        Hybrid operating room applications in the increasingly complex endovascular era: the trump card of modern vascular surgery

        Hybrid operating room represents nowadays an important tool in the management of a constantly increasing number of complex surgical procedures which necessitate appropriate settings in order to be performed safely. We herein present the peculiarities and applications of such a versatile operating environment which is capable of guaranteeing the best performances in terms of equipment and imaging tools respecting the standards of asepsis that a simple angiographic room could not offer. In particular, we focus on its relevance in the field of complex vascular pathology, and on the importance of setting an appropriate management process in order to make the most of its potentialities without sacrificing the not negligible costs connected to it.

        <P>We report on mode-locking of a Cr:YAG laser at 1516 nm using a monolayer graphene-based saturable absorber of transmission type generating 91 fs pulses with a Fourier product of 0.38 at an average output power exceeding 100 mW. Stable single-pulse mode-locked operation without any sign of Q-switching instabilities or multiple pulses is achieved.</P>

        Elective splenectomy in patients with non-Hodgkin lymphoma: Does the size of the spleen affect surgical outcomes?

        Backgrounds/Aims: Splenectomy in patients with non-Hodgkin lymphoma (NHL) is performed to relieve abdominal symptoms, treat hypersplenism or confirm diagnosis. Excision of a very large spleen is technically challenging and data on outcomes of surgery in patients with NHL are scanty. The aim of study was to evaluate the impact of spleen size on the surgical outcome of splenectomy in patients with NHL. Methods: Patients with NHL who underwent splenectomy, between 2006 and 2017, were included and divided into two groups: group 1, spleen ≤ 20 cm; group 2, spleen > 20 cm. Surgical approach, operative time, postoperative morbidity, mortality, hospital stay and re-admission rates were retrospectively compared between groups. Non-parametric data were evaluated with the Mann-Whitney U test. Differences in frequencies were analyzed with Fisher’s exact test. Results: Sixteen patients were included (group 1, 6; group 2, 10). Laparoscopy was successful in three patients of group 1, none of group 2 (p = 0.035), the intraoperative time did not differ significantly between groups. One patient in each group developed postoperative complications. The patient in group 1 died of pneumonia. Median length of stay was 8 days (range, 3–16 days) for group 1, 5.5 days (range, 3–10 days) for group 2, showing no significant difference between the two groups. No patient was readmitted to hospital. Conclusions: Spleen size does not affect the outcome of splenectomy in patients with NHL. If a mini-invasive approach is to be chosen, laparoscopy may not be feasible when the spleen size is > 20 cm.

        Relationship between the Chemical Composition and the Biological Activities of Food Melanoidins

        The relationship between the chemical compositionand the biological activities of food melanoidin-rich fractionswas investigated. Melanoidin-rich fractions were extractedusing ultrafiltration (a 10 kDa cut-off) from coffee, barleycoffee, dark beer, and traditional balsamic vinegar. All thefood melanoidin-rich fractions were formed mainly ofcarbohydrates, phenolic compounds, and proteins. In darkbeer, barley coffee, and traditional balsamic vinegarmelanoidins, glucose was the most abundant sugarincorporated into melanoidins. Coffee melanoidins containedthe largest amount of phenolic groups, followed by traditionalbalsamic vinegar melanoidins. The radical scavenging,Fe2+-chelating, and heme binding abilities of food melanoidinswere investigated under gastric conditions. The melanoidinrichfraction extracted from coffee was the most active,showing the highest radical scavenging, Fe2+-chelating,and heme binding activities, compared to barley coffee,dark beer, and traditional balsamic vinegar. The radicalscavenging and Fe2+-chelating abilities were assigned tothe phenolic groups present in food melanoidins.

