        The role of the gut microbiome and diet in the pathogenesis of non-alcoholic fatty liver disease

        Non-alcoholic fatty liver disease (NAFLD) is the leading cause of chronic liver disease, with a prevalence that is increasing in parallel with the global rise in obesity and type 2 diabetes mellitus. The pathogenesis of NAFLD is complex and multifactorial, involving environmental, genetic and metabolic factors. The role of the diet and the gut microbiome is gaining interest as a significant factor in NAFLD pathogenesis. Dietary factors induce alterations in the composition of the gut microbiome (dysbiosis), commonly reflected by a reduction of the beneficial species and an increase in pathogenic microbiota. Due to the close relationship between the gut and liver, altering the gut microbiome can affect liver functions; promoting hepatic steatosis and inflammation. This review summarises the current evidence supporting an association between NAFLD and the gut microbiome and dietary factors. The review also explores potential underlying mechanisms underpinning these associations and whether manipulation of the gut microbiome is a potential therapeutic strategy to prevent or treat NAFLD.         Curitiba, is the capital of the State Of Parana, locatedin the southern Brazil, is the seventh city in population containingapproximately 1, 8 million inhabitants and an area of 432km2. The city began planning in 1940`s where there was formulating a master plan, valuing the deployment of squares and parks in the city. All the planning converge to integrate the urban development, public transportation and environmental conservation, in order to provide quality of life for citizens of Curitiba. The environmental sustainability of the planet necessarily passes through the sustainability of cities. All this planning allowed Curitiba to have: 35 Units of Conservation, among Parks and Forests, covering an area of 11,957,837.00 m2, 3,500,000.00 of these massive forest regions, 77.7 million m2 of massive forest regions was accounted in 2005, covering an area equivalent to 18% of the total city area, 342 different species of birds observed and cataloged, 300 000 trees in the road system and 51.5 square meters of green area per inhabitant. Environmental education is always included in the preparation of projects and in their implementation.Undoubtedly, the planning of which I spoke, is of great importance, but nothing would have been worth it if we had the support of the population of Curitiba. Is not an easy task. Today, I confess to you that on some projects that we have developed, the most difficult part is finding the right approach to engage the population. Without the engagement of the population, is very likely that the project will not succeed. The objective of this presentation is to demonstrate the major environmental programs of the City of Curitibaandtoemphasizetheengagementofourpopulation.

        Primary Linked Total Elbow Arthroplasty for Acute Distal Humerus Fracture Management: A Systematic Review of Clinical Outcome

        Background: The treatment of distal humerus fractures is often challenging in osteoporotic elderly patients. Total elbow arthroplasty (TEA) is a salvage option for non-reconstructable fractures. The aim of this systematic review was to evaluate the clinical evidence for primary TEA in patients with acute distal humeral fractures. Methods: Literatures were searched through PubMed, Ovid/Medline, Cochrane, Google Scholar, and Embase databases with the keywords, “distal humerus fracture,” “total elbow arthroplasty,” and “outcome” according to the MeSH (Medical Subject Headings) index for English-language studies published from April 2009 to April 2019. We performed a systematic review using Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) guidelines. Results: Ten articles with a total of 269 patients were included in the review. The Bryan-Morrey approach was the most common surgical approach (33.7%) with triceps reflecting (42%) for triceps tendon management. The most common implant design used was the Coonrad-Morrey system (83%). The mean postoperative motion arc was 102.3° for flexion-extension and 145.8° for pronation- supination. The average functional outcome score was 89.5 with Mayo Elbow Performance Score (MEPS). An excellent MEPS was found in studies with less than 7 days of average time from injury to surgery. The overall complication rate was 21.5%. Conclusions: The current review showed favorable outcome of primary linked TEA for acute distal humerus fractures. Despite the promising functional outcomes, the complication rate was still considerably high. This systematic review will give surgeons help in explaining to patients regarding the expected outcome after primary TEA for acute distal humerus fractures.

        Rowe Kahn의 성공적 노화 개념: 문화적 요소의 결여

        연구는 Rowe와 Kahn(1997)이 제시한 성공적 노화의 개념 발달의 근간과 그 개념의 틀을 고찰하고, Rowe와 Kahn의 성공적 노화 개념이 한 개인이 속해있는 문화적 배경을 고려하지 않는 한계를 가지고 있음을 주장한다. 캐나다의 이뉴잇족(Inuit), 보츠와나의 산족(!Kung) 및 홍콩의 중국인을 대상으로 수행한 인류학 연구들에 대한 검토를 통해서, 성공적 노화에 대한 이해와 의미 부여가 문화적 배경에 따라 다름을 확인하였고, Rowe와 Kahn의 성공적 노화 개념으로는 설명될 수 없는 부분이 있음을 밝혔다. 이에 본 연구에서는 Rowe와 Kahn의 성공적 노화 개념을 수정·보완하여 기존 개념의 틀이 지녔던 문화적 한계를 극복할 수 있는 방안을 제시하고, 본 연구의 정책적, 실천적 함의를 제시하였다. This study reviews the conceptual framework of successful aging suggested by Rowe and Kahn (1997) and the roots of their conceptualization. This paper suggests that the cultural relevance of their concept is very limited. Using three qualitative studies from the anthropology literature, the limitation in its cultural relevance is examined. The anthropological accounts with the Inuit in Canada, the !Kung in Botswana, and the Chinese in Hong Kong provide evidence that Rowe and Kahn’s conceptual framework fails to consider a cultural context. The paper calls for a revision to Rowe and Kahn’s idea and refines their framework addressing its cultural shortcomings. A brief implication for policy makers, researchers, and practitioners are also discussed.

