        Double pyrolus, one of the relatively rare anomalies of the gastrointestinal tract, is a fistulous communication between the gastric antrum and the duodenal bulb. The first case of a double Pylorus was reported in 1969 by Smith, since then several cases have been reported. Recently, as the frequency of examining upper gastrointestinal series and endoscopy increases, it occurs more frequently and a few cases have been described in our country. According to the reports previously published, it resulted only from cysts, diverticla, atresia, band, membrane malformation of the pylorus, however, it might be also caused by penetration, complication of peptic ulcer. We report a case of double pylorus in a 67-year old man with prolonged administration of corticostetoids due to lumbago who has complained epigastric pain which was confirmed by endoscopy and biopsy.

      • 간 낭종을 동반한 성인 다낭신 환자에서 간 낭종의 감염 1예

        Adult polycystic kidney disease(APKD) usually accompanies with renal and extrarenal manifestations and liver cysts are present in aproximately 30% of patients with APKD. Kidney cysts often accompany with hemorrhage, rupture, infection, however, liver cysts are usually asymptomatic and do not impair liver function. Rare complication of liver cysts in patient with APKD include portal hypertension with bleeding esophageal varices and hepatic failure with encephalopathy. But, in APKD patient, infection of liver cysts without infection of renal cysts is extremely rare. We report a case of infected liver cyst in a patient with APKD who has complained generalized abdominal pain and high fever which was confirmed by ultrasound and aspiration. The patient was treated successfully with repeated aspiration and systemic antibiotics.

        Efficiency optimization of an indirect vector controlled induction motor drive is proposed. The loss models are used in the validation of the fuzzy logic based on-line efficiency optimization control. At steady state, the fuzzy controller adaptively changes the excitation current on the basis of measured input power, until the maximum efficiency point is reached. The pulsating torque, due to flux reduction, has been compensated by an ingenious feedforward scheme. During transient state, rated flux is established, to get the best transient response. Through a comprehensive simulation study, the results confirmed the validity of the proposed method.

        A cooling system of chamber is universally appliable with simple ON-OFF control algorithm. It has such disadvantages as having a delay for response time. To solve the above problem, we propose establishing the mathematical model of cooling system for chamber, simulating it with PID control algorithm and evaluating performance tests for control algorithm respectively.

        초음파를 이용한 교근의 단면두께 측정의 재현성에 대한 연구

        Purpose : To determine if ultrasonography is a reliable technique to assess masseter muscle sites within intra- and interobserver and the scanning level and/or the muscle condition affect local cross-sectional dismension (LCSD) measurements of masseter muscle. Materials and Methods : 10 subjects without sign and symptom of temporomandibular disorders and missing posterior teeth were examined by ultrasonography. Bilateral ultrasonographic examinations were performed with a linear (B-scan) 7.5 MHz small-part transducer to register LCSDs of the masseter muscle on three different levels. Scans were made on relaxed and clenching condition. Three oral and maxillofacial radiologists measured at two sessions with a time interval of at least 5 minutes. Results : Variables such as ‘condition’ and ‘level’ had a significant effect on muscle measurements (p>0.05). There was no significant difference betwee‘sessions’(p>0.05) and ‘observers’(p>0.05). LCSDs on lower scan level were significantly thinner than those on upper and middle level. Those on clenching condition were significantly thicker than those on relaxed condition (p<0.05). The scanning level with the highest reproducibility was middle with clenching condition (ICC = 0.90, MSE = 0.55%). Conclusion : The data suggested that ultrasonography was a reliable method for measuring LSCD of masseter muscle in intra- and interobserver and middle scan level showed the most reliable data.

