Thyroidectomy is a safe procedure often performed either for benign or malignant thyroid diseases. Complication rate is low and bilateral recurrent laryngeal nerve (RLN) injury associated with thyroidectomy is rarely described. The RLN may be injured bilaterally and damage is usually recognized postoperatively. With an increased use of intraoperative neural monitoring (IONM), an adaptation of the resection strategy appears to be necessary in case of an intraoperative loss of signal of the first operated side with total thyroidectomy planned. We review a case of a 21-year-old female with a history of Graves' disease who underwent a total thyroidectomy in a 2-stage procedure due to a loss of RLN function detected intraoperatively. The patient recovered uneventfully from the 2 surgeries.
The Path of European Institutional and Economic Integration: What Lessons for Latin America?

What lessons can be drawn from the European experience with regional integration that started in the 1950s, for regional integration in Latin America, which is still in its early stages? We present here a new indicator of institutional integration and study how it developed vis-a-vis diverse measures of economic integration. We find that Latin America is currently less economically integrated not only than the European Union today, but in some cases even than the EU at the beginning of its regional integration process. A cluster analysis illustrates that the link between institutional and economic integration has worked both ways throughout the European experience. The more institutional integration went beyond the creation of a customs union and moved towards a common market and an economic and monetary union, the deeper economic integration turned out. Increasing economic integration in turn corroborated and sustained the process of institutional integration.
Efficacy of Percutaneous Thermal Ablation of Papillary Thyroid Carcinoma

Percutaneous thermoablation (PTA) has become an increasingly used option in the interventional treatment of symptomatic, cytologic and sonographic benign solid thyroid gland disease. Currently, there is no high-level evidence of treatment for differentiated thyroid cancer by means of PTA. Surgery is a standard treatment for primary and also for recurrent thyroid cancer, followed by radioactive iodine and thyroid hormone therapy. PTA, however, can be used in patients at high-risk and in patients who refuse to undergo repeated surgery. The Task Force Committee of the Korean Society of Thyroid Radiology has developed recommendations for the optimal use of PTA for thyroid nodules. However, there are still several questions on safe distance to the laryngeal nerves, no data on duration of PTA application around the nerves, no intra-procedure assessment of laryngeal nerves function and position. Future experimental studies are required.

Several authors investigated the properties which are invariant under the passage from a group to its nonabelian tensor square. In the present note we study this problem from the viewpoint of the classes of groups and the methods allow us to prove a result of invariance for some geometric properties of discrete groups.

An open smooth manifold is said of \textit{finite geometric rank} if it admits a handlebody decomposition with a finite number of 1-handles. We prove that, if there exists a proper submanifold $W^{n+3}$ of finite geometric rank between an open 3-manifold $V^3$ and its stabilization $V^3 \times B^n$ (where $B^n$ denotes the standard $n$-ball), then the manifold $V^3$ has the \textit{Tucker property}. This means that for any compact submanifold $C\subset V^3$, the fundamental group $\pi_1 (V^3 - C)$ is finitely generated. In the irreducible case this implies that $V^3$ has a well-behaved compactification.

Papillary thyroid carcinoma (PTC) is a heterogeneous tumor group with differing pathogenesis and prognosis of the individual subtypes. In addition, a tumor entity has been spun off, now referred to as “noninvasive follicular neoplasm with papillary-like nuclear features (NIFTP). A recent study, based on a comprehensive evaluation of a prospective multicenter NIFTP-based retrospective study, was intended to clarify whether NIFTP can metastasize and thus justify a reduced resection rate compared to classical PTC. For 3 reasons, Authors nicely recommend caution regarding a limited thyroid procedure (i.e. lobectomy vs. completion thyroidectomy): i) In the current literature, lymph node metastases have been described in 10 patients with NIFTP, and in 1 patient lung metastases. ii) As reported for the first time in this study, nearly one-fifth of patients have NIFTP-associated PTMC with unclear potential for metastasis. iii) Observation periods are still relatively short and so far, there are no agreed follow-up standards.
Prevalence of Non-erosive Esophageal Phenotypes in Children: A European Multicenter Study

Background/AimsSince available data on pediatric non-erosive esophageal phenotypes (NEEPs) are scant, we investigated their prevalence and the phenotype-dependent treatment response in these children. MethodsOver a 5-year period, children with negative upper endoscopy, who underwent esophageal pH-impedance (off-therapy) for persisting symptoms not responsive to proton pump inhibitor (PPI)-treatment, were recruited. Based on the results of acid reflux index (RI) and symptom association probability (SAP), patients were categorized into: (1) abnormal RI (non-erosive reflux disease [NERD]), (2) normal RI and abnormal SAP (reflux hypersensitivity [RH]), (3) normal RI and normal SAP (functional heartburn [FH]), and (4) normal RI and not-reliable SAP (normal-RI-not otherwise-specified [normal-RI-NOS]). For each subgroup, treatment response was evaluated. ResultsOut of 2333 children who underwent esophageal pH-impedance, 68 cases, including 18 NERD, 14 RH, 26 FH, and 10 normal-RI-NOS were identified as fulfilling the inclusion criteria and were analyzed. Considering symptoms before endoscopy, chest pain was more reported in NERD than in other cases (6/18 vs 5/50, P = 0.031). At long-term follow-up of 23 patients (8 NERD, 8 FH, 2 RH, and 5 normal-RI-NOS): 17 were on PPIs and 2 combined alginate, 1 (FH) was on benzodiazepine + anticholinergic, 1 (normal-RI-NOS) on citalopram, and 3 had no therapy. A complete symptom-resolution was observed in 5/8 NERD, in 2/8 FH, and in 2/5 normal-RI-NOS. ConclusionsFH may be the most common pediatric NEEP. At long-term follow-up, there was a trend toward a more frequent complete symptom resolution with PPI-therapy in NERD patients while other groups did not benefit from extended acid-suppressive-treatment.
Preoperative Treatment with Lugol Solution for Graves' Disease

Graves' disease is one of the commonest causes of hyperthyroidism. Pre-treatment with Lugol solution, containing iodine/potassium iodide, to induce euthyroidism before surgery may be suggested, especially in case of adverse events due to antithyroid medication. Whether post-operative outcomes are any different following a course of Lugol solution in patient with Graves' disease is not well-documented in the literature. It is definite that large, prospective, randomized controlled trials of clinical and scientific are warranted to answer whether or not preparation with Lugol solution is necessary prior to surgery for Graves' disease.
