        Dipyridamole 부하 심근 SPECT에서 Dipyridamole의 작용과 부작용

        N/A Dipyridamole is an agent that may be used to noninvasively evaluate coronary artery disease. The effect of dipyridamole infusion its generally related to its induced peripheral vasodilatory effect. In normal person, heart rate is generally increased slightly while blood pressure decrease, but the achieved double product and related myocardial oxygen consumption have no significant change. The purpose of this study is to examine the effect and side effect of dipyridamole, and to compare different response to dipyridamole among the patients. We evaluated 847 patients who underwent dipyridamole stress myocardial SPECT. 93.6% of them had induced hypotension 0.9% showed no change of blood pressure, 5.5% had increased blood pressure 8.3% had no change of pulse rate more than 10% of basal pulse rate. Among diabetes, 16.9% was not change of pulse rate, 6.7% in non-diabetes. There was no significant correlation between age and rate pressure product rest(RPPr), in patients without perfusion defects on SPECT(y=7.1x+48.4r=0.13 p>0.01). As increasing age, RPPs/RPPr was declined(y=-11.6x+68.9 r=0.17 p〈0.01), similar results were obtained in patients with perfusion defect. The size of perfusion defect on myocardial SPECT have no correlation between RPPr and RPPs/RPPr. The side effects of dipyridamole included chest pain and chest tightness, headache, abdominal pain, dizzness, nausea, and dyspnea. As increasing age, dipyridamole-induced cardiac work at rest was increased, cardiac response to dipyridamole was decreased.

        For more effective intraperitoneal chemotherapy, the authers made 5-fluorouracil-polyglycolic acid composite disks(FU-PGA disks) with 5-fluorouracil and polyglycolic acid, a biodegradable polymer. We inserted the FU-PGA disk into the peritoneal cavites of Sprague-Dawley male rats, the control and three experimental groups; one FU-PGA disk insert group(300mg/kg), two FU-PGA disk insert group(600mg/kg), and three FU-PGA disk insert group(900mg/kg). The control group received a similar number of PGA disk inserts. A pharmacokinetic study was performed to measure the 5-fluorouracil concentration in the peritoneal fluid, blood, and tissues(liver, kidney and heart) at 24 hours, 72hours and 168 hours after insertion of the FU-PGA disk. Light and electron microscopic studies were done. The results were as follows: 1) The number of white blood cells and platelets decreased after FU-PGA insertion. The degree was proportional to the duration and amounts inserted. The change in the number of red blood cells varied slightly. 2) Light microscopically, slight changes were noted at 168 hours in the 3 disk insert group (900mg/kg). Mild fatty change and hepatocyte degeneration around the central veins of the liver were noted, with vacuolar degeneration in the proximal convoluted tubules of the kidney. 3) Electron microscopically,the liver showed focal increases of lipid droplets in the hepatocytes, and irregular nuclear membrane with focal nucleolar segregation of the fibrillar and granular elements. Also the double membranous structure of the hepatocytes and Kupffer cells was widened with bleb formation. The kidney showed wide separation of the nuclear double membrane in the proximal convoluted tubular cells and mesangial cells, with vacuolation and myelin figure formation in the mitochondria of the proximal convoluted tubular cells, at 72 hours and 168 hours in the 2 to 3 FU-PGA insert groups. The heart showed focal loss of cristae,vacuolation and myelin figure formation in the mitochondria of the 2 to 3 FU-PGA insert groups. In conclusion, despite the large amont of FU-PGA inserted, the histopathological changes in the liver, kidney and heart were slight, and consonant with the very low amount of 5-fluorouracil concentrations detected in the liver, kidney and heart. The above results suggest that the FU-PGA composite can serve as a new device for releasing drugs in a controlled manner and easily targeted to intraperitoneal organs. This device can improve the efficacy of intraperitoneal chemotherapy for gastric cancer.

