      • Disorders of Liver : Is Endoscopic Variceal Ligation Effective For The Treatment Of Bleeding From Gastroesophageal Varices (GOV)?

        Background/Aims: The optimal management of bleeding from gastroesophageal varices (GOV) remains controversial. The aim of this study was to investigate whether endoseopic variceal ligation (EVL) is effective and safe for the management of bleeding from GOV. Methods: Forty-one cirrhotic patients receiving EVL due to a history of variceal bleeding from GOV were analyzed. In twenty-two patients of them, serial endoseopic ultrasonography (EUS) were performed. GOV were grade using the system suggested by Sarin et al: type I (GOVI) and type 2 (GOV2). Results: Twenty-eight patients were GOVI and thirteen patients were GOV2. Successful treatment for controlling acute bleeding were achieved in 26/28 (93%) in GOVI and 9/13 (85%) in GOV2. Rebleeding was 15% in GOVI, while 40% in GOV2. Multivariate Cox regression indicated that type of GOV, variceal size and residual variceal size after EVL were independent factors predictive of GOV re-bleeding. Serial EUS findings demonstrated that the size of GOVI became decreased after EVL in 10/15 (67%) patients, while GOV2 did not change. Conclusions: EVL was safe and effective for the emergency treatment of acutely variceal bleedings from GOV. For the preventing variceal rebleeding, EVL was more effective in GOVI than GOV2.

      • 해안선 변화를 예측하기 위한 수치모델에 관한 연구

        본 연구는 해안선 변화에 대하여 수학적으로 검토하므로서 해안선 발달에 대한 현상을 정량화 하고 나아가 장래의 해안선 변화를 예측할 수 있는 수치모델으 ㄹ제시한다. 해안은 토지이용의 극대화에 의한 시설물 증가 등에 의해 끊임없이 영향을 받고 아울러 해안선은 퇴적, 침식으 ㄹ거듭하면서 변화되고 있다. 그러나 대부분의 시설물의 보호를 위한 방파제 등에 대한 연구는 많이 진행되고 있으나 토지 이용면에서 해안의 보호를 위한 안벽에 대한 연구는 별로 진행되고 있지 않다. 그러므로 본 연구에서는 시설물 중 안벽의 영향을 분석하고자 한다. The quantification of phenomena presented by the shoreline changes and the prediction of future changes of shoreline are investigated by using a numerical model in this study. Shoreline has been sucessively affected by the activities of reclamation to maximize the land use and it also has been changed with cycles of accumulation and erosion of deposits. Many researches were performed on the fetties constructe to protect facilities adjacent to the shore. However, few studies on a seawall of protecting the beach, being very inportant in terms of land use, were carried out. Therefore, this study is to analysze effects of a straight seawall to shoreline changes.

      • Fatigue analysis of floating wind turbine support structure applying modified stress transfer function by artificial neural network

        <P><B>Abstract</B></P> <P>The frequency-domain approach has been studied as a potential replacement modality for the time-domain method in fatigue analysis of offshore wind turbine structures. It is assumed that in the frequency-domain approach, the stress response spectra induced by wind and wave loads can be expressed by a stress transfer function. To obtain the stress transfer function, coupled analysis should be performed in advance. However, since the response of a wind turbine to different average wind speeds is non-linear, the stress transfer function is bound to change with wind speed. This means that repeated simulation is needed in order to calculate the stress transfer function according to wind speed change. The problem, though, is that if the number of simulations is large, prohibitively high computational and time costs probably will be incurred. In this study, to reduce the number of simulations and, at the same time, increase the accuracy of results, a correction factor of the stress transfer function induced by wind load was artificial-neural-network-approximated as a function of mean wind speed and frequency. Sensitivity analysis was conducted to determine how many sample points are required and how to select them. Also, a superposition model is proposed to improve the accuracy of the ANN model. This model is designed so that the peaks in the stress spectrum have a dominant influence on fatigue damage. In order to better simulate the correction factor around the peak, the model considering only the data of the periphery of the peaks and the model reflecting the whole data are superimposed. The total stress spectrum were calculated by summing stress spectrum induced by wind load from the ANN model and induced by inertia load from motion analysis based on linear wave theory. Numerical analysis for a 10 MW class wave and offshore wind hybrid power generation (WWHybrid) system, which is a kind of semi-submersible wind turbine platform, was performed to verify the performance of the proposed model. It was confirmed that the superposition model improves the accuracy by 20–50% compared with the single ANN model.</P> <P><B>Highlights</B></P> <P> <UL> <LI> This paper proposes a procedure to apply the regression model to predict the fatigue life of a floating wind turbine support structure. </LI> <LI> The proposed procedure shows the stress transfer function induced by wind loads as a function of mean wind speed and frequency through a neural network model. </LI> <LI> A sampling strategy for constructing a model and a superimposed model concept for reflecting different weights between samples to a model are suggested. </LI> </UL> </P>

