백서와 기니픽 대뇌 피질의 오피오이드 Kappa-2수용체 결합특성

Opioid-수용체에서는 μ,δ그리고 κ의 세가지 주된 형이 존재함이 알려져 있는바, 최근 수용체 동정기업과 선택적인 약물의 개발로 인해 그 아류들이 존재한다고 보고되고 있다. 본 연구에서 백서와 기니픽 대뇌 피질 세포막 표본에서 opioid μ와 δ-수용체를 각각의 선택적인 결합약물로 봉쇄한 후 수종의 κ촉진제와 길항제의〔³H〕diprenorphine(DIP) 결합 친화력 억제에 대한 Na+ion와 GTPΥS의 영향을 관찰하여 이를 지표로 각 ligand의 수용체에서의 작용양상을 구명코자 하였다. 각 표본에서 DIP는〔³H〕DIP 결합부위에서 서로 다른 Ki치를 보였고, U69는 백서 표본에서와 달리 기니픽 표본에서는〔³H〕DIP 결합부가 서로 다름을 보였다. 배양액내 NaC1을 NMDG로 대치시 기니픽에서는 EKC, bremazocine 그리고 U69593의 Ki치는 감소되었으며, 백서에서는 EKC의 Ki는 감소되었으나, bremazocine과 DIP와 Ki치의 변동은 없었다. 백서에서 배양액내 EKC의 친화력은 GTPΥS 50μM에 의해서는 변동되지 않았으나, 100μM에 의해서는 현저히 감소되었으며, DIP와 BRM의 친화력은 GTPΥS 50μM또는 100μM에 의해 현저히 감소되었고, DIP의 친화력은 변동되지 않았다. 본 실험성적은 guanine nucleotides와 Na+ion은 κ-수용체 길항제의 친화력에는 영향을 주지 않으나, 촉진제의 친화력을 감소시키며, opioid κ₂-수용체에 비해 κ₁-수용체는 guanine nucleotides와 Na+ion에 보다 민감하게 조절되며, bremazocine이 κ₂-수용체에 대해서는 길항제로써 작용할 수 있음을 증명하였다. It has been widely accepted that there are 3 types of opioid receptors in mammalian brain, namely μ,δand κ. Recently, multiplicity of each type of opioid receptor was established owing to advance of receptor identification technology and development of more selective ligands. Even though the presence of subtype of κ-opioid receptor was reported repeatedly, selective agonist of antagonist for κ₂-opioid receptor has not been found yet. In this study, the influences of GTPΥS, a nonhydrolyzable guanine nucleotide analogue, and Na+on binding affinity and density of several known κ-agonists and antagonists for〔³H〕dipernorphine(DIP) binding site in the presence of excess of DAMGO(1μM) and DPDPE(1μM) as blocking ligands for μ and δsites. Competition analysis in both rat and guinea pig cortex has shown a single population of〔³H〕DIP binding site with different Kd values. U69,593(U69) could not inhibit the〔³H〕DIP binding even at 10μM concentration. By the replacement of NaC1 with NMethyl-D-Glucamine(NMDG), the Ki values of DIP and Bremazocine(BRM) were not changed, and of ethylketocyclazocine(EKC) was decreased in the rat. But, in the guinea pig, the Ki values of EKC and BRM were decreased leaving the of DIP unchanged. This suggests the BRM would possess the decreased leaving the of DIP unchanged. This also suggests that BRM would possess the antagonistic property at κ₂site which is dominant type of κ-opioid receptor in the rat cortex. The Ki values of EKC were increased by the addition 100μM of GTPΥS but not by 50μM in the rat. In guinea pig cortex, the Ki values of U69, EKC and BRM were increased by both 50 and 100μM of GTPΥS. This results suggest that κ₁opioid receptor is more sensitive to Na+ and guanine nucleotide than κ₂site, and bremazocine apparently possesses antagonistic property at κsite.

Objectives : The aim of this study was to examine the behavioral and emotional problems associated with nocturnal enuresis in Korean children. Methods : Three hundred eighteen children with nocturnal enuresis, together with their parents, completed the Child Behavior Checklist (CBCL), Disruptive Behavior Disorder Scale according to DSM-IV (DBDS), Children's Depression Inventory (CDI), State-Trait Anxiety Inventory for Children (STAIC), and Piers-Harris Children's Self-Concept Scale (PHCSC). Ninety-three normal students were selected as the control group. Results : Compared to the normal control group, the mean scores with regard to the withdrawn, social problems, attention problems, delinquent behavior, aggressive behavior, externalizing problems and total problems profiles were significantly higher in the nocturnal enuresis group according to the CBCL results. The nocturnal enuresis group also scored significantly higher in the ADHD and ODD profiles of the DBDS. The nocturnal enuresis group was more depressed and anxious than the control group according to the results of the CDI and STAI. The mean score of the PHCSC was significantly lower in the nocturnal enuresis group when compared to the normal control group. Conclusion : The results of this study suggest that children with nocturnal enuresis in Korea have clinically relevant behavioral and emotional problems. The findings support the link between nocturnal enuresis and psychopathology in Korean children.