      • 결핵성 경부임파선염의 전산화 단층촬영 소견

        Purpose : To evaluate the computed tomography (CT) findings of the cervical tuberculous lymphadenitis Methods and materials : The CT features in 10 patients of tuberculous lymphadenitis were retrospectively reviewed. We characterized the morphologic appearances of lymphadenopathy on post-contrast CT scan and comparatively evaluated the densities of central necrotic portion and peripheral solid portion of necrotic nodes on pre-contrast CT scan. All examines were performed on a General Electric Hispeed Advantage RP System. Ten millimeter and five millimeter contiguous sections from skull base to thoracic inlet were obtained for the pre-contrast and post-contrast images respectively. Results : Most patients were young adults. A painless neck mass was the most common chief complaint and the duration of the symptoms was about 5 months. The average number of diseased lymph nodes was 3.7. Unilateral involvement was 4 times as common as bilateral location. Central necrotic lymphadenopathy was 78.3% of 37 enlarged lymph nodes and the density of the nerotic portion was isodense to the density of the peripheral wall on pre-contrast images in 71.4% of necrotic lymph nodes. The most common morphologic features of cervical tuberculous lymphadenitis was the type of a multichambered or conglomerated necrotic lymphadenopathy with a peripheral irregular enhancement. Conclusion : The tuberculous cervical lymphadenitis was highly suggestive when the diseased lymph nodes show the central necrosis, the morphologic features of multichambered or conglomerated necrotic lymphadenopathy with a strong peripheral irregular contrast enhancement, and necrotic materails isodense to peripheral solid portion on pre-contrast CT scan in young adult patients who complain of a painless neck mass.

        육상교통 인프라 구축을 위한 다중 네트워크 간 미지정수 수준 조정이 고려된 Compact Network RTK 보정정보 생성기법 연구

        정적 사용자에 한정되었던 Network RTK 서비스는, 최근 차량 사용자와 같은 동적 사용자를 위한 고 정확도 항법에 대한 요구 증가로 동적 사용자에 대한 적용 연구가 활발히 진행되고 있다. 동적 사용자는 위치 이동에 따라 보정정보가 생성되는 기준 국 조합이 변경될 가능성이 있으며, 사용자의 균일한 정확도 유지를 위해서 Compact Network RTK 보정정보는 기준 국 조합에 상관없이 일관성을 유지해야 한다. 따라서 본 논문에서는 기준 국에서 수행 가능한 네트워크 간 미지정수 수준 조정 방법을 제안하고, 이론적 수식전개를 통해 설명하였다. 전국 단위의 가상의 다중 네트워크를 설정하여 미지정수 수준 조정 방법에 대한 성능 검증을 수행하였으며, 사 용자 이동에 따른 네트워크 변경 상황을 가정하고, 사용자 위치의 보정정보를 미지정수 수준 조정 여부에 대해 비교 분석하였다. 또한, 두 네트워크 경계 지점에 있는 사용자에 대해 GPS 실측 데이터를 활용하여 사용자 성 능을 예측하였다. 그 결과, 미지정수 수준 전 수 cycle에 해당하는 두 네트워크 간 보정정보 불연속이 0.25 cycle 이내로 감소하였으며, 미지정수 수준 전, 2DRMS 40~50cm에 달했던 수평오차가 사용자는 네트워크 변경 에 상관없이 수평 정확도 8 cm (2DRMS) 이내의 일관된 위치 정확도의 연속적인 획득이 가능하였다. Network RTK is widely used especially for static applications so far, however, the demand for high accuracy positioning for kinetic users such as land vehicles is growing for safety and convenience reasons. Kinematic users move along the roads and the network where they receive corrections can be changed. Compact Network RTK corrections should keep consistency while network change. In this paper, we introduced a method of generating Compact Network RTK corrections considering network ambiguity level adjustment by formulation of corrections. We verified the proposed method for reference station networks across whole country. We also generated Compact Network RTK corrections using simulation and real GPS data from reference stations in South Korea and evaluated performance of users. As a result, the discontinuity between corrections from two networks reduced to 0.25 cycle from several cycles. And user could achieve less than 8 cm (2DRMS) horizontal position accuracy continuously regardless of network change.