        Evaluation of Deltoid Origin Status Following Open and Arthroscopic Repair of Large Rotator Cuff Tears: A Propensity-Matched Case-Control Study

        Background: The purpose of this study was to evaluate and compare deltoid origin status following large rotator cuff repair carried out using either an open or an arthroscopic method with a propensity score matching technique. Methods: A retrospective review of 112 patients treated for full-thickness, large rotator cuff tear via either a classic open repair (open group) or an arthroscopic repair (arthroscopic group) was conducted. All patients included in the study had undergone postoperative magnetic resonance imaging (MRI) and clinical follow-up for at least 12 and 18 months after surgery, respectively. Propensity score matching was used to select controls matched for age, sex, body mass index, and affected site. There were 56 patients in each group, with a mean age of 63.3 years (range, 50-77 years). The postoperative functional and radiologic outcomes for both groups were compared. Radiologic evaluation for postoperative rotator cuff integrity and deltoid origin status was performed with 3-Tesla MRI. Results: The deltoid origin thickness was significantly greater in the arthroscopic group when measured at the anterior acromion (P=0.006), anterior third (P=0.005), and middle third of the lateral border of the acromion level (P=0.005). The deltoid origin thickness at the posterior third of the lateral acromion was not significantly different between the arthroscopic and open groups. The arthroscopic group had significantly higher intact deltoid integrity with less scarring (P=0.04). There were no full-thickness deltoid tears in either the open or arthroscopic group. Conclusions: Open rotator cuff repair resulted in a thinner deltoid origin, especially from the anterior acromion to the middle third of the lateral border of the acromion, at the 1-year postoperative MRI evaluation. Meticulous reattachment of the deltoid origin is as essential as rotator cuff repair when an open approach is selected.

        Bilateral Breast Metastases from Epstein-Barr Virus-Associated Gastric Cancer during Pregnancy: Is There a Method to Its Madness?

        Breast metastases of extramammary malignant neoplasms are rare, with an incidence of 0.3%–2.7% among all malignant mammary tumors. Breast metastases from gastric carcinoma are very rare (<0.1%), and this event is even rarer during pregnancy. Herein, we describe a 39-year-old Caucasian woman with a history of an Epstein-Barr virus-associated gastric carcinoma (EBVaGC) that was characterized by prominent tumor infiltrating lymphocytes. Three years after undergoing radical surgery, the patient developed bilateral breast nodules during her pregnancy. A breast biopsy was performed, and histology confirmed a diagnosis of EBVaGC; tumor cells showed positivity for cytokeratin 8/18 and E-cadherin, and negativity for cytokeratin 7, cytokeratin 20, cytokeratin 5/6, caudal type homebox 2, androgen receptor, mammaglobin, gross cystic disease fluid protein-15, and estrogen and progesterone receptors. We also discuss the main diagnostic pitfalls. To our knowledge, this is the first report of an EBVaGC with lymphoid stroma that developed breast metastases during pregnancy.

        Consumer evaluation of customer loyalty programs: the role of customization in customer loyalty program involvement

        This study investigates the role of customizing the offerings (rewards and services) of a customer loyalty program in consumers’ responses regarding perceived value (cash value and emotional value), satisfaction with the loyalty program, and loyalty intention toward the focal retailer. It also examines the moderating role that customer loyalty program involvement may play in value–satisfaction–loyalty associations. Data were collected from 174 college students using a web-based survey. Results yield overall support for the model in which customization of a loyalty program influences perceived cash value of the loyalty program, which in turn influences perceived emotional value,satisfaction with the loyalty program, and loyalty toward the retailer. Additionally,consumer involvement in customer loyalty programs moderates the effectiveness of customer loyalty programs by differentiating the degrees to which perceived emotional value and satisfaction influence loyalty intention.

        Risk factors associated with development of coinfection in critically Ill patients with COVID-19

        Background: At outset of the coronavirus disease 2019 (COVID-19) pandemic, the significance of bacterial and fungal coinfections in individuals with COVID-19 was unknown. Initial reports indicated that the prevalence of coinfection in the general population was low, but there was uncertainty regarding the risk of coinfection in critically ill patients.Methods: Nine hundred critically ill adult patients with COVID-19 infection were enrolled in this observational case-control study. Patients with a coinfection (case) and patients without a coinfection (control) were compared using univariate and multivariable analyses. A subgroup analysis was performed on patients with coinfection, dividing them into early (infection within 7 days) and late (infection after 7 days) infection groups.Results: Two hundred and thirty-three patients (25.9%) had a bacterial or fungal coinfection. Vasopressor use (P<0.001) and severity of illness (higher Acute Physiology and Chronic Health Evaluation III score, P=0.009) were risk factors for the development of a coinfection. Patients with coinfection had higher mortality and length of stay. Vasopressor and corticosteroid use and central line and foley catheter placement were risk factors for late infection (>7 days). There were high rates of drug-resistant infections.Conclusions: Critically ill patients with COVID-19 are at risk for both community-acquired and hospital-acquired infections throughout their hospitalization for COVID-19. It is important to consider the development of a coinfection in clinically worsening critically ill patients with COVID-19 and consider the likelihood of drug-resistance when choosing an empiric regimen.
