외과적 보조의 급속상악확장술 : 치험 5예 REPORT OF 5 CASES

Five adults (3 cases of bilateral posterior crossbite, 2 cases of unilateral posterior crossbite) with intermaxillary discrepancy of over 5㎜ due to maxillary transverse deficiency were treated by surgically assisted rapid maxillary expansion (SARME). Bilateral maxillary horizontal osteotomies, osteotomies of pterygomaxillary junction and anterior lateral nasal wall, ans anterior palatal osteotomy were performed in 4 cases, and unilateral osteotomies in 1case, followed by palatal expansion with tooth-borne orthopedic applicance. No significant complication was observed. The mean palatal expansion was 7.2㎜ (5.8∼10.0㎜) in the maxillary first molar region. During the mean follow-up period of 4 years (5 months∼8 years 7 months), no recurrence of crossbite was observed in all cases. The SARME seemed to be a simple, stable, and reliable procedure for achieving a permanent increase of over 5㎜ in skeletal maxillary transverse width of adults.

As the machine tool industry progresses, the performance of cutting tools also needs to be of good quality and specialized. If the existing metal cutting tool tip is coated properly, its life would be longer and the machining of a difficult-to-cut material also could be possible. For the development of the high quality cutting tool, the applicabilities of TiCN coating deposited by PVD on the cutting insert were experimentally investigated. The various analyses such as SEM micrograph, XRD pattern, AFM surface morphology, and hardness were performed to cross-check the possibility and availability of TiCN coated tool in the field.

Successful alveolar ridge augmentation by distraction osteogenesis method has been reported previously. There are several types of devices for alveolar ridge augmentation. In this paper, a new intraoral distraction device was used on the severe atrophic mandible in order to distract an alveolar ridge vertically in 5 patients. Mean amount of distracted alveolar ridge was 7.4㎜ (5∼9.5㎜). Dental implants were placed in the distracted alveolar bone and good osseointegration was obtained in all patients. Mean follow up period is 2years 6months (7months∼4years 9months). No complications, such as infection or resorption was observed in all patients. Alveolar ridge augmentation by gradual distraction seems to be a safe, simple and reliable procedure.

Pleomorphic adenoma of the palate is the most common of all intraoral salivary tumors. It can occur at any age but it presents most commonly in the 40∼60 age group, and there is no significant sex preponderance. It grows slowly and is usually painless, firm, well-circumscribed nodule. Because of high rate of recurrence, extra-capsular excision including the overlying mucosa and margin of normal tissue is recommended. We report 10 cases of palatal pelomorphic adenoma which have been successfully treated by extra-capsular excision. The surgical defects smaller than 3㎝ in diameter were covered with palatal acrylic splint for the secondary healing. The larger defects were repaired using palatal island flap in 3 cases, rotation flap and buccal fat graft in each 1 case. No patient showed any recurrence or malignant change during the mean follow-up period of 4 years and 9 months. These repair methods seem to be simple, reliable and uncomplicated procedures in the palatal surgical defects.
S-54 : Primary Gastric Histiocytic sarcoma, similar with an inflammatory pseudotumor: A Case report

Histiocytic sarcoma is a malignant neoplasm that originated from myeloid-derived macrophages, and related to myelomonocytic or lymphoblastic leukemia. Histiocytic sarcoma rarely occurs in the stomach. In previous reports, a few cases of histiocytic sarcoma in the stomach were reported. In this case, a 45-year-old female had complained of patient epigastric pain and melena for 1 month. In her laboratory data, hemoglobin was 8 g/dl. In endoscopic finding, there was an ulcerofungating mass with irregular base coated with dirty exudates in distal antrum and body. Abdominal Computed tomography showed a large ulcerofungating mass involving from gastric antrum to duodenal bulb with suspicious invasion of pancreas. In PET CT, metastatic lymph nodes in infrapyloric, perigastric and right hilar area were found. In pathologic findings, tumor cells showed band-like infiltration mainly in the mucosa and submucosa, but also penetrated into serosa in some areas. Tumor cells largely had intermediate-size nuclei with plump, pink cytoplasm. Because there was lack of high degree of cellular atypia or pleomorphism in pathologic findings, final diagnosis as gastric Histiocytic sarcoma was confused with inflammatory pseudotumor. In immunohistochemical staining, the tumor cells were positive for CD68, CD163, LCA, lysozyme, but negative for other immunomarkers including CD21, CD1a, and MPO. The patient underwent palliative distal gastrectomy. The resected specimen after operation clearly demonstrated the malignant nature of this tumor. Based upon the examination, the authors finally diagnosed her as histiocytic sarcoma. The patient was referred to oncology department after operation. She received chemotherapy such as Etoposide, Cisplatin and Iphosphamide. She has another schedules for additional chemotherapy in the out-patient department clinics.