        GABA와 Benzodiazepine 수용체 및 그 기능

        GABA is ubiquitously distributed throughout the CNS. It is probably the major central inhibitory aminoacid neurotransmitter which hyperpolarizes the postsynapic neurons and inhibits the release of neurotransmitters. Generally inhibition of GABA activity causes excitation leading to anxiety and convulsions, whereas activation of its activity causes antiexpressive. anticonvulsive activity and sedation. The GABA receptors have been divided into two distinct groups named GABA(A) and GABA(B) receptors by their pharmacological and physiological properties. GABA(A) receptors are coupled with Bz binding site and that in conjunction with a CI­ channel they form a supramolecular receptor complex which mediates rapid increasing of CI­ influx into postsynaptic neurons. thus contributing to the prompt inhibition of cellular excitability. On the contrary, the GABA(B) receptor does not contain an integral ion channel and is responsible for slow responses through receptor-G protein-effector complexes. GABA(B) receptors appear to be localized on presynaptic nerve terminals and stimulation of presynaptic GABA(B) receptors reduced Ca²+ influx, resulting in decreased release of neurotransmitters. Stimulation of presynaptic GABA(B) receptors increases K efflux, resulting in inhibition of cell firing. A division of function among the two types of GABA receptors appears to exist : GABA(A) receptor complex mediates anxiety, anticonvulsive activity and feeding, GABA(B) receptors, on the other hand, are involved in depression and analgesia. In those cases where GABA(A) and GABA(B) receptors mediate similar functions (e.g., cardiovascular regulation), they do so by affecting different transmitter systems and cellular mechanisms. The putative involvement of GABA(A) and GABA(B) receptors in various behavioral and physiological effects is summarized in Table 3. There are two types of Bz receptors in brain which are central and peripheral type Bz receptors. Central Bz receptors are in many regions of the brain, coupled with the receptors for GABA and they mediate the acute actions of Bz in CNS. Although there is general acceptance that Bz evert their major actions via the GABA(A) receptor, more recent studies suggest that other systems may be involved. In addition, endogenous anxiogenic ligands that interact with Bz receptors in several ways have been discovered, which of these,β-carbolines and DBI may produce anxiety and convulsion in human, The recent investigation of endogenous ligands and specific receptor agonist or antagonist and inverse agonist provide important new conceptual tools for the studies of anxiety and depression mechanisms. It is expected that a better understanding of GABA and Bz receptors will eventually help to demonstrate the biology of anxiety and depression.

      • 주파수 선택성 레일리 페이딩 환경에서 부호화 기법을 이용한 직교 멀티캐리어 시스템의 오율 해석

        본 논문에서는 주파수 선택성 레일리 페이딩 환경에서 RS부호화, 컨벌루션 부호화, 연접부화를 이용하는 직교 멀티캐리어 BDPSK시스템과 QDPSK시스템의 오율특성을 구하였다. 오율특성은 정규화된 지연시간(??), 비트에너지 대 잡음전력 스펙트럼 밀도비(??), 희망 신호전력 대 지연되 비희망 신호전력비(D/U)를 파라미터로 하여 나타내었다. 검토한 결과, BDPSK 신호인 경우 D/U가 20[dB]일 대 오율을 ??으로 유지하기 위해 필요한 Eb/No는 Rs부호화, 컨벌루션 부호화 및 연접부호화에서느 각각 대략 20.8[dB], 16.0[dB], 14.5 [dB]가 필요하며, QDPSK신호인 경우 Rs부호화, 컨벌루션 부호화 및 연접부호화에서 각각 대략 24.0[dB], 18.9[dB], 13.5[dB]가 필요함을 알 수 있었다. This paper has evaluated the error performance of orthogonal multicarrier BDPSK and QDPSK systems which use Reed Solomon coding, convolutional coding, and concatenated coding technique in a frequency selective Rayleigh fading environment. The results show that when D/U (desired wave's power to undesired delayed wave's power ratio)=20dB at error rate of ??, Reed Solomeon coded BDPSK signal, convolutional coded BDPSK signal, and concatenated coded BDPSK signal need about 20.8[dB], 16.0[dB], 14.5[dB] respectively in terms of Eb/No, and Reed Solomon coded QDPSK signal, convolutional coded QDPSK signal, and concatenated coded QDPSK signal need about 24.0[dB], 18.9[dB], 13.5[dB] respectively.

        An accurate loss model of induction motor drive controlled by the converter is proposed. Steady state fundamental and hamonics loss characteristics, besides the dynamics of the machine are analyzed and incorporated in the model, resulting in a new synchronous frame dynamics d-q equivalent circuit. The converter system has been modeled accurately for condition and switching losses.
