Purpose: The gastrointestinal (GI) tract is the most common site of extranodal non-Hodgkin's lymphoma (NHL), which is increasing in incidence, but there is no established optimal treatment modality. Thus, this study was investigated the clinicohistologic feature, the therapeutic modalities, and the prognosis for GI-NHL, as well as the factors affecting it. Methods: We retrospectively analyzed 45 patients who had been diagnosed as having GI-NHL and had been followed up from July 1994 to February 2005 at Soonchunhyang University Hospital. The patients were divided into groups according to the site of origin and to various other features, and the survivals of the various groups were compared. The modified Ann Arbor system and WHO classification were adopted for staging and histopathologic classification, respectively. Results: GI-NHL of the stomach, small bowel, ileocecal region, and colon occurred in 28 patients (62.2%), 5 patients (11.1%), 3 patients (6.7%), and 8 patients (17.8%), respectively, In one patient, the entire gastrointestinal tract was diffusely involved. The median age of patients was b5 years (25~78 years), and male-to-female ratio was 1:1.1. Fourteen patients were in stage Ⅰ, 24 in stage Ⅱ, 4 in stage Ⅲ, and 3 in stage Ⅳ. Surgical resection was performed in 19 patients, and combination chemotherapy was performed in 43 patients. Surgical resection only was performed in 4 patients, Chemotherapy only was performed in 26 patients. The expected overall 5 year survival of 45 patients was 39.6%, and there was a significant survival difference between the stages, but between sites of origin (p=0.842). The most important factors influencing the survival was the stage and other factors were not significant. Conclusion: The stomach was the most common site of GI-NHL. Most GI-NHL were localized Stage was the most important prognostic factor. However, Prospective randomized studies are needed to approve the therapeutic modality.

현 정부의 대북한 정책의 내용과 방향

Present South Korean government has pursued the engagement policy toward North Korea. In his inaugural address on February 25, 1991, the President Kim Dae-Jung set the improvement of inter-Korean relations through peace, reconciliation and cooperation as the goals of the North Korea policy for his government. Instead of waiting collapse or pursuing absorption unification of North Korea, the government prefer a peaceful coexistence with North Korea. As a result of the engagement policy, there have been many desirable changes on the Korean Peninsula for last nineteen months. As examples, over 7,000 South and North Korean people visited other sides. After the historic tourism began on August 1998, about 111,800 South Korean tourists visited Mt. Kumgang crossing the militarized border. The trade has increased to reach 190 million dollars for seven months this year. For two years, South Korea has provided 74 million dollars of humanitarian assistance to North Korea. Besides, the cases of meeting between separated families have been rapidly increased. In accordance with the engagement policy, the government proposed a comprehensive approach aiming at dismantling the Cold War structure on the Korean Peninsula. All related countries including the United States, Japan, China and Russia, need to discuss political, economical, diplomatic and military issues on the negotiation table to reach an agreement. The Perry report can be said as the result of close consultations between South Korea, U.S. and Japan by South Korean government's initiative. Seeing recent improvement in the U.S.-North Korea negotiations and North Korea's more practical attitudes, I have a prospect that the two Korean relations also will develop into positive directions. The development of the U.S.-North Korea relations will ease tensions on the Korean Peninsula and make a favorable atmosphere for reopening of the South-North Dialogue. To achieve its goals such as changing the North Korea's policy, dissolving security threats and building the basis of peaceful unification, the government should maintain the engagement policy with consistency and firmness. First of all, it should focus on reopening direct talks with North Korea and implementing the South-North Basic Agreement of 1992. It should promote the diverse exchanges and cooperations between two Korean people. And it should make every efforts to relieve the pains of the separated families. The engagement policy and the comprehensive approach can be said to be two wheels of the government's North Korea policy. Whether it will succeed or not depends on North Korea's policy choice, desirable international environment and the domestic supports in South Korea. Coordinated efforts among South Korea, the U.S. and Japan are essential for the success of the comprehensive approach. The government should pay more attentions to obtain international and domestic supports toward its policy.

