Development of a gamma irradiation loop to evaluate the performance of a EURO-GANEX process

A new irradiation loop design has been developed, which provides the ability to carry out radiolyticresistance studies of extraction systems simulating process relevant conditions in an easy and simpleway. The step-by-step loop configuration permits an easy modification of settings and has a relative lowvolume requirement. This irradiation loop has been initially set up to test the main EURO-GANEX processsteps: the lanthanide (Ln) and actinide (An) co-extraction followed by the transuranic (TRU) stripping. The performance and changes in the composition have been analyzed during the irradiation experimentby different techniques: gamma spectroscopy and ICP-MS for the extraction and corrosion behavior ofthe full system, and HPLC-MS and Raman spectroscopy to determine the degradation of the organic andaqueous solvents, respectively. The Ln and An co-extraction step and the corrosion that occurred duringthe first irradiation step revealed the favorable expected results according to literature. The effects ofacidity changes occurred during the irradiation process, the presence of stainless corrosion products insolution as well as the new possible degradation compounds have been explored in the An stripping step. The results obtained demonstrate the importance of developing realistic irradiation experiments wheredifferent factors affecting the performance can be easily studied and isolated

Purpose: The surgical management of deep infiltrative endometriosis (DE) involving the rectum remains a challenge. The objective of this study was to assess the outcomes from a single tertiary center over a decade with an emphasis on the role of a protective loop ileostomy (PI).Methods: A retrospective review of outcomes for 168 patients managed between 2008 and 2018 is presented including 57 rectal shaves, 23 discoid excisions, and 88 segmental rectal resections.Results: The nodule size (mean±standard deviation) in the segmental resection group was 32.7±11.2 mm, 23.4±10.5 mm for discoid excision, and 18.8±6.0 mm for rectal shaves. A PI was performed in 19 elective cases (11.3%) usually for an ultra-low anastomosis <5 cm from the anal verge. All Clavien-Dindo grade III/IV complications occurred after segmental resections and included 5 anastomotic leaks, 6 rectovaginal fistulas, 2 ureteric fistulas, and 1 ureteric stenosis. Of 26 stomas (15.5%), there were 19 PIs, 3 secondary ileostomies (after complications), and 4 end colostomies. The median time to PI closure was 5.8 months (range, 0.4–16.7 months) in uncomplicated disease compared with 9.2 months (range, 4.7–18.4 months) when initial postoperative complications were recorded (P=0.019). Only 1 patient with a recurrent rectovaginal fistula had a permanent colostomy.Conclusion: In patients with DE and rectal involvement a PI is selectively used for low anastomoses and complex pelvic reconstructions. Protective stomas and those used in the definitive management of a major postoperative complication can usually be reversed.
Prognostic Factors and Decision Tree for Long-Term Survival in Metastatic Uveal Melanoma

Purpose The purpose of this study was to demonstrate the existence of a bimodal survival pattern in metastatic uveal melanoma. Secondary aims were to identify the characteristics and prognostic factors associated with long-term survival and to develop a clinical decision tree. Materials and Methods The medical records of 99 metastatic uveal melanoma patients were retrospectively reviewed. Patients were classified as either short ( 12 months) or long-term survivors (> 12 months) based on a graphical interpretation of the survival curve after diagnosis of the first metastatic lesion. Ophthalmic and oncological characteristics were assessed in both groups. Results Of the 99 patients, 62 (62.6%) were classified as short-term survivors, and 37 (37.4%) as long-term survivors. The multivariate analysis identified the following predictors of long-term survival: age 65 years (p=0.012) and unaltered serum lactate dehydrogenase levels (p=0.018); additionally, the size (smaller vs. larger) of the largest liver metastasis showed a trend towards significance (p=0.063). Based on the variables significantly associated with long-term survival, we developed a decision tree to facilitate clinical decision-making. Conclusion The findings of this study demonstrate the existence of a bimodal survival pattern in patients with metastatic uveal melanoma. The presence of certain clinical characteristics at diagnosis of distant disease is associated with long-term survival. A decision tree was developed to facilitate clinical decision-making and to counsel patients about the expected course of disease.
High electrical conductivity and oxygen barrier property of polymer-stabilized graphene thin films

<P>Next-generation electronics require mechanical flexibility and durability as well as electrical conductivity. In this report, few-layer graphene and polyelectrolytes were assembled into ultrathin films on flexible plastic substrates by a simple and cost-effective layer-by-layer technique. This technique integrated high electrical conductivity and excellent oxygen barrier property into a single film. These characteristics have been rarely reported for other types of thin films prepared by solution processing. The optical properties, film thickness, and mass of the films were precisely controlled by the number of bilayers. After a brief exposure to nitric acid (HNO3) vapor, the films exhibited much improved electrical conductivity while preserving their other properties. Raman, x-ray photoelectron, and FT-IR spectroscopy proved that the effects of the HNO3 treatment were due to the removal of polymeric components and restricted to the film surface, and not due to the chemical doping of graphene. Spectropolarimetric Evidence for a Siphon Flow along an Emerging Magnetic Flux Tube

<P>We study the dynamics and topology of an emerging magnetic flux concentration using high spatial resolution spectropolarimetric data acquired with the Imaging Magnetograph eXperiment on board the SUNRISE balloon-borne solar observatory. We obtain the full vector magnetic field and the line of sight (LOS) velocity through inversions of the Fe I line at 525.02 nm with the SPINOR code. The derived vector magnetic field is used to trace magnetic field lines. Two magnetic flux concentrations with different polarities and LOS velocities are found to be connected by a group of arch-shaped magnetic field lines. The positive polarity footpoint is weaker (1100 G) and displays an upflow, while the negative polarity footpoint is stronger (2200 G) and shows a downflow. This configuration is naturally interpreted as a siphon flow along an arched magnetic flux tube.</P>
