 약 50,000 dalton으로 추정된다. 이 효소의 촉매적 성질은 pH7에서 pH10 사이의 광범위한 pH 영역에서 안정하였고, 적정반응온도는 30℃에서 37℃였으며, 그 온도 안정성은 25℃에서 45℃ 사이로 나타났다. 이 효소는 20mM 이상의 MgCl_2, 25mM 이상의 NaCl에서 활성이 저해되었으며, 2-mercaptoethanol 400mM까지 첨가시 계속 활성을 보였고, 플라스미드인 pBR 322와 pUC 18, 그리고 pBS등을 절단할 수 있었다. Lamda DNA를 여러 종류의 제한효소로 절단하여 제한효소 지도를 작성한 결과 SalI의 isoschizomer로 확인되었고 염기서열의 조사 결과로도 확인하였다. A restriction endonuclease, PsyI, has been isolated from Pseudomonas syringae pv. phaseolicola, and its catalytic properties have been studied. This enzyme was purified through streptomycin sulfate and ammonium sulfate fractionation, phosphocellulose pll, DEAE-cellulose, hydroxyapatite and Sephadex G-100 column chromatography. It's molecular weight was about 50,000 dalton as determined by 7.5% polyacrylamide gel electrophoresis containing 0.1% SDS. In catalytic properties, PsyI shows stable at wide ranges of pH between 7.0 and 10.0 of temperature between 30℃ and 37℃, and its thermal stability is between 25℃ and 45℃ at the presence of 10 mM MgCl_2 PsyI essentially require Na salt for enzyme reaction, is rather inhibited in the high Na salt concentration. The presence of 2-mercaptoethanol is absolutely required for the enzyme activity. This endonuclease, PsyI was determined to be an isoschizomer of SalI from the results of the restriction mapping and DNA sequencing.
Streptomyces diastatochromogenes로부터 제한효소 SdiⅠ의 분리정제

토양에서 분리한 방선균에서 제한효소 활성을 조사하여 그 가운데 Streptomyces diastatochromogenes로부터 제한효소 활성을 확인하였다. 이 균주의 제한효소 SdiI을 균체 15g으로부터 streptomycin sulfate침전, ammonium sulfate 침전, hydroxyapatite column chromatography, Sephacryl S-200 HR column chromatography, hydroxylapatite column chromatography의 단계를 거쳐 분리하였고, SDS-polyacrylamide gel electrophoresis에 의하면 소단위체 분자량은 약 35,000 Da으로 추정되었다. About thirty bacterial strains of actinomycete isolated from the soil were examined for the presence of restriction endonuclease activity. Streptomyces diastatochromogenes, which was identified previously, was found to contain restriction endonuclease activity. The purification of this enzyme, SdiI, was carried out via streptomycin sulfate precipitation and ammonium sulfate fractionation followed by hydroxylapatite column chromatography, Sephacryl S-200 HR column chromatography and second hydroxylapatite column chromatography. SDS-polyacrylamide gel electrophoresis of the active protein (purified from various column chromatography) resulted in 35,000 Da protein.