Background and objectives : The long-term survival in patients with non-Hodgkin's lymphoma (NHL) after conventional dose chemotherapy is about 35% and the rest of the patients tend to have relapse. So, in relapsed or refractory NHL, we compared the outcome of patients undergoing high-dose chemotherapy with autologous peripheral blood stem cell transplantation(APBSCT) with only salvage chemotherapy of cisplatin, dexamethasone, mitoxantrone, and etoposide(CDME). Materials and methods : From June 1993 to December 1999, 25 patients with relapsed or resistant NHL were treated with CDME regimen as salvage chemotherapy. Twelve patients were received four cycles of CDME chemotherapy, and 13 patients were received high-dose chemotherapy with APBSCT following two cycles of CDME chemotherapy. Results : The median follow-up duration was 12.8 months(range:4-68). The overall response rate was 41.7% (complete response rate 25%, partial response rate 16.7%) in 12 patients with CDME only. Thirteen patients who were treated with high-dose chemotherapy with APBSCT achieved 61.5% complete response rate and 15.4% partial response rate, with an overall response rate of 76.9%. The estimated 3-year progression-free survival rate was significantly higher among patients who received high-dose therapy than patients who received CDME only(41.5% vs 20.0%, p<0.05). And, 3-year overall survival rate was significantly higher among patients who received high-dose therapy(51.3% vs 25.0%, p <0.05). Conclusions : In relapsed or refractory NHL, CDME chemotherapy is an effective salvage chemotherapy and allow peripheral blood stem cell collection. Also, high-dose chemotherapy with APBSCT following CDME is superior to CDME salvage chemotherapy only.

Single and 4 weeks oral administration of New Woohwangchungsimwon (NWCH) which was used l-muscone as substitutive material of musk, to beagle dogs of both sexes were performed to investigate both acute and subacute toxicity. Beagle dogs(3 males and 3 females) in acute experiments were administered orally with single dose of 2,000 ㎎/㎏ and groups of 9 male and 9 female beagle dogs in subacute experiments were given daily different dosage of NWCH, 160 ㎎/㎏/day(low dosage group), 400 ㎎/㎏/day (middle dosage group), 1,000 ㎎/㎏/day (high dosage group) once a day for 4 weeks by oral route according to the Established Regulation of Korea Food and Drug Administration (1996. 4. 16). LD_50 value for beagle dogs was more than 2,000 ㎎/㎏ per oral for both male and females. In animals administered with NWCH, there were neither dead animals nor significant changes of body weights. In addition, no differences were found between control and treated groups in clinical sign, urinalysis, eye examination, hematology, serum chemistry, organ weight and other fingings. No histological lesions were observed in both control and treatment groups. Above data strongly suggset that NWCH in beagle dogs is considered to be safe.

Thymoma is associated with myasthenia gravis, Pure red cell aplasia, and autoimmune diseases such as autoimmune hemolytic anemia, aplastic anemia, and hypogammaglobulinemia. It's association with aplastic anemia is rare and aplastic anemia appearing after surgical removal of thymoma is especially rare. The authors hereby report a case of aplastic anemia occuring in a patient who was diagnosed with thymoma and myasthenia gravis and had the tumor surgically removed. The patient was treated with anti-thymocyte globulin, cyclosporin, and prednisolone, and showed partial remission with hematologic improvements after 12 months.

RT-LAMP를 이용한 콩황화일반모자이크바이러스의 진단

Soybean yellow common mosaic virus (SYCMV) has been recently reported, it has been occurred a lot with Soybean mosaic virus (SMV) and Soybean yellow mottle mosaic virus (SYMMV) in soybean field. SYCMV belongs to genus of Sobemovirus and induced viral symptoms with yellowing, mottle and mosaic. A reverse transcription loop-mediated isothermal amplification (RT-LAMP) method allowed one-step detection of gene amplification by simple procedure and needed only a simple incubator for isothermal template. This RT-LAMP method allowed direct detection of RNA from virus-infected plants without thermal cycling and gel electrophoresis. In this study, we designed RT-LAMP primers named SYCML-F3/B3/FIP/BIP from coat protein gene sequence of SYCMV. After the reaction of RTLAMP, products were identified by electrophoresis and with the detective fluorescent dye, SYBR Green I. under daylight and UV light. Optimal reaction condition was at 63 for 60min and the primers of RTLAMP showed the specificity for only SYCMV tested in this study.