Hearing impairement in varying degrees constitutes one of the most common disabilites affecting school aged children. Eight out of every 1,000 students under the age of twenty have significant hearing loss. Hearing disabilities range from slight to total loss of sound perception. Statistics indicate that only one out of every ten hearing impaired children will become deaf. Most motor activities can be included students may possess low levels of static and/ or dynamic balance, but the greatest majority possess normal motor and physical capabilities. All the hearing impaired youngsters should be properly assessed and given the identical testing batteries as normal children. Hearing impaired students have, essentialy, a communication handicap and suffer from inability to understand verbal instructions. All physical educators should be able to use minimal finger spelling and signing for hearing impaired students.

The study objective was to prepare biodegradable branched dextran microspheres encapsulated with His-tagged interferon-alpha (BDM-hIFN-α) and evaluate its activity in vitro and in vivo. The glycidyl methacrylate derivatized dextrans (Dex-GMA) as precursor was primarily synthesized by substituting hydroxyl groups of either the branched or linear type of dextran with GMA. Dex-GMA microspheres loaded with hIFN-α was then prepared by the water-in-water emulsion technique. In vitro release and Western blotting experiments demonstrated the retained activity of hIFN-α released from branched dextran microspheres at 24 h by inducing phosphorylation of signal transducer and activator transcription-1 (STAT-1), a down-stream effector of IFN-α, in HepG2 cells. Animal data further revealed a peak of plasma levels of IFN-α in rats injected intravenously with BDM-hIFN-α at 10 min post-injection, but a sharp decline at 2 h. High plasma levels of neopterin, a plasma protein induced by IFN-α, were also detected in rats injected with BDM-hIFN-α at 10 min post-injection. Notably, plasma levels of neopterin remained high at 4 h, but largely declined thereafter.
인공슬관절 재치환술 시 경골 폴리에틸렌 삽입물 단독 치환술의 결과

목적 : 인공 슬관절 재치환술 시 경골 폴리에틸렌 삽입물 단독 치환술 시행 후 단기 추시 결과에 대해 조사하여 보았다. 대상 및 방법 : 1991년 6우러부터 2003년 8월까지 슬관절 전치환술을 시행받고 평균 69 (64-89)개월 후에 경골 폴리에틸렌 삽입물 단독 치환술을 시행받은 19명 20예 중 추시관찰이 가능했던 17명 18예를 조사했다. 모든 경골 삽입물 단독 치환술은 1998년 5월에서 2008년 10월 까지 본원에서 시행되었으며 추시 기간은 평균 48 (22-142)개월이었다. 임상적 및 방사선학적 평가를 하였으며, 임상적 평가에는 관절운동범위, HSS score 및 환자군의 하루 평균 노동 시간과 비만도를 조사하였고, 방사선학적 평가에는 대퇴 경골각, 방사선 투과선을 조사하였다. 결과 : 추시상 관절 운동 범위는 평균 110.6°, HSS score는 평균 86.9점, 대퇴경골각은 외반 5.3°로 조사되었다. 하루 평균 노동 시간은 9.7시간으로 조사되었으며 평균 비만도는 25.9로 나타났다. 방사선 투과선은 총 4예에서 보였으나 모두 2 ㎜ 미만의 비 진행성 병변으로 확인되었다. Kaplan-Meier survival analysis에서 생존율은 100%로 조사되었다. 결론 : 인공 슬관절 재치환술에서 제한된 적응증으로 경골 폴리에틸렌 삽입물 단독 치환술을 시행시 인공 슬관절 재치환술의 성공적인 방법이 될 수 있을 것이라고 사료된다. Purpose : We investigated the short term outcomes of isolated tibial polyethylene insert exchange after revision total knee arthroplasty. Materials and Methods : We selected 18 cases from 17 patients among the 20 cases from 19 patients who took isolated tibial polyethylene insert exchange after mean 69 months-follow up of TKA, which was carried out from June 1991 to August 2003. Two cases were excluded on account of loss to follow-up. In all cases, isolated tibial polyethylene insert exchange was carried out from May 1998 to October 2008 in our institute and the mean follow- up period after operation was 48 (22-142) months. We evaluated the following clinical and radiologic aspects: range of motion, HSS score, average labor time, and BMI. Together, these factors were used in clinical evaluation while femorotibial angle and radiolucent line were used in radiologic evaluation. Results : The average range of motion was 110.6 degrees and HSS score was 86.9 in follow up period. The average femorotibial angle was valgus 5.3°. The average labor time was 9.7 hours and BMI was 25.9. Although radiolucent line was found in 4 cases, there were all confirmed to be non progressive lesion less than 2 ㎜. Survival rate was 100% in Kaplan-Meier survival analysis. Conclusion : Selective isolated tibial polyethylene insert exchange can be the successful method for revision TKA in limited cases.