        전기전도도 측정을 통한 반류수 MLE공정에서 공기공급 제어기술 개발

        목적 : 반류수를 대상으로 무산소조에서 전기전도도 측정을 통해 호기조의 공기공급량을 제어할 수 있는 기술을 개발하여 공정성능 향상 및 에너지 사용량 절감을 달성하고자 하였다. 방법 : 전기전도도가 생물학적처리의 운전지표로 활용될 수 있는지를 검증하기 위해 각 공정별 수질인자와 전기전도도의 상관성 분석을 여름철(메탄올 투입), 겨울철, 가을철 시기로 나누어 실험하였다. 기존에 제시된 필요공기량 수식을 간단히 정리하여 경험식을 제시하고, 이를 활용해 전기전도도 범위에 공기공급량을 순차적으로 넣어 조견표를 작성하였다. 성능평가는 암모니아성질소와 총무기질소의 제거효율, SNR과 SDNR, 공기공급량의 변화, 유입수 변화에 대한 공정의 안정성 등을 기준으로 판단하였다. 결과 및 토의 : 계절별로 전기전도도와 각 수질항목의 상관계수가 0.5267 ~ 0.9115 범위로 암모니아성질소, 총무기질소, DOC, 인산염인 순서로 높게 산출되었다. 상관계수 0.5 이상으로 생물학적 처리공정의 운전지표로 전기전도도 가 활용될 수 있음을 알 수 있었다. 전기전도도와 암모니아성질소의 회귀식은 계절별로 재현성이 확보되어 전기전도도를 통해 암모니아성질소를 추정하는 것이 가능할 것으로 판단된다. 호기조 말단 DO가 3.4 mg/L 정도에서 겨울철 질소처리효율이 가장 양호하였다. 호기조 DO는 공기공급량으로 제어할 수 있으며 질소와 상관성이 높은 전기전도도를 직접 측정하여 공기공급량을 제어하고 질소처리효율을 향상시키는 것이 가능함을 알 수 있었다. 전기전도도와 유입유량을 통해 필요공기량을 산정할 수 있는 경험식을 제시하였고, 식 계수를 구한 결과 A", (B" + X") 는 각각 0.0589 (m³-air/h)/(m³/h)/(μS/cm), -77.562 (m³-air/h)/(m³/h)로 나타낼 수 있었다. 겨울철기간 무산소조의 전기전도도 측정값에 따라서 공기공급량을 자동 제어한 결과는 총무기질소 제거효율과 SDNR이 실플랜트 조건보다 각각 8.3%, 0.007 g-N/g-MLSS/d 높게 나타났다. 자동제어한 기간 동안에 공기공급량/유입유량 평균비가 36(m³-air/h)/(m³/h)로 실플랜트 조건보다 공기공급량을 21.7% 감소시킬 수 있었다. 결론 : 전기전도도 기반 MLE공정의 공기공급 제어기술은 질소제거 효율 향상 및 에너지 사용량 절감을 동시에 달성할 수 있을 것으로 판단된다. Objectives : This study aimed to achieve improved process performance and energy saving by developing a technology to control the air supply of an aerobic basin by measuring the conductivity in the anoxic basin. Methods : To verify whether conductivity can be used as an operation indicator of biological treatment, the correlation analysis between water quality factor and conductivity of each process was conducted by dividing into summer (methanol input), winter and autumn periods. An empirical formula was presented by briefly arranging the required air quantity formula, and a quick reference was prepared by putting air supply in the conductivity range sequentially. The performance evaluation was judged based on the removal efficiency of ammonia nitrogen and total inorganic nitrogen, SNR and SDNR, the change of air supply, the stability of the process against inflow change. Results and Discussion : The seasonal correlation coefficients of conductivity and water quality items were calculated in the order of ammonia nitrogen, total inorganic nitrogen, DOC, and phosphate in the range of 0.5267 ~ 0.9115. It was found that the conductivity could be used as an operation indicator of the biological treatment process with a correlation coefficient of 0.5 or more. The regression equations for the conductivity and ammonia nitrogen are secured by season, so it is possible to estimate the ammonia nitrogen through the conductivity. At the end of the aerobic basin DO was 3.4 mg/L, the nitrogen treatment efficiency in winter was the best. The aerobic basin DO can be controlled by the air supply, and it can be seen that it is possible to control the air supply and improve the nitrogen treatment efficiency by directly measuring the conductivity having a high correlation with nitrogen. An empirical formula for estimating the required air volume through conductivity and inflow is presented. A" and (B" + X") are 0.0589 (m³-air/h)/(m³/h)/(μS/cm) and -77.562 (m³-air/h)/(m³/h). The result of automatic control of air supply according to the measured conductivity of anoxic tank during winter season showed that total inorganic nitrogen removal efficiency and SDNR were 8.3% and 0.007 g-N/g-MLSS/d higher than the actual plant conditions, respectively. During the automatic control period, the air supply/inflow average ratio was 36 (m³-air/h)/(m³/h), which could reduce the air supply by 21.7% compared to the actual plant conditions. Conclusions : The air supply can be estimated from the flow rate and conductivity. The air supply control technology of the conductivity-based MLE process will be able to simultaneously improve nitrogen removal efficiency and reduce energy consumption.