        Integrated Level 1 – Level 2 decommissioning probabilistic risk assessment for boiling water reactors

        This article describes an integrated Level 1eLevel 2 probabilistic risk assessment (PRA) methodology toevaluate the radiological risk during postulated accident scenarios initiated during the decommissioningphase of a typical Mark I containment boiling water reactor. The fuel damage scenarios include thoseinitiated while the reactor is permanently shut down, defueled, and the spent fuel is located into thespent fuel storage pool. This article focuses on the integrated Level 1eLevel 2 PRA aspects of the analysis,from the beginning of the accident to the radiological release into the environment. The integrated Level1eLevel 2 decommissioning PRA uses event trees and fault trees that assess the accident progressionuntil and after fuel damage. Detailed deterministic severe accident analyses are performed to support thefault tree/event tree development and to provide source term information for the various pieces of theLevel 1eLevel 2 model. Source terms information is collected from accidents occurring in both thereactor pressure vessel and the spent fuel pool, including simultaneous accidents. The Level 1eLevel 2PRA model evaluates the temporal and physical changes in plant conditions including consideration ofmajor uncertainties. The goal of this article is to provide a methodology framework to perform a decommissioning ProbabilisticRisk Assessment (PRA), and an application to a real case study is provided to show the use of themethodology. Results will be derived from the integrated Level 1eLevel 2 decommissioning PSA eventtree in terms of fuel damage frequency, large release frequency, and large early release frequency,including uncertainties.

        Indications, Outcomes, and Complications of Pedicled Propeller Perforator Flaps for Upper Body Defects: A Systematic Review

        Background The aim of this investigation was to systematically review the current literature to provide the best data for indications, outcomes, survival, and complication rates of pedicled propeller perforator flaps for upper body defects. Methods A comprehensive literature review for articles published from January 1991 to December 2011 was performed using the PubMed, Medline, and Cochrane Databases. Articles without available full-text, single case reports or papers with excessive missing data were excluded. Papers reporting pedicle-perforator (propeller) flaps used for lower extremity reconstruction were excluded from meta-analysis. Results From the initial 1,736 studies our search yielded, 343 studies qualified for the second stage of selection. Of 117 full-text reports screened, 41 studies, met the definitive inclusion and exclusion criteria. Of the selected 41 articles, 26 were case series, original papers or retrospective reviews and were included, whereas 15 were case report papers and therefore were excluded. Two hundred ninety-five propeller flaps were reported to have been used in a total of 283 patients. Indications include repair of trauma-induced injuries, post-trauma revision surgery, cancer resection, chronic infection, pressure sores, and chronic ulcers with a major complication rate (3.3%) comparable to that of free flaps. No specific exclusion criteria for the procedure were presented in the studies reviewed. Conclusions Pedicled propeller flaps are a versatile and safe reconstructive option that are easy and quick to raise and that provide unlimited clinical solutions because of the theoretical possibility of harvesting them based on any perforator chosen among those classified in the body.

        Novel Molecular Mechanisms in the Development of Non-Alcoholic Steatohepatitis

        Non-alcoholic fatty liver disease (NAFLD) is one of the most common causes of chronic liver disease in adults and children worldwide. NAFLD has become a severe health issue and it can progress towards a more severe form of the disease, the non-alcoholic steatohepatitis (NASH). A combination of environmental factors, host genetics, and gut microbiota leads to excessive accumulation of lipids in the liver (steatosis), which may result in lipotoxicity and trigger hepatocyte cell death, liver inflammation, fibrosis, and pathological angiogenesis. NASH can further progress towards liver cirrhosis and cancer. Over the last few years, cell-derived extracellular vesicles (EVs) have been identified as effective cell-to-cell messengers that transfer several bioactive molecules in target cells, modulating the pathogenesis and progression of NASH. In this review, we focused on recently highlighted aspects of molecular pathogenesis of NASH, mediated by EVs via their bioactive components. The studies included in this review summarize the state of art regarding the role of EVs during the progression of NASH and bring novel insight about the potential use of EVs for diagnosis and therapeutic strategies for patients with this disease.

        Bioresorbable Scaffolds in Coronary Intervention: Unmet Needs and Evolution

        Bioresorbable scaffolds (BRS) represent a novel paradigm in the 40-year history of interventional cardiology. Restoration of cyclic pulsatility and physiologic vasomotion, adaptive vascular remodeling, plaque regression, and removal of the trigger for late adverse events are expected BRS benefits over current metallic drug-eluting stents. However, first-generation BRS devices have significant manufacturing limitations and rely on optimal implantation technique to avoid experiencing an excess of clinical events. There are currently at least 22 BRS devices in different stages of development, including many trials of device iterations with thinner (<150 µm) struts than first-generation BRS. This article reviews the outcomes of commercially available and potentially upcoming BRS, focusing on the most recent stages of clinical development and future directions for each scaffold type