      • 혈압 및 혈관손상에 대한 Candesartan과 Cilnidipine의 병용투여 효능 평가

        This study was designed to evaluate the effect of candesartan combined with cilnidipine on high blood pressure in spontaneous hypertensive rats (SHRs) and vascular injury induced by cuff-placed neointimal hyperplasia model. To measure blood pressure, low and high doses of candesartan (0.765 and 1.53 mg/kg/day) and cilnidipine (0.96 and 1.92 mg/kg/day) alone or in combination were orally administered in telemetered-SHRs for 2 weeks. To examine the protective action of vascular injury, high doses of candesartan (1.53 mg/kg/day) and cilnidipine (1.92 mg/kg/day) alone or in combination was orally administered for 2 weeks in cuff-induced neointimal hyperplasia model of C56BL/6 mice. As a result, high doses of candesartan and cilnidipine in combination decreased in systolic blood pressure (SBP) and mean arterial pressure (MAP) as compared with monotherapy of high dose of each drug, but not statistically significant. In vascular injury model, candesartan and cilnidipine in combination decreased BrdU-positive cells in neointima and vascular smooth muscle cell proliferation as compared with monotheray of each drug. Thus, these results indicate that the combination of candesartan with cilnidipine is effective for the protection of vascular injury, suggesting this combination to be a valuable tool for the management of vascular remodelling such as restenosis.

      • 濟州島民의 性格 : 濟州島民謠에 反映된 島民性

        Those interested have made efforts to research for Tamla -Jeju- Spirit lately. I tried to find out the personality of Jeju islanders and the adjustment pattern of them in the mirror of Jeju-ballads. I divided their adjustment pattern into defense mechanism, rationalization mechanism, escape mechanism, aggression mechanism, and other adjustment mechanisms. According to those I divided them as 17 subcla ?? ifications again-daydreaming, identification, compensation, sublimation, projection, sour-grape, sweet-lemon, delusion, isolation, repression, regression, fixaation, obsession, direct-aggression, indirect-aggression, self-aggression, reaction-formation, etc. As the result of making analysis, study the ballads from the side of quantity, the main contents found out are as follows: 1) General tendency of Jeju islanders in the mirror of Jeju-ballads discovered in order of percentage was to the following effect: 30% at aggression mechanism, which was chiefly formed, 23% at escape mechanism, 22% at rationalization mechanism, 15% at defense mechanism, 6% at other reaction formation, etc., and it showed that Jeju islanders depended on aggressive adjustment pattern chiefly. 2) As the result of making analysis from 17 behaviour patterns, it showed following orders: 25% at indirect-aggression, 15% at sweet-lemon, 9% at isolation, 9% at compensation, 7% at repression, 6% at reaction formation, and other behaviour patterns. According to those, it is found out that the personality of Jeju islanders is chiefly formed with indirect-aggression and sweet-lemon, etc. 3) The personality of the islanders in the mirror of defense mechanism was showed in order of the following percent : 8% at Compensation behaviour, 4% at behaviour pattern of sublimation, identification .and daydreaming. It is very desired personality in the process of its formation that they try to cover frustration in their daily life with compensation behaiviour mainly. 4) The personality of Jeju islanders in the mirror of rationalization mechanism is as following orders; 15% at sweet-lemon, 4% at sour-grape. It proved that they have depended on the sweet-lemon-they ascribe their ill luck to fate if frustration happened to them. 5) The personality of Jeju islanders in the mirror of escape mechanism in showed as behaviour pattern of the following percentage: 9% at escape, 7% at repression, 3% at regression, and fixation, etc. It proved that they have solved their frustration in their daily life with escaping contact with others (isolation) or with patience (repression). 6) The personality of Jeju islander in the mirror of aggression mechanism is showed 25% at indirect-aggression, which is occupied chiefly, self-aggression, direct-aggression in order. It proved that ancestors of the islander did not criticize others for their conscience' sake directly, but make cynical remarks on them: by which they solved their daily problems-tension, anxiety, complication, etc. In conclusion the process of forming the personality of Jeju islander has chiefly depended on behabiour pattern of sweet lemon which ascribe their ill luck to fate, and the personality of them in the mirror of Jeju-ballads is indirect-aggression and reaction consciousness in a word.