      • Chunghyul-Dan(Qingxie-Dan) Improves Arterial Stiffness in Patients with Increased baPWV

        Arterial stiffness is an important, independent determinant of cardiovascular risk. Pulse wave velocity (PWV) has been used as a valuable index of arterial stiffness and as a surrogate marker for atherosclerosis. Chunghyul-dan (CHD) has anti-hyperlipidemic activity, anti-inflammatory activity and anti-atherogenic effects. To determine its clinical effect on increased arterial stiffness, we examined whether CHD improves arterial stiffness in patients with increased brachial-ankle PWV (baPWV). Thirty-five subjects with increased baPWV (> 1400 cm/sec) were recruited and randomized to a treatment group (20 subjects) or a control group (15 subjects). The treatment group was administered CHD at a dose of 600 mg three times a day for 8 weeks, and the control group received no medication (observation only). baPWV was assessed using a pulse pressure analyzer at baseline and after 8 weeks. Blood pressure and serum lipid profile were monitored in the treatment group. Our results indicate that baPWV was lowered significantly in the treatment group after 8 weeks of medication (p < 0.05), but not in the control group. Moreover, there were no significant changes in blood pressure and serum lipids profile except triglyceride level suggesting that the effect is largely independent of CHD's lipid-lowering effect or a blood pressure change. In conclusion, CHD appears to improve arterial stiffness in patients with increased PWV.

        The Korean hemorrhagic fever (KHF) is an acute febrile disease with characteristic of fever, bleeding tendency, and renal failure. There are many complications of Korean hemorrhagic fever such as infection, anemia, internal bleeding, hypopituitarism, respiratory, and neurologic complication. A few cases were reported on acute pancreatitis with hemorrhagic fever abroad, but there was no case about Korean hemorrhagic fever with acute pancreatitis in this country. We experienced a case of Korean hemorrhagic fever associated with suspected acute pancreatits. With review of articles, we report a case of 51 year-old woman with KHF, where acute pancreatitis developed during management, (Korean J Infect Dis 33:376∼379, 2001)

        뇌혈관 협착 환자에서 SPM과 확률뇌지도를 이용한 기저/아세타졸아미드 SPECT의 정량적 분석법의 유용성

        목적: 뇌혈관질환에 있어서 뇌혈류와 뇌혈류예비능을 기저/아세타졸아미드 뇌혈류SPECT로 평가 가능하나 재관류 수술효과를 비교 평가하기 위해서는 정량적인 평가가 필요하다. 대상 및 방법: 12명의 환자(51±15세, 남:녀=6:6)의 수술 전후 기저 시와 아세타졸아미드 부하 뇌관류 SPECT를 SPM에서 공간정규화하고 소뇌의 계수를 기준으로 계수정규화한 후 확률뇌지도(statistical probabilistic anatomical map, SPAM)를 이용하여 부위별 혈류를 정량화하였다. 이 결과로부터 수술 전후 및 정상대조군(59±15세, 남:녀=10:11)과의 비교를 McNemar test와Mann-Whiteny test를 이용하여 비교하였다. 혈류예비능은 기저 시와 부하시의 차이를 기저 시 계수를 기준으로 백분율로 표현하였다. 결과: 수술 후 수술 부위의 기저 시와 아세타졸아미드 부하 시 혈류 그리고 혈류예비능 모두가 유의하게 향상되었다(p<0.05). 정상군과의 비교에 있어서는 기저 시 혈류는 정상군과 차이가 없을 정도로 호전되었으나, 아세타졸아미드 부하시 혈류와 혈류예비능은 수술 후에도 유의한 차이를 보였다(p<0.05). 혈류예비능은 주로 수술부위를 중심으로 향상되었다. 결론: SPM 및 SPAM을 이용하여 기저/아세타졸아미드 뇌혈류 SPECT의 수술 전후결과를 정량적, 객관적으로 쉽게 비교 평가할 수 있다. Purpose: While cerebral blood flow and cerebrovascular reserve could be evaluated with basal/acetazolamide Tc-99m-HMPAO SPECT in cerebrovascular disease, objective quantification is necessary to assess the efficacy of the revasculariztion. In this study we adopted the SPM method to quantify basal cerebral blood flow and cerebrovascular reserve on basal/acetazolamide SPECT in assessment of the patients who underwent bypass surgery for linternal carotid artery (ICA) stenosis. Materials and Methods: Twelve patients (51±15 years) with ICA stenosis were enrolled. Tc-99m-HMPAO basal/acetazolamide perfusion SPECT was performed before and after bypass surgery. After spatial and count normalization to cerebellum, basal cerebral blood flow and cerebrovascular reserve were compared with 21 age-matched normal controls and postoperative changes of regional blood flow and reserve were assessed by Statistical Parametric Mapping method. Mean pixel values of each brain region were calculated using probabilistic anatomical map of lobes. Perfusion reserve was defined as the % changes after acetazolamide over basal counts. Results: Preoperative cerebral blood flow and cerebrovascular reserve were significantly decreased in involved ICA territory, comparing with normal control (p<0.05). Postoperative improvement of cerebral blood flow and cerebrovascular reserve was observed in grafted ICA territories, but cerebrovascular reserve remained with significant difference with normal control. Improvement of the cerebrovascular reserve was most prominent in the superior temporal and the angular gyrus, nearest to the anastomosis sites. Conclusion: Using SPM quantification method on basal/acetazolamide Tc-99m-HMPAO SPECT, the cerebral blood flow and cerebrovascular reserve could be assessed before revascularization and so could the efficacy of the bypass surgery.         사회인지이론 기반 중산층 노인 식품안전 교육 프로그램의 효과 평가