Background/Aims Probiotics are expected to modify the composition of gut microbiota. We aimed to investigate the changes in the composition and diversity of gut microbiota by the administration of probiotics in healthy individuals. Methods Twelve healthy volunteers with age range of 30-42 years provided baseline fecal samples. Subsequently, they took commercially available probiotic capsules (a mixture for Bifidobacterium, Lactobacillus , and Enterococcus ) for 4 weeks. Fecal samples were collected at 4 weeks of administration and 2 weeks after the stop of administration. Fecal microbiota was analyzed via 16S ribosomal RNA gene sequencing. Results The mean Shannon index was not significantly altered by the 4-week administration of probiotics (4.365 vs 4.556, P > 0.05). The proportion of Bacteroidetes, Actinobacteria, Firmicutes, and Proteobacteria was not significantly changed by the 4-week administration of probiotics. At the genus level, the proportions of Lactobacillus (2.138% vs 2.773%, P = 0.028) and Enterococcus (0.022% vs 2.758%, P = 0.004) significantly increased 4 weeks after the administration of probiotics, but reduced 2 weeks after the stop of administration (2.773% vs 3.292%, P = 0.064 and 2.758% vs 0.001%, P = 0.001). Conclusions The diversity of fecal microbiota is not significantly affected by 4 weeks of probiotics administration. The proportion of fecal microbiota at the genus level is significantly altered by the administration of probiotics. However, this effect does not seem to last long, probably because of homeostasis or dietary influence. (J Neurogastroenterol Motil 2018;24:452-459)
Electrochemical Atomic Layer Processing of Compound Semiconductors

We present two electrochemical methods of atomic layer processing of compound semiconductors. One is electrochemical formation of compound semiconductors, based on the principles of atomic layer epitaxy (ALE). This method is referred to ”Electrochemical Atomic Layer Epitaxy (ECALE)”. The other is electrochemical removal of atomic layers in a compound semiconductor in a sequential way. This method is called ’’Electrochemical Digital Etching (ECDE)”. The principle of the electrochemical atomic layer processing methods is associated with a famous phenomena in electrochemistry, which is “Underpotential Deposition (UPD)”. When one element deposits on a second metal surface, the deposition occurs at a potential prior to (under) the potential of bulk deposition (i.e. deposition of the element on itself). The UPD process occurs when compound formation is favored thermodynamically. In fact, the amount of underpotentially deposited element is limited to submonolayer or full monolayer. In the ECALE method, individual solutions of depositing elements are sequentially exposed to a substrate as in gas-phase ALE. One element is deposited on the substrate surface at an appropriate underpotential to obtain one monolayer. Then, a second element is deposited on the substrate whose surface is already covered with one monolayer of the first element. In this way, a compound semiconductor can be formed. The selection of deposition potentials is critical because one of the elements should be deposited without stripping the already-deposited atomic layers. In the ECDE method, the top layer element of a compound semiconductor is stripped underpotentially since it has dangling bonds (i.e. the corresponding element in the bulk is more stable). Upon removing one element in the top layer, a second element, which is exposed to electrolyte, is stripped in the same way. Accordingly, layer-by-layer etching results from a sequential application of underpotentials for each elements with removing the dissolved ion. In this seminar, experimental evidences for the electrochemical atomic layer processing will be presented. The ECALE method will be exemplified with deposition of ZnTe and CdTe on polycrystalline Au electrodes. Likewise, the ECDE method will be explained with etching of CdTe(100) surfaces. In addition, the problems associated with the methods will be discussed.