        안면신경 마비 환자에 있어서의 측두근 및 근막피판을 이용한 안면근 기능 회복 증례보고

        Generally, the totally paralyzed face can never be made normal by any of the current methods of reconstruction. Careful selection of patients based on sound judgment of what can and cannot be achieved by the proposed surgical technique is paramount to a successful operation and a satisfied patient. The results are related to time of delayed between injury and repair : the shorter the delay the better are the results. The objectives in correcting facial paralysis are to achieve normal appearance at rest ; symmetry with voluntary motion ; control of the ocular, oral, and nasal sphincter; symmetry with involuntary emotion and controlled balance when expressing when expressing emotion ; and no significant functional deficit secondary to the reconstructive surgery. It must be employed a number of concepts, for treatment of the paralyzed face by surgeon, depending on the cause, time interval, and wound characteristics, as well as the availability of and necessity for neuromuscular substitution. Nerve grafts, crossovers, muscle transfers, free muscle and nerve-muscle grafts, micronuerovascular muscle transfers, and regional muscle transpostion are the principal methods being developed. We applied the tmporal musle transposition for reanimation of unilatrally paralyzed faces for long times on two patients. The results of muscle transposition can be enhanced by the patient's learning to activate the transposed muscle by voluntary effort, and are best in patients who are motivated to learn the necessary motor-sensory coordination techniques.

        This study is intended to approach the artistic system of Sparring arroung it s artistic facts. To find out the connection between the Taekwondo Sparring and aesthetic facts, I studie Sparring's aesthetic facts in space, art and drama. Taekwondo is the activity to concerte the desire of body and mind based on the protective instinct. Sparring is the activity to attack and defend oneself using hand and feet. It needs momentary technique according to opponent's acting. Psychological warfare with opponent, strategy, technique are needed in space and aesthetic characteristic is realized when the technique is carried out in this restricted space. The end of sparring is the expression of aesthetic and it has the ability to overcame unexpected situation effectively by using various technique and proper method. It also has dramatic fact due to being limited to time. It is three-game contest and each game takes three minutes, and so player had better not to finish the game in on early state but to penetrate the opponent. Therefore even though there are great differences in score, Sparring has dramatic fact to reversed.

      • Losartan과 Amlodipine의 병용투여에 의한 고혈압 및 혈관재협착 보호효과

        Abstract - To examine the protective effect of combination treatment of angiotensin II AT1-receptor blockers (ARBs) with Ca2+ channel blockers (CCBs) for high blood pressure and neointima hyperplasia, losartan and amlodipine were orally injected into telemetered-spontaneous hypertensive rats and vascular injury induced by cuff-inserted neointimal formation model in vivo. For the measurement of blood pressure, random orders of losartan 0, 4.79, 9.58 mg/kg/day, amlodipine 0, 0.48, 0.96 mg/kg/day, or higher/lower doses in combination were administered in spontaneous hypertensive rats (SHRs; n=10). For the measurement of neointimal hyperplasia, 9.58 mg/kg/day of losartan or 0.96 mg/kg/day of amlodipine in alone or combination were given for 2 weeks to cuffed C57BL/6 mice (n=6). Our results suggest that the combination therapy of high doses of losartan and amlodipine are effective in lowering systolic blood pressure (SBP) and mean arterial pressure (MAP) as compared with monotherapy of each drug, but not in combined therapy of low doses of two drugs. In protection for vascular remodeling, the combination therapy of losartan with amlodipine synergistically decreased BrdU-positive cells in neointima and DNA synthesis in [3H]-thymidine incorporation by reducing cell proliferation. Therefore, present study implies that combination of losartan with amlodipine may be effective for the treatment of hypertension and vascular remodeling such as restenosis.