        현대의학의 발달은 인간의 수명을 연장시키고 그 결과 노인인구 증가를 가져와 노인 의료비 문제가 사회적으로 대두되고 있다. 이에 적절한 방안으로는 노인들의 건강한 노화(healthy aging)를 도모하는 것이며, 이는 사회의 의료비용을 감소시킴으로써 사회 전체의 효율성을 제고시킬 수 있다. 본 연구는 중산계층 노인을 대상으로 사회인지이론 기반의 식품안전 관리 교육 프로그램을 적용하여 시범실시를 하여 교육 효과 평가를 실시하였다. 본 연구에서는 교육군 212명, 대조군 42명의 자료가 분석되었으며, 사전 사후 지식 총점의 변화는 교육군이 2.80점에서 3.80점으로 유의적으로 상승하였다(P<0.001). 교육군의 식품안전 식태도/건강신념의 교육 전과 후의 총점은 20점 만점에 15.28점에서 16.50점으로 유의적인 상승을 보였다(P<0.001). 대조군은 사전 15.21점에서 사후 15.29점으로 상승하였으나 유의적인 차이를 보이지 않았다. 노인의 식품안전 식행동은 교육군의 교육 전과 후의 총점이 28점 만점에 20.07점에서 22.50점으로 2.43점 상승하여 유의적인 향상을 보였다(P<0.001). 가정 내 식품 취급실태를 살펴보면 식품을 직접 구매한다고 한 응답자가 교육군의 87.3%, 대조군의 95.2%로 나타났으며, 식품 조리 여부 항목에 대해서는 교육군의 87.7%, 대조군의 95.2%가 직접 가정에서 조리한다고 응답하였다. 식태도/건강신념 변화와 식행동 변화값 간에 상관관계가 가장 높으며(r=0.447, P<0.001), 식행동 변화값과 지식의 변화 값과의 상관관계가 높은 것으로 나타났다(r=0.152, P<0.05). 경로별 교육 효과를 보기 위하여 사전, 사후 점수와 점수변화량을 비교 분석한 결과 지식, 식태도/건강신념, 식행동의 모든 경로에서 유의적인 향상을 보였다. 개발된 교육 프로그램의 효과 평가는 2차 시 교육의 사전, 사후 평가를 대조군의 평가 결과와 비교 분석하였는데, 교육의 기간이 2차시로 제한되어 있고 교육 후 행동 변화의 유지를 평가하지 못하였다. 또한, 교육경로와 대상자의 수가 적어 연구 결과를 일반화하기에는 무리가 있으며, 향후 교육의 확대를 통한 후속연구들이 진행되어야 할 것으로 사료된다. This study assessed the educational effectiveness of a developed food safety management education program based on social cognitive theory for elderly in the middle class. The subjects participated in the food safety education program two times at a public health center, community welfare center, senior citizens’ center, and center for children’s foodservice management. For food safety intervention education, data from the intervention group (212 subjects) and control group (42 subjects) were analyzed. Significant (P<0.001) differences in the scores for food safety knowledge, food attitude, and food behavior were observed in the intervention group before and after the study, but no significant difference was observed in the control group. A significant positive correlation also detected between knowledge, food attitude/health belief, and food behavior in the intervention group, which indicates that the study contributed to perception changes in the intervention group and eventually behavioral changes. Accordingly, this program can induce cognitive and behavioral changes by conducting food safety training for the elderly.
