        Role of crystallinity on the thermal and viscous behaviour of polyethylene glycol-in-silicone oil (o/o) phase change emulsions

        Stable polyethylene glycol-in-silicone oil phase change emulsions were successfully prepared with aselected silicone surfactant. These oil-in-oil (o/o) emulsions have been scarcely examined up, despitethe highly remarkable properties of both phases for energy recovery applications and cosmetic/pharmaceuticalcompositions. The objective of the present work was to study the effect of composition (surfactantand disperse phase concentrations) and the post-processing conditions (further agitation at theprocessing temperature for 24 h) on the final features of the emulsion. For this purpose, optical morphology,thermo-physical and the rheological behaviour of the emulsions and binary blends of their compoundswere measured and analysed. Special attention has been paid to structural changes andcrystallinity modifications of the emulsion disperse phase. The observed miscibility of the silicone surfactantand silicone chains with the disperse phase reduces its crystallinity degree and modifies in the crystallizationmechanism of the polyethylene glycol, from heterogeneous to homogeneous nucleation. Interestingly, the change in the disperse phase crystallinity remarkably modifies final thermal and rheologicalproperties of the emulsion.

        The purpose of the investigation project described herewas to develop an intangible heritage inventoryingmethodology suitable for Portugal and in accordance withthe provisions of the UNESCO 2003 Convention, theOperational Directives and the recommendations ofexperts. Within the project, the inventorying methodologyand the instruments developed were tested on a sample ofpeople involved with specific elements of ICH from the fiveConvention domains, and with an ICH element from thedomain of traditional craftwork – the production of inlaidceramics in Nisa, a small Portuguese village in NorthAlentejo.

        Intersections of Gender, Books, and Power : Converging Interests in Narratives by/on Women of East Timor

        This essay discusses some political and theoretical texts on gender in Southeast Asia in general, and in East Timor in particular, with a view to examine values related to traditional female roles, as well as the strategies used to recognize women in political and social fields that are conventionally dominated by males. In the post-colonial space of East Timor, the intersections of past and present, of the global and the local, of war and peace, define the guidelines to explore the negotiation and evolution of gender concepts. Listening actively to the narratives of women from Southeast Asian transitional territories like East Timor, requires translation, equivalence, dialogue and negotiation resources, in order to find points of convergence and divergence with dominant knowledge and thus build and implement concrete and specific strategies, while exercising the hermeneutics of suspicion on the alleged universality of western thought. Este artigo discute alguns textos políticos e teóricos sobre condição de género no Sudeste Asiático em geral, e em Timor Leste, em particular, tentando analisar os valores relacionados com os papéis femininos tradicionais, bem como as estratégias utilizadas para reconhecer as mulheres em campos políticos e sociais que são convencionalmente caracterizados pelo domínio masculino. No espaço pós-colonial de Timor Leste, as intersecções de passado e presente, global e local, guerra e paz, definem as directrizes utilizadas para explorar a negociação e a evolução dos conceitos de género. Escutar activamente as narrativas de mulheres de territórios de transição do Sudeste Asiático, como Timor Leste, exige capacidades de tradução, equivalência, diálogo e negociação, a fim de encontrar pontos de convergência e de divergência com o conhecimento dominante e, assim, construir e implementar estratégias concretas e específicas, ao mesmo tempo que se exerce a hermenêutica da suspeita sobre a alegada universalidade do pensamento ocidental.

        Late avulsion of a free flap in a patient with severe psychiatric illness: Establishing a successful salvage strategy

        Post-traumatic defects of the distal third of the leg often require skipping a few steps of thewell-established reconstructive ladder, due to the limited local reliable reconstructive options. In rare cases, the reconstructive plan and flap choice may encounter challenges when the patienthas psychiatric illness affecting compliance with postoperative care. We describe a caseof a patient with severe intellectual disability and an open fracture of the distal lower limb. After fracture management and debridement of devitalized tissues, the resultant soft tissuedefect was covered with a free gracilis flap. On postoperative day 7, the patient ripped outthe newly transplanted flap. The flap was too traumatized for salvage, so a contralateral freegracilis muscle flap was used. The patient showed good aesthetic and functional outcomes ata 1-year follow-up. When planning the postoperative management of patients with psychiatricillness, less complex and more robust procedures may be preferred over a long and complexsurgical reconstruction requiring good compliance with postoperative care. The medicalteam should be aware of the risk of postoperative collapse, focus on the prevention of pain,and be wary of drug interactions. Whenever necessary, free tissue transfer should be performeddespite potential compliance issues.

        Emergency Department Visits Can Be Reduced by Having a Regular Doctor for Adults with Diabetes Mellitus: Secondary Analysis of 2013 Korea Health Panel Data

        Chronic diseases pose a major challenge to population health worldwide. Diabetes is a major chronic disease that is managed overwhelmingly in primary care. There is an increasing recognition of the role that primary care physicians play to achieve high-quality care for patients with diabetes. By analyzing 2013 Korean Health Panel data, the authors aimed to determine the current status of having a regular doctor (RD) for adults (aged 18 years or older) with diabetes. In addition, the association of having a RD with the experience of emergency department (ED) visits was determined in this study. Among adults with diabetes, those with RD accounted for 41.0%. The older the age group and the higher the Charlson comorbidity index score, the higher the percentage of adults with diabetes had RD. Even for those with RD, coordination of care was very poor (positive answer: 27.1%). After adjustment for confounding variables, those having (vs. not having) a RD (odds ratio [OR], 0.57; 95% confidence interval [CI], 0.35–0.94), especially those whose RDs delivered good comprehensiveness of care (OR, 0.47; 95% CI, 0.26–0.84) or worked at a primary care clinic (OR, 0.43; 95% CI, 0.22–0.81), and those whose longitudinal relationship with a RD was 5 years or less (OR, 0.45; 95% CI, 0.22–0.91) were less likely to have ED visits within the last year. In conclusion, health care policies that promote having a RD who delivers high-quality primary care could decrease unnecessary ED visits by diabetic adults. This can partly reduce ED overcrowding in Korea.

        Breastfeeding for One Month or Longer is Associated with Higher Risk of Osteoarthritis in Older Adults: NHANES 1999-2012

        Breastfeeding is associated with many health benefits to the mother but the association between osteoarthritis (OA) is not known. Menopause, a state of rapid estrogen loss, is associated with OA. To test whether lactation, another physiological state of low estrogen status, is associated with OA, a nationally representative dataset National Health and Nutrition Examination Survey (NHANES) 1999-2012 was analyzed. Information of OA diagnosis history and lactation for at least one month was self-reported by women 50 years and older that had given birth to at least one child. Women that breastfed for less than one month had a higher proportion of those that were 60 years and older, Hispanic (than non-Hispanic Black), and higher current/self-reported greatest body mass index. Women that breastfed for one month or longer had a higher proportion of those that had 3 or more children, higher poverty-income ratio, were post-menopausal and performed vigorous physical activity. Weighted percentage of OA patients was 22%, and did not differ between the two groups. However, when logistic regression was performed adjusting for multiple covariates, lactation for one month or longer was positively associated with OA. Women that breastfed for one month or longer had an adjusted odds ratio of 1.21 (95% confidence interval, 1.05-1.40) for OA compared to those that breastfed for less than one month. The results indicate that women that breastfed for at least one month have a higher risk of OA than women that delivered a child but breastfed for less than one month.

        Virulence factors and genetic variability of uropathogenic Escherichia coli isolated from dogs and cats in Italy

        In this study, the association between virulence genotypes and phylogenetic groups among Escherichia (E.) coli isolates obtained from pet dogs and cats with cystitis was detected, and fingerprinting methods were used to explore the relationship among strains. Forty uropathogenic E. coli (UPEC) isolated from dogs (n = 30) and cats (n = 10) in Italy were analysed by polymerase chain reaction (PCR) for the presence of virulence factors and their classification into phylogenetic groups. The same strains were characterized by repetitive extragenic palindromic (REP)- and enterobacterial repetitive intergenic consensus (ERIC)-PCR techniques. We found a high number of virulence factors such as fimbriae A, S fimbriae (sfa) and cytotoxic necrotizing factor 1 (cnf1) significantly associated with phylogenetic group B2. We demonstrated a high correlation between α-hemolysin A and pyelonephritis C, sfa, and cnf1 operons, confirming the presence of pathogenicity islands in these strains. In addition, UPEC belonging to group B2 harboured a greater number of virulence factors than strains from phylogenetic groups A, B1, and D. REP- and ERIC-PCR grouped the UPEC isolates into two major clusters, the former grouping E. coli strains belonging to phylogenetic group B2 and D, the latter grouping those belonging to groups A and B1. Given the significant genetic variability among the UPEC strains found in our study, it can be hypothesized that no specific genotype is responsible for cystitis in cats or dogs.

        Reliability and Validity of the Appropriateness Evaluation Protocol for Public Hospitals in Korea

        Objectives: This study was conducted to assess the applicability of the Appropriateness Evaluation Protocol (AEP) for public hospitals in Korea. Methods: In May 2016, 1500 admission claims were collected from Korean public district hospitals using stratified random sampling. Of these claims, 560 admissions to 37 hospitals were retrieved for analysis. Medical records administrators determined the appropriateness of admission using the criteria detailed in the AEP, and a physician separately assessed the appropriateness of admission based on her clinical judgment. To examine the applicability of the AEP, the concordance of the decisions made between a pair of AEP reviewers and between an AEP reviewer and a physician reviewer was compared. Results: The results showed an almost perfect inter-rater agreement between the AEP reviewers and a moderate agreement between the AEP reviewers and the physician. The sensitivity and specificity of the AEP were calculated as 0.86 and 0.56, respectively. Conclusions: Our findings suggest that the AEP could potentially be applied to Korean public hospitals as a reliable and valid instrument for assessing the appropriateness of admissions.
